本発明の部位特異的HER2 ADCの調製において有用な薬物には、細胞傷害剤、細胞分裂停止剤、免疫調節性剤および化学療法剤が含まれるがこれらに限定されない、がんの処置において有用な任意の治療剤が含まれる。細胞傷害効果とは、標的細胞(即ち、腫瘍細胞)の枯渇、排除および/または死滅を指す。細胞傷害剤とは、細胞に対して細胞傷害効果を有する薬剤を指す。細胞分裂停止効果とは、細胞増殖の阻害を指す。細胞分裂停止剤とは、細胞に対して細胞分裂停止効果を有し、それによって、細胞(即ち、腫瘍細胞)の特定のサブセットの成長および/または拡大増殖を阻害する薬剤を指す。免疫調節性剤とは、サイトカインおよび/もしくは抗体の産生ならびに/またはT細胞機能をモジュレートすることを介して免疫応答を刺激し、それによって、直接的、または別の薬剤をより効果的にすることによって間接的のいずれかで、細胞(即ち、腫瘍細胞)のサブセットの成長を阻害するまたは低減させる薬剤を指す。化学療法剤とは、がんの処置において有用な化学化合物である薬剤を指す。薬物は、薬物誘導体であってもよく、薬物は、本発明の抗体とのコンジュゲーションを可能にするために官能化されている。
本発明の部位特異的HER2 ADCは、薬物をHER2抗体に連結またはコンジュゲートするためのリンカーを使用して調製される。リンカーは、薬物と抗体とを連結して抗体薬物コンジュゲート(ADC)を形成するために使用され得る二官能性化合物である。かかるコンジュゲートは、薬物の腫瘍細胞への選択的送達を可能にする。適切なリンカーには、例えば、切断可能および切断不能リンカーが含まれる。切断可能リンカーは、典型的には、細胞内条件下での切断に対して感受性である。コンジュゲートされた薬物が抗体から切断される主要な機構には、リソソームの酸性pH中での加水分解(ヒドラゾン、アセタールおよびcis−アコニテート様アミド)、リソソーム酵素(カテプシンおよび他のリソソーム酵素)によるペプチド切断、およびジスルフィドの還元が含まれる。切断のためのこれらの機構が様々であることの結果として、薬物を抗体に連結する機構もまた非常に様々であり、任意の適切なリンカーが使用され得る。
本発明の抗体薬物コンジュゲートを調製するための方法もまた提供される。例えば、本明細書に開示される部位特異的HER2 ADCを産生するためのプロセスは、(a)リンカーを薬物に連結するステップ;(b)リンカー−薬物部分を抗体にコンジュゲートするステップ;および(c)抗体薬物コンジュゲートを精製するステップを含み得る。

本発明のADCは、HER2発現がんを処置する際に有用である。一実施形態では、HER2発現がんは、固形腫瘍である。より具体的な実施形態では、HER2発現固形腫瘍には、乳がん(例えば、エストロゲンおよびプロゲステロン受容体陰性乳がん、三重陰性乳がん)、卵巣がん、肺がん(例えば、非小細胞肺がん(腺癌、扁平上皮癌および大細胞癌を含む)および小細胞肺がん)、胃がん、食道がん、結腸直腸がん、尿路上皮がん(例えば、微小乳頭型尿路上皮がん(micropapillary urothelial cancer)および定型尿路上皮がん)、膵がん、唾液腺がん(例えば、粘表皮癌、腺様嚢胞癌および終末導管腺癌(terminal duct adenocarcinoma))および脳腫瘍、または上述のがんの転移(即ち、HER2+乳がんからの肺転移)が含まれるがこれらに限定されない(Martinら、2014、Future Oncol.10(8):1469〜86)。
腫瘍上のHER2発現レベルを評価するための最良の方法に関する態様は議論されており、臨床的意義は概説されている(Sauterら、2009、J Clin Oncol.27:1323〜33;Wolffら、2007、J Clinical Oncology 25:118〜45;Wolffら、2013、J Clinical Oncology 31:3997〜4014)。現在、HER2状態は、免疫組織化学(IHC)、蛍光in situハイブリダイゼーション(FISH)および発色性in situハイブリダイゼーション(CISH)によって評価され得る。
本発明は、本明細書に開示される部位特異的HER2抗体薬物コンジュゲートのいずれかと薬学的に許容できる担体とを含む医薬組成物を提供する。さらに、組成物は、本明細書に開示される部位特異的HER2 ADCのうち1つよりも多くを含み得る。
in vivo適用のために、部位特異的HER2 ADCは、有効投薬量で提供または投与される。語句「有効投薬量」または「有効量」とは、本明細書で使用する場合、直接的または間接的のいずれかで任意の1つまたは複数の有益なまたは所望の治療結果を達成するために必要な薬物、化合物または医薬組成物の量を指す。例えば、がん保有対象に投与する場合、有効投薬量は、がん細胞細胞溶解、がん細胞増殖の阻害、がん細胞アポトーシスの誘導、がん細胞抗原の低減、遅延された腫瘍成長および/または転移の阻害を含む抗がん活性を惹起するのに十分な量を含む。腫瘍の縮小は、有効性についての臨床的な代用マーカーとして十分に受容されている。有効性についての別の十分に受容されたマーカーは、無増悪生存期間である。
本発明の一部の態様では、本明細書に記載される方法は、さらなる形態の治療で対象を処置するステップをさらに含む。一部の態様では、さらなる形態の治療は、化学療法、放射線照射、手術、ホルモン治療、および/またはさらなる免疫療法が含まれるがこれらに限定されないさらなる抗がん治療である。
部位特異的コンジュゲーションのためのトラスツズマブ誘導抗体の調製
A.システインを介したコンジュゲーション
システイン残基を通しての部位特異的コンジュゲーションのためのトラスツズマブ誘導体を調製する方法は、一般的にPCT出願国際公開第2013/093809号(全体が本明細書に組み込まれている)に記載されるように実施した。軽鎖(Kabat番号付けスキームを使用して183位)または重鎖(Kabat番号付けスキームのEUインデックスを使用して290、334、392、および/または443位)のいずれかの1つまたは複数の残基を、部位指定変異誘発によってシステイン(C)残基に変更した。
グルタミン残基を通しての部位特異的コンジュゲーションのためのトラスツズマブ誘導体を調製する方法は、一般的にPCT出願国際公開第2012/059882号全体が本明細書に組み込まれている)に記載されるように実施した。トラスツズマブを、3つの異なる方法で、コンジュゲーションのために使用されるグルタミン残基を発現するように操作した。
トラスツズマブ誘導抗体を発現する安定にトランスフェクトされた細胞の産生
A.システイン変異体
一重および二重システイン操作トラスツズマブ誘導抗体バリアントが、細胞において安定に発現され、大規模に産生され得ることを決定するために、CHO細胞に、9個のトラスツズマブ誘導抗体バリアント(T(kK183C)、T(K290C)、T(K334C)、T(K392C)、T(kK183C+K290C)、T(kK183C+K392C)、T(K290C+K334C)、T(K334C+K392C)およびT(K290C+K392C))をコードするDNAをトランスフェクトして、安定な高い産生プールを、当技術分野で周知の標準的な手順を使用して単離した。コンジュゲーション試験のためにT(kK183C+K334C)を産生するために、HEK−293細胞(ATCC受託番号CRL−1573)に、標準的な方法を使用してこの二重システイン操作抗体バリアントをコードする重鎖および軽鎖DNAを一過性に同時トランスフェクトした。2カラムプロセス、即ち、プロテインA親和性による捕捉の後にTMAEカラム、または3カラムプロセス、即ちプロテインA親和性による捕捉の後にTMAEカラム、次いでCHA−TIカラムを使用して、濃縮CHOプール出発材料からこれらのトラスツズマブバリアントを単離した。これらの精製プロセスを使用すると、全てのシステイン操作トラスツズマブ誘導抗体バリアント調製物は、分析的サイズ排除クロマトグラフィーによって決定した場合に>97%の目的のピーク(POI)を含んだ(表5)。表5に示すこれらの結果は、10個全てのトラスツズマブ誘導システインバリアントに関して、プロテインA樹脂からの溶出後に許容可能なレベルの高分子量(HMW)凝集種が検出されたこと、およびこの望ましくないHMW種を、サイズ排除クロマトグラフィーを使用して除去することができることを証明する。加えて、データは、ヒトIgG1定常領域におけるプロテインA結合部位が、システイン操作残基の存在によって変更されないことを証明した。
トラスツズマブ誘導抗体の完全性
トラスツズマブ野生型抗体と比較して重要な生物物理的特性を評価するため、およびバリアントが標準的な抗体製造プラットフォームプロセスに従うことを確実にするために、システイン操作およびトランスグルタミナーゼバリアントの分子評価を実施した。
安定なCHO発現を介して産生された精製システイン操作抗体バリアント調製物の完全性を決定するために、非還元キャピラリーゲル電気泳動(Caliper LabChip GXII:Perkin Elmer Waltham,MA)を使用してピークのパーセント純度を計算した。結果から、システイン操作抗体バリアントT(kK183C+K290C)およびT(K290C+K334C)が、トラスツズマブ野生型抗体と類似のように、断片および高分子量種(HMMS)の両方を低レベルで含むことが示される。これに対し、T(K334C+K392C)は、評価した他の二重操作システインバリアントと比較して高レベルの断片化抗体ピークを含んだ(表6)。これらの結果は、操作されるシステインの特定の組合せが、部位特異的コンジュゲーションに関して意図される抗体の完全性に影響を及ぼし得ることを示唆している。

トラスツズマブ誘導抗体のバイオコンジュゲーション
本発明のトラスツズマブ誘導抗体を、リンカーを介してペイロードにコンジュゲートし、ADCを生成した。使用したコンジュゲーション法は、部位特異的(即ち、特定のシステイン残基または特定のグルタミン残基を介して)であるか、または従来のコンジュゲーションのいずれかであった。
表8のADCを、以下に記載のシステイン部位特異的方法を介してコンジュゲートした。

トラスツズマブ−メイタンシノイドコンジュゲート(T−DM1)は、トラスツズマブ−エムタンシン(Kadcyla(登録商標))と構造的に類似である。T−DM1は、二機能性リンカーであるスルホスクシンイミジル 4−(N−マレイミドメチル)シクロヘキサン−1−カルボキシレート(スルホ−SMCC)を通してDM1メンタンシノイドに共有結合したトラスツズマブ抗体で構成される。スルホ−SMCCを最初に、50mMリン酸カリウム、2mM EDTA、pH6.8中、10:1の反応化学量論で抗体上の遊離のアミンに25℃で1時間コンジュゲートし、次に非結合リンカーをコンジュゲート抗体から脱塩する。次に、この抗体−MCC中間体を、50mMリン酸カリウム、50mMNaCl、2mM EDTA、pH6.8中、10:1の反応化学量論でMCCリンカー抗体上の遊離のマレイミド末端でDM1スルフィドに25℃で一晩コンジュゲートする。次に、残っている未反応のマレイミドを、L−システインでキャッピングし、Superdex200カラムを通してADCを分画し、非モノマー種を除去する(Chariら、1992,Cancer Res 52:127〜31)。
ADCの精製
ADCを以下に記載のサイズ排除クロマトグラフィー(SEC)を使用して一般的に精製および特徴付けした。意図されるコンジュゲーションの部位への薬物の担持を、以下により詳しく記載するように、質量分析(MS)、逆相HPLC、および疎水性相互作用クロマトグラフィー(HIC)を含む多様な方法を使用して決定した。これら3つの分析方法の組合せは、抗体上のペイロードの担持を確認および定量する多様な方法を提供し、それによってそれぞれのコンジュゲートに関してDARの正確な決定を提供する。
タンパク質凝集体を除去するため、および反応混合物に残っている微量のペイロード−リンカーを除去するために、Akta Explorer FPLCシステムにおいてWaters Superdex200 10/300GLカラムを使用するSECクロマトグラフィーを使用して、ADCを一般的に精製した。場合により、ADCは、SEC精製前に凝集体および低分子を含まず、したがって分取用SECに供しなかった。使用した溶出液は、流速1mL/分のPBSであった。これらの条件下で、凝集材料(室温で約10分で溶出)は、非凝集材料(室温で約15分で溶出)から容易に分離された。疎水性ペイロード−リンカーの組合せによりしばしば、SECピークの「右シフト」が起こった。いかなる特定の理論にも拘束されたくはないが、このSECピークシフトは、リンカー−ペイロードと静止相との疎水性相互作用によるものであり得る。いくつかの例において、この右シフトにより、コンジュゲートタンパク質を非コンジュゲートタンパク質から部分的に分離することができた。
分析的SECを、溶出液としてPBSを使用して、Agilent 1100 HPLCで実施し、ADCの純度およびモノマー状態を評価した。溶出液を、220および280nmでモニターした。カラムがTSKGel G3000SWカラム(7.8×300mm、カタログ番号R874803P)である場合、使用した移動相は、流速0.9mL/分のPBSで30分間であった。カラムがBiosepSEC3000カラム(7.8×300mm)である場合、使用した移動相は、流速1.0mL/分のPBSで25分間であった。
ADCの特徴付け
A.質量分析(MS)
試料およそ20μl(PBS中でおよそ1mg/mL ADC)を20mMジチオスレイトール(DTT)20μlと混合することによって、LCMS分析のための試料を調製した。混合物を室温で5分間静置した後、試料を、Agilent Poroshell300SB−C8(2.1×75mm)カラムを備えたAgilent 110 HPLCシステムに注入した。システムの温度を60℃に設定した。20%〜45%アセトニトリル水溶液(0.1%ギ酸修飾剤を含む)の5分間の勾配を利用した。溶出液を、UV(220nm)およびWaters Micromass ZQ質量分析計(ESIイオン化、コーン電圧:20V、イオン源温度:120℃;脱溶媒和温度:350℃)によってモニターした。多重荷電種を含む粗製スペクトルを、MassLynx4.1ソフトウェアパッケージ内のMaxEnt1を製造元の指示に従って使用して逆畳み込みした。
ADCを作製するための抗体へのペイロードの総担持を、薬物抗体比またはDARと呼ぶ。作製したADCのそれぞれに関して、DARを計算した(表12)。

システイン変異体ADCに関して、抗体への求電子ペイロードのいかなる非特異的担持も、「内部」システイン残基(即ち、典型的に、HC−HCまたはHC−LCジスルフィド架橋の一部である残基)とも呼ばれる「鎖間」で起こると想定される。Fcドメインにおける操作されたシステインへの求電子剤の担持と、内部システイン残基への担持(そうでなければ典型的にHC−HCまたはHC−LC間でS−S結合を形成する)とを区別するために、コンジュゲートを、抗体のFabドメインとFcドメインの間を切断することが知られているプロテアーゼによって処置した。そのような1つのプロテアーゼは、システインプロテアーゼIdeSであり、Genovis社によって「FabRICATOR(登録商標)」として販売され、von Pawel−Rammingenら、2002,EMBO J.21:1607に記載されている。
試料およそ20μl(PBS中でおよそ1mg/mL)を20mMジチオスレイトール(DTT)20μlと混合することによって、試料を逆相HPLC分析のために調製した。混合物を室温で5分間静置した後、試料を、Agilent Poroshell300SB−C8(2.1×75mm)カラムを備えたAgilent 1100 HPLCシステムに注入した。システムの温度を60℃に設定して、溶出液をUV(220nmおよび280nm)によってモニターした。20%〜45%のアセトニトリル水溶液(0.1%TFA修飾剤を含む)による20分間の勾配を利用した:T=0分、25%アセトニトリル;T=2分、25%アセトニトリル;T=19分、45%アセトニトリル;およびT=20分、25%アセトニトリル。これらの条件を使用して、抗体のHCおよびLCをベースラインで分離した。この分析の結果は、LCがほとんど修飾されないままであるが(T(kK183C)およびT(LCQ05)含有抗体を除く)、HCは修飾される(データは示していない)ことを示している。
試料をPBSでおよそ1mg/mLに希釈することによって、化合物をHIC分析のために調製した。試料を、TSK−GELブチルNPRカラム(4.6×3.5mm、孔径2.5μm;Tosoh Biosciences、部品番号14947)を備えたAgilent 1200 HPLCへの15μlの自動注入により分析した。システムは、サーモスタットを備えた自動サンプラー、カラムヒーター、およびUV検出器を含む。
移動相A:1.5M硫酸アンモニウム、50mMリン酸水素二カリウム(pH7);移動相B:20%イソプロピルアルコール、50mMリン酸水素二カリウム(pH7);T=0分、100%A;T=12分、0%A。
示差走査熱量測定(DCS)を使用して、操作されたシステインおよびトランスグルタミナーゼ抗体バリアント、ならびに対応するAur−06380101部位特異的コンジュゲートの熱安定性を決定した。この分析に関して、PBS−CMF pH7.2中で調合した試料を、オートサンプラーを備えたMicroCal VP−Capillary DSC(GE Healthcare Bio−Sciences,Piscataway,NJ)の試料トレイに分配し、10℃で5分間平衡にした後、100℃/時間の速度で110℃まで走査した。16秒間のフィルタリング期間を選択した。生データをベースラインで補正して、タンパク質濃度を標準化した。Originソフトウェア7.0(OriginLab Corporation,Northampton,MA)を使用して適切な転移回数でデータをMN2−Stateモデルに適合させた。
HER2に対するADCの結合
A.直接結合
BT474細胞(HTB−20)をトリプシン処理して、遠心沈降させ、新しい培地に再懸濁した。次に、細胞を、出発濃度1μg/mLのADCまたは非コンジュゲートトラスツズマブのいずれかの連続希釈液と共に4℃で1時間インキュベートした。次に、細胞を氷冷PBSによって2回洗浄し、抗ヒトAlexafluor 488二次抗体(カタログ番号A−11013、Life technologies)と共に30分間インキュベートした。次に、細胞を2回洗浄した後、PBSに再懸濁した。平均蛍光強度を、Accuriフローサイトメーター(BD Biosciences San Jose、CA)を使用して読み取った。
BT474細胞をトリプシン処理して、遠心沈降させ、新しい培地に再懸濁した。次に、細胞を、1μg/mLトラスツズマブ−PE(eBiosciences (San Diego, CA)によりカスタム合成された1:1 PE標識トラスツズマブ)と混合した、ADCまたは非コンジュゲートトラスツズマブのいずれかの連続希釈液と共に4℃で1時間インキュベートした。次に、細胞を2回洗浄した後、PBSに再懸濁した。平均蛍光強度を、Accuriフローサイトメーター(BD Biosciences San Jose、CA)を使用して読み取った。
ヒトFcRnに対するADCの結合
当技術分野において、FcRnはサブタイプによらず、IgGとpH依存的に相互作用し、それが分解されるリソソーム区画に入るのを防止することによって抗体を分解から保護すると考えられている。したがって、反応性システインを野生型IgG1−Fc領域に導入する位置の選択に関して検討したのは、操作されたシステインを含む抗体のFcRn結合特性および半減期が変化するのを回避することであった。
Fcγ受容体に対するADCの結合
ヒトFcγ受容体に対する部位特異的コンジュゲーションを使用したADCの結合を、ペイロードに対するコンジュゲーションが抗体依存性細胞傷害(ADCC)などの抗体関連機能性特性に影響を及ぼし得る結合を変更するか否かを理解するために評価した。FcγIIIa(CD16)はNK細胞およびマクロファージ上で発現し、抗体結合を介してのこの受容体と標的発現細胞との同時会合がADCCを惹起する。BIAcore(登録商標)分析を使用して、トラスツズマブ誘導モノクローナル抗体およびそのそれぞれのADCの、Fcγ受容体IIa(CD32a)、IIb(CD32b)、IIIa(CD16)、およびFcγRI(CD64)に対する結合を調べた。
ADCC活性
ADCCアッセイにおいて、Her2発現細胞株BT474およびSKBR3を標的細胞として使用し、NK−92細胞(Conkwest社によって50歳の白人男性の末梢血単核球細胞から誘導されたインターロイキン−2依存的ナチュラルキラー細胞株)または健康なドナー(#179)から新たに採取した血液から単離したヒト末梢血単核球(PBMC)を、エフェクター細胞として使用した。
「エフェクター自然放出」は、PBMC単独の存在下で測定したシグナルに対応する。
「標的自然放出」は、標的細胞単独の存在下で測定したシグナルに対応する。
「標的最大」は、洗浄剤溶解標的細胞単独の存在下で測定したシグナルに対応する。
In vitro細胞傷害アッセイ
抗体−薬物コンジュゲートを実施例3に記載のように調製した。細胞を96ウェルプレートに低密度で播種し、翌日にADCおよび非コンジュゲートペイロードを10濃度の連続3倍希釈液によって2回の反復実験で処置した。細胞を湿潤37℃/5%CO2インキュベータ内で4日間インキュベートした。プレートをCellTiter(登録商標)96 AQuecus One MTS溶液(Promega,Madison,WI)と共に1.5時間インキュベートすることによって採取し、Victorプレートリーダー(Perkin−Elmer,Waltham,MA)において波長490nmで吸光度を測定した。IC50値を、XLfit(IDBS,Bridgewater,NJ)による4パラメータロジスティックモデルを使用して計算し、図5にnMペイロード濃度で、および図6にng/mL抗体濃度で報告した。IC50値は、括弧内に独立した測定回数と共に±標準偏差で示す。
異種移植片モデル
本発明のトラスツズマブ誘導ADCを、N87胃がん異種移植片モデル、37622肺がん異種移植片モデル、および多数の乳がん異種移植片モデル(即ち、HCC 1954、JIMT−1、MDA−MB−361(DYT2)、および144580(PDX)モデル)で試験した。以下に記載のそれぞれのモデルに関して、最初の用量を1日目に与えた。腫瘍を少なくとも週に1回測定し、その体積を式:腫瘍体積(mm3)=0.5×(腫瘍の幅2)(腫瘍の長さ)によって計算した。最大で8〜10匹、最少で6〜8匹の動物を含む各処置群の平均腫瘍体積(±S.E.M)を計算した。
トラスツズマブ誘導ADCの効果を、免疫欠損マウスにおいて、高レベルのHER2発現を有するN87細胞株(ATCC CRL−5822)から確立したヒト腫瘍異種移植片のin vivo成長に関して調べた。異種移植片を生成するために、雌性ヌード(Nu/Nu,Charles River Lab,Wilmington,MA)マウスに、50%Matrigel(BD Biosciences)中で7.5×106個のN87細胞を皮下移植した。腫瘍の体積が250〜450mm3に達すると、様々な処置群における腫瘍量の均一性を確保するために、腫瘍を病期分類した。N87胃がんモデルに、PBSビヒクル、トラスツズマブADC(0.3、1、および3mg/kg)またはT−DM1(1、3、および10mg/kg)を4日間離して4回(Q4d×4)静脈内投与した(図7)。
HCC1954(ATCC# CRL−2338)は、高HER2発現乳がん細胞株である。異種移植片を生成するために、雌性SHOマウス(Charles River,Wilmington,MA)に、50%Matrigel(BD Biosciences)中で5×106個のHCC1954細胞を皮下移植した。腫瘍の体積が200〜250mm3に達すると、様々な処置群における腫瘍量の均一性を確保するために、腫瘍を病期分類した。HCC1954乳がんモデルに、PBSビヒクル、トラスツズマブ誘導ADC、および陰性対照ADCをQ4d×4で静脈内投与した(図8A〜8E)。
JIMT−1は、中等度/低いHer2を発現し、トラスツズマブに対して本質的に抵抗性である乳がん細胞株である。異種移植片を生成するために、雌性ヌード(Nu/Nu)マウスに、50%Matrigel(BD Biosciences)中で5×106個のJIMT−1細胞(DSMZ#ACC−589)を皮下移植した。腫瘍の体積が200〜250mm3に達すると、様々な処置群における腫瘍量の均一性を確保するために、腫瘍を病期分類した。JIMT−1乳がんモデルに、PBSビヒクル、T−DM1(図9G)、部位特異的コンジュゲーションを使用したトラスツズマブ誘導ADC(図9A〜9E)、従来のコンジュゲーションを使用したトラスツズマブ誘導ADC(図9F)、および陰性対照huNeg−8.8ADCをQ4d×4で静脈内投与した。
MDA−MB−361(DYT2)は、中等度/低Her2発現乳がん細胞株である。異種移植片を生成するために、雌性ヌード(Nu/Nu)マウスに100cGy/分を4分間照射し、3日後に50%Matrigel(BD Biosciences)中で1.0×107個のMDA−MB−361(DYT2)細胞(ATCC# HTB−27)を皮下移植した。腫瘍の体積が300〜400mm3に達すると、様々な処置群における腫瘍量の均一性を確保するために、腫瘍を病期分類した。DYT2乳がんモデルに、PBSビヒクル、部位特異的コンジュゲーションおよび従来のコンジュゲーションを使用したトラスツズマブ誘導ADC、T−DM1、および陰性対照ADCをQ4d×4で静脈内投与した(図10A〜10D)。
トラスツズマブ誘導ADCの効果を、免疫欠損マウスにおいて、適切な同意手順に従って得た新たに切除した144580乳房腫瘍の断片から確立したヒト腫瘍異種移植片のin vivo成長に関して調べた。新しい生検を採取した際の144580の腫瘍特徴付けは、トリプルネガティブ(ER−、PR−、およびHER2−)乳がん腫瘍であった、144580乳がん患者由来異種移植片を、雌性ヌード(Nu/Nu)マウスにおいて動物から動物への断片としてin vivoで皮下に継代した。腫瘍の体積が150〜300mm3に達すると、様々な処置群における腫瘍サイズの均一性を確保するために、腫瘍を病期分類した。144580乳がんモデルに、PBSビヒクル、部位特異的コンジュゲーションを使用したトラスツズマブADC、および従来のコンジュゲーションを使用したトラスツズマブ誘導ADC、および陰性対照ADCを4日毎に4回(Q4d×4)静脈内投与した(図11A〜11E)。
適切な同意手順に従って得た37622の患者由来非小細胞肺がん異種移植片モデルにおいていくつかのADCを試験した。37622患者由来異種移植片を、雌性ヌード(Nu/Nu)マウスにおいて動物から動物への断片としてin vivoで皮下に継代した。腫瘍の体積が150〜300mm3に達すると、様々な処置群における腫瘍サイズの均一性を確保するために、腫瘍を病期分類した。37622PDXモデルに、PBSビヒクル、部位特異的コンジュゲーションを使用したトラスツズマブ誘導ADC、T−DM1、および陰性対照ADCを、4日毎に4回(Q4d×4)静脈内投与した(図12A〜12D)。
トラスツズマブおよび抗HER2 ADCを、適切な同意手順に従って得た患者由来胃がん異種移植片モデル(GA0044)において試験した。GA0044患者由来異種移植片を、雌性ヌード(Nu/Nu)マウスにおいて動物から動物への断片としてin vivoで皮下に継代した。腫瘍の体積が150〜300mm3に達すると、様々な処置群における腫瘍サイズの均一性を確保するために、腫瘍を病期分類した。GA0044PDXモデルに、PBSビヒクル、トラスツズマブ、T−DM1、またはvc0101に対する部位特異的コンジュゲーションを使用したトラスツズマブ誘導ADCを、4日毎に4回(Q4d×4)静脈内投与した(図30)。
T−DM1 ADCの放出された代謝物は、膜不透過性化合物であるリジンキャップmcc−DM1リンカーペイロード(即ち、Lys−mcc−DM1)であることが示されている(Kovtunら、2006,Cancer Res 66:3214〜21;Xieら、2004,J Pharmacol Exp Ther 310:844)。しかし、T−vc0101 ADCから放出された代謝物は、オーリスタチン0101であり、Lys−mcc−DM1より膜透過性が高い化合物である。放出されたADCペイロードが隣接する細胞を死滅させる能力は、バイスタンダー効果として知られている。膜透過性のペイロードの放出により、T−vc0101は、強いバイスタンダー効果を惹起することができるが、T−DM1は惹起することができない。図13は、T−DM1の6mg/kg(図13A)またはT−vc0101の3mg/kg(図13B)のいずれかを1回投与した後、採取し、後に96時間のホルマリン固定で処理したN87細胞株異種移植片腫瘍の免疫組織化学を示す。腫瘍の切片を、両方のADCのペイロードについて提唱される作用機序の読み出しとして、腫瘍細胞に結合したADCを検出するためにヒトIgGに関して染色し、分裂細胞を検出するためにホスホ−ヒストンH3(pHH3)に関して染色した。
In vitroT−DM1抵抗性モデル
A.in vitroでのT−DM1抵抗性細胞の生成
N87細胞を2つの個別のフラスコに継代し、生物学的複製実験を可能にするために、それぞれのフラスコを抵抗性生成プロトコールに関して同一に処置した。細胞を、T−DM1コンジュゲートのおよそIC80濃度(10nMペイロード濃度)に3日間曝露した後、およそ4〜11日間処置を行わずに回復させることを5サイクル行った。T−DM1コンジュゲートの10nMで5サイクル後、細胞を、100nM T−DM1のさらに6回サイクルに同様に曝露した。手順は、診療所で細胞傷害性治療薬に関して典型的に使用される、最大忍容量の後に回復期間を設ける長期的なマルチサイクル(on/off)投与を模倣することを意図した。N87から誘導した親細胞をN87と呼び、T−DM1に長期的に曝露した細胞をN87−TMと呼ぶ。中等度から高レベルの薬物抵抗性が、N87−TM細胞に関して4カ月以内に発生した。抵抗性レベルが持続的な薬物曝露後にもはや増加しなくなるサイクル処置の3〜4カ月後に、薬物の選択圧を除去した。応答および表現型は、その後およそ3〜6カ月間、培養細胞株において安定なままであった。その後、細胞傷害アッセイによって測定した抵抗性表現型の大きさの低減が時に観察され、この場合には、継代初期に凍結保存したT−DM1抵抗性細胞を追加の試験のために融解した。細胞の安定化を確実にするために、T−DM1選択圧の除去後少なくとも2〜8週間の間に、報告された全ての特徴付けを実施した。結果の一貫性を確実に得るためにモデルの作製後およそ1〜2年の間に、1回の選択に由来する融解した様々な凍結保存集団からデータを収集した。胃がん細胞株N87を、それぞれの細胞株に関しておよそIC80(約10nMペイロード濃度)である用量での処置サイクルによって、トラスツズマブ−メイタンシノイド抗体−薬物コンジュゲート(T−DM1)に対する抵抗性に関して選択した。親N87細胞は、コンジュゲートに対して本質的に感受性であった(IC50=1.7nMペイロード濃度、62ng/mL抗体濃度)(図14)。親N87細胞の2つの集団を処置サイクルに曝露して、100nM T−DM1でのおよそ4カ月間の曝露サイクル後、これらの2つの集団(以降、N87−TM−1およびN87−TM−2と呼ぶ)はそれぞれ、親細胞と比較してADCに対して114倍および146倍の不応性となった(図14および図15A)。興味深いことに、対応する非コンジュゲートメイタンシノイド遊離薬に対して最小の交差抵抗性(約2.2〜2.5倍)が観察された(図14)。
ADCを実施例3に記載のように調製した。非コンジュゲートメイタンシンアナログ(DM1)およびオーリスタチンアナログは、Pfizer Worldwide Medicinal Chemistry(Groton,CT)が調製した。他の標準治療化学療法剤は、Sigma(St.Louis,MO)から購入した。細胞を96ウェルプレートに低密度で播種した後、翌日、ADCおよび非コンジュゲートペイロードの10濃度の3倍連続希釈液によって2回の反復実験で処置した。細胞を湿潤37℃/5%CO2インキュベータ内で4日間インキュベートした。CellTiter(登録商標)96AQueous One MTS溶液(Promegam Madison,WI)と共に1.5時間インキュベートすることによってプレートを採取し、Victorプレートリーダー(Perkin−Elmer,Waltham,MA)において波長490nmで吸光度を測定した。IC50値を、XLfit(IDBS,Bridgewater,NJ)による4パラメータロジスティックモデルを使用して計算した。
細胞培養において観察された抵抗性がin vivoで再現されるか否かを決定するために、親N87細胞およびN87−TM−2細胞を拡大増殖させて、The Jackson Laboratory(Bar Harbor,ME)から得た雌性NOD scidガンマ(NSG)免疫欠損マウス(NOD.Cg−Prkdcscid Il2rgtm1Wjl/SzJ)の脇腹に注射した。
マウスにN87またはN87−TM細胞(7.5×106個/注射、50%Matrigel中)のいずれかの浮遊液を右脇腹に皮下注射した。腫瘍が約0.3g(約250mm3)に達すると、マウスを試験群に無作為化した。T−DM1コンジュゲートまたはビヒクルを0日目に食塩水中で静脈内投与し、全体で4用量を4日間離して繰り返した(Q4D×4)。腫瘍を毎週測定して、量を体積=(幅×幅×長さ)/2として計算した。time to event分析(腫瘍の倍加)を実施して、ログランク(Mantel−Cox)検定によって有意性を評価した。これらの試験の全ての処置群のマウスにおいて、体重減少は観察されなかった。
In vivoT−DM1抵抗性モデル
A.In vivoでのT−DM1抵抗性細胞の生成
全ての動物試験は、確立されたガイドラインに従って、Pfizer Pearl River Institutional Animal Care and Use Committeeによって承認された。異種移植片を生成するために、雌性ヌード(Nu/Nu)マウスに、50%Matrigel(BD Biosciences)中で7.5×106個のN87細胞を皮下移植した。平均腫瘍体積が約300mm3に達すると、動物を2つの群に無作為化した:1)ビヒクル対照(n=10)および2)T−DM1処置(n=20)。T−DM1 ADC(6.5mg/kg)またはビヒクル(PBS)を食塩水中で0日目に静脈内投与した後、動物に6.5mg/kgを30週まで毎週投与した。腫瘍を週に2回または週に1回測定して、量を体積=(幅×幅×長さ)/2として計算した。これらの試験において全ての処置群のマウスにおいて、体重減少は観察されなかった。
T−DM1処置から再発し、in vitroで培養した(本実施例の節Aに記載のように)細胞を96ウェルプレートに播種して、翌日にADCまたは非コンジュゲートペイロードの4倍連続希釈液を投与した。細胞を湿潤37℃/5%CO2インキュベータ内で96時間インキュベートした。CellTiter Glo溶液(Promega,Madison,WI)をプレートに添加して、Victorプレートリーダー(Perkin−Elmer,Waltham,MA)において波長490nmで吸光度を測定した。IC50値を、XLfit(IDBS,Bridgewater,NJ)による4パラメータロジスティックモデルを使用して計算した。
T−DM1処置から再発し、in vitroで培養した(本実施例の節Aに記載のように)細胞において、Her2発現を特徴付けした。FACS分析に関して、細胞をトリプシン処理して遠心沈降させ、新しい培地に再懸濁した。次に細胞を、5μg/mLトラスツズマブ−PE(eBiosciences (San Diego, CA)によりカスタム合成された1:1PE標識トラスツズマブ)と共に4℃で1時間インキュベートした。Accuriフローサイトメーター(BD Biosciences San Jose,CA)を使用して平均蛍光強度を読み取った。
細胞株は、ウェスタンブロットによりMDR1を発現せず(図24A)、細胞は、MDR−1基質である遊離の薬物0101に対して抵抗性ではない(図24B)。ドキソルビシンに対する抵抗性は観察されず(図24C)、抵抗性機構がMRP1を通して起こるのではないことが示された。しかし、細胞はなおも遊離のDM1に対して抵抗性である(図24D)。
薬物動態(PK)
従来のまたは部位特異的vc0101抗体薬物コンジュゲートに対する曝露を、5または6mg/kgのいずれかの用量をカニクイザルにIVボーラス投与後に決定した。総抗体(総Ab;コンジュゲートmAbおよび非コンジュゲートmAbの両方の測定)およびADC(少なくとも1つの薬物分子にコンジュゲートされたmAb)濃度を、リガンド結合アッセイ(LBA)を使用して測定した。ADCは、AcLysvc0101を使用したT(LCQ05)を除き、全ての例においてvc0101を使用して作製した。従来のコンジュゲーション(部位特異的コンジュゲーションではない)を使用してトラスツズマブからADCを作製した。
疎水性相互作用クロマトグラフィーによる相対的保持の値とラットにおける曝露(AUC)との比較
疎水性は、疎水性相互作用クロマトグラフィー(HIC)によって評価することができるタンパク質の物理特性であり、タンパク質試料の保持時間は、その相対的疎水性に基づいて異なる。ADCのHIC保持時間をそれぞれの抗体のHIC保持時間によって除算した比率である相対的保持時間(RRT)を計算することによって、ADCをそのそれぞれの抗体と比較することができる。非常に疎水性が高いADCは、より高いRRTを有し、これらのADCはまた、より多くの薬物動態不安定性、特に小さい曲線下面積(AUC、または曝露)を有する可能性がある。様々な部位変異を有するADCのHIC値を、ラットにおける測定されたAUCと比較すると、図26の分布が観察された。
毒性試験
2つの独立した探索的毒性試験において、全体で10匹の雄性および雌性カニクイザルを5つの用量群(1/性別/用量)に分類し、3週間毎に1回(試験日1、22、および43日)IV投与した。試験46日目(3回の用量投与後3日目)に、動物を安楽死させ、プロトコールに明記された血液および組織試料を採取した。臨床所見、臨床病理学、肉眼的および顕微鏡病理評価を、生存時および剖検後に実施した。解剖学的病理評価に関して、組織病理学所見の重症度を主観的、相対的、試験特異的に基づいて記録した。
ADC結晶構造
結晶構造を、T(K290C+K334C)−vc0101、T(K290C+K392C)−vc0101、およびT(K334C+K392C)−vc0101に関して得た。これらの特定のADCは、K290C+K334CおよびK334C+K392C二重システインバリアントとのコンジュゲーションがADCC活性を消失させるが、K290C+K392Cとのコンジュゲーションは活性を消失させなかったことから、結晶学のために選択した。
異なるコンジュゲーション部位により異なるADC特性が起こる
A.システイン変異体ADCの合成のための一般的手順
1つまたは複数の操作されたシステイン残基(表22に示す)を組み入れるトラスツズマブ溶液を、50mMリン酸緩衝液、pH7.4中で調製した。PBS、EDTA(0.5M保存溶液)、およびTCEP(0.5M保存溶液)を、タンパク質の最終濃度が10mg/mL、EDTAの最終濃度が20mM、およびTCEPの最終濃度がおよそ6.6mM(100モル当量)となるように添加した。反応を室温で48時間静置した後、GE PD−10 Sephadex G25カラムを使用して、製造元の指示に従ってPBSに緩衝液交換した。得られた溶液をおよそ50当量のデヒドロアスコルビン酸塩(1:1EtOH/水中の50mM保存溶液)によって処置した。抗体を4℃で一晩静置した後、GE PD−10 Sephadex G25カラムを使用して、製造元の指示に従ってPBSに緩衝液交換した。いくつかの変異体について、上記の手順をわずかに変更したものを使用した。
LCMS:カラム=Waters BEH300−C4、2.1×100mm(P/N=186004496);機器=SQD2質量分析検出器を備えたAcquity UPLC;流速=0.7mL/分;温度=80℃;緩衝液A=水+0.1%ギ酸;緩衝液B=アセトニトリル+0.1%ギ酸。勾配は、3%Bから95%Bを2分間、95%Bで0.75分間保持した後、3%Bで再度平衡にした。試料を注入直前にTCEPまたはDTTによって還元した。溶出液をLCMS(400〜2000ダルトン)によってモニターし、タンパク質ピークを、MaxEnt1を使用して逆畳み込みした。DARを重量平均担持として報告した。
表22のADCを、様々なコンジュゲートの相対的疎水性を決定するために、疎水性相互作用クロマトグラフィー(上記の方法)によって評価した。ADCの疎水性は、総抗体曝露と相関することが報告されている。
ADC試料(約1.5mg/mL)を、マウス、ラット、またはヒト血漿で希釈して、血漿中で50μg/mL ADCの最終溶液を得た。試料を37℃、5%CO2下でインキュベートし、アリコートを3回の時点(0、24時間、および72時間)で採取した。血漿のインキュベーション(25μL)からのそれぞれの時点のADC試料を、IgG0によって37℃で1時間脱グリコシル化した。脱グリコシル化後、捕捉抗体(マウスおよびラット血漿に対して特異的な1mg/mLのビオチン化ヤギ抗ヒトIgG1 Fcγ断片、またはヒト血漿に関して1mg/mLのビオチン化抗トラスツズマブ抗体)を添加して、混合物を37℃で1時間加熱した後、室温でさらに1時間軽く振とうさせた。Dynabead MyOne Streptavidin T1磁性ビーズを試料に添加して、室温で軽く振とうさせながら1時間インキュベートした。次に、試料プレートをPBS 200μL+0.05%Tween−20、PBS 200μL、およびHPLC等級の水で洗浄した。結合したADCを、2体積%ギ酸(FA)55μLによって溶出した。各試料の50μLアリコートを、新しいプレートに移した後、200mM TCEPをさらに5μL添加した。
ADC試料を、グルタチオン水溶液で希釈して、リン酸緩衝液、pH7.4中で最終GSH濃度0.5mMおよび最終タンパク質濃度約0.1mg/mLを得た。次に、試料を37℃でインキュベートし、DAR(T−0、T−3日、T−6日)を決定するために、アリコートを3つの時点で除去した。それぞれの時点のアリコートをTCEPで処理し、実施例20Aに記載の方法に従ってLC−MSによって分析した。
非担癌無胸腺雌性nu/nu(ヌード)マウス(6〜8週齢)を、Charles River Laboratoriesから得た。マウスを使用する手順は全て、確立されたガイドラインに従って施設内動物飼育使用委員会の承認を受けた。マウス(n=3または4)に、抗体構成要素に基づいてADCの3mg/kg用量を1回静脈内投与した。血液試料を各マウスの尾静脈を介して、投与後0.083、6、24、48、96、168、および336時間に採取した。ヒツジ抗ヒトIgG抗体を捕捉のために使用し、ヤギ抗ヒトIgG抗体をTabの検出のために使用し、または抗ペイロード抗体をADCの検出のために使用するLBAによって、総抗体(Tab)およびADC濃度を決定した。各動物の血漿中濃度データを、Watson LIMSバージョン7.4(Thermo)を使用して分析した。曝露は部位に基づいて変化した。290Cおよび443C変異体から作製したADCは、最低の曝露を示したが、183Cおよび392C部位から作製したADCは、最高の曝露を示した。多くの応用に関して、治療剤の持続の増加に至ることから、高い曝露を有する部位が好ましい場合があり得る。しかし、ある特定の応用に関して、より低い曝露を有するコンジュゲート(290Cおよび443Cなど)を使用することが好ましい場合があり得る。特に、より低い曝露(即ち、より低いPK)の応用は、限定されないが脳、CNS、および眼での使用を含み得る。適応は、がん、特に脳、CNS、および/または眼のがんを含む。
カテプシンBを、150mM酢酸ナトリウム、pH5.2中で6mMジチオスレイトール(DTT)を使用して37℃で15分間活性化した。次に活性化カテプシンB 50ngを1mg/mL ADC 20μLと、2mM DTTの最終濃度、50mM酢酸ナトリウム、pH5.2で混合した。反応を、250mMホウ酸緩衝液、pH8.5中での10μM E−64システインプロテアーゼ阻害剤を使用して37℃で20分間、1時間、2時間、および4時間インキュベートすることによってクエンチした。アッセイ後、TCEPを使用して試料を還元し、実施例21Aに記載の条件を使用してLC/MSによって分析した。データは、リンカー切断速度がコンジュゲーションの部位に大きく依存することを示した。443C、388C、および290Cなどの特定の部位は非常に速やかに切断されるが、334C、375C、および392Cなどの他の部位は、非常にゆっくりと切断される。一部の態様において、遅い切断を受ける部位にコンジュゲートすることが有利であり得る。他の態様において、急速な切断が好ましい。例えば、エンドソームで費やされる時間を低減させるためにペイロードを急速に放出することが好ましくなり得る。さらなる態様において、急速なペイロード切断によって、特定の固形腫瘍などのように、コンジュゲートした分子が侵入することができない場所で、ペイロードが有利に侵入することが可能となる。さらなる態様において、急速な切断によって、ペイロードを抗体の抗原を発現しない隣接する細胞に送達することができ、このように例えば不均質な腫瘍の処置を可能にする。
ADCを、10mM EDTAを含むPBS(pH7.4)中で0.2mg/mLに希釈した。ADCを密封バイアルに入れて、45℃に加熱した。経時的に形成される高分子量種(HMWS)および低分子量種(LMWS)のレベルをサイズ排除クロマトグラフィー(SEC)によって評価するために、アリコート(10μL)を1週間毎に除去した。SEC条件を実施例21Aに概要する。これらの条件下で、モノマーは、およそ3.6分で溶出した。モノマーピークの左に溶出するいかなるタンパク質材料もHMWSとして計数され、モノマーピークの右に溶出するいかなるタンパク質材料もLMWSとして計数された。結果を以下の表27に示す。183C、375C、および334Cなどの選択されたADCは、優れた熱安定性を示したが、443Cおよび392C+443Cなどの他のADCは、有意な分解を示した。
抗体−薬物コンジュゲートのin vivo有効性試験を、N87細胞株を使用する標的発現異種移植片モデルにおいて実施した。50%matrigel中でおよそ7.5百万個の腫瘍細胞を6〜8週齢のヌードマウスの皮下に移植し、腫瘍サイズを250〜350mm3に達成させる。薬物を、尾静脈注射によりボーラス投与した。動物に10、3、または1mg/kg抗体薬物コンジュゲートを、4日毎に1回、全体で4回(1、5、9および13日目)注射した。全ての実験動物を体重変化に関して毎週モニターする。腫瘍の体積をキャリパー装置によって最初の50日間、週に2回測定し、その後は週に1回測定し、以下の式:腫瘍体積=(長さ×幅2)/2によって計算する。腫瘍体積が2500mm3に達すると、動物を人道的に屠殺した。腫瘍サイズは、一般的に処置の最初の1週間後に減少することが観察される。処置の中止後(処置後100日まで)、腫瘍の再成長に関して動物を継続的にモニターした。3mpk投与群からのデータを図29に示す。388Cおよび347C変異体から生成したADCは、334C、183C、392C、および443C変異体からのADCよりわずかに低い効力を示した。
Claims (9)
- 式:Ab−(L−D)の抗体薬物コンジュゲートであって、 (a)Abが、HER2に結合し、配列番号18のアミノ酸配列を含む重鎖と配列番号42のアミノ酸配列を含む軽鎖とを含む抗体であり; (b)L−Dがリンカー−薬物部分であり、Lがマレイミドカプロイル−バリン−シトルリン−p−アミノベンジルオキシカルボニル(vc)のリンカーであり、Dが、2−メチルアラニル−N−[(3R,4S,5S)−3−メトキシ−1−{(2S)−2−[(1R,2R)−1−メトキシ−2−メチル−3−オキソ−3−{[(1S)−2−フェニル−1−(1,3−チアゾール−2−イル)エチル]アミノ}プロピル]ピロリジン−1−イル}−5−メチル−1−オキソヘプタン−4−イル]−N−メチル−L−バリンアミドまたはその薬学的に許容できる塩もしくは溶媒和物である、抗体薬物コンジュゲート。
- 請求項1に記載の抗体薬物コンジュゲートと薬学的に許容できる担体とを含む医薬組成物。
- 請求項1に記載の複数の抗体薬物コンジュゲートと、任意選択により薬学的担体とを含む組成物であって、4のDARを有する組成物。
- HER2発現がんを処置するための、請求項2に記載の医薬組成物。
- 前記がんが固形腫瘍である、請求項4に記載の医薬組成物。
- 前記固形腫瘍が、乳がん、卵巣がん、肺がん、胃がん、食道がん、結腸直腸がん、尿路上皮がん、膵がん、唾液腺がんおよび脳腫瘍からなる群から選択される、請求項5に記載の医薬組成物。
- 式:Ab−(L−D)の抗体薬物コンジュゲートであって、 (a)Abが、HER2に結合し、ATCC受託番号PTA−122673で寄託された微生物株により産生される重鎖ポリペプチドとATCC受託番号PTA−122672で寄託された微生物株により産生される軽鎖ポリペプチドとを含む抗体であり; (b)L−Dがリンカー−薬物部分であり、Lがマレイミドカプロイル−バリン−シトルリン−p−アミノベンジルオキシカルボニル(vc)であり、Dが、2−メチルアラニル−N−[(3R,4S,5S)−3−メトキシ−1−{(2S)−2−[(1R,2R)−1−メトキシ−2−メチル−3−オキソ−3−{[(1S)−2−フェニル−1−(1,3−チアゾール−2−イル)エチル]アミノ}プロピル]ピロリジン−1−イル}−5−メチル−1−オキソヘプタン−4−イル]−N−メチル−L−バリンアミドまたはその薬学的に許容できる塩もしくは溶媒和物である、抗体薬物コンジュゲート。
