Description
(i)式(I)
(ii)少なくとも1つの化学療法剤および/または少なくとも1つの免疫療法剤と
を含む抗腫瘍医薬組み合わせ(antitumor pharmaceutical combination)に関する。一実施形態では、前記抗腫瘍医薬組み合わせは、腫瘍抗原をさらに含む。特定の一実施形態では、式(I)
(i)1以上の用量単位の式(I)
(ii)1以上の用量単位の少なくとも1つの化学療法剤および/または1以上の用量単位の免疫療法剤と
を含む組み合わせ製剤に関する。
本発明に従って用いられる式(I)
本明細書で用いられるとき、用語「化学療法剤」は、抗がん化学療法に用いられる任意の細胞増殖阻害化合物または細胞傷害性化合物を指す。かかる化合物が抗原でないことは理解される。かかる化学療法剤は、当業者にとって周知であり、
− アルキル化剤、例えばナイトロジェンマスタード、例えばシクロホスファミド、イホスファミド、メクロレタミン、クロラムブシルおよびメルファラン;エチレンアミンおよびメチルメラミン、例えばチオテパ;メチルヒドラジン誘導体、例えばプロカルバジン;スルホン酸アルキル、例えばブスルファン;ニトロソウレア、例えばカルムスチンまたはロムスチン;トリアゼン、例えばダカルバジンおよびテモゾロミド;白金配位複合体、例えばシスプラチン、カルボプラチンおよびオキサリプラチン;

− ホルモンおよび拮抗剤、例えば副腎皮質抑制薬、例えばアミノグルテチミド;副腎皮質ステロイド、例えばプレドニゾン;プロゲスチン、例えば酢酸メゲストロールおよびメドロキシプロゲステロン;エストロゲン、例えばジエチルスチルベストロール;抗エストロゲン、例えばタモキシフェン、イドキシフェン、ドロロキシフェン、ジンドキシフェン、トリオキシフェン、ICI182、780、EM−800およびトレミフェン;アロマターゼ阻害剤、例えばアナストロゾール、レトロゾールおよびエキセメスタン;アンドロゲン、例えばプロピオン酸テストステロン;抗アンドロゲン、例えばフルタミド;およびゴナドトロピン放出剤、例えばリュープロリド
を含む。
本明細書で用いられるとき、用語「免疫療法剤」は、がん細胞に対して新規の免疫応答を開始させるか、または既存の免疫応答を追加免疫することにより抗新生物効果を媒介する抗がん剤、例えば腫瘍抗原を標的にする抗体またはリンパ球を指す。免疫療法剤は、悪性細胞に対する宿主免疫系を(再)活性化するその能力に基づき、「活性型」または「受動型」として分類され得る。
本発明は、上の「抗腫瘍医薬組み合わせ」セクションに定義される通り、治療有効量の、
(i)式(I)
(ii)上の「化学療法剤」セクションに定義されるような少なくとも1つの化学療法剤および/または上の「免疫療法剤」セクションに定義されるような少なくとも1つの免疫療法剤と
を含む抗腫瘍医薬組み合わせの、それを必要とする患者における投与を含む、がんを治療する方法に関する。一実施形態では、前記ABX196化合物は、上の「ワクチン組成物」セクションに定義されるような腫瘍抗原をさらに含むワクチン組成物中に含まれる。

(ii)上の「化学療法剤」セクションに定義されるような少なくとも1つの化学療法剤および/または上の「免疫療法剤」セクションに定義されるような少なくとも1つの免疫療法剤と
の使用にも関する。一実施形態では、前記ABX196化合物は、上の「ワクチン組成物」セクションに定義されるような腫瘍抗原をさらに含むワクチン組成物中に含まれる。
本実施例は、化合物ABX196および腫瘍抗原を含むワクチン組成物がドキソルビシンなどの化学療法剤または抗PD−1抗体などの免疫療法剤の抗がん効果を高めることを示す。
1.1.化合物
用いたワクチン組成物は、ペプチドCl II−TRP2180−188およびABX196アジュバントを含む。
ドキソルビシンをNaCl0.9%で希釈した。
ペプチドCl II−TRP2は、1μgのアジュバントABX196とともに50μg/マウスの用量で投与した。
ワクチン組成物は、静脈内経路によりマウスの尾側静脈に注射した(IV、ボーラス)。
1.5.1.がん細胞株
用いた細胞株は、下の表1に詳細に示す。
細胞を、それら各々の培地において、加湿雰囲気中(5%CO2、95%空気)、37℃、単層で増殖させた(下表を参照されたい)。培地は、10%ウシ胎仔血清(参照:3302,Lonza)を添加した、2mMのL−グルタミン(参照:BE12−702F,Lonza,Verviers,Belgium)を含有するDMEMであった。腫瘍細胞は、プラスチックフラスコに接着する。実験使用のため、腫瘍細胞を、カルシウムまたはマグネシウムを含まないハンクス培地(参照:BE10−543F,Lonza)中、トリプシン−バーゼン液(参照:BE17−161E,Lonza)で5分間処理することにより培養フラスコから剥離し、完全培地の添加により中和した。細胞を血球計数器で計数し、それらの生存度を0.25%トリパンブルー排除アッセイにより評価した。
1.6.1.動物
6〜7週齢の健常な雌C57BL/6(C57BL/6J)マウス95匹をJANVIER LABS(Le Genest−Saint−Isle,France)から入手した。動物は、FELASAガイドラインに従い、SPF健康状態で維持した。
動物は、次の制御された環境条件下において収容室内で維持した。
− 温度:22±2℃、
− 湿度55±10%、
− 光周期(12時間明/12時間暗)、
− HEPA濾過空気、
− 再循環なしで空気交換15回/時間
− 上面フィルター、ポリカーボネート製Eurostandard Type IIIまたはIVケージ、
− トウモロコシ穂軸のベッディング(参照:LAB COB12,SERLAB,France)、
− 25kGyの照射食餌(Ssniff(登録商標)Soest,Germany)、
− 免疫担当齧歯類用の完全食物−R/M−H Extrudate、
− 水0.2μmで滅菌濾過、
− 環境エンリッチメント(SIZZLE−dri kraft−D20004 SERLAB,France)
2.1.皮下B16−F10メラノーマ保有マウスにおけるPD−1標的化抗体またはドキソルビシンと組み合わせた新規ワクチンの抗腫瘍活性試験
2.1.1.動物におけるB16−F10腫瘍の誘発
PBS緩衝液200μL中、1×106個のB16−F10細胞のC57BL/6雌動物95匹の右側腹部への皮下注射により、腫瘍を誘導した。右側腹部への腫瘍細胞注射の日をD0とみなした。
次にD3に95匹中90匹を、それらの体重に従い、各群が動物15匹からなる6群(群1〜6)に無作為化した。
・群1からの動物は、3日目にワクチン組成物用に用いる媒体の1回のIV注射と、10、14、17および21日目に抗PD−1抗体用に用いる媒体の4回のIP投与とを受けた。
・群2からの動物は、3日目に1μgのABX196とともに50μgのCl II−TRP2の1回のIV注射を受けた。
・群3からの動物は、10、14、17および21日目に抗PD−1抗体の4回のIP投与を受けた。
・群4からの動物は、3日目に1μgのABX196とともに50μgのCL II−TRP2の1回のIV注射と、10、14、17および21日目に抗PD−1抗体の4回のIP投与とを受けた。
・群5からの動物は、7日目に12mg/kgのドキソルビシンの1回のIV注射を受けた。
・群6からの動物は、3日目に1μgのABX196とともに50μgのCL II−TRP2の1回のIV注射と、7日目に12mg/kgのドキソルビシンの1回のIV注射とを受けた。
2.2.1.臨床的監視
生存度および挙動を毎日記録した。体重は、腫瘍細胞の注射まで週2回、次に週3回測定した。腫瘍の長さおよび幅をノギスで週3回測定し、腫瘍の体積を式:
− 疼痛、苦痛または窮迫の徴候:痛み姿勢、痛みフェイスマスク(pain face mask)、挙動、
− 正常体重の10%を超える(個別腫瘍を考慮)が、1,500mm3を超えない(右側腹部の腫瘍体積/MFPと左側腹部の腫瘍体積/MFPとの和を考慮)腫瘍、
− 異所運動または栄養に干渉する腫瘍、
− 潰瘍化腫瘍または組織侵食、
− 2連続日での20%の体重減少状態、
− 身体状態不良、るいそう、悪液質、脱水、
− 外部刺激に対する自発的応答不在の遷延、
− 迅速な努力性呼吸、貧血、有意な出血、
− 神経性徴候:旋回、痙攣、麻痺、
− 持続的な体温低下、
− 腹部膨満。
実験を終了したすべての動物、可能であれば、すべての安楽死・瀕死動物または死亡が認められた動物に対して剖検(肉眼検査)を実施した。
2.3.1.麻酔
腫瘍接種および静脈内注射においてイソフルランガス麻酔を用いた。
動物の安楽死は、過剰用量(イソフルラン)によるガス麻酔、その後の頸部脱臼または全採血により実施した。
3.1.健康パラメータ
Vivo Manager(登録商標)ソフトウェア(Biosystemes,Couternon,France)を用いて以下の健康評価基準を決定した。
− 動物の各体重および平均体重を準備した。
− 平均体重変化(MBWC):パーセントでの治療動物の平均体重変化(B日目の体重からA日目の体重を引いてA日目の体重で除したもの)を算出した。MBWCを算出した区間を体重曲線および体重測定日の関数として選択した。
対照動物と比べた治療動物の腫瘍体積に対するワクチン組成物の効果の観点で治療有効性を評価した。Vivo Manager(登録商標)ソフトウェア(Biosystemes,Couternon,France)を用いて、抗腫瘍有効性の以下の評価基準を決定した。
− 個別の腫瘍体積の平均および中央値を準備した。
− 腫瘍を有しないマウスの数を準備した。
マウスの生存をさらに監視し、有効性パラメータとして用いた。生存曲線を描いた。
規定の時点での平均腫瘍体積は、すべての治療において、クラスカル・ウォリス検定を用いるGrapadPrismソフトウェア(バージョン6.07)で分析した。すべての治療間の有意差(P<0.05)に続き、ダンの多重比較検定を用いてペアワイズ比較を行った。GrapadPrismソフトウェア(バージョン6.07)でのログランク(マンテル・コックス)検定を用いて、生存を分析した。すべての治療間の有意差(P<0.05)に続き、ログランク(マンテル・コックス)検定を用いてペアワイズ比較を行った。
4.1.皮下B16−F10メラノーマ保有マウスにおけるPD−1標的化抗体またはドキソルビシンと組み合わせた本発明のワクチン組成物(TRP2−ABX196)の抗腫瘍活性試験
4.1.1.毒性パラメータ
平均体重曲線を決定し、マウス個別体重に対する治療の効果を試験した。
個別の腫瘍体積の平均および中央値曲線を図1および3に示す。
B16−F10腫瘍保有マウスをD3にTRP2−ABX196ワクチン、D10、D14、D17およびD21に抗PD−1抗体または両方の組み合わせで治療した。腫瘍増殖の動力学を図1に示す。
B16−F10腫瘍保有マウスをD3にTRP2−ABX196ワクチン、D7にドキソルビシンまたは両方の組み合わせで治療した。腫瘍増殖の動力学を図3に示す。
本実施例の目的は、TRP2−ABX196ワクチンと抗PD−1抗体または化学療法剤ドキソルビシン(免疫原性細胞死を誘導することで知られる)のいずれかとを組み合わせることであった。
本実施例は、アジュバントとして他のα−ガラクトシルセラミド誘導体を含む他のワクチン組成物と比べた、アジュバントとしてABX196を含む本発明のワクチン組成物を用いて得られる具体的効果を説明する。
実験は、マウスに5×105個のB16−F10細胞を投与した以外、実施例1に記載のように実施した。
・T細胞傷害性集団:CD45+/CD3+/CD8+/TNFα/パーフォリン/グランザイム
・Treg集団:CD45+/CD3+/CD4+/CD8−/FoxP3+
1.試験目的
異所性B16−F10メラノーマモデルにおけるABX196および実施例2に定義のようなα−Gal−Cerの抗腫瘍活性を評価する。異所性B16−F10メラノーマモデルにおけるドキソルビシン治療と組み合わせたABX196およびα−Gal−Cerの抗腫瘍活性を評価する。
実験は、以下を除いて実施例1と同様である。
ドキソルビシンをNaCl0.9%で希釈した。ABX196を250μg/mLの溶液として提供した。α−Gal−Cerを1mg/mLの溶液として提供した。ABX196またはαGalCerの希釈をPBS緩衝液で実施した。7日目に群1で用いる媒体溶液は、ABX196試験アイテムと同じ最終濃度においてリン酸緩衝食塩水(PBS)で希釈したDMSOを含有した。
ABX196およびα−Gal−Cer化合物を10または100ng/マウスの用量で投与した。ドキソルビシンを12mg/kgの用量で投与した。
試験物質は、静脈内経路によりマウスの尾側静脈に注射した(IV、ボーラス)。ドキソルビシンは、マウスの尾側静脈に静脈内注射した(IV、ボーラス)。
3.1.1.動物におけるB16−F10腫瘍の誘導
PBS緩衝液200μL中、5×105個のB16−F10細胞の195匹のC57BL/6雌動物の右側腹部への皮下注射により、腫瘍を誘導した。右側腹部への腫瘍細胞注射の日をD0とみなした。
次に、D7において、195匹中150匹の動物を、それらの腫瘍体積に従い、各群が動物15匹からなる10群(群1〜10)に無作為化した。D7の体積が小さすぎる場合、特にマウスが測定可能な腫瘍を示さない場合、それらの体重に従い、無作為化を実施した。
・群2からの動物は、10日目に10ngのABX196の1回のIV注射を受けた。
・群3からの動物は、10日目に10ngのα−Gal−Cerの1回のIV注射を受けた。
・群4からの動物は、10日目に100ngのABX196の1回のIV注射を受けた。
・群5からの動物は、10日目に100ngのα−Gal−Cerの1回のIV注射を受けた。
・群6からの動物は、7日目に12mg/kgのドキソルビシンの1回のIV注射を受けた。
・群7からの動物は、7日目に12mg/kgのドキソルビシンの1回のIV注射と、10日目に10ngのABX196の1回のIV注射とを受けた。
・群8からの動物は、7日目に12mg/kgのドキソルビシンの1回のIV注射と、10日目に100ngのABX196の1回のIV注射とを受けた。
・群9からの動物は、7日目に12mg/kgのドキソルビシンの1回のIV注射と、10日目に10ngのα−Gal−Cerの1回のIV注射とを受けた。
・群10からの動物は、7日目に12mg/kgのドキソルビシンの1回のIV注射と、10日目に100ngのα−Gal−Cerの1回のIV注射とを受けた。
1.試験目的
パート1:異所性B16−F10メラノーマモデルにおけるワクチンCL II−TRP2/ABX196またはCL II−TRP2/α−Gal−Cerと抗PD−1抗体との組み合わせの抗腫瘍活性を評価する。
パート2:CL II−TRP2/ABX196ワクチンの単独または抗PD−1抗体治療との組み合わせで治療したマウスからのB16−F10腫瘍における免疫浸潤物の特徴付け。
実験のパート1は、以下を除き、実施例1と同様である。
製造業者の推奨に従い、抗PD−1抗体をリン酸緩衝食塩水(PBS)または他の好適な媒体中で調製した。Cl II−TRP2ペプチド(5mg/チューブ)を50mg/mLの濃度でDMSOに再懸濁した。アジュバントABX196を250μg/mLの溶液として準備した。アジュバントα−Gal−Cerを1mg/mLの溶液として準備した。Cl II−TRP2ペプチドおよびABX196を含有するワクチンの最終製剤をリン酸緩衝食塩水(PBS)中で調製した。3日目に群1で用いた媒体溶液は、CL II−TRP2/ABX196ワクチンについて同じ最終濃度においてリン酸緩衝食塩水(PBS)で希釈したDMSOを含有した。
ペプチドCl II−TRP2を100ngのアジュバントABX196またはα−Gal−Cerとともに50μg/マウスの用量で投与した。抗PD−1抗体を10mg/kgの用量で投与した。
試験物質は、静脈内経路によりマウスの尾側静脈に注射した(IV、ボーラス)。抗PD−1抗体は、マウスの腹膜腔に注射した(腹腔内、IP)。
パートI:皮下B16−F10メラノーマ保有マウスにおけるPD−1標的化抗体と組み合わせたワクチンの抗腫瘍活性試験
i.動物におけるB16−F10腫瘍の誘導
PBS緩衝液200μL中、5×105個のB16−F10細胞の156匹のC57BL/6雌動物の右側腹部への皮下注射により、腫瘍を誘導した。右側腹部への腫瘍細胞注射の日をD0とみなした。
次に、D3において、156匹中120匹の動物を、それらの体積に従い、動物15匹からなる6群(群1〜6のACT2)および動物10匹からなる3群(群1〜3のACT3)の9群に無作為化した。
・群1からの動物は、3日目に試験物質媒体の1回のIV注射と、10、14、17および21日目に試験参照媒体(抗体)の4回のIP投与とを受けた。

1.試験目的
試験は、同系インビボメラノーマB16F10腫瘍保有マウスモデルで実施した。B16F10腫瘍細胞を免疫担当C57BL/6マウスに皮下接種した。
1群あたりマウス14〜15匹
インビボ有効性評価において全部でマウス90匹の場合、
− 対照媒体治療群(抗PD−1治療スケジュールに基づくPBS)
− 抗PD−1治療群(1用量での腹腔内投与)
− ABX196治療群(10日目に1用量での静脈内(iv)投与)
− 組み合わせ治療群(抗PD−1抗体の1用量での腹腔内(ip)投与と組み合わせた10日目にABX196の1用量での静脈内(iv)投与)
− ABX196治療群(10日目に1用量での腫瘍内(it)投与)
− 組み合わせ治療群(抗PD−1抗体の1用量での腹腔内(ip)投与と組み合わせた10日目にABX196の1用量での腫瘍内(it)投与)
2.1 動物
マウス(Mus musculus)、C57BL/6株、雌
提供者:Charles River Laboratories−BP0109−F69592 L’Arbresle Cedex。
動物の腫瘍体積および体重は、週3回、測定し、記録した。2000mm3を超える腫瘍体積または動物の初期体重と比べて15%を超える体重減少をエンドポイントとみなした。
手術ステップ(腫瘍細胞の皮下注射)では、麻酔は、腹腔内注射により、ケタミン(0.33mg/ml)およびキシラジン(33.6μg/ml)を用いて行った。
− がん細胞株:
メラノーマB16F10細胞を、提供者の仕様に従い、10%の最終濃度のFBSを添加したRPMI1640でインビトロ培養した。マウスへの移植前、トリパンブルー排除を用いて細胞生存度を評価し、細胞懸濁物を生菌数に従って調製した。
− マウスにおけるメラノーマB16F10腫瘍の誘導:
B16F10細胞をC57BL/6免疫担当マウス(8週齢雌)の右側腹部に皮下移植した。移植手順は以下の通りであった(すべてのステップを無菌層流条件下で実施した):マウス(約20gの体重)を90μLの麻酔剤(すなわち1.5mg/kgのケタミンおよび150μg/Kgのキシラジン)の腹腔内注射で麻酔した。移植される細胞を滅菌PBSに再懸濁し、移植に必要な量を25G針で1mlのシリンジに充填した(1000,000細胞/100μL)。
− 抗PD−1抗体治療
腹腔内注射による100μgでの抗PD−1モノクローナル抗体(a−PD−1)治療スケジュールを適用した。治療は、6、9、12、15および18日目に4回反復した。腹腔内注射用に27Gゲージ針を用いた。
材料:
− 抗PD−L1モノクローナル抗体(a−PD−1)−(クローンRMP1−14)
− 腹腔内投与用の抗PD−1モノクローナル抗体製剤
− リン酸緩衝食塩水(PBS)溶液に溶解した抗PD−1 mAb
ダルベッコPBSを用いて、抗PD−1抗体を1,0mg/mLの濃度(100uLの体積)で溶解した。次に、保存液をアリコートし(1日治療用分量)、−20℃で貯蔵した。
− ABX196治療
10日目にABX196を、
− 100ng/マウスの用量での静脈内(i.v)注射により;または
− 10ng/マウスの用量での腫瘍内(i.t)注射により
投与した。
材料:
i.v投与用のABX196
ABX196は、PBS中、250μg/mLの溶液として準備した。1μg/mLのABX196溶液を調製し、4℃で貯蔵した。10日目、1μg/mLのABX196溶液100μLを3および4群からのマウスの尾静脈に注射した。
i.t投与用のABX196
ABX196は、PBS中、250μg/mLの溶液として準備した。0,4μg/mLのABX196溶液を調製し、4℃で貯蔵した。10日目、5および6群からの動物を麻酔し、0,4μg/mLのABX196溶液25μLを腫瘍に注射した。
すべての実験動物群は、以下のパラメータについて、B16F10細胞接種後6日目から4週間にわたり週に3回監視した。
− 腫瘍サイズ:身体検査により週に3回測定、
− 体重:週に3回監視、および
− 生存:週に3回、カプラン・マイヤープロットで表現
a)腫瘍体積
結果は、腫瘍負荷後、異なる治療に曝露されたB16F10保有マウスの平均腫瘍体積(mm3)を示す図5および6に示す。
結果は、腫瘍負荷後、異なる治療に曝露されたB16F10保有マウスの生存を示す図7および8に示す。
本試験は、メラノーマの同系マウスモデルにおけるPD−1の遮断と組み合わせたABX196の利益を評価することを目的とする。
1.材料および方法
1.1.試験および参照物質
1.1.1.試験物質
ABX196。
抗PD−1抗体(参照:BE0146,BioXcell;クローン:RMP1−14、反応性:マウス;アイソタイプ:ラットIgG2a;貯蔵条件:+4℃)。
製造業者の推奨に従い、抗PD−1抗体をリン酸緩衝食塩水(PBS)または他の好適な媒体中で調製した。ABX196を250μg/mLの溶液として準備した。
ABX196を100ng/マウスの用量で投与した。抗PD−1抗体を10mg/kgの用量で投与した。
試験物質は、静脈内経路によりマウスの尾側静脈に注射した(IV、ボーラス)。抗PD−1抗体は、マウスの腹膜腔に注射した(腹腔内、IP)。すべての群において、ABX196は、100μLの一定用量ボリューム(すなわち体重が20gのマウス1匹に対して約5mL/kg/投与)で投与した。抗PD−1抗体は、10mL/kg/投与の用量ボリュームで投与した。
1.4.1.がん細胞株
用いた細胞株を下表に詳細に示す。
・マウスMBT−2細胞株は、C3H/HeJマウスにおける発がん物質誘導性膀胱腫瘍に由来した。MBT−2細胞株は、Dr Cozzi,Memorial Sloan Kettering Cancer Center(New York,USA)から入手した。
腫瘍細胞は、加湿雰囲気下(5%CO2、95%空気)において37℃で単層として増殖させた。培地は、10%ウシ胎仔血清(参照:3302,Lonza)を添加した、2mMのL−グルタミン(参照:BE12−702F,Lonza,Verviers,Belgium)を含有するRPMI1640であった。腫瘍細胞は、プラスチックフラスコに接着した。実験使用のため、腫瘍細胞を、カルシウムまたはマグネシウムを含まないハンクス培地(参照:BE10−543F,Lonza)中、トリプシン−バーゼン液(参照:BE02−007E,Lonza)で5分間処理することにより培養フラスコから剥離し、完全培地の添加により中和した。
6〜7週齢の健常雌BALB/cマウス63匹を、マウスのこの株(すなわちCT−26)に同系の各モデルについてCHARLES RIVER(L’Arbresles)から入手し、6〜7週齢の健常雌C3H/HeJ(C3H/HeOuJ)マウス68匹をMBT−2モデルについてThe Jackson Laboratory(Bar Harbor,Maine)から入手した。
2.1.動物におけるCT−26およびMBT−2腫瘍の誘導
RPMI1640の200μL中、1×106個のCT−26細胞の63匹の雌BALB/Cマウスの右側腹部への皮下注射により、腫瘍を誘導した。MBT−2腫瘍は、RPMI1640の200μL中、1×106個の細胞の68匹の雌動物の右側腹部への皮下注射により誘導した。
腫瘍が80〜120mm3の平均体積に達した時、治療を開始した。Vivo Manager(登録商標)ソフトウェア(Biosystemes,Couternon,France)を用いて、CT−26モデルにおける63匹またはMBT−2モデルにおける68匹からの48匹の動物を、それら個別の腫瘍体積に従い、各群が動物12匹からなる4群に無作為化した。統計学的試験(分散の分析、ANOVA)を実施し、群間の均一性について試験した。治療スケジュールは以下の通りであった。
− 群1からの動物は、ABX196媒体のIV注射および抗PD−1抗体媒体のIP注射を受けた。
− 群2からの動物は、100ngのABX196のIV注射を受けた。
− 群3からの動物は、抗PD−1抗体の週2回投与を受けた。
− 群4からの動物は、100ngのABX196のIV注射および抗PD−1抗体の週2回投与を受けた。
2.3.1.臨床的監視
動物の体重測定値、腫瘍体積、臨床および死亡率記録ならびに治療を含むすべての試験データをスケジュール化し、Vivo Manager(登録商標)データベース(Biosystemes,Dijon,France)に記録した。
すべての統計学的解析は、Vivo Manager(登録商標)ソフトウェア(Biosystemes,Couternon,France)を用いて実施した。ANOVAを用いて、平均体重、MBWC、無作為化時の平均腫瘍体積、平均腫瘍体積V、Vに達するための平均時間および平均腫瘍倍加時間の統計分析を実施した。有意なANOVA結果の場合、ボンフェローニ/ダン補正を用いてペアワイズ検定を実施した。ログランク(カプラン・マイヤー)検定を用いて生存曲線を比較した。P値<0.05を有意とみなした。
人道的エンドポイントは、実施例1と同様であった。
試験において終了したすべての動物、可能であれば、すべての安楽死・瀕死動物または死亡が認められた動物に対して剖検(肉眼検査)を実施した。
すべての手順:腫瘍接種および静脈内注射において、イソフルランガス麻酔を用いた。
すべての痛みを伴う処置に対して、非薬理学的ケアを準備した。加えて、主治獣医師の推奨により、試験(局所治療)に干渉しない薬理学的ケアを準備することができた。
ガス麻酔の過剰用量(イソフルラン)、その後の頸部脱臼または全採血により、動物の安楽死を実施した。
3.1.皮下CT−26結腸直腸がんを有するマウスにおける試験治療の抗腫瘍活性
結果は、腫瘍負荷後、異なる治療に曝露されたCT−26保有マウスの平均腫瘍体積(mm3)を示す図9に示す。特に、結果によると、抗PD−1抗体と本発明に従うABX196との組み合わせは、ABX196単独または抗PD−1抗体単独よりも腫瘍体積の低減に有効であることが示される。
結果は、腫瘍負荷後、異なる治療に曝露されたMBT−2保有マウスの平均腫瘍体積(mm3)を示す図10に示す。特に、結果によると、抗PD−1抗体と本発明に従うABX196との組み合わせは、ABX196単独または抗PD−1抗体単独よりも腫瘍体積の低減に有効であることが示される。
結果は、腫瘍負荷後、異なる治療に曝露されたMBT−2保有マウスの生存を示す図11、および下表に示す。
試験治療は、単独または組み合わせのいずれかであり、動物で十分に耐容性があり、薬物に関連した重篤な毒性も死亡も記録されなかった。
ABX196+抗PD−1で治療したマウスの腫瘍増殖遅延は、媒体治療群と比べて有意に改善される(図9を参照されたい)。T/CおよびTGI比は、ABX196と抗PD−1抗体との間の相乗効果を明示する。ABX196+抗PD−1治療を有する群のみがT/C<42%を示す。
ABX196+抗PD−1抗体の組み合わせは、腫瘍増殖を有意に低減する(図10を参照されたい)。T/CおよびTGI比は、ABX196と抗PD−1抗体との間の相乗作用を明示する。ABX196+抗PD−1で治療したマウスの生存は、媒体治療群と比べて有意に改善される(図11を参照されたい)。組み合わせ群では、抗腫瘍活性は、ABX196単独または抗PD−1単独と比べてさらに改善され、抗PD−1抗体と組み合わせるとき、27または28日の生存中央値は、34日まで増加する。
1.材料および方法
1.1 試験および参照物質
1.1.1 試験物質
ABX196。
ドキソルビシン:DOXO CELL,Cell Pharm
ソラフェニブ(Nexavar(登録商標),Bayer Pharma,200mg/丸薬)。
抗PD−1抗体(参照:BE0146,BioXcell;クローン:RMP1−14、反応性:マウス;アイソタイプ:ラットIgG2a;貯蔵条件:+4℃)。
ドキソルビシンをNaCl0.9%で希釈した。
ABX196を100ng/マウスの用量で投与した。ドキソルビシンを12mg/kgの用量で投与した。抗PD−1抗体を10mg/kgの用量で投与した。ソラフェニブを100mg/kgの用量で投与した。
ABX196は、静脈内経路によりマウスの尾側静脈に注射した(IV、ボーラス)。ドキソルビシンは、マウスの尾側静脈に静脈内投与した(IV、ボーラス)。抗PD−1抗体は、マウスの腹膜腔に注射した(腹腔内、IP)。ソラフェニブは、カニューレを介した経口経管栄養により投与した(経口、PO)。
1.5.1.がん細胞株
用いた細胞株を下表に詳細に示す。
腫瘍細胞は、加湿雰囲気下(5%CO2、95%空気)において37℃で単層として増殖させた。培地は、10%ウシ胎仔血清(参照:3302,Lonza)を添加した、4mMのLグルタミン(参照:BE12−604F,Lonza,Verviers,Belgium)、4.5g/lのグルコースおよび1mMのNaPyrを含有するDMEMであった。この細胞は、プラスチックフラスコに接着した。
5〜6週齢の健常雌C57BL/6(C57BLl6J)マウス100匹をJANVIER LABS(Le Genest−Saint−Isle)から入手した。
すべての画像化実験は、能動的遮蔽勾配システムを備えた4.7Tの水平磁気(PharmaScan,Bruker Biospin GmbH,Germany)上で実施した。すべてのMR画像は、ParaVision(PV5.1,Bruker Biospin)下で取得した。
マウスは、Pharmascan内部のボリュームコイル(38mmの内直)内をスライドする専用のマウス身体用クレイドル内に腹臥位にした。
すべてを取得する間、マウスは、鼻部分を介し、空気の混合物中のイソフルラン(Minerve,Bondoufle,France)を用いて継続的に麻酔した。マウスの呼吸速度は、その腹部にテープ固定した圧力センサを用いて継続的に監視した。生理学的シグナルは、MRIワークステーションに併置され、光ファイバケーブルによりセンサに接続されたラップトップを介して監視した(SA Instruments,USA)。
較正および位置決め
磁気における動物の位置決め後、スカウト画像を較正目的に取得した。矢状、冠状および軸スライスを取得した。この取得の開始時、自動調節を実施し、シム、RF力およびMRシグナルの増幅を最適化した。
T2w RAREシーケンスを用いて解剖学的画像を取得した。この工程間に用いたシーケンスは、以下の特徴を有する。
すべてのMR画像は、ImageJ下で分析するため、Windows(登録商標)に基づくワークステーションに移した。腫瘍浸潤は、全肝臓における腫瘍の百分率の目視評価により半定量的に評価した。
2.1.脾臓内注射による動物におけるHepa1−6腫瘍の誘導
RPMI1640培地50μL中の100万(1×106)個の腫瘍細胞を、脾臓内注射を通じて100C57BL/6マウスに移植した。すなわち、左肋骨下側腹部を少し切開した。脾臓を露出させた。脾臓を滅菌ガーゼパッド上に曝露し、視覚制御下で細胞懸濁液ともに27ゲージ針を用いて注射した。細胞接種後、脾臓を切除した。腫瘍細胞注射日をD0とみなした。
治療はD5に開始した。Vivo Manager(登録商標)ソフトウェア(Biosystemes,Couternon,France)を用いて、100匹中99匹の動物を、それら個別の体重に従い、動物13匹での3群および動物12匹での5群に無作為化した。統計学的試験(分散の分析、ANOVA)を実施し、群間の均一性について試験した。治療スケジュールは以下の通りであった。
− 群1からの動物は、5日目にABX196媒体の1回のIV注射を受けた。
− 群2からの動物は、5日目から21連続日、100mg/kg/投与のソラフェニブの1日1回のPO投与を受けた。
− 群3からの動物は、5日目に100ngのABX196の1回のIV注射と、および5日目から21連続日、100mg/kg/投与のソラフェニブの1日1回のPO投与とを受けた。
− 群4からの動物は、5日目に12mg/kgのドキソルビシンの1回のIV注射を受けた。
− 群5からの動物は、5日目に100ngのABX196の1回のIV注射と、12mg/kgのドキソルビシンの1回のIV注射とを受けた。
− 群6からの動物は、7、10、14および17日目(週2回×2)に抗PD−1抗体の4回のIP投与を受けた。
− 群7からの動物は、5日目に100ngのABX196の1回のIV注射と、7、10、14および17日目(週2回×2)に抗PD−1抗体の4回のIP投与とを受けた。
D7およびD22において、約120μLの血液を、頸静脈穿刺により、血餅アクチベーターを有する血液捕集管に収集した。室温、1,300gで10分間のサンプリングから30分後、チューブを遠心分離し、血清を得た。血清試料をプロピレンチューブ内、−80℃で貯蔵した。D22において、すべての収集した血清試料を、ELISA分析により、循環するAFPレベルの判定のために分析した(Dosage Mouse a−フェトプロテイン/AFP,参照:MAFP00,RD Systems)。
D19およびD20において群1〜8からのマウス5匹/群を画像化した(動物40匹/日)。次に、半定量分析を実施した。
最終のマウス終了の時点で(D60頃)、肝臓を秤量した。転移の数を肉眼で評価し、限局化、外観(形状、色、一貫性)およびそれら各々の大きさを記録した。肝臓の肉眼写真を撮った。
2.6.1.臨床的監視
動物の体重測定値、腫瘍体積、臨床および死亡率記録ならびに治療を含むすべての試験データをスケジュール化し、Vivo Manager(登録商標)データベース(Biosystemes,Dijon,France)に記録した。
25mmを上回る腹部直径、
疼痛、苦痛または窮迫の徴候:痛み姿勢、痛みフェイスマスク、挙動、
異所運動または栄養に干渉する腫瘍、
3連続日での20%の体重減少状態、
身体状態不良、るいそう、悪液質、脱水、
外部刺激に対する自発的応答不在の遷延、
迅速な努力性呼吸、貧血、有意な出血、
神経性徴候:旋回、痙攣、麻痺、
持続的な体温低下、
腹部膨満。
試験におけるすべての終了した動物、可能であれば、すべての安楽死・瀕死動物または死亡が認められた動物に対して剖検(肉眼検査)を実施した。
手術方法は、IACUCによって認可された手術法に記載された。
すべての処置:手術および採血においてイソフルランガス麻酔を用いた。
マルチモーダルなカプロフェン/ブプレノルフィンまたはキシロカイン/ブプレノルフィン鎮痛プロトコルは、外科手技の厳密さに適応した。すべての痛みを伴う処置に対して、非薬理学的ケアを施した。加えて、主治獣医師の推奨により、試験(局所治療)に干渉しない薬理学的ケアを施すことができた。
動物の安楽死は、過剰用量(イソフルラン)によるガス麻酔、その後の頸部脱臼または全採血により実施した。
4.1.健康パラメータ
Vivo Manager(登録商標)ソフトウェア(Biosystemes,Couternon,France)を用いて以下の健康評価基準を決定した。
− 動物の個別および/または平均(または中央値)体重、
− 平均体重変化(MBWC):パーセントでの治療動物の平均体重変化(B日目の体重からA日目の体重を引いてA日目の体重で除したもの)を算出した。MBWCを算出した区間を体重曲線および体重測定日の関数として選択した。
対照動物と比べた治療動物の腫瘍体積に対する試験物質の効果の観点で治療有効性を評価した。抗腫瘍有効性の以下の評価基準を決定した。
− D19〜20、MRI画像化を用いた肝臓内部の腫瘍浸潤の個別および/または平均(または中央値)測定、
− D22、血漿中の循環するa−フェトプロテインの測定、
− 終了時の肝臓重量測定、
− 生存曲線、
− 生存時間中央値。
腫瘍浸潤
結果は、20日目の各治療群からの肝臓の腫瘍浸潤の百分率を示す図12に示す。
この実験は、化学療法剤または免疫療法剤およびABX196を含む組み合わせが単独に摂取される組み合わせの各成分よりも強力であることを示す。
Claims (14)
- 前記化学療法剤は、ドキソルビシン、シクロホスファミド、エピルビシン、イダルビシン、ミトキサントロンおよびオキサリプラチンからなる群から選択される、請求項1〜4のいずれか一項に記載のがんの治療における使用のための抗腫瘍医薬品。
- 前記化学療法剤は、ドキソルビシンまたはソラフェニブである、請求項1〜4のいずれか一項に記載のがんの治療における使用のための抗腫瘍医薬品。
- 前記免疫療法剤は、Her2/neu、EGFR、VEGF、CD20、CD52、CD33、TACE、カテプシンS、uPA、uPAR、PD−1、グリピカン3、クローディン3、クローディン4、BMCAおよびCTLA4からなる群から選択される腫瘍抗原に特異的な抗体である、請求項1〜4のいずれか一項に記載のがんの治療における使用のための抗腫瘍医薬品。
- 前記免疫療法剤は、モノクローナル抗PD1抗体である、請求項1〜4のいずれか一項に記載のがんの治療における使用のための抗腫瘍医薬品。
- 前記がんは、メラノーマ、肝細胞がん、結腸直腸がんおよび膀胱がんからなる群から選択される、請求項1〜4のいずれか一項に記載のがんの治療における使用のための抗腫瘍医薬品。
- 前記少なくとも1つの化学療法剤は、ドキソルビシン、シクロホスファミド、エピルビシン、イダルビシン、ミトキサントロン、オキサリプラチンおよびソラフェニブからなる群から選択される、請求項5に記載のがんの治療における使用のための組み合わせ製剤。
- 前記少なくとも1つの免疫療法剤は、Her2/neu、EGFR、VEGF、CD20、CD52、CD33、TACE、カテプシンS、uPA、uPAR、PD−1、グリピカン3、クローディン3、クローディン4、BMCAおよびCTLA4からなる群から選択される腫瘍抗原に特異的な抗体である、請求項5に記載のがんの治療における使用のための組み合わせ製剤。
- 前記少なくとも1つの免疫療法剤は、モノクローナル抗PD−1抗体である、請求項5に記載のがんの治療における使用のための組み合わせ製剤。
