キメラ抗原受容体(CAR)は、T細胞に抗原特異性を付与するために設計された人工的抗体様分子である。CARを発現するT細胞は、特定タイプのがんの処置に関して長期有効性を示している(Eshhar, 1997, Cancer Immunol Immunother 45(3-4)131-1)36(非特許文献1)、Eshhar et al, 1993, Proc Natl Acad Sci U S A 90(2):720-724(非特許文献2)、Brocker and Karjalainen, 1998, Adv Immunol 68:257-269(非特許文献3))。第一世代のCARは、T細胞を活性化して特異的免疫を提供するように選択された抗原結合ドメイン、膜貫通ドメインおよびCD3ζなどの細胞内ドメイを含む。しかし、これらのCAR改変T細胞のインビボでの拡大増殖(expansion)および存続は、標的抗原とのエンゲージメント後の共刺激シグナルの欠如によって妨げられた。多くの腫瘍細胞は、最適で持続的なT細胞の機能、増殖(proliferation)および存続に必要な共刺激分子の腫瘍細胞における発現を、ダウンレギュレートするからである。T細胞応答を強化するために第2世代および第3世代のCARコンストラクトが作製された。これらはCD3ζとタンデムに1つまたは2つの二次共刺激シグナルを含んでいた。共刺激分子はCD28、4-1BB、OX-40およびCD27などの「第2シグナル」を模倣し、この第2シグナルは、前臨床モデルでも(Carpenito et al., 2009, Proc. 1. TCR陽性細胞を消滅させるための方法であって、以下の工程を含む方法:

T細胞に導入される先行技術のキメラ抗原受容体は、シグナリングドメインを次々と順に追加していくことを必要とする単鎖ポリペプチドで形成されていた。しかしシグナリングドメインを、その天然の膜近傍位置から移動させることは、それらの機能を妨害することになりうる。この欠点を克服するために、本出願人らは最近、すべての関連シグナリングドメインが通常の膜近傍位置につけるように、FCERIに由来する多鎖CARを設計した。この新しい構成では、FCERIアルファ鎖の高アフィニティーIgE結合ドメインが、T細胞特異性を細胞標的に対してリダイレクトするためにscFvなどの細胞外リガンド結合ドメインで置き換えられ、FCERIベータ鎖のN末端テールおよび/またはC末端テールが、通常の膜近傍位置に共刺激シグナルを置くために使用される。
一般に、mRNAは、リボソーム結合、翻訳の開始および細胞におけるmRNAの安定性を増加させる5’端のキャップと3’ポリ(A)テールとを、どちらも有する。リードアウト(半減期)は、3’ポリ(A)テールがないmRNAと比較して、少なくとも2倍は増加する。
本発明の抗TCR mRNAトランスフェクション法は、本質的に一過性であり、ベクターフリーである。インビトロ転写したmRNAを使ったトランスフェクションは、当技術分野において公知の任意の手段によって達成することができる。
抗TCR CARをコードするmRNAが細胞表面に一過性に発現する、CARを発現させるT細胞集団を生成させるために、CAR含有T細胞を生成させる。細胞(例えばPBMC、T細胞、例えばTIL、CD4+またはCD8+細胞)は、標準的な手順に従って、自然源、例えば転移性黒色腫患者から精製し、培養し、かつ/または拡大増殖させることができる。細胞は、例えば米国特許出願公開第20080311095号に記載されているように刺激しうる。細胞にはCARが形質導入される。あるいは、CARの形質導入に先だって、抗TCR CARをコードするmRNAが導入される。本発明の範囲内にあるCARの例として、抗CD38 CAR、抗CD19 CAR、抗CD123 CAR、抗CD30 CAR、または抗CD22 CARが挙げられる。ただし本発明は、標的分子としてのこれらの抗原に限定されるべきではない。むしろ、一過性に共発現される抗TCR CARCARをコードするmRNAとの関連において、任意の標的分子に対する抗原結合ドメインを含む任意のCARを使用することができる。

一特定態様では、簡単に述べると、細胞をTCR遺伝子の内在性発現についてノックアウトした後、2日の休息期間を置き、次に、本明細書に記載する抗TCR CARをコードする核酸を、細胞に形質導入し、インターロイキン-2(IL-2)の存在下で2日間にわたって拡大増殖させる。さらに、T細胞をモノクローナル抗体リツキシマブに対して感受性にすることを目的として、安全機構としてのRQR8遺伝子を共発現するようにT細胞を操作する。RQR8は、インビボでの形質導入細胞の選択的欠失を可能にする自殺遺伝子である。抗TCR CAR mRNAをCAR T細胞に導入すると、その結果生じる細胞の表現型は抗TCR CAR+T-になる。次に4~11日にわたって培養下で細胞を拡大増殖させる。一態様では、4日にわたって、または5日にわたって、または7日にわたって、細胞を拡大増殖させる。
本発明のTCR陰性CAR+免疫細胞の有効性は、関心対象の特定適応症に関して当技術分野において認識されている動物モデルを使って試験することができる。例えば、がんの場合、確立されたがんマウスモデルを、関心対象の特定のがんに関して、広く利用することができる。
本発明は、TCR-neg CAR+免疫細胞の少なくとも1つの集団を含む薬学的組成物に関する。そのような薬学的組成物は、がん、感染症または免疫疾患の処置における使用のために考えられる。
本発明の態様において、本発明の方法によって生成させた細胞は、哺乳動物に送達される。細胞送達媒体は当技術分野において公知であり、それらを本発明の細胞を送達するために使用しうる。本発明の方法で改変されたT細胞またはNKは、通常、静脈内に注入されるか、具体的疾患の体腔部位に注入され、注入前に食塩溶液に再懸濁される。

抗CD3 CARの配列は、5’から3’に向かって、ヒトCD8aからのシグナル配列(SEQ ID NO:1)、抗CD3 scFV(SEQ ID NO:2)、CD8aヒンジドメイン(SEQ ID NO:3)、CD8a膜貫通ドメイン(SEQ ID NO:4)、4-1BB/CD3z共刺激および活性化細胞内ドメイン(SEQ ID NO:5)を、2A要素(SEQ ID NO:6)およびBFPレポーター遺伝子(SEQ ID NO:7)を含有するプラスミド中に集合させて、30527(SEQ ID NO:8)とすることによって得られた。この配列を、5’にT7プロモーター(SEQ ID NO:9)を含有し、3’にマウスhba 3’UTR(SEQ ID NO:10)および120ヌクレオチド長ポリA(SEQ ID NO:11)を含有するプラスミドに、さらにサブクローニングすることで、配列30697(SEQ ID NO:12)とした。

活性化の4日後に、AgilePulse MAXシステム(Harvard Apparatus)を用いる電気導入(electrotransfer)によって、ヒトTリンパ球のトランスフェクションを行った。細胞をペレット化し、サイトポレーション(cytoporation)培地Tに>28×106細胞/mlの密度で再懸濁した。三つ一組にして、5×106細胞を合計5μgのTRAC TALEN mRNA(各2.5μgの右および左TALENアーム)と混合し、0.4cmキュベットに入れた。並行して2つのモックトランスフェクション(mRNAなし)も実施した。エレクトロポレーションは、800Vで2回の0.1msパルスと、それに続く130Vで4回の0.2msパルスからなった。エレクトロポレーション後に、細胞を2mLの培養培地に希釈し、37℃/5%CO2でインキュベートした。TRAC TALENトランスフェクション後の2日目、7日目または9日目に、TRAC TALEN-トランスフェクト細胞の別々のアリコートに、30527(SEQ ID NO:8)から得られたPCR産物(SEQ ID NO:22)とオリゴ1(SEQ ID NO:13)およびオリゴ2(SEQ ID NO:14)とからmMessage mMachine T7 Ultraキット(Thermo fisher scientific)を使ってEPAP媒介ポリアデニル化で作製された20μgの抗CD3 CAR mRNA、またはTALENコード配列の後ろにマウスhba 3’UTR(SEQ ID NO:10)および120ヌクレオチド長ポリA(SEQ ID NO:11)を含有しポリAの下流で線状化されたプラスミドDNAテンプレート30697(SEQ ID NO:12)からのT7 RNAポリメラーゼ転写によって得られた20μgの抗CD3 CAR mRNAを、再びエレクトロポレーションした。
抗CD3 CAR発現に関するBFPの検出は各CD3 CAR mRNAエレクトロポレーションの24時間後に実施した。efluor780(ebioscience 65-0865-18)を使って、細胞生存率をPBS中、4℃で20分間、モニターした後、PBS中の2%FBSでの洗浄工程を行い、PFA 4%で固定した。2日後、3日後または4日後に、TCRおよびCD3陽性細胞の頻度を評価した。PBS中の2%FBS、EDTA 2mM、アジド0.1%に希釈した抗ヒトTCRab(Miltenyi)(アルファベータTCR特異的)および抗ヒトCD3(Miltenyi)抗体を使って4℃で20分間、二重標識を行った後、PBS中の2%FBS、EDTA 2mM、アジド0.1%による洗浄工程を行った。生存率をVicellで評価した。フローサイトメトリーはMACSQUANT(Miltenyi Biotec)を使って実施し、データ解析はFlowJoソフトウェアで実施した。TRAC TALENのみで処理された細胞と比較してCD3/TCR陽性細胞の90~98%の枯渇が測定されたので、収集されたデータにより、抗CD3 mRNAの極めて効率のよい枯渇が実証された(図1)。
TALEN処理の7日後にTRAC TALEN処理細胞に抗CD3 CAR mRNA(3’UTR-120Aフォーマット)(SEQ ID NO:12)をエレクトロポレーションした。24時間後に、さまざまな比のTRAC TALENだけで処理した細胞とTRAC TALEN+抗CD3 CAR二重処理細胞とを、さまざまな比で混合した。48時間の共培養後に、TCRおよびCD3の表面発現を上記のように評価した(図2)。
供給工程のための細胞は、使用前に融解され、80%超の生存細胞を含む、白血球アフェレーシスなどによる個別健常ドナーからの凍結ヒト末梢単核球である。
アルファベータTCRを発現する細胞の任意の調製物を使用しうる。
活性化後に、上記の細胞100万個あたり1μgのTRAC TALENコードmRNAの電気導入によるトランスフェクションを、細胞に行うか、または行わなかった。1.5時間後に、培養物に、CARを含むrAAV6ドナーベクターを3×104vg/細胞の感染多重度で加えるか、または加えなかった。CARは、任意のCAR(例えば本明細書に記載したものの中では、好ましくは、CD123、CD22、CS1、CLL-1 CAR)であってよく、CAR発現は、CD4、CD8、TCRαβ mAb、組換えタンパク質(CARの完全長標的)を、生細胞/死細胞マーカーと組み合わせて使用することにより、生存T細胞でのフローサイトメトリーによって評価した。
GVHDの評価
抗TCR CARをコードするmRNAを発現することにより、同種異系CAR Tの品質が完了されうるかどうか、特にTCR+T細胞の存在による副作用が減少するかどうかを調べるために、TCR陰性細胞を上述(実施例1)のように調製し、GVHD測定を行うために、さまざまな用量をマウスに投与した。
動態実験により、本発明の抗CD3 CAR(SEQ ID 8または12)の発現は、トランスフェクションの3時間後から5日後(最高用量の場合)およびヒトグロビン配列を構築に加えた場合は7日後まで検出されうることが示された。
T細胞増殖
T細胞は、5%ヒト血清hAB(Gemini)および20ng/ml IL-2(Miltenyi)を補足したX-Vivo 15(Lonza)中、1x106細胞/mlの密度で培養した。
mRNAは、CD3 CARをコードするPCR産物から、EPAP媒介ポリアデニル化により、mMessage mMachine T7 Ultraキット(Thermo fisher scientific)を使って作製するか、EPAP媒介ポリアデニル化なしで、CD3 CAR、マウスhba 3’UTRおよび120-ヌクレオチド長ポリAをコードする線状化したプラスミドDNAテンプレートから作製した。
10%FBS(Gibco)、1%HEPES(Gibco)、1%L-グルタミン(Gibco)および1%ペニシリン/ストレプトマイシン(Gibco)を補足したRPMI 1640培地(ThermoFisher)で培養した293FT細胞(ThermoFisher)において、Opti-MEM培地(Gibco)およびLipofectamine 2000(ThermoFisher)を使用し、標準的なトランスフェクション手順に従って、レンチウイルス粒子を生成させた。トランスフェクションの48時間後および/または72時間後に、上清を回収し、超遠心分離によって濃縮した。
凍結保存されたヒトPBMC(ALLCELLS)を融解し、5%hAB血清(Gemini)またはCTS免疫細胞SR(ThermoFisher)および20ng/ml IL-2(Miltenyi Biotech)を補足したX-vivo-15培地(Lonza)に、1×106細胞/mlでプレーティングして、37℃で一晩培養した。翌日、IL-2を含まない無血清X-vivo-15培地中で、ヒトTアクチベーターCD3/CD28(Life Technology)を使ってPBMCを活性化した。30μg/mlのレトロネクチン(Takara)でプレコートした無処理12ウェルプレートにおいて、CD22標的CARをコードするレンチウイルス粒子の存在下、37℃で2時間、100万個の活性化PBMC(600μl中)を、ビーズを除去することなく直ちにインキュベートした。2~3時間後に600マイクロリットルの2×X-vivo-15 培地(X-vivo-15、10%hAB血清および40ng/ml IL-2)を加え、細胞を37℃で72時間インキュベートした。
活性化/形質導入の4日後に、AgilePulse MAXシステム(Harvard Apparatus)を用いる電気導入によって、ヒトTリンパ球のトランスフェクションを行った。細胞をペレット化し、サイトポレーション培地Tに再懸濁した。5×106細胞を合計5μgのTRAC TALEN mRNA(各2.5μgの右および左TALENアーム)と混合し、0.4cmキュベットに入れた。TRAC TALENトランスフェクト細胞またはモックトランスフェクト細胞の別々のアリコートに、さまざまな時点(TRAC TALENトランスフェクション後の2日目、7日目または9日目)に、20μgのCD3 CAR mRNAをエレクトロポレーションした。次に、操作されたT細胞を培養中に最大4日間保ってから、40mlの完全X-vivo-15培地中、G-Rex10(Wilson Wolf)において、6~7日間の拡大増殖を行った。
次に、CARをその表面に発現しているT細胞の比率を、以下の抗体を使って定量した:CD3:クローンBW264/56、Vioblue(Miltenyi #130-094-363)、TCRαβ:クローンREA652、PE(Miltenyi #130-109-920)、CD4:クローンVIT4、PEVio770(Miltenyi #130-096-552)、CD8:クローンSK1、BV510(Biolegend # 344732)、CD62L:クローン145/15、APC(Miltenyi # 130-113-617)、CD45RA:クローンT6D11、Vioblue(Miltenyi # 130-113-360)、PD1:クローンREA1165、PE(Miltenyi # 130-120-388)およびLAG3:クローン11C3C65、BV421(Biolegend # 369313)。
Raji細胞をCellRad X線照射システム(Faxitron、米国アリゾナ州トゥーソン)を使って、20Gyで処理し、2回洗浄し、計数し、24ウェルプレートにおいて、二つ一組にして、500,000個を500,000個のT細胞(1:1)と共に、5%hAB血清を含むがIL-2は含まない最終体積1mlのX-vivo-15培地に入れてプレーティングした。4日目および7日目に、細胞をVicellで計数し、500,000細胞/0.5ml培地を48ウェルプレートに継代した。10日目に、細胞を混合し、最終時点として計数した。
12ウェルプレートにおいて、形質導入されたT細胞(1.5×106細胞)を、5%hAB血清を含みIL-2を欠くX-vivo-15培地中で、CAR標的抗原を提示しルシフェラーゼを発現する標的細胞(Raji)(0.5×106細胞)と共に3:1(T細胞:標的)の比でインキュベートした。24時間後に、細胞を収集し、混合し、100μlの細胞をルシフェラーゼ定量(OneGlo、Promega)に使用した。残りの細胞をペレット化し、新鮮なX-vivo15培地、5%hAB血清、IL-2なしに再懸濁し、0.5×106個の標的細胞を追加した。この工程を2日連続して繰り返した。
動物に関わる手順はすべて規制および確立された指針に従って実施され、Cellectisの施設内動物実験委員会(IACUC)によって精査され承認された。
Claims (16)
- 操作された細胞を製造するための方法であって、
内在性T細胞受容体(TCR)構成要素をコードする少なくとも1つの遺伝子を破壊することによってドナーからの免疫細胞が操作される、破壊工程、
次に、または同時に、
第1の組換えキメラ抗原受容体(CAR)をコードする少なくとも1つの外因性ポリヌクレオチドを該細胞のゲノムに導入することによって細胞が改変される、第1形質転換工程、
次に、
TCRのエピトープ、TCRサブユニットのエピトープ、TCRサブユニットの組合せ、またはCD3のエピトープに特異的である、第2のキメラ抗原受容体(CAR)を一過性に発現させるための第2形質転換工程
を少なくとも含む、方法。 - 第2形質転換工程が、
前記第2のCARをコードする合成mRNA、またはコンディショナルプロモーターの制御下に前記第2のCARをコードする配列を含むDNAを、細胞に導入すること
を含む、請求項1記載の方法。 - 前記第2のCARが、
CD3サブユニットのエピトープに特異的である、
TCRアルファサブユニットのエピトープに特異的である、
TCRベータ1またはTCRベータ2サブユニットのエピトープに特異的である、または
TCRアルファベータサブユニットの(共通)エピトープに特異的である、
請求項1または2記載の方法。 - 前記第2のCARをコードする前記合成mRNAまたは前記DNAが、SEQ ID NO:2の配列、または以下の一連の配列:SEQ ID NO:1、SEQ ID NO:2、SEQ ID NO:3、SEQ ID NO:4、およびSEQ ID NO:5を含む、請求項2~3のいずれか一項記載の方法。
- あるゲノム配列に特異的なレアカッターエンドヌクレアーゼ、例えばTALエフェクタータンパク質またはCRISPR CAS9をコードするmRNAを導入することを含む、少なくとも1つの破壊工程を含む、請求項1~4のいずれか一項記載の方法。
- 第1形質転換工程において、第1のCARをコードする外因性遺伝子が、ウイルスベクター、好ましくはAAV6ウイルスベクターを含むウイルスベクターを用いて、細胞に導入される、請求項1~5のいずれか一項記載の方法。
- 前記破壊工程の1工程が、TCR遺伝子に含まれる配列に特異的である、第1のレアカッターエンドヌクレアーゼをコードするmRNAを導入することを含む、請求項5記載の方法。
- 前記破壊工程のさらなる1工程が、薬物に対する感受性または抵抗性を付与する遺伝子、サイトカイン遺伝子、およびそれらの組合せからなる群より選択される遺伝子に含まれる配列に特異的である、第2のレアカッターエンドヌクレアーゼをコードするmRNAを導入することを含む、請求項7記載の方法。
- 破壊工程の少なくとも1工程が行われる前記免疫細胞が、T細胞、炎症性Tリンパ球、細胞傷害性Tリンパ球、制御性Tリンパ球またはヘルパーTリンパ球、NK T細胞を含むか、またはそれらに由来する、請求項1~8のいずれか一項記載の方法。
- 第1のCARが、ROR1、EGFRvIII、BCMA、CD33、GD3、CD19、CD38、HSP70、CD30、FAP、HER2、CD79a、CD79b、CD123、CD22、CLL-1、MUC-1 GD2、OアセチルGD2、およびCS1からなる群より選択される細胞表面抗原標的に特異的である、請求項1~9のいずれか一項記載の方法。
- アロ反応性に関与する少なくとも1つの遺伝子、例えば、ベータ2M、制御因子X関連アンキリン含有タンパク質(RFXANK)、制御因子5(RFX5)、制御因子X関連タンパク質(RFXAP)、およびクラスIIトランス活性化因子(CIITA)、TAP-1、またはそれらの組合せを不活化するさらなる工程、および/または
PDL1、プログラム細胞死1(PD-1)、細胞傷害性Tリンパ球抗原4(CTLA-4)、LAG3、Tim3、BTLA、BY55、TIGIT、B7H5、LAIR1、SIGLEC10、および2B4からなる群より選択される少なくとも1つの遺伝子を不活化するさらなる追加の破壊工程、および/または
デオキシシチジンキナーゼ(dCk)、ヒポキサンチングアニンホスホリボシルトランスフェラーゼ(HPRT)、グルココルチコイド受容体(GR)、CD52、およびそれらの組合せから選択される薬物耐性に関与する少なくとも1つの遺伝子を不活化するかまたは過剰発現させるさらなる工程、および/または
薬物過敏症に関与する免疫細胞中の少なくとも1つの遺伝子、例えばGGH、RhoA、CDK5、CXCR3、NR1H2、URG4、PARP14、AMPD3、CCDC38、NFU1、またはCACNG5タンパク質をコードする遺伝子を不活化するさらなる工程、
を含む、請求項1~10のいずれか一項記載の方法。 - 請求項1~11のいずれか一項記載の方法に従って得ることができるTCR陰性CAR発現免疫細胞の集団であって、TCRのエピトープ、TCRサブユニットのエピトープ、TCRサブユニットの組合せ、またはCD3のエピトープに特異的である、キメラ抗原受容体(CAR)を一過性に発現する細胞を含む、集団。
- 3%未満のアルファベータTCR+細胞、0.03%未満のアルファベータTCR+細胞、0.01%未満のアルファベータTCR+細胞、0.001%未満のアルファベータTCR+細胞、または0.00001%未満のアルファベータTCR+細胞を含む、請求項12記載のTCR陰性CAR発現免疫細胞の集団。
- 医薬の調製における、請求項12または13記載のTCR陰性CAR発現免疫細胞の集団の使用。
- 請求項12~13のいずれか一項記載のTCR陰性CAR発現免疫細胞の集団と、薬学的に許容される媒体とを含む、薬学的組成物。
- がん、感染症、または免疫疾患の処置のための医薬の調製における、請求項12~13のいずれか一項記載のTCR陰性CAR発現免疫細胞の集団の使用または請求項15記載の薬学的組成物の使用。
