Description
「n」は、上記ポリマーにおける繰り返し単位の数を指し、nは、2~1000、例えば2又は5から10、20、30、40、50、60、70、80、90、100、150、200、250、300、350、400、450、500、600、700、800又は900まで変化することができ、
G1及びG2は、独立に、ビグアニド又はグアニジンを含むカチオン性基を表し、L1及びL2は、上記グアニドの窒素原子に直接結合されている。従って、上記ビグアニド基又はグアニジン基は、上記ポリマー骨格に不可欠である。このビグアニド基又はグアニジン基は、式1aにおける側鎖部分ではない。
背景
炎症及び/又は疼痛の治療並びに管理のための治療剤としてどれを進めるのが最良かを決定するために、研究プログラムを、Nanocin(登録商標)(Tecrea Ltd(テクレア)、英国)(ポリヘキサメチレンビグアニド(PHMB))と配合したいくつかの抗炎症剤をスクリーニングするように選んだ。活性医薬成分(API)/Nanocin選択プロセスを以下の研究プログラムによって決定した。
・水及びエタノールのビヒクルの中での溶解度。
・粒子形成、サイズ及び品質に注目したときの選んだAPIとNanocinとの配合。
・注目する配合物のナノ粒子形成を確認するための電子顕微鏡観察の使用。
・APIの抗炎症活性の測定及びnanocinがこの効果に拮抗しないことの確認。
ジクロフェナク(D)が最も可溶性であったので、ジクロフェナク(D)を最初にNanocinと配合した。Nanocinに対するジクロフェナクの比(D:N)を試験し、比が異なると粒子サイズが変化することが示された。しかしながら、図1に示すように、多分散性指数(混合物中のナノ粒子サイズのばらつきの尺度)は、1:1mg/mlの混合物においてのみ「良好」として報告された。
上記配合物を、EM Support Systems Ltd(イーエム・サポート・システムズ)(英国)によって走査電子顕微鏡(SEM)を用いても調べた。最初に、乾燥試料として金でコーティングし、WET SEMも使用した。
他のAPIを0.33:1mg/ml又は1:1mg/ml(API:Nanocin)の濃度でNanocinと配合した場合、両方においてナノ粒子が形成される証拠があった。
図5~図7は、THP-1細胞(ヒト単球細胞株)を用いて確立された抗炎症アッセイの結果を示す。リポ多糖(LPS)で刺激した後のサイトカイン(特に炎症に関連するものであるTNF-α、IL-8、IL-1αに注目)の放出は、化合物がもつ抗炎症作用の潜在的能力を試験するための有効なモデル系である(参考文献1)。
各APIの0~30μg/mlの用量範囲をTHP-1細胞アッセイで試験した(図8a参照)。ラパマイシン、ジクロフェナク、シクロスポリン及びタクロリムスは、LPS炎症反応の阻害においてイブプロフェンよりも強力であると思われた。タクロリムス及びシクロスポリンのMICは0.3μg/mlであり、ジクロフェナク及びラパマイシンは1μg/mlであった。
両方のサイトカイン測定で、ジクロフェナク単独及びNanocin単独がLPS刺激性炎症反応を低下させることが示された。一緒に処方した場合、分泌されたサイトカインのレベルは、LPS刺激応答よりも依然としてかなり少なかった。このデータは、Nanocinが単独である程度の抗炎症作用を有し得ることを示唆する。
例2でより詳細に記載されているように、ブタの耳の皮膚を用いて、インビトロ浸透研究を実施した。
背景
これらの実験の目的は、浸透促進剤としてのNanocinを伴う及び伴わない、ブタ皮膚切片上の選択された活性医薬成分(API)の「インビトロ」浸透を評価することであった。
フランツセル分析に用いたブタの耳は、地元の食肉処理場から入手した。屠殺されたブタの月齢は4~6ケ月齢の間であった。耳を脱イオン水で洗浄し、外側の皮膚を下にある軟骨から注意深く取り外した。次いで、切除した皮膚を、使用するまで-20℃で保存した。浸透実験に使用した全ての耳は、調達後6か月以内であった。
ブタの皮膚を入手し、上記の方法1と同じ様式で予備調製を行った。耳を脱イオン水で洗浄し、次いで使用まで-20℃で保存した。浸透実験に使用した全ての耳は、調達後6か月以内であった。この実験設定のために、軟骨に付着した内側皮膚を使用して、安定性が増強された切片を作製した。
ブタの皮膚を入手し、処理後にフランツセルから試料を取り出すまで、方法1(上記)に記載したのと同じステップに従って、標準調製及びフランツセル処理を行った。
全てのToF-SIMS試料切片分析は、以下のような操作パラメーターを用いて超高真空条件下でToF-SIMS IV機器(ION-TOF GmbH(イオン-トフ)、ミュンスター(Muenster)、ドイツ)上で実施した。
・一次イオンビーム:ビスマス液体金属イオンガン(Bi3+) 25kV(約1.0のパルスターゲット電流)
・スパッタイオンビーム:NA
・分析装置:単段リフレクトロン
・電荷補償:pA。低エネルギー電子(20eV)
・データの取得及び分析:SurfaceLab6ソフトウェア(IONTOF GmbH)を使用して実施した。
・分析した面積:500μm×500μmの面積にわたってデータを取得した。
・解像度:256×256画素
・走査数:20スキャン
CoolLED pE-4000蛍光照明、pE-100明視野照明及びニコンプランフルオール(plan Fluor)10x(0.30 NA)対物レンズを装着した倒立型ニコンEclipse T1及びQIMAGING optiMOSカメラを用いて、蛍光顕微鏡法用の試料断面を撮像した。蛍光は、490nmでの励起によって、410~500(露光時間50μs)、500~550(露光時間200μs)、550~650(露光時間200μs)、650~750(露光時間200μs)の発光を収集することで捕捉した。10μsの露光時間で明視野を捕捉した。全ての蛍光画像及び明視野画像を12ビット画像(0~4095)に補正した。
プロジェクトの開始時の実験計画は、以下の項目を調製し分析することであった。
1. ToF-SIMSのためのAPI+Nanocin断面

> タクロリムス+Nanocin断面

> ジクロフェナク+FITC-Nanocin断面
シクロスポリン+Nanocin試料はすべて角質層剥離を伴い不首尾であった。イブプロフェン+Nanocin試料はすべて角質層剥離を伴い不首尾であった。ラパマイシン+Nanocin(2つの試料は成功裏に調製した)の例示的なデータを図19に示す。皮膚表面又は角質層内での活性ワークアップにおいて特定されたラパマイシンマーカーの証拠はなく、これは浸透を暗示し得る。しかしながら、これは、検出アッセイの感度に起因していた可能性がある。ほとんどの空間的分散を示すイオンはOCT及び皮膚化学に関連し、対照試料(ブランク)と一致した。ラパマイシンの完全な欠如(検出不能)が示唆された。
1つの試料は成功裏に調製され、1つの試料は重度の汚染のため除外した。データを図20に示す。皮膚表面、又は角質層内でのAPIワークアップにおいて、タクロリムスイオンマーカーの証拠は確認されず、これは浸透を暗示する。ほとんどの分散を示すイオンは、ここでも皮膚化学及びOCT媒体に関連しているようであり、対照試料(ブランク)と一致した。タクロリムスの完全な欠如(又は検出不能)が示唆された。
2つの試料を成功裏に調製した。図21にデータを示す。皮膚表面、又は角質層内でのAPIワークアップにおいて特定されたジクロフェナクイオンマーカーの証拠はなく、これは浸透を暗示する。しかしながら、基準ワークアップでは見られなかった他のイオン(C14H27O2-、C16H31O2-、C14H29O8-及びC22H43O2-)を見ることができ、角質層への局在と一致する空間分散を実証する。これらのイオンは(200~400m/z)の質量範囲にあると見られ、対照試料(未処理)には存在しなかった。これは、nanocin-ジクロフェナクの共配合物が化合物化学のイオン化マトリックスに対して、著しく異なった二次イオンフィンガープリントを生成するのに十分な影響を与えるということを示唆している可能性がある。そうであるならば、この分析で見られるイオンは、ジクロフェナク-nanocin複合体の浸透を反映している可能性があるが、これには研究をさらに広げる必要がある。
・この分析のためのブタの皮膚試料の調製の一貫性は不良であった。角質層の剥離は、持続的な問題であることが見出され、これらの処置を受けた試料の構造的完全性が低いことを特定した。従って、データは、全てのAPIについて収集されたわけではない。
・リファレンスから特定された二次イオンマーカーに基づいて、APIの局在化について成功裏に生成された試料について証拠を集めることはできなかった。
・しかしながら、ジクロフェナクの場合、注目する新しい二次イオンが角質層に関連することがコントラストサーチによって見られた。
・ジクロフェナクとnanocinとの共配合物は、上記化合物のイオン化パターンの実質的な変化を引き起こし、これが異なるフィンガープリントをもたらすと仮定した。
・この仮定が妥当であれば、これらの新しいマーカーは、ジクロフェナクの皮膚への浸透を示唆している可能性がある。
・しかしながら、API/nanocinの検出が検出限界を反映しているかどうかも不明である。
以下の実験は、API/nanocinが検出可能であるべき分析領域を最大にし、検出の欠如が閾値の問題ではないことを確実にするために使用した。

図22aは、シクロスポリン+Nanocinの結果を示す。イブプロフェン+Nanocin試料はすべて角質層剥離を伴い不首尾であった。図22bは、ラパマイシン+Nanocinの結果を示す。図22cは、タクロリムス+Nanocinの結果を示す。ジクロフェナク+Nanocin試料では、すべてが角質層剥離を伴い不首尾であった。
・この分析のためのブタの皮膚試料の調製の一貫性は、ここでも問題であることが見出された。角質層の剥離は持続しており、これらの処置を受けた試料の構造的完全性が低いことを特定した。従って、データは、全てのAPIについて収集されたわけではない。
・シクロスポリン、ラパマイシン及びタクロリムスの配合物(nanocinを伴う)について収集したデータは、S1.1で決定された代表的な二次イオンのイオン強度が、対照(ブランク試料)で見られるものとほぼ変わらないこと(テープストリップ1及び2において)を示した。
・これは、いかなる浸透についての証拠の欠如、及び上面での存在の証拠が限られている/全くないことを支持した。
FMイメージングがToF-SIMSデータと矛盾する明らかな浸透を示すかどうかを評価するために、2つのAPI(ジクロフェナク及びタクロリムス)FITC-nanocin試料を選択した。

ジクロフェナクは、浸透の何らかの示唆を特定する(断面分析)ことができる唯一の活性成分であったので、nanocinを伴って及び伴わずに、このAPIのみに焦点を当てることを決めた。検出効力を高めるために配合物中のAPIの高められた濃度(1mg/ml)を使用することも決めた。

3種類のジクロフェナク及び3種類のジクロフェナク+nanocinから得た上から3つのテープストリップ(テープ片)のToF-SIMS分析は、組み合わせ配合物は、活性成分単独では浸透しない角質層の最上層への活性成分の浸透を誘発したことを示唆しているように思われた。
フランツセル実験法により、API処理皮膚試料の全範囲を成功裏に生成した。しかしながら、その後の断面スライスへの試料の進行は、最も一般的には角質層の炎症及び剥離に起因するいくつかの試料の不備を伴い一貫性がないことが判明した。(FITC標識されていない)nanocin-API配合物で処理した試料に対してToF-SIMSを用いて収集した初期データは、特定されたイオンマーカーを用いてAPIもnanocinも検出できないという証拠を提供しなかった。API単独のToF-SIMS及びAPI+FITC-Nanocin FMを調べる実験は実施せず、このデータに基づいて方法調整を開始し、テープストリッピングを介して皮膚表面の何らかの横分析を行い、より大きな予想表面積を見た場合にイオンマーカーが検出され得るかどうかを確認した。
ヒト皮膚研究により、健常なヒトの皮膚へのNSAID(ジクロフェナク)の薬物送達増強が確認された(図31及び図32を参照)。ヒトの腹部の皮膚は、健康なヒトドナーから倫理的に入手した。三連の皮膚ディスクを、表皮面を最上にして静的拡散セル(フランツセル)中に配置した。薬物溶液(ジクロフェナク単独、又はナノ粒子を形成するためにポリヘキサニドと配合したジクロフェナク)を、フランツセルの上側チャンバーに添加した。このフランツセルを完全に組み立て、次いで、32℃で24時間インキュベーションした後、分析した。この時点で、フランツセルを分解し、皮膚ディスクを取り出した。このディスクを洗浄し、ティッシュで軽くたたくことによって乾燥させた。次いで、皮膚の上層を、粘着テープを使用して3回連続して剥離した。試料は、フランツセルの上側チャンバー及び下側チャンバーからも採取した。

Claims (15)
- ナノ粒子を形成することができるポリマーと、抗炎症剤及び/又は鎮痛剤とを含む炎症及び/又は疼痛の局所的な治療に使用するためのナノ粒子組成物であって、前記ポリマーが、直鎖状及び/若しくは分枝状若しくは環状のポリモノグアニド/ポリグアニジン、ポリビグアニドを含み、前記抗炎症剤及び/又は鎮痛剤が、以下の、ラパマイシン、タクロリムス、イブプロフェン、シクロスポリン、ジクロフェナク、及びナプロキセンから選択される1種以上を含む、前記ナノ粒子組成物。
- 前記ポリマーがポリヘキサメチレンビグアニドを含む請求項1に記載の組成物。
- 前記ナノ粒子が前記抗炎症剤及び/若しくは鎮痛剤と共に、並びに/又は前記抗炎症剤及び/若しくは鎮痛剤の存在下で形成される請求項1又は請求項2に記載の組成物。
- 前記抗炎症剤が、非ステロイド系抗炎症剤(NSAID)を含む請求項1から請求項3のいずれか1項に記載の組成物。
- 前記組成物が、緩衝剤、賦形剤、結合剤、油、水、乳化剤、グリセリン、酸化防止剤、防腐剤及び芳香剤のうちの1種以上の成分をさらに含む請求項1から請求項4のいずれか1項に記載の組成物。
- 前記炎症及び/又は疼痛が筋肉又は骨格のものである請求項1に記載の組成物。
- 前記組成物が、腱、靱帯、筋肉及び関節の外傷、リウマチ、関節痛若しくは関節炎の治療において使用するためのものである請求項6に記載の組成物。
- 前記組成物が、クリーム、ゲル、軟膏剤、スプレー、粉末、泡沫又はムースの形態にある請求項1から請求項7のいずれか1項に記載の組成物。
- 炎症及び/又は疼痛の治療に使用するための局所医薬の調製における、ラパマイシン、タクロリムス、イブプロフェン、シクロスポリン、ジクロフェナク、及びナプロキセンから選択される抗炎症剤及び/若しくは鎮痛剤を有する、又はラパマイシン、タクロリムス、イブプロフェン、シクロスポリン、ジクロフェナク、及びナプロキセンから選択される抗炎症剤及び/若しくは鎮痛剤と会合する1以上のナノ粒子を形成するためのポリヘキサメチレンビグアニド(PHMB)の使用。
- 前記抗炎症剤及び/又は鎮痛剤が非ステロイド系抗炎症剤(NSAID)を含む請求項9に記載のPHMBの使用。
- 前記ナノ粒子が、患部への抗炎症剤及び/又は鎮痛剤のための送達用媒体として使用される請求項9に記載のPHMBの使用。
- 前記患部が筋肉領域又は骨格領域である請求項11に記載のPHMBの使用。
- 前記炎症及び/又は疼痛が、腱、靱帯、筋肉及び関節の外傷、リウマチ、関節痛又は関節炎を含む請求項9から請求項12のいずれか1項に記載のPHMBの使用。
- 炎症及び/又は疼痛の局所的な治療のためのナノ粒子組成物の製造方法であって、直鎖状及び/若しくは分枝状若しくは環状のポリモノグアニド/ポリグアニジン、ポリビグアニドをラパマイシン、タクロリムス、イブプロフェン、シクロスポリン、ジクロフェナク、及びナプロキセンから選択される1種以上の抗炎症剤及び/又は鎮痛剤と混合する工程を備える方法。
- 前記方法が、請求項1から請求項8のいずれか1項に記載の組成物を製造するために使用される請求項14に記載の方法。
