[実施例]
動物研究
Pex7ホモ接合性ハイポモルフ動物(Pex7neo/neo)を肢根型点状軟骨異形成症(RCDP)のモデルとして使用した。マウスに、PPI-1011又はPPI-1040のいずれかを、それぞれ100mg/Kg及び89.7mg/Kgの等モル濃度で投薬した。PPI-1011は、12.5mg/mLの濃度でNeobee M-5中で製剤化し、最初の処置日まで-20℃で保管し、その時点の後、残った製剤を4℃で保管した。強制経口容積を、100mg/kg用量を保証するように、マウスの体重によって調整した。PPI-1040は、使用直前まで、アルゴンガス下、ジクロロメタンでの溶液で、密封されたアンプル中で保管した。処置の時に、アンプルを開封し、ジクロロメタンを窒素ガス下で10分間蒸留去した。次いで、Neobee M-5を添加し、溶液をボルテックス(vortex)して、22.4mg/mLの製剤を作成した。動物に、それぞれの動物に対して89.7mg/Kg用量を保証するように、体重によって調整した用量で、腹腔内注射を介して投薬した。動物を9回の処置(1週目は月曜日から金曜日、2週目は月曜日から木曜日)の24時間後、屠殺した。血液試料をEDTAチューブに収集し、次いで、臨床の遠心分離で遠沈した。血漿試料を分析のために解凍するまで-80℃で保管した。組織試料を採取し、液体窒素中で急速凍結させ、次いで、-80℃で保管した。すべての組織試料を、液体窒素を使用してホモジナイズし、微粉末を作成した。

本明細書において議論するように、肢根型点状軟骨異形成症(RCDP)は、プラスマロゲン生合成に必須のペルオキシソーム遺伝子の変異によって引き起こされる壊滅的でまれな遺伝性障害である。プラスマロゲンは、sn-1位の脂肪アルコールに連結したビニルエーテルによって特徴付けられるグリセロリン脂質のクラスである。ビニルエーテル結合は、それらが、小胞輸送、膜タンパク質機能及びフリーラジカルスカベンジングなどの膜媒介機能を正常化するために絶対的であるように、プラスマロゲンに特有の生理化学的性質を与える。以下の研究において、環状ホスホエタノールアミンのビニルエーテルプラスマロゲン前駆体中間体であるPPI-1040の経口での生物学的利用率を研究して、RCDP1のPex7ハイポモルフモデルにおいてプラスマロゲンレベルを増大させ、行動的表現型を正常化するPPI-1040の能力を調査した。
薬物:
PPI-1011、PPI-1040及びPPI-1050を、0.1%のチオグリセロール(99%、Sigma社)を有するNeobee M-5(Stepan Liquid Nutrition)中、10mg/mL又は25mg/kg(PPI-1050)の濃度で製剤化した。PPI-1011は-4℃で保管したが、PPI-1040及びPPI-1050は、安定性の低下に起因して-80℃で保管した。PPI-1050は、図8において「*」で示される位置が13Cで標識化された、PPI-1040の13C標識化バージョンである。薬物の製剤は、処置の前に室温に平衡化させた。強制経口容積を、示されたmg/kg用量を保証するように、マウスの体重によって調整した。
胃の酸性の性質に耐えるPPI-1040中のビニルエーテルの能力を試験するために、化合物を幅広い酸の強度(pH1~5)に曝露した。ビニル結合の安定性を、(上記に示すようにして製剤化した)PPI-1040を1~5のpH範囲に曝露することによって試験した。このpHの溶液は、pH4に達するまで、HPLCグレードの水中の1MのHCLの10倍の連続希釈によって作製した。純粋なHPLCグレードの水を対照溶液として使用した。最終希釈は、20μlの一定分量を200μlのPPI-1040製剤に添加することによって完了し、1~5のpH範囲をもたらした(それぞれの条件を三反復で試験した)。混合物を室温で1時間ボルテックスした。1時間のインキュベーションの後、それぞれの溶液を酢酸エチルに再懸濁させて、10μl/mlの溶液を得て、これを、Agilent 1100 HPLCポンプ及びオートサンプラーと連結されたAPI4000(商標)質量分析計(Applied Biosystems社)におけるフローインジェクションタンデム質量分析(FI-MS/MS)を使用して分析した。
PPI-1050と呼ばれるPPI-1040の13C標識化バージョンを作成し、腸の境界を通過し、血流にインタクトで入る、インタクトな化合物の能力を調べた。PPI-1050は3個のグリセロール炭素及びsn-1パルミチン酸の3個の炭素に炭素-13標識を含有する。野生型C57/Bl6マウスを、100mg/kgの(0.1%のチオグリセロールを有するNeobee-M5中で製剤化された)PPI-1050又は媒体の単回経口投与で処置した。次いで、動物を屠殺し、血液を、処置の1、3及び6時間後に心穿刺によって採取した(n=3)。次いで、血清を、グリセロ脂質への標識の組み込みの存在について分析した。血清の抽出を、1.4mlのThermoマトリックスチューブで20μlの一定分量に対して行った。脂質を、それぞれの血清試料にHPLCグレードの水(50μl)及び酢酸エチル(500μl)を添加することによって抽出し、次いで、1750rpmで1時間混合し、続いて、相分離のために3500rpmで2分間遠心分離した。酢酸エチル層の100μlの一定分量を、Agilent 1100 HPLCポンプ及びオートサンプラーと連結されたAPI4000(商標)質量分析計(Applied Biosystems社)におけるFI-MS/MSを使用して分析した。それぞれの推移を、試料あたり2分の総捕捉時間で50ミリ秒間スキャンした。600μL/分の流速で、80:15:5の酢酸エチル:メタノール:水の比を、移動相として使用した。親のPlsEtnの質量を、定量的MS/MSの推移ペアを得るために、対応する娘イオン(sn2の脂肪酸の喪失)と一緒に予測される親における13C標識のインタクトな数を組み込むことによって決定した。吸収の際にエタノールアミン基がインタクトなままであることを確認するために、我々は、ポジティブモードでFI-MS/MS分析を使用して、標識化アルキル-アシル及びビニル-アシルグリセロールの存在について、血清も分析した。すべての測定された推移を下記の表1に見ることができる。
Pex7ハイポモルフ/ヌルモデルは、混合バックグラウンド(C57BL/6NCrl及び129S6/SvEvTac)におけるハイポモルフ(B6;129S6-Pex7tm2.3Brav/s9)マウスモデルである。Pex7ハイポモルフ/ヌルにおける発現低下アレルは、イントロン2、及びエクソン3周辺のloxPサイトへの逆neoカセットを挿入することによって作成した。Pex7ヌルアレルは、(B6.C-Tg(cmv-cre)1Cgn/J)マウスを使用して作成した。Pex7ハイポモルフ/ヌルマウスモデルは、90%の生存率、0.016~0.257%の野生型Pex7のmRNAレベル及び20~30%のプラスマロゲンレベルのPex7ハイポモルフ/ヌルマウス(3~4カ月)を有しており、野生型動物の雄及び雌を処置及び行動試験に含めた。Pex7ハイポモルフ/ヌルマウスを、ベースラインのオープンフィールド試験の後、無作為に、PPI-1040、PPI-1011及び媒体対照の3つの処置群に割り当てた(群あたりn=6マウス)。50mg/kgのPPI-1011又はPPI-1040を、週5日(月曜日~金曜日)、4週間、強制経口によって与えた。処置期間の間、動物は、毎週体重を測定し、苦痛の兆候について観察した。オープンフィールド試験を処置の終わりに行った。選択バイアスを回避するために、データ解析を研究の終わりにのみ行った。動物を、最終投薬の後、24時間後に屠殺した。組織をLC/MSMS分析のために採取した。
オープンフィールド試験は、正常な環境に応答して、マウスの一般活動性、探索行動及び不安様行動を調べるために使用される行動試験である。動物を、40×40×40cmの寸法の特注の正方形の灰色のアクリレートボックスの内側に置き、オーバーヘッドカメラ(カメラモデル)によって記録しながら、5分間自由に動き回らせた。映像を、総移動距離(メートル)及び活動性(秒での移動度)時間について、自動追跡システム(Any-maze Video Tracking Software、Stoelting Co社、Wood Dale、IL、USA)によって解析した。総移動距離は、試験の間に動物が移動した総距離であり、活動性は、動物が試験の間に活動的であった時間の量を測定する。
液体窒素中に沈めることによって凍結させた組織試料を、Covaris Cryoprepを使用してホモジナイズして、微粉末を得た。次いで、粉末を、帯電防止ポリプロピレン使い捨てミリグラムスクープ(TWD Tradewinds社)を使用して等分し、1.4mlのThermoマトリックスチューブあたり4~6mgの組織とした。重量を記録し、代謝物レベルを、組織1mgあたりで正規化した。HPLCグレードの水(50μl)をそれぞれのチューブに添加し、試料を液体窒素中で瞬間凍結し、次いで、抽出まで-80℃で保管した。抽出の日に、組織試料を室温に解凍し、氷浴中で15分間超音波処理した後、2000rpmで5分間混合し、次いで、600μLの酢酸エチルを添加した。脂質を、1750rpmで1時間混合し、続いて3500rpmでの10分間の遠心分離によって、酢酸エチルに抽出して、透明な酢酸エチル層を得た。組織の脂質抽出物を、標識化内部標準[13C-PlsEtn(C37 13C6H74NO7P)]を含有する酢酸エチルストックに希釈した(脳領域を1:10で希釈、末梢組織を1:5で希釈)。水(40μL)を希釈した抽出物に添加し、試料を1500rpmで1時間撹拌し、続いて、3500rpmで2分間遠心分離した。
データは平均±SDで表す。チューキーの事後検定を使用する一元配置分散分析(ANOVA)を使用して、プラスマロゲンレベル及び行動データを解析した。基本の線形回帰を使用して、プラスマロゲンレベルと行動スコアを比較した。0.05未満のp値を、統計学的に有意であるとみなした。
PPI-1040の酸安定性:
プラスマロゲンレベルを増大させるための経口療法としてPPI-1040をさらに調査するために、酸性条件下でのビニルエーテル結合の酸安定性を研究した。PPI-1040を、水性条件(対照)及び酸性条件(pH1~5)の両方に1時間曝露して、胃の環境をシミュレーションした。PPI-1040のsn-3位の環化されたエタノールアミン基は、環化型のレベルはインキュベーション後にすべての試料においてほとんど検出されず、水性環境又は酸性環境に曝露されると容易に開環することを示した(図9A)。PPI-1040の開環バージョンは、内因性のPlsEtn16:0/22:6として特定され、pH2で観察され、pH1での非常に少量が検出されるわずかな低下で、pH3まで安定であり、低pHでのビニル結合の分解を示唆している(図9B)。sn-1のビニルエーテル結合の切断を受けたPPI-1040の開環のレベルを分析することによって、制御条件及びpH3~5の下で、sn-1基の最小の喪失が存在したことを確認した。しかしながら、ビニル基の切断は、pH2で開始が広まり、pH1まで、分子の全体的な分解があるように見える(図9C)。
経口での生物学的利用率を、PPI-1050と呼ばれるPPI-1040の13C標識化バージョンを野生型マウスに投薬し、インタクトな標識化合物について血清を調べることによって、研究した。加えて、薬物の代謝を、置換されたsn-2構成物を有するPlsEtnを測定することによって追跡した。インタクトな閉環を有するPPI-1040は、いかなる試料中でも検出可能ではなく(データは示さない)、摂取の際に、環が自発的に開環しないことを示す。明確な時間依存性の増加が、標的の13C6標識化PlsEtn16:0/22:6において観察された(図9A)。sn-2構成物のリモデリングが直ぐに生じることが報告され、したがって、sn-1に16:0を有する13C6標識化PlsEtn、及び5つの最も一般的に生じるsn-2構成物が測定された(図9B)。すべての種は、標的のPlsEtnとともに見られる時間依存性の増加を反映した。ビニルエーテル結合が、加水分解されないか、又はsn-1での再配置に付されないことを検証するために、同じPlsEtn種のレベルを測定したが、ビニルエーテルが壊れ、且つ標識化されたままのsn-1のパルミチン若しくはグリセロールのみか、両方ともそうではない場合に予測される13C3標識化物であった。媒体対照に対して、いかなる時点でもいかなる種の増加も観察されず、ビニルエーテル結合がインタクトなままで残っていることを確認した。最後に、sn-3のホスホエタノールアミン基が吸収の前に除去されなかったことを確認するために、ビニル-アシル及びアルキル-アシルの16:0/22:6の存在を試験し、このいずれもが対照に対して上昇しなかった。これらのデータの組み合わせは、PPI-1040が経口で生物学的に利用可能であり、腸の内壁をインタクトで通過し、予測されるsn-2の再配置のみが生じたことを例証する。
媒体、PPI-1011又はPPI-1040の経口投与の4週間後、血漿及び組織レベルを、プラスマロゲンレベルについて試験した。Pex7ハイポモルフ/ヌル動物は、測定されたすべてのプラスマロゲンの有意に減少したレベルを有し、平均で、野生型対照レベルのおよそ25%であった。50mg/kgのPPI-1011での処置は、測定されたいかなるプラスマロゲン種の有意な増大をもたらさなかった。しかしながら、PPI-1040の処置は、プラスマロゲンレベルを増大させることが可能であった。標的の16:0/22:6プラスマロゲンのレベルを正常化することに加えて、sn-2位での再配置は、16:0/22:4を除くすべての測定された16:0プラスマロゲンにおける増加を生じ、これは、少ない割合の総16:0プラスマロゲンのプールを表す(図10)。sn-1に18:0及び18:1を含有するプラスマロゲンも測定したが、予想されたように、これらの種のいずれにおいても増大は観察されなかった。加えて、試料を、ホスホエタノールアミン基が除去された場合に増加すると予測される、ビニル-アシル及びアルキル-アシルグリセロールのレベルについて分析した。アルキル-アシル又はビニル-アシルグリセロール種は、処置された動物において増加しなかった。
Pex7ハイポモルフ/ヌルマウスをオープンフィールド内で追跡して、総移動距離(メートル)及び活動時間(秒)によって測定される活動性のレベルを評価した。媒体処置動物は、いずれかの測定によって評価されるように、野生型対照に対して、有意なレベルの活動亢進を示した。PPI-1040での処置は、活動時間及び移動距離の両方によって評価されるように、活動亢進性表現型の正常化をもたらした(図11A)。それぞれの処置群についての代表的な追跡画像を示す。加えて、血漿プラスマロゲンレベルは、行動的表現型と強く相関することを示した。標的の16:0/22:6プラスマロゲンの血漿レベルは、移動距離(R2=0.36、F=7.93、p=0.014)及び活動時間(R2=0.54、F=16.37、p=0.0012)の両方と相関があった(図11C)。血漿中の総16:0プラスマロゲンレベルは、処置後に増大するプラスマロゲンの総プールの評価を可能にし、移動距離(R2=0.37、F=8.37、p=0.011)及び活動時間(R2=0.55、F=17.28、p=0.00096)の両方とも相関があった。
酸安定性の研究及びC13標識化PPI-1050経口処置研究に基づいて、ビニルエーテル結合が胃の酸性環境において安定であり、経口投与後にGI管から直ぐに吸収されることは明らかである。加えて、C13標識化PPI-1050のデータは、望ましい環状ホスホエタノールアミン環の開環以外に、分子の構造がインタクトで吸収されることを実証する。Pex7ハイポモルフ/ヌルマウスの肝臓、小腸及び骨格筋を含む末梢器官におけるさまざまな程度への血漿中でのプラスマロゲンレベルの正常化及びプラスマロゲンレベルの増加は、欠乏個体におけるプラスマロゲンレベルの増大が、PPI-1040処置によって可能であり得るという仮定を裏付ける。皮質脳組織においてプラスマロゲンの増大を検出することは可能ではなかったにもかかわらず、PPI-1040で処置されたマウスは、モデルの典型的な活動亢進性表現型において有意な改善を示し(p<0.05)、行動及び血漿プラスマロゲンレベルの間の強い相関が観察された。
脳のプラスマロゲンレベルが神経伝達に影響を与えると仮定されている。したがって、この研究において、野生型マウスの海馬脳切片を、媒体又はPPI-1040のいずれかを含有する人工脳脊髄液中でインキュベートした。切片を、100μm離れて電極を有する、マルチ電極アレイ(MEA、multi-electrode array)上でインキュベートした。単一電極を選択して、放射状層の興奮性シナプス後場電位(fEPSP、field excitatory post-synaptic potential)を作動させるシェファー側枝細胞を刺激した。対照の記録の10分後、PPI-1040又は媒体のいずれかの存在中で、長期増強(LTP)を、200ミリ秒の間隔でそれぞれ100Hzの4つの刺激で構成される10バーストの単一列によって誘導した。次いで、誘発反応の増強を60分間監視した。PPI-1040(0.5μM)で処置された切片は、60分の間にわたって媒体で処置された対照と比較して、LTPが向上し、向上したプラスマロゲンレベルが神経可塑性を向上させることを示唆し、神経機能におけるプラスマロゲンの重要性を裏付ける。結果を図14に示す。
PPI-1011及びPPI-1040は両方とも、内因性16:0/22:6エタノールアミンプラスマロゲン種に変換されるように設計されたプラスマロゲン前駆体である。両方の分子は、グリセロール骨格とsn-1位にパルミチルアルコール(16:0)及びsn-2位にDHA(22:6)脂肪酸で構成される。
インビボでのPPI-1040の変換を調査し、生成したPlsEtnが血清中で検出され得ることを確認するために、PPI-1040の13C標識化バージョンを設計し、PPI-1050と指定した。PPI-1050は、[13C3]パルミチン酸及び[13C3]グリセロールで標識化される。
インビボでのPPI-1011の変換を検証するために、PPI-1011の13C標識化バージョンを設計し、PPI-1038と指定した。PPI-1038は、PPI-1050と同様に、[13C3]パルミチン酸及び[13C3]グリセロールで標識化されるが、追加で、sn-2のDHAにおいて[13C3]標識化された。この追加は、PPI-1050処置動物において行わなかった、投与の後のsn-2基における追跡を可能にした。
上記に記載したPPI-1040及びPPI-1011のPKプロファイルにおいて見られた相違に加えて、2つの分子は、欠乏動物モデルにおけるプラスマロゲンレベルを増大させる異なる能力を示す。欠乏マウスへの媒体、PPI-1011又はPPI-1040の経口投与の4週間後、血漿及び組織レベルを、プラスマロゲンレベルについて試験した。媒体で処置されたPex7ハイポモルフ/ヌル動物は、測定されたすべてのプラスマロゲンの有意に減少したレベルを有し、平均で、野生型対照レベルのおよそ25%であった。過去の研究において、PPI-1011は、動物においてプラスマロゲンレベルを増大させる中程度の能力を実証したが、Pex7ハイポモルフ/ヌルマウスにおける50mg/kgのPPI-1011での処置は、プラスマロゲンレベルの増大に無効であり、標的プラスマロゲン又は試験されたその他の16:0プラスマロゲン種において変化がなかった。しかしながら、PPI-1040の処置は、プラスマロゲンレベルを効果的に増大させた。標的の16:0/22:6プラスマロゲンのレベルを正常化することに加えて、sn-2位での再配置は、16:0/22:4を除くすべての測定された16:0プラスマロゲンにおける増加を生じ、これは、少ない割合の総16:0プラスマロゲンのプールを表す(図20)。sn-1に18:0及び18:1を含有するプラスマロゲンも測定したが、思った通り、これらの種においても増大は観察されなかった。最後に、試料を、sn-3のホスホエタノールアミン基が除去された場合に増加すると予測される、ビニル-アシル及びアルキル-アシルグリセロールのレベルについて分析した。測定されたアルキル-アシル又はビニル-アシルグリセロール種は、処置された動物において増加せず、上記に示したPPI-1040のPKデータと一致した。

- 対象が、肢根型点状軟骨異形成症(RCDP)又はツェルウェーガースペクトラム障害に罹っている、請求項1~6のいずれかに記載の上昇剤。
