Description

痙攣という用語は、発作と相互交換可能に使用される場合が多い。痙攣は、人の身体が迅速にかつ制御されずに震える際に生じる。痙攣の間、人の筋肉は、反復して収縮および弛緩する。多くの異なる型の発作が存在する。一部は、震えを伴わない軽度の症状を有する。一部の発作は、人に意識障害(staring spells)を有させるだけである。これらは、気づかれないままの場合がある。痙攣の具体的な症状は、脳のどの部分が関与するかに依存し得る。症状は通常、突然生じ、これには、以下が含まれ得るがこれらに限定されない:短時間のブラックアウトとその後の錯乱の期間(この人は、短時間記憶していることができない);行動における変化、例えば、人の服をいじる;よだれを垂らすまたは口から泡を吹く;眼の動き;唸り声をあげるおよび鼻を鳴らす;膀胱または腸の制御の喪失;気分の変化、例えば、突然の怒り、説明のつかない恐れ、パニック、歓喜または笑い;全身の震え;突然の転倒、苦味または金属風味の感覚;歯の食いしばり;呼吸における一時的な停止、ならびに/またはピクピクするおよび引き付けを起こす四肢を伴う制御不能な筋肉けいれん。症状は、数秒もしくは数分後に停止し得る、または最大で15分間にわたって継続し得るが、まれに、より長く継続し得る。
低カルシウム血症に関連する痙攣を有する4~7日齢の乳児への、グルコン酸カルシウム、硫酸マグネシウムまたはフェノバルビトン(phenobarbitone)の投与が報告されている。3つ全ての組成物が、痙攣の率を低減させるようであり、処置内比較は、硫酸マグネシウムが、グルコン酸カルシウムまたはフェノバルビトンのいずれよりも有効であったことを示した。しかし、参考文献は、カチオンベースの治療を必要としない乳児におけるグルコネート組成物の有効性に関して、いずれの開示も提供していない。Turner, T., “Comparisons of phenobarbitone, magnesium sulphate, and calcium gluconate in treatment of neonatal hypocalcaemic convulsions” Paediatric Research Society Abstracts, pg 244。
一実施形態では、本発明は、電位依存的CLC-3 Cl-チャネルが、特に新生仔てんかんの間に、細胞内Cl-濃度([Cl-]i)を制御する際に役割を果たし得ることを企図する。例えば、CLC-3チャネルは、成体脳ではなく新生仔脳において大きい電位依存的外向き整流Cl-電流を媒介し得るが、これは、グルコン酸によって阻害することができる。例えば、データは、グルコネートが、新生仔てんかん活動を強力に抑制し、成体動物では効果がより少なかったことを示している。さらに、CLC-3ノックアウトマウスは、新生仔脳における電位依存的外向き整流Cl-電流の非存在、および低減された再発性てんかん活動を示した。EEG記録もまた、グルコネートが、成体動物ではより有効性が低いが、新生仔動物において発作バースト活動を阻害するのにより有効であったことを確認した。最後に、新生仔てんかんの間に観察されたCLC-3チャネル活性化は、[Cl-]iを顕著に増加させたが、グルコネートを使用してCLC-3チャネルを遮断することは、[Cl-]i蓄積を阻害した。一実施形態では、本発明は、グルコネート(天然の有機化学物質)が、有効なCLC-3 Cl-チャネル遮断剤であり、新生仔てんかんを処置するための抗痙攣薬として有効であり得ることを企図する。
てんかん発生の間のCl-の機能的役割を、バス溶液中の細胞外Cl-(すなわち、例えば、137mMのCl-)を、グルコン酸などの大きいアニオンで置き換えることによって調査した。驚くべきことに、バス溶液中のおよそ5~20mMの間の濃度のグルコン酸ナトリウム(NaGcA)が、培養された皮質ニューロンにおいて、自発的てんかん様活動を阻害した。図1Aを参照のこと。バス溶液中に117mMを超えるCl-がなおも存在したので、低濃度のNaGcAによるてんかん様活動のかかる強力な阻害は、Cl-濃度変化によって単純に説明することはできなかった。NaGcAがてんかん様活動を直接阻害し得るかどうかを決定するために、シクロチアジド(CTZ)を使用して、ニューロン培養物においてロバストなてんかん様バースト活動を惹起した。Qi et al., “Cyclothiazide induces robust epileptiform activity in rat hippocampal neurons both in vitro and in vivo” The Journal Of Physiology 571:605-618 (2006)。データは、CTZ誘導性てんかん様活動が、10mMのNaGcAによって完全に遮断され、阻害が用量依存的であったことを実証した。それぞれ、図1Bおよび図1Cを参照のこと。バースト活動は、NaGcAを洗い流した後に、対照レベルまで回復したので、NaGcAの効果は可逆的であった。図1Aおよび図1Cを参照のこと。これらのデータは、NaGcAが、培養されたニューロンにおけるてんかん様バースト活動に対して強力な阻害効果を発揮することを示している。
ニューロン死は、てんかんの重篤な副作用である。Sagar et al., “Hippocampal neuron loss in temporal lobe epilepsy: correlation with early childhood convulsions” Annals of Neurology 22:334-340 (1987);およびSass et al., “Verbal memory impairment resulting from hippocampal neuron loss among epileptic patients with structural lesions” Neurology 45:2154-2158 (1995)。本明細書に提示されるデータは、グルコネートが、抗てんかん効果を有するだけでなく、KA誘導性ニューロン死に対する神経保護効果を発揮することを実証している。

培養されたニューロンに対するグルコネートの上記効果がアーチファクトであった可能性、NaGcAの抗てんかん活性を、比較的インタクトなニューロン回路を有する海馬スライスにおいて試験した。フィールド電位を、CA3錐体層において記録し、Mg2+なしの人工脳脊髄液(aCSF)中でてんかんバースト活動を誘導した。安定なてんかんバースト活動の誘導後(約30分)、20mMのNaGcAを、Mg2+なしのaCSF中に適用して、てんかん活動に対するその影響を試験した。データは、NaGcAが、P6~P12の新生仔動物由来の脳スライスにおいて抗てんかん効果を発揮したが(図2Aおよび2c)、およそ1カ月年かさの動物では、中程度の効果を発揮したことを示している。図3Eと比較して図3Aおよび3Cを参照のこと。次いで、フィールド電位のパワースペクトルを、NaGcA適用の前、その間およびその後に解析した。パワーの振幅は、新生仔動物ではNaGcAの適用後に顕著に低減されたが、年かさの動物(例えば、P26)ではわずかにのみ低減された。図3Fと比較して図3Bおよび3Dを参照のこと。定量的に、NaGcAは、新生仔動物においててんかん活動の平均パワーの60%を阻害したが(P6~8、59.6±4.3%、n=5匹の仔からの10個のスライス;P10~12、62.1±3.8%、n=5匹の仔からの10個のスライス;P<0.001、対応のあるt検定)、年かさの動物では20%だけ低減させた(P21~33、23.8±4.1%、n=4匹の仔からの7個のスライス;p<0.005、対応のあるt検定)。図3Gを参照のこと。これらの結果は、NaGcAが、新生仔脳において強力な抗てんかん活性を有し得ることを示唆している。
上で議論したように、NaGcAは、培養されたニューロンにおいてCl-電流を阻害した。図2を参照のこと。以前の研究は、海馬の培養されたニューロンが、電位依存的CLC-3 Cl-チャネルを有したことを報告した。Wang et al., “CLC-3 channels modulate excitatory synaptic transmission in hippocampal neurons” Neuron 52:321-333 (2006)。本明細書に示されるように、グルコネートは、CLC-3チャネルによって媒介される外向き整流Cl-電流に対する強い阻害効果を有する。CLC-3チャネルを介したシナプス後Cl-流入は、NMDA受容体媒介性興奮性電流を強化することができ、NMDA受容体の過剰活性化は、発作誘導性ニューロン細胞死に関与する。従って、グルコネートの神経保護効果は、NMDA受容体機能の間接的抑制を介してであり得る。
CLC-3チャネルがてんかん発生に関与するかどうかを決定するために、CLC-3発現レベルを、P8~P12新生仔脳スライスにおいててんかん活動の誘導の前および後に決定した。海馬スライスを、Mg2+なしのaCSF中で1時間インキュベートして、てんかん活動を誘導し、次いで、CLC-3免疫染色を実施して、CLC-3発現レベルを試験した。対照スライスは、それに応じて正常aCSF中で1時間インキュベートした。興味深いことに、海馬CA3錐体層におけるCLC-3免疫反応性は、対照と比較して、0 Mg2+処理したスライスにおいて有意な増加を示した。図6A、Bを参照のこと;対照、11.2±1.5a.u.、n=4匹の仔からの10個のスライス;0 Mg2+、20.4±2.8a.u.、n=4匹の仔からの11個のスライス;P<0.02、スチューデントt検定。かかる上昇した発現レベルのCLC-3を、ウエスタンブロットによってさらに確認した。図6C、Dを参照のこと。従って、CLC-3チャネルは、新生仔てんかんスライスにおいて上方調節される。
新生仔発作を誘導するために、一般に使用されるKAモデルを使用した。P10~P12新生仔マウス中へのKAの注射(2mg/kg、i.p.)は、in vivo EEG記録によって明らかなように、ロバストなてんかん発作バーストを惹起した。図7A、Bを参照のこと。NaGcA(2g/kg、i.p.)をKA注射の10分後に注射した場合、てんかん発作活動は、本質的に無効にされた。図7C、Dを参照のこと。パワー解析は、パワーの振幅が、NaGcA注射によって顕著に阻害されたことを確認した。図7E、Fを参照のこと。これらのデータは、NaGcAが強力なin vivo抗新生仔発作薬物であることを示している。
NaGcAを、この化合物が成体てんかんを処置するためにも使用することができるかどうかに関して決定した。予備データは、KA痙攣モデルが高い死亡率を有することを示した。しかし、類似のPTZモデルは、成体マウスにおいて信頼性のあるてんかん発作を生じた。図8Aを参照のこと。データは、NaGcAが、PTZによって誘導されたてんかん発作バーストの潜時を延長させたが、新生仔動物において見られるものと同様に有効には、てんかんバースト活動を抑制しなかったことを示している。図8を参照のこと。従って、NaGcAは、新生仔発作を処置するにあたり、成体てんかんについてよりも強力な独自の抗てんかん薬であり得る。
関連の疑問は、新生仔発作をこれもまた阻害する他のCl-チャネル遮断剤、例えば、NPPBおよびNFAもまた、Cl-イオンチャネル関連活動を遮断するということである。図9および図10を参照のこと。一実施形態では、本発明は、グルコネート、NPPB、ニフルミン酸、DIDS、フルフェナム酸が含まれるがこれらに限定されない化合物が、新生仔発作を阻害するにあたり同様に作用し得ることを企図する。発明の機構を理解することは必要ではないが、新生仔発作阻害は、クロライドイオンチャネル阻害によって生じ得ると考えられる。
一実施形態では、本発明は、グルコースオキシダーゼが、発作活動を阻害するための抗痙攣薬として使用され得ることを企図する。発明の機構を理解することは必要ではないが、グルコースオキシダーゼは、グルコースを、てんかん様活動を低減させることが本明細書で示されたグルコネート(上記)に変換すると考えられる。本明細書に提示されるデータは、グルコン酸が、おそらくはCLC-3 Cl-チャネルを遮断することによって、発生中の脳における発作活動に対して強い阻害効果を有することを実証している。データは、グルコースをグルコン酸へと酸化する酵素であるグルコースオキシダーゼ(GOx)もまた、新生仔マウス由来の海馬スライスにおいて抗てんかん様活性を有することもまた示している。
GABA機能の変更は、てんかんと密接に関連している。GABA機能は、細胞内Cl-濃度([Cl-]i)によって決定され得るが、それは、GABAA受容体(GABAA-R)が、リガンド開口型Cl-チャネルであると考えられるからである。例えば、以前の研究は、[Cl-]iおよび従ってGABA機能を調節する際の、Cl-トランスポーター、例えば、KCC2およびNKCC1の役割を大々的に調査した。しかし、他のCl-チャネルがどのように[Cl-]iおよびGABA機能に影響を与え得るかは、十分には理解されていない。
新生仔てんかんにおけるCLC-3チャネルの機能的役割を理解するために、CLC-3チャネル発現レベルを、新生仔脳スライスを0 Mg2+人工脳脊髄液(aCSF)で処理することによるてんかん様活動の誘導後に決定した。
一実施形態では、本発明は、新生仔てんかんをグルコネート化合物で処置するステップを含む方法を企図する。一実施形態では、グルコネート化合物は、全身投与される。
発明の機構を理解することは必要ではないが、CLC-3チャネルは電位依存的外向き整流Cl-チャネルであるので、[Cl-]iにおける変化は、新生仔てんかんにおけるCLC-3チャネルの少なくとも1つの分子機構を代表し得ると考えられる。例えば、てんかんバースト活動は、10秒よりも長く持続する場合が多かった。図16A。
本発明は、医薬組成物(例えば、上記化合物を含む)をさらに提供する。本発明の医薬組成物は、局所処置が所望されるか全身処置が所望されるか、および処置される領域に依存して、いくつかの方法で投与され得る。投与は、外用(点眼、ならびに膣および直腸送達を含む粘膜へのものを含む)、肺(例えば、ネブライザーによるものを含む、粉末またはエアロゾルの吸入または吹き入れによる;気管内、鼻腔内、表皮および経皮)、経口または非経口であり得る。非経口投与には、静脈内、動脈内、皮下、腹腔内もしくは筋肉内注射もしくは注入;または頭蓋内、例えば、くも膜下腔内もしくは脳室内投与が含まれる。特に、グルコン酸ナトリウムの筋肉内注射および/または静脈内注射は、無菌食塩水溶液中で、1~100mMのグルコネートのおおよその濃度範囲で送達され得る。発明の機構を理解することは必要ではないが、これらの濃度範囲は、グルコン酸ナトリウムが高度に水溶性であるために達成可能であると考えられる。
本発明は、大まかに均一な分布を提供するいくつかの薬物送達系が、制御可能な速度の放出を有することを企図する。薬物送達系を創出する際に有用な種々の異なる媒体が、以下に記載される。いずれか1つの媒体または担体が本発明を限定することは意図しない。任意の媒体または担体が、別の媒体または担体と組み合わされ得ることに留意されたい;例えば、一実施形態では、化合物に結合されたポリマーミクロ粒子担体は、ゲル媒体と組み合わされ得る。

本発明の一実施形態は、本明細書に記載される治療剤を結合および放出することが可能なリポソームを企図する。リポソームは、リン脂質などの両親媒性分子でできた、水性コアを囲む微視的球状脂質二重層である。例えば、リポソームは、リン脂質ミセルの疎水性テイル間に治療剤を捕捉し得る。水溶性薬剤は、コア中に捕捉され得、脂溶性薬剤は、シェル様二重層中に溶解され得る。リポソームは、水溶性化学物質および水不溶性化学物質が界面活性剤の使用も他の乳化剤の使用もなしに媒体中で一緒に使用されるのを可能にするという点で、特別な特徴を有する。リポソームは、リン脂質(phosopholipid)を水性媒体中に力強く混合することによって、自発的に形成し得る。水溶性化合物は、リン脂質を水和させることが可能な水溶液中に溶解される。従って、リポソームの形成の際に、これらの化合物は、水性リポソーム中心内に捕捉される。リン脂質膜であるリポソーム壁は、油などの脂溶性材料を保持する。リポソームは、組み込まれた化合物の制御放出を提供する。さらに、リポソームは、薬物動態学的半減期を増加させるために、ポリエチレングリコールなどの水溶性ポリマーでコーティングされ得る。本発明の一実施形態は、リポソーム産生を洗練するための超高せん断テクノロジーを企図し、具体的に設計された構造的特徴を有する安定な単層(単一層)リポソームを生じる。リポソームのこれらの独自の特性は、通常は不混和性の化合物の同時貯蔵、およびそれらの制御放出の能力を可能にする。
ミクロスフェアおよびマイクロカプセル剤は、一般に均一な分布を維持するそれらの能力に起因して有用であり、安定な制御された化合物放出を提供し、産生および調剤するのに経済的である。好ましくは、関連した送達ゲルもしくは化合物含浸ゲルは、透明であり、またはあるいは、前記ゲルは、医療関係者による容易な視覚化のために着色される。
細胞培養および脳スライスのための動物プロトコールは、National Institutes of Health Guide for the Care and use of Laboratory Animalsに従って、Pennsylvania State University IACUCによって承認された。成体マウスまたは新生仔ラットに対するin vivo実験について、全ての手順は、動物研究のための倫理ガイドラインに従って、それぞれ、Fudan UniversityまたはSouth China Normal UniversityのCommittee of Animal Use for Research and Educationによって承認された。動物部屋は、12時間の明/暗サイクルで自動的に制御し、水および食物は、自由に利用可能にした。
細胞培養およびトランスフェクション
マウス皮質ニューロンを、以前に記載されたように、新生C57BL/6マウスから調製した。Qi et al., “Cyclothiazide induces robust epileptiform activity in rat hippocampal neurons both in vitro and in vivo” The Journal Of Physiology 571:605-618 (2006)。簡潔に述べると、新生マウス大脳皮質を、氷冷HEPES緩衝食塩水溶液中でばらばらにし、洗浄し、0.05%のトリプシン-EDTAで37℃で20分間消化した。血清含有培地によるトリプシンの非活性化後、細胞を遠心分離し、再懸濁し、24ウェルプレート中に、10,000細胞/cm2の密度で、皮質アストロサイトの単層上に播種した。ニューロン培養培地は、MEM(500ml、Invitrogen)、5%の胎仔ウシ血清(Atlanta Biologicals)、10mlのB-27サプリメント(Invitrogen)、100mgのNaHCO3、2mMのGlutamax(Invitrogen)ならびに25単位/mlのペニシリンおよびストレプトマイシンを含有した。AraC(4μM、Sigma)を添加して、アストロサイトの過剰な増殖を阻害した。細胞培養物を、5%CO2加湿インキュベーター中で37℃で14~21日間維持した。
細胞生存度アッセイ
エチジウムホモダイマー-1およびカルセイン-AMを含有するLIVE/DEAD(登録商標)Viability/Cytotoxicity Assay Kit(L3224、Life Technologies)を使用して、細胞生存度を試験した。エチジウムホモダイマー-1は、細胞DNAに結合し、典型的には、死細胞を赤色蛍光で標識するが、カルセイン-AMは、生細胞中のエステラーゼによって切断されて、強い緑色蛍光を生じ得る。薬物処理後、ニューロンを、1μMのカルセイン-AMおよび4μMのエチジウムホモダイマー-1を含有するバス溶液中で、室温で40分間インキュベートした。細胞の生存および死の比率を、それぞれ、緑色蛍光細胞および赤色蛍光細胞のパーセンテージを定量化することによって測定した。各群について、各カバーガラスの少なくとも5つの視野を、データ解析のためにイメージングした。
マウス脳スライス調製
脳スライスを、C57BL/6マウス(雄性および雌性)から調製した。動物を、Avertin(トリブロモエタノール、250mg/kg)で麻酔し、断頭した。海馬水平切片(400μm)を、以下の氷冷人工脳脊髄液(aCSF)中で、Leica VT1200Sビブラトームによって調製した(mMで):125 NaCl、26 NaHCO3、10 グルコース、2.5 KCl、2.5 CaCl2、1.25 NaH2PO4および1.3 MgSO4、モル浸透圧濃度290~300mOsm、95%のO2/5%のCO2を通気。次いで、スライスを、カルボゲン(95%のO2/5%のCO2)で飽和させた正常aCSFを含有するインキュベーションチャンバーに33℃で30分間移し、その後、使用前に室温で1時間回復させた。個々のスライスを、浸された記録チャンバーに移し、これらを、95%のO2/5%のCO2で飽和させたaCSFで31~33℃(TC-324B、Warner instruments Inc)で持続的に灌流した(2~3ml/分)。スライスを、DICオプティックを備えたOlympus顕微鏡を使用して、赤外線光学で可視化した。
電気生理学
細胞培養
培養されたニューロンを、以下からなるバス溶液の連続的灌流を伴って、記録チャンバー中に配置した(mM):128 NaCl、10 グルコース、25 HEPES、5 KCl、2 CaCl2、1 MgSO4、NaOHでpH7.3に調整、モル浸透圧濃度約300mOsm。電流クランプモード下で自発的発火を記録するために、ピペットに、以下を含有する内部溶液を満たした(mMで):125 K-グルコネート、5 Na-ホスホクレアチン、5 EGTA、10 KCl、10 HEPES、4 Mg-ATP、0.3 Na-GTP、280~290mOsm、KOHでpH7.3に調整。培養されたニューロンにおけるてんかん様活動を、10μMのCTZで24時間、または1μMのKAで2時間、または50μMの4-APで2時間、誘導した。バースト活動は、以前に記載されたように定義した。Qi et al., “Cyclothiazide induces robust epileptiform activity in rat hippocampal neurons both in vitro and in vivo” The Journal Of Physiology 571:605-618 (2006)。簡潔に述べると、大きい脱分極シフト(≧10mVの脱分極、持続時間≧300ミリ秒)の上を覆う少なくとも5つの連続した作用電位。
フィールド電位記録を、外部溶液を満たしたガラス電極(2~4ΜΩの先端抵抗)を用いて実施した。ガラス電極を、CA3錐体層中に配置した。電流クランプ(I=0)記録のために、増幅器を、0.1~5KHzのバンドパスフィルターと共に、100×に設定した。全ての記録を、31~33℃で実施した。てんかん活動を、Mg2+なしのaCSFによって、またはaCSF中50μMの4-APもしくは8.5mMのK+の添加によって惹起した。
免疫染色およびウエスタンブロット
脳スライスを、免疫染色のために200μmの厚さ、ウエスタンブロットのために400μmの厚さで、上記電気生理学プロトコールに従って調製した。一般に、スライスを、室温で1時間回復させ、次いで、2つの群にランダムに分割した。スライスの1つの群を、正常aCSF中で33℃で1時間インキュベートした。スライスの別の群を、0 Mg2+aCSF中で33℃で1時間インキュベートした。次いで、スライスを、4%のPFAによって4℃で一晩固定した。
CLC-3染色のために、スライスを、ブロッキング溶液(0.1MのPBS中、0.3%のTriton-Xならびに5%の正常ロバおよびヤギ血清)で2時間事前処理し、次いで、CLC-3一次抗体(ウサギ、1:200、Alomone、ACL-001)と共に72時間インキュベートした。一部のスライスを、対照としてウサギIgGと共にインキュベートした。0.01%のtriton-Xを有するPBS中で3回洗浄した後、脳切片を、Cy3にコンジュゲートしたヤギ抗ウサギ二次抗体(1:500、Jackson ImmunoResearch)と共に、室温で2時間インキュベートした。脳切片を、退色防止マウント溶液(Invitrogen)と共に、ガラススライド上にマウントした。蛍光画像を、Olympus共焦点顕微鏡システム(FV1000)で獲得した。各スライスについて、CA3の少なくとも2~3個の視野をイメージングした。CA3錐体層におけるCLC3蛍光強度を定量化するために、共焦点画像を、NIH Image Jソフトウェアを使用して解析した。
ウエスタンブロット
脳波(EEG)記録
新生仔および成体げっ歯類におけるグルコネートのin vivo抗てんかん効果を試験するために、P8~12のSprague-Dawleyラットまたは2月齢の雄性C57BL/6マウスを、ペントバルビタールナトリウム(新生仔ラットに対して50mg/kg、成体マウスに対して100mg/kg、腹腔内注射)で深く麻酔した。2つのステンレス鋼スクリュー(直径1mm)を、EEG記録電極として、皮質の上の頭蓋骨中に挿入し、1つの接地電極ならびに1つの参照電極を、十字縫合の+1.8mm前、正中線に対して±0.5mm側方、および皮質表面の下1mmに位置づけた。全ての電極を、マイクロコネクターに取り付け、歯科用セメントで頭蓋骨上に固定した。手術後に、新生仔ラットを母親の元に戻し、引き続くEEG記録の前に2日間にわたって回復させた。成体マウスを、移植した電極の損傷を予防するために1匹で収容し、EEG記録の前に少なくとも5日間回復させた。
EEGの解析
発作行動試験
雄性マウスを、以下の3つの群に分割した:ビヒクル対照群(食塩水、n=9)、1g/kgのD-グルコン酸ナトリウム群(n=10)、および2g/kgのD-グルコン酸ナトリウム群(n=11)。PTZ(50mg/kg)を、薬物処置の1時間後に腹腔内投与した。PTZ注射後、行動を、1時間にわたってモニタリングおよび観察した。発作行動を、Racineスコアに従って分類した:ステージ0、応答なし;ステージ1、顔面のピクつき;ステージ2、うなずき;ステージ3、前肢間代性、直立した尾;ステージ4、前肢間代性を伴って立ち上がる;ステージ5、立ち上がりおよび転倒、ジャンプ、全般間代性発作、全般間代性および強直性発作または死。Racine et al., “Modification of seizure activity by electrical stimulation. 3. データ解析
データを、平均±s.e.m.として示した。スチューデントt検定(対応のあるまたは対応のない)を、2群比較のために実施し、χ2検定を使用して、2つの群間のパーセンテージの差異を比較した。複数の群中での比較のために、事後検定と共に一元配置ANOVAを使用した。統計的有意性を、P<0.05に設定した。
グルコースオキシダーゼによるてんかん様活動の低減
この実施例は、グルコースオキシダーゼがてんかん様活動を有効に阻害することができることを実証する予備データを示している。
Claims (17)
- 新生児痙攣の処置を必要とする対象における、新生児痙攣を処置するための医薬品の製造のための、グルコースオキシダーゼを含む組成物の使用。
- 前記組成物が、新生児痙攣の処置を必要とする前記対象において、グルコースをグルコネートへと変換する、請求項1に記載の使用。
- 非新生児と比較して上方調節されたCLC-3 Cl-チャネルを有する新生児における痙攣活性の低減において、補足的グルコースと組み合わせて医薬として使用するための、グルコースオキシダーゼを含む経口医薬組成物。
- 前記新生児が、循環グルコースを有する、請求項3に記載の経口医薬組成物。
- 前記グルコースオキシダーゼが、前記循環グルコースをグルコネートへと変換する、請求項4に記載の経口医薬組成物。
- 前記グルコネートが、前記CLC-3 Cl-チャネルを阻害する、請求項5に記載の経口医薬組成物。
- 前記CLC-3 Cl-チャネルの阻害が、前記新生児における前記痙攣活性を低減させる、請求項6に記載の経口医薬組成物。
- 前記新生児が、新生仔てんかんと診断されている、請求項3に記載の経口医薬組成物。
- 前記新生児が、1月齢未満である、請求項3に記載の経口医薬組成物。
- 薬学的に許容される担体をさらに含む、請求項3に記載の経口医薬組成物。
- 新生児における痙攣活性の処置における医薬品として使用するための医薬製剤であって、固定用量のグルコースオキシダーゼおよび不活性担体を含む、医薬製剤。
- 前記不活性担体が、食塩水である、請求項11に記載の医薬製剤。
- 前記新生児が循環グルコースを有し、前記グルコースオキシダーゼが、前記循環グルコースをグルコネートへと変換する、請求項11に記載の医薬製剤。
- 前記グルコースオキシダーゼが、補足的グルコースと組み合わせて前記新生児に投与されるものであり、前記補足的グルコースが、7.3mg/kg/分のグルコース注入速度の固定用量で静脈内注射により投与されるものである、請求項11に記載の医薬製剤。
- 前記新生児が、新生仔てんかんと診断されている、または前記新生児が、非新生児と比較して上方調節されたCLC-3 Cl-チャネルを有する、請求項11に記載の医薬製剤。
- 経皮パッチ剤によって前記新生児に投与されるものである、請求項11に記載の医薬製剤。
- 新生児痙攣の処置を必要とする対象における、新生児痙攣の処置のための、グルコースオキシダーゼを含む組成物。
