Description
1.動物
BioLasco Taiwan Co., Ltd.(台湾、台北)から購入した8〜10週齢の成熟オスC57BL/6マウスをレシピエントとして用いた。本研究は台北栄民総医院の実験動物委員会により認可され(2012年1月1日、IACUC 2012−016)、かつ米国科学アカデミー(National Academy of Science,USA)により制定された「実験動物管理及び使用指針」に従って実施された。
単離された低酸素培養のMSCの作製および性質については先行研究で説明されている(Tsai et al., Hypoxia inhibits senescence and maintains mesenchymal stem cell properties through down−regulation of E2A−p21 by HIF−TWIST, Blood, 2011, 117:459−469; Yew et al., Efficient expansion of mesenchymal stem cells from mouse bone marrow under hypoxic conditions, Journal of tissue engineering and regenerative medicine, 2013, 7:984−993)。低酸素培養について、MSCを94%のN2、5%のCO2、1%のO2で構成される気体の混合中で培養した。簡単に説明すると、インフォームドコンセント後に治験審査委員会により承認されたプロトコルに基づいて、正常な成人ドナーの腸骨稜から骨髄穿刺液を採取した。有核細胞を密度勾配(Ficoll−Paque;Pharmacia;ニュージャージー州ピーパック)により単離し、コンプリート培地[CCM:10.0%のウシ胎児血清(FBS)、100単位/mLのペニシリン、100μg/mlのストレプトマイシン、2mMのL−グルタミンを添加したα−MEM(α−最小必須培地;Gibco−BRL、メリーランド州ゲイザースバーグ)]に再懸濁され、週2回CCM培地を交換して増殖させた。
コンカナバリンA(Con A)の投与はマウスにおいて用量依存的な肝損傷を引き起こす(Tiegs et al., 1992, A T cell−dependent experimental liver injury in mice inducible by concanavalin A. J Clin Invest 90, 196−203)。先行研究に基づき、Con A(Sigma Chemical Co.、セントルイス)20mg/kgの用量を200μlのパイロジェンフリーリン酸緩衝生理食塩水(PBS)に溶解させて尾部静脈に静脈注射することにより、急性肝障害を誘発した(Erhardt et al., 2007, IL−10, regulatory T cells, and Kupffer cells mediate tolerance in concanavalin A−induced liver injury in mice. Hepatology 45, 475−485)。Con A投与の4時間後、2つの用量の間葉系幹細胞(継代4〜6)(200μLのPBS中に1×104、1×105、1×106細胞)(MSC群)または200μLのPBS(PBS群)を尾部静脈から投与するためにマウスが無作為に分けられた。しかし、1×106MSCの投与は、細胞移植後数分以内にほとんどのマウスを肺塞栓で死亡させた。薬物投与されていないマウスの一群が正常の対照群とされた(n=6)。Con A投与の6、12、24および48時間後、すべての群のマウス(各群n=6)を殺した。実験の時間枠を図1Aに示す。
上述の実験で取得された血清のアスパラギン酸アミノ基転移酵素(AST)、アラニンアミノ基転移酵素(ALT)を、Rochi/Hitachi Modular Analytics Systems(Roche Diagnostics GmbH、ドイツ、マンハイム)により測定した。
血清中のマウス腫瘍壊死因子α(TNF−α)、インターフェロンγ(INF−γ)、インターロイキン−2、4、5、6、10、17A(IL−2、IL−4、IL−5、IL−6、IL−10、IL−17A)、顆粒球・マクロファージコロニー刺激因子、エオタキシンの量を、マルチプレックスサイトカインアッセイ(BioCat GmbH、ドイツ、ハイデルベルク)を使用して、メーカーのプロトコルに従って測定した。INF−γおよびIL−10の濃度はELISA法(eBioscience、カリフォルニア州サンタクララ)により再測定した。
TUNELアッセイを、インサイチュウ細胞死検出キット(Roche Diagnostics GmbH、マンハイム)を使用して実施した。1スライド当たり少なくとも7つの視野で肝細胞核をカウントした。
RNAzol Bee試薬(Tel−Test、テキサス州フレンズウッド)を使用してトータルRNAを肝臓および肺の試料から単離した。各50mgの試料が1mLのRNAzol Bee試薬で均質化され、200μLのクロロホルムが添加された。15分間ボルテックスにかけた後、均質化された溶液を12000gにて、4℃で10分間遠心分離した。その後、300μLの上清を、−20℃にて700μLのイソプロパノール溶液とともに30分間インキュベートした。続いて、この溶液を再度遠心分離した。上清を除去した後、1mLの75%エタノールを添加した。その後試料を再度遠心分離し、乾燥させた後、RNaseフリー水で処理した。RNAの純度および濃度を分光法で測定した。相補的DNAを、MMLV逆転写酵素第一鎖cDNA合成キット(MMLV reverse transcriptase 1st−strand cDNA Synthesis Kit、EPICENTRE、ウィスコンシン州マディソン)を用いて、1μgのトータルRNAの逆転写により合成した。cDNA溶液を100μLに希釈し、−20℃にてアッセイまで貯蔵した。本研究で用いたプライマーのヌクレオチド配列を表1に示す。量的遺伝子発現を、ABI PRISM 7900HT配列検出システム(Applied Biosystems Inc.、カリフォルニア州フォスターシティ)でSYBR Green技術を使用して実行した。
肝臓の一部をそれぞれ切り取り、氷上でポッター型テフロンガラスホモジナイザーを使用して、RIPA緩衝液(50mM Tris−HCl、pH8.0、150mM塩化ナトリウム、1.0%Nonidet P−40、0.5%デオキシコール酸ナトリウム含有)、ホスファターゼ阻害剤カクテル1(P−2850、Sigma Co.、ミズーリ州セントルイス)、プロテアーゼ阻害剤カクテル(P−8340、Sigma Co.)、ホスファターゼ阻害剤カクテル2(R−5726、Sigma Co.)を含む溶液中で均質化した。細胞を激しい撹拌と超音波処理により可溶化した。均質化溶液中の不溶性物質を、10000gで15分間遠心分離した。その後、上清を12000gで15分間遠心分離した。各試料のタンパク質濃度をBradford法で測定した。精製されたタンパク質をドデシル硫酸ナトリウム−ポリアクリルアミドゲル電気泳動(SDS−PAGE)で分析し、PVDFメンブレン(Millipore、米国マサチューセッツ州)にウェット式エレクトロブロッティングで転写した。メンブレン上の非特異的な部位をTBST中5%無脂肪乳乾燥粉末を用いてブロッキングした。ブロットを表2に示す一次抗とともにインキュベートした。さらにホースラディッシュペルオキシダーゼ標識二次抗体(Jackson immunoresearch laboratories, Inc.、ペンシルベニア州ウェストグローブ)を用いて高感度ケミルミネッセンス法(ECL Western Blotting Analysis System、英国アマーシャム)で検出した。
肝組織を10%パラホルムアルデヒドを用いて室温で24時間固定した後、脱水し、パラフィン包埋されたものから4μm厚の薄切標本を作成し、ヘマトキシリンおよびエオシンで染色した。免疫組織化学染色のため、スライドを表2に示す一次抗体とともに4℃で一晩インキュベートした。一晩インキュベート後、スライドを二次抗体で30分間インキュベートした。その後、スライドをSuper Sensitive Polymer−HOURP IHC検出システム(BioGenex Laboratories Inc.、カリフォルニア州フリーモント)で染色した後、マイヤーヘマトキシリンで対比染色した。免疫蛍光二重染色のため、組織を濯ぎ、2つの非標識の一次抗体(表2)と共に4℃で一晩インキュベートした。2日目に、組織を蛍光染料で標識された二次抗体とともに120分間インキュベートした。蛍光顕微鏡(オリンパス、AX−80)またはレーザー走査型共焦点顕微鏡(オリンパスFV1000)を使用して画像を取得した。
TRCライブラリ用のトランスフェクション品質のDNAを作製するため、インターロイキン1受容体アンタゴニスト(IL1Ra)(DRCN67154)の短ヘアピンRNA(shRNA)をRNAiコンソーシアム(RNAi consortium)shRNAライブラリ(RNAi Core Lab、台湾、中央研究院)より発注入手した。細菌クローンを画線し、単一コロニーが形成された。16時間インキュベート後、プラスミドpLKO.1(Amp+)を選択されたコロニーから抽出し、配列決定に送った。LKO_shRNAのフォワードプライマーは、5’−acaaaatacgtgacgtag−3’(shRNAのフォワード鎖シーケンシング用);LKO_shRNAのリバースプライマーは、5’−ctgttgctattatgtctac−3’(shRNAのリバースシーケンシング用)である。その後、プラスミドがレンチウイルス内で構築された。レンチウイルス感染を行うために、5mLのFBS添加α−MEM中の1×105MSCのアリコートを6cmプレートに播種し、一晩インキュベートした(37℃、5%CO2、1%O2)。2日目、培地を新鮮なポリブレン含有(8μg/ml)培地に交換し、細胞を30分間インキュベートした。その後、9.8μLの構築されたレンチウイルス(最終感染多重度:3)を培地に添加した。24時間培養後、感染した細胞を選択するためにピューロマイシン(3μg/ml)を72時間添加した。RNA干渉効果を確認するため、ノックダウンMSC(sh_IL1Ra MSC)および野生型MSC(ナイーブMSC)の安定クローンから抽出されたタンパク質溶解物をウェスタンブロットにより定量化した。
データはGraphPad Prism 4(GraphPad Software、サンディエゴ)により分析され、平均±S.E.M.で表された。各群の統計的有意性がone−way ANOVAを使用して判定され、事後多重比較がニューマン=コイルス検定またはスチューデントT検定により実行された。パラメトリックテストの基準に違反があった場合、マン・ホイットニーのU検定が実行された。有意性はp値<0.05で判定された。
1.MSCがアポトーシスの減少によりCon A媒介肝障害を軽減した
105または5×105MSCでの細胞療法が、Con A投与の12および24時間後、血清中のアラニンアミノ基転移酵素(ALT)/アスパラギン酸アミノ基転移酵素(AST)濃度を減少させることが分かった(図1B)。MSC処理されたマウスの肝臓における壊死部分の有意な減少が見られた(図1C)。陽性のKi−67染色肝細胞はMSC処理とPBS処理マウスの両方で非常に少なかった(図1D)。しかし、MSC処理はアポトーシスを起こした肝細胞の数を有意に減少することが分かった(図1E)。MSC処理後、これらマウスの肝臓におけるカスパーゼ−3、カスパーゼ−8、カスパーゼ−9の活性型の発現レベルが有意に減少し(図1E)、これはMSC処理が抗アポトーシス効果によりCon A誘発性肝障害を軽減できることを示している。
シグナル伝達兼転写活性化因子1および3(STAT1およびSTAT3)は、肝臓におけるCon A誘発アポトーシス媒介において重要な役割を担っていることが報告されている(Hong et al., 2002, Opposing roles of STAT1 and STAT3 in T cell−mediated hepatitis: regulation by SOCS. J Clin Invest 110, 1503−1513)。Con A障害後、肝臓リン酸化STAT1、インターフェロン調節因子1(IRF1)、切断型カスパーゼ−3の濃度が有意に増加した(図2A)。MSC処理はこれら分子の発現を有意に減少することが分かった。一方で、肝臓リン酸化STAT3はMSC処理後増加することが分かった(図2A)。
Con A誘発性肝障害の減少に血清IL10とIFN−γがどの程度寄与するかを判定するため、リコンビナントマウスIL10の注射または抗IFN−γ抗体中和による治療を含む一連の動物実験が実施された。静脈IL10処理は血清中のALT/AST濃度を有意に減少させた(p=0.011/0.0379)(図3A)。しかし、抗IFN−γ処理では血清中のALT/AST濃度が減少しなかった(p=0.62/0.59)(図3B)。注意すべきは、MSCの治療効果が抗IL10中和抗体での処理によって有意に減少した点である(図3C)。さらに、Con A処理とリコンビナントIL10投与を受けたマウスの肝臓におけるIL10のmRNAとタンパク質発現レベルが測定された(図3Dと図3E)。これらの実験はMSC処理群に見つかった条件を模擬した。図2Dと図2Eでの発見と同じように、IL10のmRNAとタンパク質の肝臓中のレベルは外因性IL10投与後のPBS群のそれよりも高くないことが分かった。これらの結果は、循環性IL10の増加がCon A誘発性肝障害に対するMSCの治療効果において主に寄与しており、Con A誘発性肝障害は循環性IL10のレベルを増加することにより肝臓の内因性IL10を上方制御することなく改善可能であることを示唆している。
Con A肝損傷に対するMSCの治療効果を媒介する根底にある機序を解明するため、注射されたMSCの肝臓および肺への集積(homing)を調査した。標識されたMSCの分布を図4Aに示す。標識されたMSCは正常またはCon A障害マウスの肺に主に滞留した。肝臓ではわずかなMSCが検出されたのみであった。さらに、Con Aによる障害が、肺または肝臓のどちらにおいてもより多くのMSCを集めることはなかった(図4A)。
上述の発見は、MSCのほぼすべてが肺に滞留し、肝臓にはほとんど滞留せず、したがって血清IL10の変化は肺自体に起因する可能性が示唆される。興味深いことに、MSC群における肺のIL10は、PBS群におけるそれよりも有意に高かった(図4Bと図4C)。
IL10の起源を検出するために、さらに肺免疫細胞集団を定義する研究が行われた。図5Aに示すように、測定された免疫細胞のマーカーのいずれもMSC投与後に有意に増加しなかった。しかし、肺のF4/80免疫組織化学染色を実施したとき、Con A処理後、およびMSC群でも、マクロファージ数の増加が示された(図5B)。MSCがM2表現型へのマクロファージスイッチングをもたらすことができることが報告されており、このM2表現型マクロファージは体外および体内で大量のIL10を産生することができる(Nemeth et al., 2009, Bone marrow stromal cells attenuate sepsis via prostaglandin E(2)−dependent reprogramming of host macrophages to increase their interleukin−10 production. Nat Med 15, 42−49; Zhang et al., 2010, Human gingiva−derived mesenchymal stem cells elicit polarization of m2 macrophages and enhance cutaneous wound healing. Stem Cells 28, 1856−1868)。従って、肺における誘導型一酸化窒素合成酵素(iNOS、M1マーカー)とアルギナーゼ1(Arg1、M2マーカー)の発現が測定され、移植されたMSCが肺でマクロファージを再プログラム化し、IL10を産生させたか否かが評価された。二重染色画像はIL10とF4/80の共局在がMSC群より取得された肺において有意に増加したことを示す(図5C)。さらに、MSC投与はiNOSのmRNAおよびタンパク質レベルを有意に減少し、Arg1のmRNAおよびタンパク質レベルを有意に増加した(図5Dと図5E)。総合すると、Con A処理された肺におけるMSC処理がM1マクロファージからM2マクロファージへのスイッチングを引き起こし、それが肺におけるIL10の産生増加に寄与して、血清中濃度の増加につながった可能性があることが分かった。
先行研究によると、インドールアミン2,3−ジオキシナーゼ、TNF−活性化遺伝子6、シクロオキシゲナーゼ2(COX2)、腫瘍増殖因子−β、肝細胞増殖因子、IL1RaはMSCの主な傍分泌因子であり、T細胞およびマクロファージなどの免疫細胞と相互作用することが報告されている(Le Blanc and Mougiakakos, 2012, Multipotent mesenchymal stromal cells and the innate immune system. Nat Rev Immunol 12, 383−396; Ortiz et al., 2007, Interleukin 1 receptor antagonist mediates the antiinflammatory and antifibrotic effect of mesenchymal stem cells during lung injury. Proc Natl Acad Sci U S A 104, 11002−11007)。従って、肺組織におけるこれら因子の遺伝子発現が比較された。IL1Raを除き、PBS群とMSC群間でこれら因子のmRNA発現に有意な変化はなかった(図6A)。注意すべきは、MSC群の肺組織はPBS群のそれよりも高いIL1Ra mRNA発現を示した点である(図6Aと図6B)。
shRNA媒介RNA干渉によりMSCのIL1Ra発現がノックダウンされた(図6C)。ノックダウンMSCがCon A障害マウスに投与された。Con AとナイーブMSCで処理されたマウスと比較したとき、Con AとIL1RaノックダウンMSCで処理されたマウスは、肝臓の壊死とアポトーシスにおいて有意に高い重篤度(図6D)、血清中ALT/AST濃度(図6E)、iNOS mRNA発現(図6F)、および有意に低い血清中および肺のIL10濃度と肺のArg1 mRNA発現を示した(図6Eと図6F)。
Con A処理マウスにおけるIL1Raによる肺マクロファージのスイッチングを再現するため、生体外マウス肺マクロファージ共培養モデルが使用された。フローサイトメトリーの結果では、MSCmがMHS(マウス肺胞マクロファージ細胞株)細胞のM2表現型へのスイッチングをIL1Ra依存的に促進できることが示された(図7)。スクランブルshRNAトランスフェクションMSCは、IL1RaノックダウンMSCよりもMHS細胞からのIL10産生をより刺激した(図7B)。スクランブルshRNAトランスフェクションMSCは、M2表現型を誘導できるメディエーターとして知られるIL4に匹敵する効果を生じた。同様に、IL1RaはクロファージM2スイッチングを刺激し、IL10産生を増加させた(図7Cと図7D)。これらの発見は、IL1RaがCon A処理肺マクロファージのMSC誘導マクロファージM2スイッチングにおいて重要な役割を果たすことを示唆している。総合すると、投与されたMSCがIL1Raを分泌し、宿主の肺マクロファージの再プログラム化を刺激してIL10を産生させ、血清中のIL10増加を生じ、これが肝障害の軽減に寄与する可能性がある(図8)。このほか、肺組織におけるCD4とIFN−γの二重染色は、CD4細胞数がCon A投与後有意に増加し、MSC処理後減少したことを示した(図9)。
Claims (8)
- 自己免疫性肝炎、A型肝炎、B型肝炎およびC型肝炎からなる群より選択される肝疾患治療のための低酸素培養された間葉系幹細胞(MSC)を含む組成物であって、該低酸素培養されたMSCが、酸素10%未満の低酸素条件下で自家または同種MSCを培養することにより取得される、組成物。
- 静脈注射、筋肉注射、腹腔内注射、皮内注射、または皮下注射で投与される、請求項1に記載の組成物。
- 該組成物の投与後に、対象におけるIL−10、インターロイキン1受容体アンタゴニスト(IL1Ra)、およびリン酸化STAT3の濃度が増加する、請求項1または2に記載の組成物。
- 該組成物の投与後に、リン酸化STAT1、インターフェロン調節因子1(IRF1)、切断型カスパーゼ−3、IFN−γの濃度が減少する、請求項1から3のいずれか一項に記載の組成物。
- 該低酸素培養されたMSCが、酸素0%〜7%の範囲の低酸素条件下で自家または同種MSCを培養することにより取得される、請求項1から4のいずれか一項に記載の組成物。
- 該低酸素培養されたMSCが、酸素0%〜7%の範囲の低酸素条件下で同種MSCを培養することにより取得される、請求項5に記載の組成物。
- 該MSCが、骨髄組織、脂肪組織、筋肉組織、角膜基質、乳歯の歯髄、臍帯組織、または臍帯血由来である、請求項1から6のいずれか一項に記載の組成物。
- 該MSCが骨髄MSCである、請求項7に記載の組成物。
