本出願には、米国特許法第119(e)条(35 U.S.C. § 119(e))の下で、2017年5月4日に提出された米国仮特許出願第62/501,371号に対する優先権が与えられ、その出願はその全体が参照により本明細書に組み入れられる。

癌の治療に関するキメラ抗原受容体(CAR)T細胞の初期の臨床データは有望な結果を示しているが、癌および感染症に対するCAR免疫療法の進展には、すぐに利用可能で効力が強く、抗原および患者に対して特異的であるTリンパ球が無いことが妨げとなっている。高度に個別化されたCAR T細胞免疫療法による患者の治療は、費用および時間がかなりかかる恐れがある。進行疾患を有する患者由来の自己T細胞は、機能が損なわれていて所望の抗原に対して寛容である可能性があり、そのため同種ドナー由来T細胞の改変が余儀なくされる。さらに、注入された同種T細胞上の内在性αβT細胞受容体は、レシピエントにおいて主要組織適合性抗原および副組織適合性抗原を認識しうることから、移植片対宿主病(GVHD)を招く可能性がある。結果として、自己CAR T細胞を注入する現行の臨床試験の大半は、養子移入後に正常組織がTCR媒介性に有害に認識されることを防ぐのを免疫寛容に頼っている。このアプローチは初期の臨床的成果を上げてはいるものの、患者特異的なT細胞製品を製造するために必要な時間および費用が制約となっている。このため、患者特異的なT細胞製品を製造するための時間および費用を最小限に抑えながらT細胞を改変する、より安全な方法に対する需要が存在する。

別に定める場合を除き、本明細書で用いる技術用語および科学用語はすべて、本発明が属する技術分野の当業者によって一般的に理解されているものと同じ意味を有する。本明細書中に記載されたものと同様または同等な任意の方法および材料を本発明の試験のために実地に用いることができるが、本明細書では好ましい材料および方法について説明する。本発明の説明および特許請求を行う上では、以下の専門用語を用いる。
本発明は、遺伝子改変細胞の作製および使用のための組成物および方法を提供する。遺伝子改変細胞は、ステムループCRISPR(Clustered Regularly Interspaced Short Palindromic Repeats)RNA(st-crRNA)をコードする外来性核酸、およびCpf1酵素をコードする外来性核酸を含む。本発明のある局面は、細胞における遺伝子編集の方法、および改変細胞を作製する方法を含む。また、養子療法のため、および疾患または状態を治療するための改変細胞を含む方法および薬学的組成物も含まれる。
異質核酸に対する適応免疫を細菌に与える、CRISPR(Clustered Regularly Interspaced Short Palindromic Repeats)およびCR1SPR関連(Cas)タンパク質は、ヒト細胞および他の種類の細胞における、さらには動物および植物における、標的化されたゲノム編集に用いるという別の目的に役立てられている。CRISPR/Cas9技術はII型CRISPR/Casシステムから始まっており、これは1つのDNAエンドヌクレアーゼタンパク質Cas9、ならびにCRISPR RNA(crRNA)およびトランス活性化crRNA(tracrRNA)という2つの小型RNAからなる。これらの小型RNAまたはキメラ性単一ガイドRNA(sgRNA)はCas9と結合し、それによってRNAガイド(RNA-guided)DNAエンドヌクレアーゼ(RGEN)複合体を形成して、特異的DNA標的を切断する。染色体の二本鎖平滑末端切断(DSB)は、続いて相同組換え(HR)または非相同末端結合(NHEJ)を介して修復されて、遺伝子改変が生じる。
本発明のある局面は、遺伝子改変細胞を含む。1つの局面において、本発明は、st-crRNAをコードする外来性核酸およびCpf1酵素をコードする外来性核酸を含む、遺伝子改変細胞を含む。遺伝子改変細胞は、Tリンパ球、Bリンパ球、NK細胞、単球、マクロファージ、好中球、上皮細胞、造血幹細胞、および誘導多能性幹細胞(iPS)を非限定的に含む、任意の種類の細胞でありうる。
1つの局面において、本発明は、遺伝子編集の方法を含む。本方法は、ステムループCRISPR(Clustered Regularly Interspaced Short Palindromic Repeats)RNA(st-crRNA)を含む外来性核酸およびCpf1酵素を含む外来性核酸を細胞に投与する段階を含む。1つの態様において、編集しようとする細胞はT細胞である。もう1つの態様において、細胞は初代T細胞である。
核酸を細胞に導入する方法には、物理的、生物学的および化学的な方法が含まれる。宿主細胞にRNAなどのポリヌクレオチドを導入するための物理的方法には、リン酸カルシウム沈殿法、リポフェクション、微粒子銃法、マイクロインジェクション、エレクトロポレーションなどが含まれる。RNAは、エレクトロポレーションを含む市販の方法を用いて、標的細胞に導入することができる(Amaxa Nucleofector-II(Amaxa Biosystems, Cologne, Germany))、(ECM 830(BTX)(Harvard Instruments, Boston, Mass.)またはGene Pulser II(BioRad, Denver, Colo.)、Multiporator(Eppendort, Hamburg Germany)。また、RNAを、リポフェクションを用いるカチオン性リポソーム媒介性トランスフェクションを用いて、ポリマーカプセル封入を用いて、ペプチド媒介性トランスフェクションを用いて、または「遺伝子銃」などのバイオリスティック粒子送達システムを非限定的に含む市販の方法(例えば、Nishikawa, et al. Hum Gene Ther., 12(8):861-70 (2001)を参照)を用いて、細胞に導入することもできる。
1つの態様において、RNAが標的細胞に導入される。もう1つの態様において、RNAは、インビトロ転写されたRNAまたは合成RNAを含むmRNAである。RNAは、ポリメラーゼ連鎖反応(PCR)により生成されるテンプレートを用いるインビトロ転写によって生成される。任意の供給源からの関心対象のDNAを、PCRにより、適切なプライマーおよびRNAポリメラーゼを用いるインビトロmRNA合成のためのテンプレートに直接変換することができる。DNAの供給源は、例えば、ゲノムDNA、プラスミドDNA、ファージDNA、cDNA、合成DNA配列、またはDNAの他の任意の適切な供給源でありうる。インビトロ転写のための所望のテンプレートは、キメラ性膜タンパク質である。一例を挙げると、テンプレートは、抗体、抗体の断片または抗体の一部分をコードする。もう1つの例を挙げると、テンプレートは、抗CD3などの抗体の単鎖可変ドメインを含む細胞外ドメインと、共刺激分子の細胞内ドメインとを含む。1つの態様において、RNAキメラ性膜タンパク質用のテンプレートは、共刺激分子に対する抗体に由来する抗原結合ドメインを含む細胞外ドメインと、CD28および4-1BBの細胞内ドメインの一部分に由来する細胞内ドメインとを含むキメラ性膜タンパク質をコードする。
本明細書に記載の改変細胞を、治療法のための組成物中に含めることができる。組成物は薬学的組成物を含むことができ、かつ、薬学的に許容される担体をさらに含む。改変細胞を含む薬学的組成物の治療的有効量を投与することができる。
ある態様においては、T細胞の供給源を対象から入手する。対象の非限定的な例には、ヒト、イヌ、ネコ、マウス、ラット、およびそれらのトランスジェニック種が含まれる。好ましくは、対象はヒトである。T細胞は、末梢血単核細胞、骨髄、リンパ節組織、脾臓組織、臍帯血、および腫瘍を含む、数多くの供給源から入手することができる。ある態様においては、当技術分野において利用可能な任意のさまざまなT細胞株を用いることができる。ある態様において、T細胞は、フィコール分離などの、当業者に公知の任意のさまざまな手法を用いて対象から収集された血液ユニットから得ることができる。1つの好ましい態様において、個体の流血由来の細胞はアフェレーシスまたは白血球アフェレーシスによって得られる。アフェレーシス製剤は、典型的には、T細胞、単球、顆粒球、B細胞を含むリンパ球、他の有核白血球、赤血球、および血小板を含有する。アフェレーシスによって収集された細胞を、血漿画分を除去するために洗浄した上で、その後の処理段階のために細胞を適切な緩衝液もしくは培地、例えばリン酸緩衝食塩水(PBS)など、またはその後の処理段階のための、カルシウムを含まないか、もしくはマグネシウムを含まないか、もしくはすべてではないものの多くの二価カチオンを含まない洗浄液の中に入れることができる。洗浄の後に、細胞を、例えば、Ca非含有、Mg2非含有のPBSといった、種々の生体適合性緩衝液中に再懸濁させることができる。または、アフェレーシス試料の望ましくない成分を除去して、細胞を培養培地中に直接再懸濁させてもよい。
ある態様においては、T細胞を増大させることができる。増大により、T細胞を、約10倍、20倍、30倍、40倍、50倍、60倍、70倍、80倍、90倍、100倍、200倍、300倍、400倍、500倍、600倍、700倍、800倍、900倍、1000倍、2000倍、3000倍、4000倍、5000倍、6000倍、7000倍、8000倍、9000倍、10,000倍、100,000倍、1,000,000倍、10,000,000倍、またはそれを上回って、ならびにそれらの間の整数の全体および部分のいずれかおよびすべての倍数に増やすことができる。1つの態様において、T細胞は約20倍~約50倍の範囲に増大される。
本発明の薬学的組成物は、本明細書に記載するような改変細胞を、1つまたは複数の薬学的または生理的に許容される担体、希釈剤または添加剤と組み合わせて含むことができる。そのような組成物は、中性緩衝食塩水、リン酸緩衝食塩水などの緩衝液;グルコース、マンノース、スクロースまたはデキストラン、マンニトールなどの炭水化物;タンパク質;ポリペプチドまたはグリシンなどのアミノ酸;酸化防止剤;EDTAまたはグルタチオンなどのキレート剤;アジュバント(例えば、水酸化アルミニウム);ならびに保存剤を含みうる。本発明の組成物は、静脈内投与用に製剤化されることが好ましい。
本発明を、以下の実験例を参照することによってこれから説明する。これらの実施例は例示のみを目的として提供されるものであり、本発明はこれらの実施例に限定されず、むしろ、本明細書で提供される教示の結果として明らかになる全ての変動も包含する。
初代ヒトCD4 T細胞およびCD8 T細胞を、健常志願者ドナーから、RosetteSepキット(Stem Cell Technologies, Vancouver BC, Canada)を用いた陰性選択により白血球アフェレーシス後に単離した。以前に記載された通りに(Barrett et al. (2011) Human Gene Therapy 22(12): 1575-1586)、初代リンパ球を、CD3およびCD28刺激性抗体でコーティングしたマイクロビーズ(Life Technologies, Grand Island, NY, Catalog)によって刺激した。第10日に、90%ウシ胎児血清および10%ジメチルスルホキシド(DMSO)の溶液中にあるT細胞を、細胞1×108個/バイアルで凍結保存した。
初代ヒトT細胞を、10% FCS、100-U/mlペニシリン、100-g/ml硫酸ストレプトマイシン、10-mM Hepesを加えたRPMI 1640中で培養し、抗CD3/抗CD28でコーティングした磁気ビーズにより細胞 対 ビーズ比1:3で刺激した。2日毎に細胞の計数および栄養素添加を行い、増殖動態低下および細胞サイズの両方による判断でT細胞が休止状態に入ったと思われる時点で、T細胞を機能的アッセイのために用いるか、または凍結保存した。
NTTT PAM部位を有するNTTTN20によってCrRNAを選択した。Cpf1およびcrRNAが標的とする、TCR α、β鎖およびβ-2ミクログロビンの定常領域を、インビトロで転写させた。CrRNAを、TCR α定常領域のエクソン1中の配列、またはTCR β定常領域1および2の両方ならびにB2mのエクソン1に共通するコンセンサス配列のいずれかを標的とするように設計した。mRNAは、単回用途のヌクレアーゼ非含有バイアル中にて-80℃で保存した。
表記の特異性を有する以下のモノクローナル抗体および試薬を、適切なアイソタイプ対照とともに用いた。BD Biosciences(San Jose, CA)より:APC結合抗CD3(555335)、PE-抗β-2ミクログロビン(551337)、FITC-抗HLA(555552)。データは、FACS Accuri(BD Biosciences, San Jose, CA)によりCellQuestバージョン3.3(BD Biosciences, San Jose, CA)を用いて取得し、FCS Expressバージョン3.00(De Novo Software, Los Angeles, CA)またはFlowJoバージョン7.6.1(Tree Star, Inc. Ashland, OR)によって分析した。
エレクトロポレーションの前に、T細胞をCD3/CD28ダイナビーズによって3日間刺激した。1000万個の初代T細胞の脱ビーズ処理を行い、その後に、BTX830によって360V、1msのパラメーターで、20μgのCpf1種(Cas9またはeSpCas9)、10μgのcrRNA(またはsgRNA)種の細胞内へのエレクトロトランスファーを行い、その後に5μgのcrRNA(またはsgRNA)の2回目のエレクトロトランスファーを行った。またはT細胞に対して、20ug Cpf1 mRNA+10ugの化学修飾crRNAを同時にエレクトロポレーションした。エレクトロポレーションの後に、細胞を直ちに2mLの予熱培地に入れて、37℃、5% CO2または32℃、5% CO2で1日培養し、続いて37℃、5% CO2に戻した。
Cpf1およびCas9 mRNAを、mMESSAGE mMACHINE T7 ULTRAキット(Life Technologies, AMI 345, Carlsbad, CA)により、インビトロで転写させた。SgRNAおよびcrRNAは、MEGAscript T7転写キットを用いてインビトロで転写させた。化学修飾crRNAは、Integrated DNA Technologiesから購入した。T細胞をCD3/CD28ダイナビーズで刺激した。第3日に、T細胞にCas9またはCpf1 mRNAをエレクトロポレーションした。手短に述べると、T細胞をOPTI-MEMで3回洗浄し、OPTI-MEM(Invitrogen)中に最終濃度1~3×108個/mlで再懸濁させた。その後に、0.1mlの細胞懸濁液をIVT RNAと混合して、2mmキュベット内でエレクトロポレーションを行った。続いて、20μgのCas9またはCpf1 mRNAおよび5μgのガイドRNAを、BTX830システム(Harvard Apparatus BTX)を360Vおよび1msで用いて、細胞内にエレクトロポレーションした。エレクトロポレーションの後に、細胞を直ちに2mlの予熱培地に入れて、IL-2(100 IU/ml)の存在下で37℃、5% CO2で培養した。第4日に、さらに5μgのガイドRNAをT細胞内にエレクトロポレーションした。
T細胞におけるTRAC、TRBCおよびB2mのゲノム破壊のレベルを、T7E1 Nucleaseアッセイ(NEB)を用いて決定した。標的破壊率を濃度測定によって定量した。
以前に、さまざまな属に由来するCpf1タンパク質が、哺乳動物細胞におけるゲノム編集効率に関して試験されている(Zetsche et al. Cell. 2015; 163(3): 759-771)。試験された8種のCpf1タンパク質のうち、それぞれアシダミノコッカス属(As)およびラクノスピラ科(Lb)の属に由来する2種のタンパク質、AsCpf1およびLbCpf1のみが、哺乳動物細胞において検出可能な突然変異を生じさせることが見いだされた。このことから、本明細書では、AsCpf1およびLbCpf1の遺伝子ターゲティング効率をT細胞において評価した。初代T細胞におけるCRISPR媒介性ゲノム編集の効率を、効率的なT細胞遺伝子破壊のために以前に開発されたプロトコール(Ren et al. (2016) Clinical Cancer Research (2016): clincanres-1300)を用いて、AsCpf1、LbCpf1またはCas9の間で比較した。Cas9またはCpf1を、ガイドRNAとともに、単回のエレクトロポレーションで初代T細胞に送達した。ターゲティングされた突然変異誘発を、TCR α鎖定常領域(TRAC)、TCR β定常領域(TRBC)およびβ-2ミクログロブリン(B2m)という3つの内在性標的部位で測定した。Cas9を用いると10%~20%のターゲティングされた突然変異誘発が観察されたが、AsCpf1およびLbCpf1のいずれも検出可能な突然変異誘発は観察されなかった。ガイドRNAの複数回の送達を用いた場合には、Cas9を用いると、3つの遺伝子座位すべてで80~90%の遺伝子破壊が達成された。AsCpf1を用いると50%を上回る遺伝子破壊が達成され、LbCpf1を用いるとおよそ30%が達成された(図4)。遺伝子破壊の有効性の向上は、Cas9およびCpf1エレクトロポレーションの後に、ガイドRNAの2回目の送達を遅らせて行ったことに付随してみられた。この知見により、sgRNAおよびcrRNAを含む小型ガイドRNAが、Cas9およびCpf1 mRNAよりも分解をはるかに受けやすいことが指し示された。AsCpf1は、LbCpf1よりも実質的に高い遺伝子破壊効率を示したことから、以後の実験ではAsCpf1に注目した。
sgRNAの安定性を高めるための化学修飾は、遺伝子破壊効率を強化することが示されている。crRNAの安定性をさらに高めるために、TRACを標的とする化学修飾crRNAを本明細書において作製した。また、単回エレクトロポレーションによって効率的な遺伝子破壊を達成することができ、それ故に第2のエレクトロポレーションによって引き起こされる毒性を減少させうるか否かについても判定した。化学修飾がcrRNAの機能を消失させないことを確かめるために、修飾crRNAを二通りのエレクトロポレーションプロトコールによって調べた。crRNAハンドルにおける尾部「AA」ヌクレオチドの修飾はその機能を消失させ、そのことは「AA」が適正なハンドル構造形成のために不可欠であることを指し示している(図5)。この影響を回避するために、ハンドル尾部に3つの「G」残基を付加し、続いてこの3つの「G」および3つの尾部プロトスペーサーヌクレオチドに対して2'-O-メチル3'ホスホロチオエート化(MS)を施した(図1C)。この場合には、修飾はcrRNAの機能を消失させなかったが、Cpf1およびMS-crRNAの単回エレクトロポレーション後に遺伝子破壊は依然として観察されなかった。部分的にホスホロチオエート化したプロトスペーサーMS-crRNA(PMS-crRNA)を用いると、TRACの9.1%(9.7±1.7、n=3)の遺伝子破壊が達成された。しかし、crRNAの完全なホスホロチオエート化(FPMS-crRNA)ではその機能が消失し、このことはcrRNAがそのハンドル構造の修飾に対して脆弱であることを指し示している(図1D)。
T細胞におけるCpf1の特徴について検討するために、初代T細胞においてCpf1およびst-crRNAを用いて遺伝子破壊を行った。sgRNAではなくcrRNAを用いることだけではなく、Cpf1はTTTNのよりストリンジェントなPAM配列も認識するが、一方、Cas9はNGGのPAM配列を認識するという点でも、Cpf1はCas9とは異なる。Cpf1はCas9と比較して、標的遺伝子を、効率については同等であるがよりストリンジェントな様式で破壊する。TRAC、TRBCおよびB2m遺伝子座位を標的とする10種類のcrRNAまたはsgRNAを用いて、T細胞におけるCpf1、Cas9、および高忠実度Cas9であるeSpCas9の成績について調べた。過半数で、sgRNAはCas9による効率的な遺伝子破壊を媒介したが、一方、Cpf1は10種のcrRNAのうち1~2種のみで同等の遺伝子破壊を達成する。これらの結果により、Cpf1によるcrRNAの選択はCas9による選択よりもストリンジェントであることが指し示された。興味深いことに、Cpf1のこの特徴は、同等の遺伝子破壊を示し、よりストリンジェントなsgRNA選択も示した、高忠実度Cas9であるeSpCas9のそれと同様であった(図2A)。より短い短縮型sgRNAの使用による特異性の強化が報告されている。このため、遺伝子ターゲティング特異性を高めるために同様の戦略をCpf1-crRNAによって用いうるかを調べた。16bpほどの短さのsgRNAを用いて遺伝子破壊効率が幾分低下しただけであったCas9とは対照的に、17bpに短縮したcrRNAでさえもCpf1の機能を完全に消失させ、これはeSpCas9に関して観察されたものと同様の結果であった(図2B)。このように、Cpf1はガイド選択性および短縮型RNAの両方の点でeSpCas9と同様に振る舞い、このことはCpf1を高忠実度遺伝子編集ツールとしてCas9の代わりに用いうることを実証している。
Cpf1の安全性をさらに調べるために、TRAC、TRBCおよびB2mをターゲティングする上で遺伝子破壊効率が最も高い3種のcrRNAを用いてオフターゲットイベントを測定した。各遺伝子について10個のオフターゲット部位を測定した。ストリンジェントなガイド選択性と一致して、野生型20bpまたは短縮型18bpのガイドRNAのいずれを用いても、Cpf1およびeSpCas9については、Cas9と比較して、はるかに低いレベル、およびより少ないオフターゲットイベントが観察された(図3A)。
CRISPRシステムの最も関心を引く用途の一つとして、高効率の遺伝子編集は、T細胞を利用する養子免疫療法を進展させる上で大いに有望である。しかし、Cas9ホモログであるCpf1およびそのガイドcrRNAの特徴はほとんど明らかになっていない。現在までに、Cpf1を用いたT細胞におけるターゲティングされたゲノム編集の可能性はまだ探究されていない。本研究は、Cpf1-crRNAを用いて効率的かつ特異的な遺伝子破壊を達成しうることを実証した。

Claims (31)
- (a)ステムループCRISPR(Clustered Regularly Interspaced Short Palindromic Repeats)RNA(st-crRNA)を含む外来性核酸であって、該st-crRNAが、
(i)細胞中の標的核酸配列にハイブリダイズする配列を含み、5’末端または3’末端にハンドルステムループ構造を含む、crRNA、および
(ii)該crRNAの5’末端または3’末端に結合した追加のステムループ構造
を含む、
外来性核酸、ならびに
(b)Cpf1酵素をコードする外来性核酸
を細胞に導入する段階を含む、エクスビボの遺伝子編集の方法であって、
該st-crRNAと該Cpf1酵素が、RNAガイド(RNA-guided)DNAエンドヌクレアーゼ(RGEN)複合体を形成して、標的核酸配列を編集する、
