Description
本出願は、同日に出願された「Estimating Cardiac Catheter Proximity to the Esophagus」と題する代理人整理番号第1002-1660号の米国特許出願に関連し、その開示内容は、参照により本明細書に組み込まれる。
本発明は一般に、侵襲的な医療装置及び処置方法、具体的には、生体内のアブレーションプローブの位置のモニタリングに関する。
心臓の左心房に対する食道の近接は、心房細動の治療に使用する肺動脈の分離手技などの、左心房における標的組織のカテーテルアブレーションにおいて問題を引き起こし得る。食道は左心房の後側にあり、右若しくは左肺動脈、又は心臓の後側壁に隣接している左心房に対して、様々な経路を通る。したがって、アブレーションが左心房の後側で行われるときに起こる高温により、食道が損傷するリスクがある。

(1) 装置であって、
プロセッサであって、前記プロセッサが、
患者の心臓内の組織をアブレーションするために高周波(RF)エネルギーを適用する1つ以上の電極の、垂直軸を有する座標系での前記心臓内における位置を受信し、
前記受信した位置に基づいて、前記垂直軸に沿った前記患者の食道より上方の前記電極のそれぞれの高さのうち、1つ以上の最小高さのサブセットを計算し、かつ、
前記食道より上方の前記電極及び前記それぞれの高さを可視化する、グラフィカルユーザーインターフェースを生成するように構成されている、プロセッサと、
前記グラフィカルユーザーインターフェースをユーザーに提示するように構成されている、出力デバイスと、を含む、装置。
(2) 前記グラフィカルユーザーインターフェースが、前記1つ以上の電極の幾何学的配置を表示する図形を含む、実施態様1に記載の装置。
(3) 前記1つ以上の電極の前記幾何学的配置が、前記食道に対する前記電極の実際の物理的配置を可視化する、実施態様2に記載の装置。
(4) 前記グラフィカルユーザーインターフェースが、電極ごとにそれぞれの識別子を表示する、実施態様1に記載の装置。
(5) 前記プロセッサが、所定の割合で又は要求によって、前記グラフィカルユーザーインターフェースを更新するように構成されている、実施態様1に記載の装置。
前記食道の少なくとも一部と、前記心臓の少なくとも一部と、を画像化する1つ以上の医用画像を受信し、
前記位置及び前記医用画像に基づいて、前記最小高さのサブセットを計算し、
前記最小高さのサブセットのうち、前記高さのそれぞれの値を推定し、かつ、
前記グラフィカルユーザーインターフェースを生成するように構成されており、前記グラフィカルユーザーインターフェースは、前記計算された高さのそれぞれに対して推定された前記値を表示する、実施態様1に記載の装置。
(7) 方法であって、
プロセッサにおいて、患者の心臓内の組織をアブレーションするために高周波(RF)エネルギーを適用する1つ以上の電極の、垂直軸を有する座標系での前記心臓内における位置を受信することと、
前記受信した位置に基づいて、前記垂直軸に沿った前記患者の食道より上方の前記電極のそれぞれの高さのうち、1つ以上の最小高さのサブセットを計算することと、
前記食道より上方の前記電極及び前記それぞれの高さを可視化する、グラフィカルユーザーインターフェースを生成することと、
前記グラフィカルユーザーインターフェースをユーザーに表示することと、を含む、方法。
(8) 前記グラフィカルユーザーインターフェースを提示することが、前記1つ以上の電極の幾何学的配置を表示する図形を提示することを含む、実施態様7に記載の方法。
(9) 前記グラフィカルユーザーインターフェースを表示することが、前記食道に対する前記電極の実際の物理的配置を可視化することを含む、実施態様7に記載の方法。

前記受信した位置及び前記医用画像に基づいて、鉛直軸に沿った前記患者の食道より上方の前記電極のそれぞれの高さのうち、1つ以上の最小高さのサブセットを計算し、かつ、
前記食道より上方の前記電極及び前記それぞれの高さを可視化する、グラフィカルユーザーインターフェースを生成するように構成されている、プロセッサと、
前記グラフィカルユーザーインターフェースをユーザーに提示するように構成されている、出力デバイスと、を含み、
前記プロセッサが前記最小高さのサブセットに対応する前記電極のアブレーションパラメータを調整するように構成されている、装置。 - 前記グラフィカルユーザーインターフェースが、前記電極の幾何学的配置を表示する図形を含む、請求項1に記載の装置。
- 前記幾何学的配置が、前記食道に対する前記電極の実際の物理的配置を可視化する、請求項2に記載の装置。
- 前記グラフィカルユーザーインターフェースが、前記電極ごとにそれぞれの識別子を表示する、請求項1に記載の装置。
- 前記プロセッサが、所定の割合で又は要求によって、前記グラフィカルユーザーインターフェースを更新するように構成されている、請求項1に記載の装置。
- 前記プロセッサが、
前記最小高さのサブセットの高さのそれぞれの値を推定し、かつ、
前記グラフィカルユーザーインターフェースを生成するように構成されており、前記グラフィカルユーザーインターフェースは、計算された前記最小高さのそれぞれに対して推定された前記値を表示する、請求項1に記載の装置。
