Description
[技術的課題]
ハンター症候群の治療について、治療薬剤を脳に有効に送達する大きな必要性が依然として存在する。くも膜下腔内(IT)注射、または脳脊髄液(CSF)への治療タンパク質の投与は、ハンター症候群の患者を治療するために最近試みられているが、多数の有害事象が臨床試験において報告されている[8]。有害事象の大部分が、IT薬物送達装置の機能不全に関連しているにもかかわらず、脳室内(ICV)投与などの別のアプローチがハンター症候群の患者の治療に必要とされている。ICV投与は、患者の脳室への治療薬剤の直接送達に有効なアプローチであろうし、現在、ICV投与によるハンター症候群の治療について、承認された製品および/または現在開発中の製品はない。
1.治療を必要とする対象に、およそ0.1mg/mlからおよそ60mg/mlの範囲の濃度でイデュルスルファーゼ−ベータ(IDS−β)タンパク質、およそ150mMの濃度で塩化ナトリウム、およそ0.05mg/mlの濃度でポリソルベート20、およびおよそ6のpHを含む治療有効量のICV製剤を脳室内投与する(ICV投与)工程を含む、ハンター症候群を治療する方法。
2.ICV製剤がおよそ15mg/mlの濃度でイデュルスルファーゼ−ベータ(IDS−β)タンパク質、およそ150mMの濃度で塩化ナトリウム、およそ0.05mg/mlの濃度でポリソルベート20、およびおよそ6のpHを含む、技術的解決策1に記載の方法。
3.治療有効量がおよそ1mgからおよそ30mgの範囲である、技術的解決策1に記載の方法。
4.治療有効量がおよそ10mgである、技術的解決策1に記載の方法。
5.ICV投与が3週間毎に1回行われる、技術的解決策1に記載の方法。

本発明の脳室内投与されたIDS−βは、MPS IIマウスにおける脳および脳脊髄液(CSF)におけるヘパラン硫酸(HS)およびグリコサミノグリカン(GAG)レベルを減少させた。
下記に詳細に記載されるように、本発明者らは、イデュルスルファーゼ−ベータ(IDS−β)タンパク質の有効な脳室内(ICV)投与のための安定な製剤を開発することに成功した。
以下、実施例を参照して本発明をさらに詳細に説明する。これらの実施例は説明の目的のみであり、かつ本発明の範囲を限定するものと解釈されるべきではないことは、当業者に明らかである。
1−1:概要
本研究は、MPS IIマウスにおけるイデュルスルファーゼベータ(IDS−β)の単回脳室内(ICV)注射の薬理学的な効果および用量応答関連性を研究するために行った。加えて、我々はCSFにおけるHS濃度を測定し、かつMPS IIマウスのCSF HSと脳組織HSおよびGAG間の相関を研究した。
動物
我々は、以前報告したIDSノックアウト(KO)マウスを使用した。簡潔に述べると、Ids遺伝子をエクソン2からエクソン3まで除去した[15]。IDS KOマウスは、C57BL/6.129Sバックグラウンド株から飼育し、Ids遺伝子においてヌル変異を有していた。野生型(WT)コントロールマウスを、C57BL/B6.129S株から飼育した。テールスニップ(tail snip)から得られたDNAのポリメラーゼ連鎖反応によりすべてのマウスの遺伝子型を確認した。この研究は動物実験委員会(Institutional Animal Care and Use Committee)により認められ、(承認番号20140925005)、かつ韓国ソウルSamsung Biomedical Research Instituteの動物愛護ポリシーに従って行われた。
6週齢の動物を層別無作為化によって5群(各群12動物)に割り当てた。IDS KOマウスを4群:ビヒクル注射されたIDS KOマウスおよびIDS−β(Green Cross Corp.、龍仁、韓国)注射の3つの異なる用量(3μg、10μg、および30μg)のIDS KOマウスに割り当てた。各群における4動物をICV注射後毎7、14、および28日に屠殺した。注射後7、14、および28日に、多様な組織(脳、心臓、肺、肝臓、脾臓、および腎臓)のGAG濃度を解析し、かつCSFおよび脳からのHS濃度を測定した。
ビヒクル溶液は150mM塩化ナトリウム 0.05mg/mL Tween 20溶液(Merck Millipore,Darmstadt,Germany)であった。濃縮したIDS−β(50mg/mL)薬物溶液をビヒクルで希釈し、0.6、2、および6mg/mLの濃度を作った。
マウスの単回ICV注射は6週齢で行った。各薬物溶液またはビヒクルを5μLの総容積でマウスにICV投与した。投薬日に、マウスをイソフルラン(Hana Pharm.,Korea)吸入で麻酔し、定位装置に入れた。小さい切り込みを入れた後、頭蓋骨を露出させ、清掃した。ICV注射は、以前に報告された改変された方法に従って行われた[16、17]。IDS−βまたはビヒクルは、コーディネート(ベンチマーク、Neurolab、St.Louis、MO):ブレグマ(bregma)へ0.58mm尾側、矢状縫合へ1.25mm側方、および深さ1.77mmを使用して、シリンジポンプ(Harvard Apparatus、Holliston、MA、USA)により制御されて10mL/分の割合で31ゲージ針で右側脳室に注射した。注射部位を破裂した血管または顔面腫脹についてモニターした。その後、逆流を防ぐためにプランジャー移動の中止後15秒で、針を除去した。切り込みを創傷クリップで閉じ、かつマウスを37℃等温パッド上に置き、手術後、回復まで観察した。全体のプロトコールは、1動物について10〜15分かかった。成功したICV注射技術を実証するために、色素溶液をICV注射した。脳を注射後の異なる時点で収集し、可視化した。脳室の1つの適切な注射は、注射後およそ10〜15分に脳の注射された側にトリパンブルー(0.05%)の分布を可能にした。大脳半球におけるトリパンブルーの広範な分布は注射後およそ1時間で見えた。不正確な注射は、大脳半球における青色の欠如により区別できる(図5)。
注射後7、14、および28日で、過剰な量のアルファキサロン(Jurox/name:Alfaxan)溶液(15mg/kg)の注射によりマウスを安楽死させた。CSFをホウケイ酸ガラスにより大槽から収集し(O.D.:10mm、I.D.:0.75mm)、HS濃度測定のために凍結した。マウス脳組織における血液を15−20分間リン酸緩衝生理食塩水(PBS)で経心臓灌流により洗浄した。脳組織を収集し、ドライアイス上で凍結した。そして、試料をホモジナイズし、4分割した(GAG測定について半分およびHS測定について半分使用した)。他の体細胞組織(心臓、肺、肝臓、脾臓、および腎臓)もまた、収集し、PBS中でホモジナイズした。
ホモジナイズされた組織試料を4℃で一晩振とうし、12,000xgで15分間遠心し、その後上清を収集した。総GAGレベルを、sGAGアッセイキット(Kamiya Biochemicals,Japan)を使用して測定した。第一に、50μLのホモジナイズされた試料を50μLの8mol/Lグアニジン−HClで、15分間室温(RT)でインキュベートした。次いで、50μlのSTA溶液(0.3%H2SO4、0.75%TritonX−100)をRTで15分間加え、およびアルシアンブルーの溶液を、15分間その溶液に加えた。その後試料を15分間、12,000rpmで遠心し、DMSO溶液(40%DMSO、0.05mol/L MgCl2)ですすいだ。最終的に、500μLのGu−prop溶液(4mol/Lグアニジン−HCl、33% 1−プロパノール、0.25%TritonX−100)を、ペレットに加え、混合物を完全に溶解した。あるいは吸光度を600nmでX−Mark(Bio−Rad、Hercules、CA)で読んだ。GAG濃度を、BCA Protein Assay kit(ThermoFisher、Waltham、MA)で測定したタンパク質濃度に正規化した。GAG濃度は、GAG基質、コンドロイチン硫酸−6の検量線を通じて計算されたGAG/mgタンパク質のgとして表した。各試料のデータは重複測定の平均であった。
マウスCSFおよび脳組織試料におけるHSレベルをLC−MS/MSを使用して決定した。5mg/mLの各較正標準(STD)ストック溶液を水にHSナトリウム塩またはDSを溶解させることにより調製した。STDストック溶液を適切な容積の水で希釈し、0.1、0.2、0.5、1.0、2.0、5.0、10および20μg/mLのSTDおよび0.2、2および15μg/mLの質コントロール(QC)試料を調製した。また、5mg/mLのSTDストック溶液を重水素標識して、HS−d6およびDS−d6を内部標準(IS)とした。20μLのSTD溶液を、ガラス試験管に加え、窒素下で蒸発させた。残渣を200μLのメタノール−d4−塩化アセチル(400:64、v/v)と混合し、65℃で90分間加熱することによりメタノール化した(methanolyzed)。メタノリシス後、溶剤を窒素下で蒸発させた。残渣を1mLの水において再構成させ、水−メタノール−ギ酸(950:50:1、v/v/v:緩衝液A)で希釈し、調製した溶液をISストック溶液として使用した。マウスCSF試料を4℃で5分間2100×gで遠心し、上清を等量のPBSで希釈した。マウス脳組織を0.01mol/Lの水酸化ナトリウム(脳重量について50または100倍の容積)でホモジナイズした。ホモジネートを室温で24時間インキュベートし、20μLのホモジネートを、180μLの水−クロロホルム(4:5、v/v)に加えた。混合後、試料を4℃で5分間10000×gで遠心し、上清を等量のPBSで希釈した。CSFおよび脳から調製したこれらの試料をLC−MS/MSの解析のための試験試料として使用した。試験管中のCSF(脳から20μL)、STDまたはQCからの4μLの試験試料を窒素下で蒸発させた。残渣に50μLの3M HCl−MeOHおよび5μLの2,2−ジメトキシプロパンを加え、3分間超音波処理し、65℃で90分間加熱し、かつ蒸発させた。残渣を200μLのISストック溶液で再構成し、3分間超音波処理し、遠心フィルターに移し、4℃で3分間10000×gで遠心し、得られた濾液を、解析した。5μLの各試料をACQUITY UPLCシステム(Waters)を備えたトリプル四重極質量分析計API5000(AB/MDS Sciex)に注入した。試験試料およびISを40℃で加熱したACQUITY UPLC HSS T3カラム(100A 1.8μm、2.1mmx100mm)上で分離した。最初の移動相は、0.4mL/minの流速で勾配溶出を伴う100:0(v/v)緩衝液A:緩衝液B[水−メタノール−ギ酸(500:500:1、v/v/v)]で構成された。溶出は直線勾配であり、緩衝液Bは0.5−4分間に0%−45%増加し、次いで4.01分で60%に増加し、1分間60%に維持され、ついで5.01分で0%に減少し、次いで、1分間0%に維持された。質量分析計をイオン化方法についてエレクトロスプレーイオン化およびイオン極性についてポジティブを選択した設定下で行った。窒素をカーテンガス(40psi)として使用し、空気をネブライザーガス(50psi)およびヒーターガス(40psi)として使用した。イオンモニタリング条件は、4.5kVのイオンスプレー電圧および600℃のターボプローブ(Turbo probe)温度として定義した。デクラスタリング電位、エントランス電位および衝突エネルギーのこれら設定は、それぞれ110V、8Vおよび22eVであった。HSについて質量電荷比(m/z)384→162、DSについてm/z426→236、HS−d6についてm/z390→162およびDS−d6についてm/z432→162を使用して、多重反応モニタリング(MRM)によりデータを取得した。ピーク面積、STD曲線、および測定濃度をAnalyst ver.1.5.1(AB Scix)で計算した。
統計学的解析をGraphPad Prism 6により行った。MannWhitney U試験を使用して、ノックアウトマウスにおける各薬物−治療群およびビヒクル−治療群間の差を比較した。0.05より小さいP値を有する差は、統計学的に有意であると考えられた。データを平均およびSEMとして示した。CSF HSと脳HS間およびCSF HSと脳GAG間の関連性を決定するために、我々は、マウスCSFおよび脳組織の73試料を評価し、かつデータを、スピアマン(Spearman)のローおよび直線回帰を使用して解析した。級内相関係数(ICC)および95%信頼区間を計算した。
体重
すべての実験群の体重は、研究期間中に有意に変化しなかった。コントロール群(WTおよび非治療IDS KOマウス)におけるものと比較してICV ERT群におけるマウスの重量には有意差はなかった。我々はまた、ERT群のいずれにおいても実験中異常な臨床兆候は何も見いださなかった。
ビヒクル注射群におけるノックアウトマウスの脳組織における総GAGは、WTマウスのものと比較して有意に高かった(図1)。すべてのIDS−β−治療群の脳組織における総GAGは投薬後7日の疾患コントロールマウスのものと比較して有意に減少した(図1)。しかしながら、3および10μg注射群において注射後14日にGAGの再蓄積を観察した。総GAGレベルは7日および14日のものと比較してわずかに再増加したにもかかわらず、注射後28日では、30μg注射群において有意なGAG減少を維持した(図1)。
HSレベルはWTコントロールマウスのCSFおよび脳組織と比較してIDS KOマウスのCSFおよび脳組織において有意に増加した(図2)。ICV注射後7日でCSFにおけるHS含量および脳組織はすべての3つのIDS−β−治療群において有意に減少した。脳組織におけるHSの有意な減少は、28日にわたり維持した(図2)。CSFにおけるHS含量は、ICV注射後14および28日で減少したままであった。しかしながら、統計学的有意差は、ICV注射後28日で30μg IDS−β−治療群においてのみ見られた(図2)。
有意な正の相関は、マウス試料の脳組織における、CSFにおけるHS含量およびHS濃度間に見出された(r=0.785、P<0.0001)(図3A)。さらに、CSFにおけるHS含量はまた、脳組織のGAG濃度と有意な正の相関を有していた(r=0.703、P<0.0001)(図3B)。
我々は、脳組織および他の体細胞組織(心臓、肺、肝臓、脾臓、および腎臓)の両方における単回ICV注射後の総GAG濃度を測定した。WTマウスと比較して、GAGの蓄積はビヒクル注射でのIDS KOマウスのすべての解析された組織において見出された(図4)。ICV注射後28日で、30μgのIDS−βでのICV投与は、すべての調べた組織における総GAGの有意な減少を維持した(図4)。ICV注射後7および14日での体細胞組織のGAG濃度は、図6−10に示される。
MPS IIは、アジアで最も一般的なタイプのMPSであり、かつMPS IIの患者のおよそ70%は、重度の形態を有する[18、19]。従って、脳の病態の修正は、MPS IIの患者の治療における最も重要かつ挑戦的な問題の1つである。組み換え酵素のくも膜下腔内または脳室内の注射は、脳に治療薬を送達する戦略として示唆されてきた。本研究では、我々は、6週齢IDS KOマウスにおける3つの異なる用量のIDS−βの単回ICV注射を行い、薬理効果の用量応答関連性および時間経過を評価した。

Claims (17)
- 15mg/mlの濃度でイデュルスルファーゼ−ベータ(IDS−β)タンパク質、150mMの濃度で塩化ナトリウム、0.05mg/mlの濃度でポリソルベート20、5mM以下の濃度でリン酸、および6のpHを含む治療有効量のICV製剤を含む、ハンター症候群を治療するための医薬組成物であって、前記医薬組成物が治療を必要とする対象に脳室内投与(ICV投与)される、医薬組成物であって、
ICV投与がリザーバーおよびリザーバーに結合したカテーテルを含む脳室内カテーテル系を通じる、医薬組成物。 - 治療有効量が1mgから30mgの範囲である、請求項1に記載の医薬組成物。
- 治療有効量が10mgである、請求項1に記載の医薬組成物。
- ICV投与が3週間毎に1回行われる、請求項1に記載の医薬組成物。
- ICV投与が毎月1回行われる、請求項1に記載の医薬組成物。
- リザーバーが、治療を必要とする対象の頭皮および脳の間に配置され、かつカテーテルの端が対象の脳室の内側に配置され、そしてリザーバーの内空がカテーテルの内空を通じて脳室の内空に結合され、そして脳脊髄液が脳室からリザーバーへ流れ、リザーバーを満たすように脳室内カテーテル系が外科的に移植され;
0.1−60ml/分の流速でリザーバーから0.1−5mlの脳脊髄液が引き抜かれ;
0.1−60ml/分の流速でリザーバーへ0.1−5mlのICV製剤が注射され;かつ
ICV製剤が、カテーテルを通じてリザーバーから脳室へ流れることができる、
請求項1に記載の医薬組成物。 - ICV投与が、ハンター症候群のための酵素補充療法治療の少なくとも1つの追加的な形態の組み合わせで行われる、請求項1に記載の医薬組成物。

- ICV投与が毎月1回行われ、静脈内投与が1週間毎に1回行われる、請求項8に記載の医薬組成物。
