Description
本出願は、2018年6月4日に出願された米国仮特許出願第62/680232号の利益を主張するものであり、この文献の開示は全体が引用により本明細書に組み入れられる。
これらの命題を満足させようとする上で、MDIに関連するVHCなどの装置は、(1)VHCと共に使用されるMDIを正しく識別し、(2)MDIが作動した時点を正しく識別し、(3)ユーザの正しい吸入法をモニタしてこれに関するフィードバックをユーザに提供し、(4)処方者及び/又は提供者に患者固有のデータを提供する、ことのうちの1つ又は2つ以上を実行するように構成することができる。図19及び図22~図24を参照すると、実施形態の1つの態様はデータの取り扱いに関連する。VHC及び/又はMDIによってログ記録されたデータは、スマートフォン、タブレット、パーソナルコンピュータなどの外部装置に転送することができる。このような外部装置が利用できない場合、データは、VHC及び/又はMDIのデータ記憶モジュール又はその他のメモリに内部的に記憶して、VHC/MDIと外部装置との間の次の同期時に転送することができる。VHC/MDIは、データの転送及び分析を実行するソフトウェアを伴うことができる。
多くの患者は、異なる時点又は状況で使用すべき複数の吸入器又は薬剤(例えば、救急薬及び/又は制御薬)を有しているので、客観的な患者固有の順守データを提供するためには、どの吸入器すなわち薬剤容器28が使用されているかを識別する必要性が重要である。一般的なタイプの非順守は、救急吸入器(rescue inhaler)28の方が症状を素早く緩和するとの理由で制御吸入器28’の代わりに救急吸入器を定期的に使用することである。医療提供者は、どの吸入器が使用されたかが分からなければ、患者が処方された治療プロトコルを遵守していたかどうかを判断することはできない。
スマート薬剤送達装置の1つの重要な側面は、吸入器が作動した時点を識別する能力である。本明細書で説明する解決策は、微小電気機械システムMEMS流量センサ150を使用して吸入器の作動を検出し、これによって両機能を果たすために必要とされるセンサの数を減少させる。図6~図8に、明確にするためにいくつかのコンポーネントを除去した装置の1つの実施形態を示す。図7及び図8には、MEMS流量センサを通じた吸気及び吸入器作動のための流路をそれぞれ示す。呼気のチャンバ内への、従ってMEMS流量センサ150への進入は、VHCの近位部の一方向吸入バルブ16によって防がれる。
1つの実施形態では、図15に示すように、MEMS流量センサ150を使用して、例えば欠陥バルブ16などの欠陥のある送達装置の指標を提供することもできる。通常の使用方法では、保持チャンバに呼気が入り込まないように構成されたマウスピース内の一方向バルブ16を通じて患者の呼気空気流が装置から排出される。一方向バルブ16が緩み、破れ又は開固着した場合には、保持チャンバ2の内部4に呼気が入り込むことによって装置のデッドスペースが増加し、チャンバから残りの薬剤が排出される恐れがある。この状況では、MEMS流量センサ150が呼気空気流を検出し、装置が、バルブが誤動作している恐れがあるため交換すべきであることを示す。アルゴリズムは、(短い急激なスパイクを生じる)作動と(より長く緩やかな曲線を生じる)呼気空気流とを識別する。
1つの実施形態では、図16及び図17A~図17Cを参照して分かるように、吸入器のプルーム(plume)に反射、透過又は遮断された光を測定することによって吸入器の作動を検出することができる。光検出技術を使用して、装置をいつ洗浄又は交換すべきであるかなどのフィードバックをユーザに提供することができる。1つの実施形態では、チャンバが新品又は洗浄したての時に光透過の基準レベルが設定される。作動毎に保持チャンバの内側に薬剤残留物が蓄積してこれを覆うことにより、送信機(例えば、LED158)によって送信されて光検出器160によって検出される光の量を減少させる。光検出器160の検出される光、従って出力信号の基準状態からの大幅な変化は、チャンバを洗浄する必要があり、或いは洗浄によって残留物を十分に除去できない場合には交換する必要があることを示す。図16には、新品の/清浄状態の装置(182)とチャンバ内に目に見えて薬剤が蓄積した装置(180)とを比較した、光検出器信号を介した光透過の基準測定値を示す。
治療順守を評価する上で重要な側面は、ユーザの吸入特性を含む吸入器の吸入法である。1つの実施形態では、図18を参照して分かるように、バルブ付き保持チャンバ上の流量インジケータ190の動きが、マウスピースを介したエアロゾル製剤の吸入を示す。流量インジケータ190は、(吸入、呼気及び/又はこれらの両方による)チャンバを通る流れが存在している間には動き、流れが存在しない期間中はその静止位置に戻る。図18に示すようなFED194及びフォトダイオード192を使用する場合、フォトダイオード192は、流量インジケータ190’が下がっている吸入中にFED194又はその他の発信源からの光を受け取る。吸入が停止すると、流量インジケータ190が直立位置に戻ってフォトダイオードによるFEDの観察を妨げる。この2値出力(開又は閉)を検出することで、各吸入器の作動について行われる吸入の長さ及び呼吸回数が識別される。この記録データから、患者の行動及び吸入法を推測することができる。このFED及びフォトダイオードを含む構成は、ハウジング内に収容してこれらのコンポーネントを周囲光から隔離することができる。流量インジケータが動く覗き窓198を真っ暗にすることもできると理解されたい。
1つの実施形態では、図1~図3C、図6及び図19を参照して分かるように、スマートバルブ付き保持チャンバ(VHC)が、バルブ付き保持チャンバ2と、電子バックピース8と、識別付属品120付きの吸入器又はMDIとを含む。スマートVHCの3つの主な特徴は、各吸入器作動の日付、時間及び識別を検出して記録し、作動に至るまでの吸入特性を記録し、正しい吸入法に関するフィードバックを提供する点である。

Claims (16)
- 薬剤送達システムであって、
入力端と、出力端と、前記入力端と前記出力端との間に延びる内部空間を定める壁部とを有し、薬剤が前記内部空間において前記入力端から前記出力端へ流れる保持チャンバと、
前記保持チャンバの前記入力端に結合されるバックピースであって、電気回路と、MDIの挿入部を受け入れるように形成された開口部とを含むバックピースと、
薬剤容器を受け入れるように形成された空洞を定める煙突部分と、前記薬剤を前記薬剤容器から前記保持チャンバの前記内部空間へ放出するオリフィスと、挿入部が前記開口部に収容される係合位置と前記挿入部が前記開口部から除去される解放位置との間で移動可能な前記挿入部と、前記挿入部が前記係合位置にある時に前記電気回路を完結する少なくとも1つの接点とを含むMDIと、
を備え、
前記MDIは、前記挿入部を定めるアクチュエータブーツを含むと共に、前記アクチュエータブーツに解除可能に結合されたアダプタをさらに含み、前記アダプタは、1対の前記接点を含み、
この1対の接点は、1対の磁石を含み、前記アクチュエータブーツは、前記挿入部が前記係合位置にある時に前記バックピースに磁気的に結合される、

前記MDIの煙突部分と係合するように構成されたアダプタと、
前記アダプタに解除可能に結合され、前記MDIの前記挿入部を受け入れるように形成された中央開口部を有するリングを含むフェイスプレートと、を備え、
前記アダプタと前記フェイスプレートは、前記フェイスプレートが前記バックピースに結合されている係合位置と、前記フェイスプレートが前記バックピースに結合されていない解放位置との間で移動可能であり、
前記フェイスプレートは、前記アダプタと前記フェイスプレートが係合位置にある時に、電気回路を完結させる少なくとも1つの接点を含む、
