図33aから33dは、2つのらせん状のトラス部材3515a、3515bを有するトラスを形成する例示的な方法のステップを示している。図33aに示されている第1のステップでは、縫合糸又は他の生体吸収性ポリマーフィラメントの形態の、第1のトラス部材3515aが、一端でクランプ3544によってクランプされ、かつ棒状のマンドレル3530の周りに第1のピッチ長P1でらせん状に巻かれている。次に、2つの細長補強部材3516も、それらの端部でクランプ3544によってクランプされ、マンドレルの相対する両側に沿って、らせん状のトラス部材の上に置かれる。次に、図33cに示されるように、第2のトラス部材3515bが、クランプ3544によってクランプされ、第1のらせん状の部材3515a、補強部材3516、及びマンドレル3530の周りに巻かれる。次に、マンドレル3530又は環境が、加熱されて、補強部材を、第1のらせん状の部材に、それらが重なり合う点で、溶着させる。トラス3507が冷却されてトラス部材の形状がセットされ、それからクランプ3544及びマンドレル3530が取り外されて、図33dに示されるように中空のトラス3507が残る。方法のステップの順序は、様々であってもよく、かつすべてのステップが必要というわけではないことは明らかであろう。
本発明のデバイスは、生体吸収性材料で形成されている。典型的には、2つのタイプの生体吸収性材料が、使用され、1つは、可撓性シート及びあらゆる織物のためのものであり、もう1つは、トラスのためのものである。
あらゆる所望の生物活性分子を、ECM、若しくはポリマー材料、又はトラス部材材料自体に組み込むことができる。適切な分子には、例えば、小分子、ペプチド類、若しくはタンパク質類、又はそれらの混合物が含まれる。生物活性材料は、ECM内部に生じるものであり得、又は埋込み片を製造するプロセスの間若しくは後に、ECM及び/若しくはポリマー材料の中に組み込まれる材料であり得る。いくつかの実施形態では、2以上(例えば、2、3、4、5、6、7、8、9、10又はそれを超える)の異なる生物活性分子を、非共有結合的に、ECM又はポリマーの中に組み込むことができる。生理活性分子は、懸濁液、カプセルに入れられた粒子、微粒子、及び/若しくはコロイドとして、又はそれらの混合物としてのいずれかで、非共有結合的に材料の中に組み込むことができる。生物活性分子は、ECM/ポリマー材料層間で分配されることができる。生物活性材料は、ドキシサイクリン、テトラサイクリン、銀、FGF-2、TGF-B、TGF-B2、BMR7、BMP-12、PDGF、IGF、コラーゲン、エラスチン、フィブロネクチン、及びヒアルロナンを含むタンパク質類、成長因子、抗菌剤、及び抗炎症剤を含む場合があるが、これらに限定されない。
デバイスの外科的配置は、図4に最もよく示されている。図4では、デバイス101は、例えば腹腔内の、筋肉103に固定されて示されている。デバイス101のポート111は、切開部142から外へ出て示されている。切開部142は、主要な外科的切開部134とは別個のものであり、デバイス101が皮膚106の外面上に置かれることを可能にする。デバイス層119、121を分離するために使用されるデバイス101の構造は、皮膚106の外面上に見ることができ、かつ、この特定の図では、組織皮下層104を通過するように示されており、そこでは、デバイス101の下面が、下で筋肉103に固定されている。
癌性組織の外科的切除後又は進行中の感染が憂慮される場合の多くの臨床的手法において、手術後、流動性の細胞ベースの流体を、処置部位に制御可能に注入及び投与する能力が望ましい。投与濃度、契約時間(contract time)、投与量、及び処置部位における部位などの様々なパラメータを精密に制御する能力は、投与プロファイルについて材料の分解特性にしばしば依存する既存の薬剤溶出、又は投与される埋込みデバイスに対する優位性も与える。
実施例1:外科的切開部の下の皮下組織死腔を閉じる
大量の脂肪(adipose)(脂肪(fat))組織が関わる手法において、閉じられた外科的切開部の下の皮下組織の管理は、臨床的に困難な場合がある。脂肪組織は、縫合を維持するその能力において、機械的強度に乏しいことが知られており、皮膚と下にある筋肉との間の距離の増加は、手術後に流体を蓄積させる、死腔の形成につながる場合がある。このことは、後の、創傷離開及び手術部位の感染などの合併症につながる場合がある。以下で与える実施例は、このデバイスが、外科的切開部の下の外科的死腔を排除するためにどのように利用され得るかを示している。
主要な外科的切開部の表面に陰圧を加える、局所的に適用される創傷処置デバイスは、創傷離開及び手術部位の感染などの外科的合併症の予防のために、広く採用されるようになった。これらの局所的に適用されるデバイスは、主に、副次的な機械的保持を提供して主要な縫合線上の張力を低減することによって、並びに皮膚からの過剰な滲出液を覆いかつ除去して浸軟及び感染を防ぐことによって、治癒を助ける。
デバイストラス部材の製造の1つの実施例は、図33a~図33dに関連して上記で全体的に説明されており、トラス部材3515a、3516、及び3515bは、それらの第1の端部でクランプされ、中央マンドレル3530の周りにきつく巻かれ、それらの反対側の端部でクランプされている。
トラスを連続プロセスで製造する代替的方法が、以下で与えられる。図34aを参照すると、長手方向の2つの「補強」トラス部材3616が、中央成形マンドレル3630に沿って供給される。第1のトラス部材3615は、局所的な熱を加えることにより、端部3607aに沿って、長手方向の2つの「補強」トラス部材3607aに取り付けられる。この領域のその後の冷却により、この局所的な区間の溶着が完了し、トラス部材が互いに固定され、回転する巻取フィーダが第1のトラス部材3615を中央マンドレル3630及び補強トラス部材3616の周りに連続的に巻き付けるようにさせると同時に、補強トラス部材3616が中央成形マンドレル3630に沿って供給される。
体内の標的の処置部位に薬剤及び液体を投与する能力は、特に痛み、局所感染、又は疾患の処置のために、医療の分野で重要なツールとなっている。薬剤の日常的な投与は、世界中で多くの患者に一般的であるが、処置期間は、短期間から生涯依存する患者まで、大きく異なり得る。
Claims (17)
- 処置部位から流体を排出し、又は処置部位に流体を送達するための、患者の体内の処置部位への埋込み用の生体吸収性デバイスであって、
骨組みを形成するために別個の接合箇所で互いに接合された第1の及び第2の可撓性の細長トラス部材を含み、2つの組織表面を離間して保持することによりチャネルを画定するための、生体吸収性弾性トラス構造であって、該処置部位からの流体が該チャネル内へ流出でき、又は該チャネルから該処置部位へ流体が送達されることができる、生体吸収性弾性トラス構造と、
1つ以上の該チャネルと流体連通しており、陰圧源又は陽圧源に接続可能なポートと、
を含み、
少なくとも1つのトラス部材は、実質的にらせん状である、
デバイス。 - 少なくとも1つのトラス部材は、前記チャネルの壁に沿って配置されている、請求項1に記載のデバイス。
- 前記トラス構造は、前記チャネルを画定する可撓性チューブを形成する、請求項1から2のいずれか一項に記載のデバイス。
- 前記第1のトラス部材は、第1のピッチ長を有して実質的にらせん状であり、
前記第2のトラス部材は、第2のピッチ長を有して実質的にらせん状であり、第1のトラス部材と反対の方向にらせん状に巻かれており、該第2のピッチ長は、該第1のピッチ長と同じである、
請求項1から3のいずれか一項に記載のデバイス。 - それぞれ前記チャネルの側面に沿って長手方向に延びており、かつ別個の接合箇所で前記第1のトラス部材及び/又は前記第2のトラス部材に接合されている、2つの可撓性の細長側部トラス部材をさらに含む、請求項4に記載のデバイス。
- 可撓性生体吸収性シートをさらに含み、前記チャネルは、前記可撓性シートの表面と、前記処置部位の組織の表面又は前記処置部位の骨の表面との間に形成されているように、前記シートは、前記チャネルの壁の少なくとも一部を形成している、請求項1から5のいずれか一項に記載のデバイス。
- 前記可撓性シートは、前記トラス構造の周りに巻き付けられている、請求項6に記載のデバイス。
- 2つの可撓性生体吸収性シートをさらに含み、前記チャネルは、該2つの可撓性シートの相対する表面の間に形成されている、請求項1から7のいずれか一項に記載のデバイス。
- 前記チャネル内への流体の流れを可能にするための、可撓性シート又は少なくとも1つの可撓性シートの、複数の開口部を前記チャネルの壁に沿ってさらに含む、請求項6から8のいずれか一項に記載のデバイス。
- 少なくとも1つのトラス部材が、少なくとも1つの可撓性シートに織り込まれ若しくは少なくとも1つの可撓性シートの中を縫って通り、又は少なくとも1つの可撓性シートに縫い込まれ若しくは縫い付けられている、所定の長さの糸又はテープを含む、請求項6から9のいずれか一項に記載のデバイス。
- 前記可撓性シート又は各可撓性シートは、細胞外マトリックス(extracellular matrix:ECM)又はポリマー材料の、1つ以上の層を含む、請求項7から10のいずれか一項に記載のデバイス。
- 前記ECMは、反芻動物の前胃の脱細胞化された固有粘膜下組織で形成されている、請求項11に記載のデバイス。
- 前記ECMは、ドキシサイクリン、テトラサイクリン、銀、FGF-2、TGF-B、TGF-B2、BMR7、BMP-12、PDGF、IGF、コラーゲン、エラスチン、フィブロネクチン、及びヒアルロナンを含む群から選択される、生物活性剤を含有する、請求項12に記載のデバイス。
- 前記トラス構造は、前記チャネルを画定する細長可撓性チューブを形成し、
前記デバイスは、少なくとも所定の長さの該可撓性チューブを波状の形状に保持する、1つ以上の接合器を含む、請求項1から13のいずれか一項に記載のデバイス。 - 患者の体内で、処置部位から流体を排出し、又は処置部位に流体を送達するためのシステムであって、
(i)請求項1から14のいずれか一項に記載のデバイスと、
(ii)デバイスの前記ポートに流体を送達するように該デバイスの前記ポート又は流体不透過性被覆材のいずれかに解放可能に結合された導管と、
(iii)該患者の該体の外部に配置されたリザーバであって、該導管から流体を受け又は該導管に流体を送達するために、該導管と流体連通している、リザーバと、
(iv)該デバイスに陽圧又は陰圧を送達するために該導管に結合された圧力源と、
を含む、システム。 - 前記圧力源は、流体が前記処置部位から前記デバイス内に排出されて、前記導管を通して前記リザーバに移送されるように、陰圧を前記デバイスに与えることができる、請求項15に記載のシステム。
- 流体を、前記処置部位から運ぶために、及び前記処置部位に運ぶために、圧力源が、該デバイスに陰圧を送達するように構成される、請求項15に記載のシステム。
