Description
拡張可能なアブレーションバルーンは、心臓血管系を通ってナビゲートされ、例えば肺静脈の口をアブレーションするために心臓内に挿入されるカテーテルの遠位端に装着されてもよい。バルーンは、静脈に不注意に入らないように十分に大きくするべきであるが、狭い血管を通してバルーンを前進させることを可能にする十分にコンパクトな形態で詰め込まれる必要がある。更なる課題は、治療後にバルーンを身体から除去するために、このようなバルーンの安全な畳み込み及びカテーテルシースへの後退を確実にすることである。本明細書で任意の数値や数値の範囲について用いる「約」又は「およそ」という用語は、構成要素の部分又は構成要素の集合が、本明細書で述べるその所望の目的に沿って機能することを可能とするような適当な寸法の許容誤差を示すものである。より具体的には、「約」又は「およそ」は、挙げられた値の±10%の値の範囲を指し得、例えば、「約90%」は、81%~99%の値の範囲を指し得る。更に、本明細書で使用するとき「患者」、「ホスト」、「ユーザ」、及び「被験者」という用語は、任意の被験者又は被験動物を指し、システム又は方法をヒトにおける使用に限定することを目的としたものではないが、ヒト患者における本発明の使用は、好ましい実施形態を代表するものである。
図1は、本発明の実施形態による、カテーテルに基づく位置追跡及びRFアブレーションバルーン40を備えるアブレーションシステム20の概略絵画図である。システム20は、カテーテル21を備え、挿入図25に見られるように、カテーテル21のシャフト22の遠位端22aは、シース23を通って台29の上に横になっている患者28の心臓26の中へ挿入される。挿入図25に更に示されるとおり、遠位端22aは、バルーン40のすぐ近位の遠位端22a内に入れられている磁気センサ39を備える。
図2は、本発明の実施形態による、図1からのバルーンカテーテル40の概略絵画図である。見られるように、バルーン40は、シース23から突出する(シャフト22の)遠位端22aに装着されている。磁気位置センサ39は、バルーン40のすぐ近位で遠位端22a内に収容される。拡張可能なバルーン40は、例えばポリエチレンテレフタレート(PET)、ポリウレタン、又はPEBAX(登録商標)などのプラスチックから形成された生体適合性材料からなる外壁又は膜43を有する。アブレーション電極46は、可撓性基材44上のバルーン40の上に円周状に配設される。
(1) バルーンカテーテルであって、
患者の心臓に挿入するように構成されているシャフトと、
前記シャフトの遠位端に装着された、拡張可能な膜で作製されたバルーンと、
前記膜上に配設されている可撓性基材と、
前記可撓性基材の上に配設され、魚骨形の構成を有する1つ以上の電極と、
前記拡張可能な膜に結合されている1つ以上の放射線不透過性フラグであって、前記拡張可能な膜が圧縮構成又は折り重ねられた構成に畳み込まれる際に、前記放射線不透過性フラグが可撓性基材と合わせて折り重ねられるように、蛇行パターンを含む、1つ以上の放射線不透過性フラグと、を備えるバルーンカテーテル。
(2) 前記膜の上に配設された灌注孔を含み、前記灌注孔の一部は、前記電極で覆われた領域上に分配され、前記灌注孔のうちの他のものは、前記電極で覆われた領域の間に分配されている、実施態様1に記載のバルーンカテーテル。
(3) 前記放射線不透過性フラグが、X線撮像されたときに前記バルーンカテーテルの向きを示すために、異なる形状でパターン化された少なくとも第1のフラグと第2のフラグとを含む、実施態様1に記載のバルーンカテーテル。
(4) 前記バルーンの近位に配設された磁気位置センサを備える、実施態様1に記載のバルーンカテーテル。
(5) 前記膜と前記可撓性基材との間に配設された糸を含む、実施態様1に記載のバルーンカテーテル。
(7) 前記可撓性基材が、前記可撓性基材の前記膜への接着を増大させるように構成されているパターン化されたトポグラフィーを含む、実施態様1に記載のバルーンカテーテル。
(8) バルーンカテーテルを製造するための方法であって、
患者の心臓に挿入するように構成されているシャフトを提供することと、
前記シャフトの遠位端に、拡張可能な膜で作製されたバルーンを装着することと、
前記膜上に可撓性基材を配設することと、
前記可撓性基材の上に、魚骨形の構成を有する1つ以上の電極を配設することと、
前記可撓性基材の上に、蛇行パターンを有する1つ以上の放射線不透過性フラグを配設することと、
前記電極及び蛇行放射線不透過性フラグを、圧縮構成において前記膜とぴったり一致させることと、を含む方法。
(9) 前記膜の上に灌注孔を配設することを含み、前記灌注孔の一部は、前記電極で覆われた領域上に分配され、前記灌注孔のうちの他のものは、前記電極で覆われた領域の間に分配される、実施態様8に記載の方法。
(10) 前記放射線不透過性フラグを配設することが、X線撮像されたときに前記バルーンカテーテルの向きを示すために、異なる形状でパターン化された少なくとも第1の放射線不透過性フラグと第2の放射線不透過性フラグとを配設することを含む、実施態様8に記載の方法。
(12) 前記膜と前記可撓性基材との間に糸を配設することを含む、実施態様8に記載の方法。
(13) 前記可撓性基材内にパターン化されたトポグラフィーを含むことによって、前記可撓性基材の前記膜への接着を増大させることを含む、実施態様8に記載の方法。
Claims (11)
- バルーンカテーテルであって、
患者の心臓に挿入するように構成されているシャフトと、
前記シャフトの遠位端に装着された、拡張可能な膜で作製されたバルーンと、
前記拡張可能な膜上に配設されている可撓性基材と、
前記可撓性基材の上に配設された1つ以上の電極と、
前記拡張可能な膜に結合されている1つ以上の放射線不透過性フラグであって、前記拡張可能な膜が圧縮構成又は折り重ねられた構成に畳み込まれる際に、前記放射線不透過性フラグが前記可撓性基材と合わせて折り重ねられるように、蛇行パターンを含む、1つ以上の放射線不透過性フラグと、を備え、
前記放射線不透過性フラグが、X線撮像されたときに前記バルーンカテーテルの向きを示すために、異なる形状でパターン化された少なくとも第1のフラグと第2のフラグとを含む、バルーンカテーテル。 - 前記拡張可能な膜の上に配設された灌注孔を含み、前記灌注孔の一部は、前記電極で覆われた領域上に分配され、前記灌注孔のうちの他のものは、前記電極で覆われた領域の間に分配されている、請求項1に記載のバルーンカテーテル。
- 前記バルーンの近位に配設された磁気位置センサを備える、請求項1に記載のバルーンカテーテル。
- 前記拡張可能な膜と前記可撓性基材との間に配設された糸を含む、請求項1に記載のバルーンカテーテル。
- 前記糸が、超高分子量繊維又は液晶ポリマー繊維のうちの1つから選択される、請求項4に記載のバルーンカテーテル。
- 前記可撓性基材が、前記可撓性基材の前記拡張可能な膜への接着を増大させるように構成されているパターン化されたトポグラフィーを含む、請求項1に記載のバルーンカテーテル。
- バルーンカテーテルを製造するための方法であって、
患者の心臓に挿入するように構成されているシャフトを提供することと、
前記シャフトの遠位端に、拡張可能な膜で作製されたバルーンを装着することと、
前記拡張可能な膜上に可撓性基材を配設することと、
前記可撓性基材の上に、1つ以上の電極を配設することと、
前記可撓性基材の上に、蛇行パターンを有する1つ以上の放射線不透過性フラグを配設することと、
前記電極及び蛇行放射線不透過性フラグを、圧縮構成において前記拡張可能な膜とぴったり一致させることと、を含み、
前記放射線不透過性フラグを配設することが、X線撮像されたときに前記バルーンカテーテルの向きを示すために、異なる形状でパターン化された少なくとも第1の放射線不透過性フラグと第2の放射線不透過性フラグとを配設することを含む、方法。 - 前記拡張可能な膜の上に灌注孔を配設することを含み、前記灌注孔の一部は、前記電極で覆われた領域上に分配され、前記灌注孔のうちの他のものは、前記電極で覆われた領域の間に分配される、請求項7に記載の方法。
- 前記バルーンの近位に磁気位置センサを配設することを含む、請求項7に記載の方法。
- 前記拡張可能な膜と前記可撓性基材との間に糸を配設することを含む、請求項7に記載の方法。
- 前記可撓性基材内にパターン化されたトポグラフィーを含むことによって、前記可撓性基材の前記拡張可能な膜への接着を増大させることを含む、請求項7に記載の方法。
