本発明のいくつかの実施形態は、抗体を含む。本明細書で用いているように、「抗体」という用語は、抗体が抗原認識部位を含み、所望の生物学的活性を示す限り、完全ポリクローナル抗体、完全モノクローナル抗体、抗体断片(Fab、Fab’、F(ab’)2およびFv、ならびに単鎖Fv(scFv)断片、単一ドメイン抗体(sdAbまたはナノボディ)など)、抗体の抗原決定部分を含む融合タンパク質、少なくとも2つの完全抗体から作製された二重特異性抗体、キメラ抗体、ヒト化抗体、ヒト抗体および抗原認識部位を含む他の改変免疫グロブリン分子(複数可)を含む。様々な異なる種類の抗体断片ならびにそのような抗体断片を製造し、使用する方法は、当技術分野で公知である。ナノボディレパートリー多重特異性抗体の製造の説明については、例えば、Fridy et al., Nature Methods. 2014 Dec;11(12):1253-60(その内容が参照により本明細書に組み込まれる)を参照のこと。抗体は、それぞれアルファ、デルタ、イプシロン、ガンマおよびミューと呼ばれるそれらの重鎖定常ドメインの同一性に基づく、IgA、IgD、IgE、IgGおよびIgMという5種の主要なクラスの免疫グロブリンまたはそのサブクラス(アイソタイプ)(例えば、IgG1、IgG2、IgG3、IgG4、IgA1およびIgA2)のいずれかでありうる。異なるクラスの免疫グロブリンは、異なる、周知のサブユニット構造および三次元配置を有する。抗体は、裸でありうるかまたは毒素、放射性同位体などの他の分子、もしくは本明細書で列挙した他の特定の分子のいずれかにコンジュゲートさせうる。
がん免疫療法は、患者の腫瘍を認識し、攻撃するように患者自身の免疫細胞を操作することを含み、これは、養子細胞移植(ACT)としばしば呼ばれるアプローチである。そのような方法は、リンパ腫の処置のためのものを含む、現在までの臨床試験において有望な結果をもたらした。ACTにおいて、患者自身の血液から採取したT細胞を遺伝子操作して、キメラ抗原受容体または「CARs」と呼ばれるそれらの表面上の組換え受容体を生成させる。CARsは、患者の腫瘍細胞上の特定の細胞表面抗原を認識し、それに結合するように設計された抗原結合ドメインを含む。操作されたCAR T細胞をインビトロで増殖させ、次いで患者に注入する。注入後、T細胞は、患者の体内で増殖し、該細胞表面抗原を発現する患者におけるがん細胞を認識し、殺滅することができる。リンパ腫に関する数件を含む、進行中の数件のCAR T細胞臨床試験が存在する。数件のリンパ腫試験は、CD19がリンパ腫細胞の表面上に高頻度で発現することから、CD19抗原に結合するように設計されたCAR(すなわち、CD19特異的CARs)を発現するCAR T細胞の使用を含む。CD20特異的、CD22特異的またはCD30特異的CAR T細胞を利用する進行中のリンパ腫試験も存在する。リンパ腫のCD19−CAR T細胞療法を含む、CAR T細胞療法および臨床試験に関するさらなる説明についてはBrentjens, Riviere et al. 2011、Brentjens, Davila et al. 2013、Sadelain 2015、Jackson, Rafiq et al. 2016、Ramos, Heslop et al. 2016を参照のこと。これらの参考文献のそれぞれの内容は、それらの全体が参照により本明細書に組み込まれる。
いくつかの実施形態において、本発明は、リンパ腫細胞への本明細書で述べた活性物質(可溶性HVEMエクトドメインポリペプチドおよび抗HVEMまたは抗BTLA抗体)の標的送達に有用なある特定の非CARベースの組成物および方法を提供する。そのような組成物および方法は、例えば、対象の腫瘍における、リンパ腫細胞上にまたはその近傍に発現する分子に結合しうる適切な「ターゲティング剤」を用いることを含む。いくつかのそのような実施形態において、ターゲティング剤は、抗体または抗体の抗原結合ドメインでありうる。例えば、いくつかの実施形態において、本発明は、(a)可溶性HVEMエクトドメインポリペプチド(または抗HVEMもしくは抗BTLA抗体)および(b)B細胞性リンパ腫細胞(例えば、BTLA+リンパ腫細胞)上の細胞表面抗原に対して特異的である抗体または抗体の抗原結合ドメインの両方を含む組成物を提供する。いくつかのそのような実施形態において、組成物は、融合タンパク質であるかまたは含むものであって、該融合タンパク質は、(a)可溶性HVEMエクトドメインポリペプチド(または抗HVEMもしくは抗BTLA抗体)および(b)B細胞性リンパ腫細胞(例えば、BTLA+リンパ腫細胞)上の細胞表面抗原に対して特異的である抗体または抗体の抗原結合ドメインの両方を含む。しかし、他の実施形態において、当組成物は、所望のリンパ腫細胞に活性物質を特異的に送達するために組成物の抗体成分を用いることができるナノ粒子、リポソーム、ポリマーミセル、リポタンパク質ベースの薬物担体、デンドリマー中、または他の適切なビヒクル中などに、両成分を個別に含みうる。いくつかの実施形態において、細胞表面抗原は、CD19、CD20、CD22、CD30、BTLA、IgkおよびROR1からなる群から選択される。いくつかの実施形態において、ターゲティング剤は、リツキシマブ(CD20特異抗体)またはその抗原結合ドメインでありうる。
本発明の実施形態のいくつかは、処置の方法を含む。本明細書で用いているように、「処置する」、「処置すること」および「処置」という用語は、対象におけるB細胞性リンパ腫の1つまたは複数の臨床指標または症状の検出可能な改善をもたらす処置的手段を意味する。例えば、そのような用語は、少なくとも1つの臨床指標または症状を一時的にまたは永続的に改善すること、緩和すること、減弱させること、回復させること、軽減すること、低下させること、阻害すること、予防すること、もしくは遅くすること、さらなる臨床指標または症状を予防すること、1つもしくは複数の臨床指標または症状の進行を低減または緩徐化すること、1つもしくは複数の臨床指標または症状の後退をもたらすこと等を含む。例えば、本発明によりB細胞性リンパ腫を「処置すること」は、B細胞性リンパ腫(またはB細胞性リンパ腫細胞)の増殖の速度を低下させること、B細胞性リンパ腫(またはB細胞性リンパ腫細胞の)の増殖を停止させること、B細胞性リンパ腫(またはB細胞性リンパ腫細胞の)の退縮をもたらすこと、B細胞性リンパ腫の腫瘍のサイズ(例えば、腫瘍容積または腫瘍質量について測定した)を低下させること、B細胞性リンパ腫の腫瘍の悪性度を低下させること、B細胞性リンパ腫腫瘍(またはB細胞性リンパ腫腫瘍細胞)を除去すること、B細胞性リンパ腫の腫瘍の再発(逆戻り)を予防すること、遅らせることまたは遅くすること、B細胞性リンパ腫に随伴する症状を改善すること、B細胞性リンパ腫患者の限られた生存期間(survival timed)を改善すること、B細胞性リンパ腫の広がり(例えば、転移)を阻害するもしくは低下させること等を含むが、これらに限定されない。同様に、B細胞性リンパ腫を「処置すること」は、患者の腫瘍における、B細胞受容体の活性化を低下させること、IL−21分泌濾胞性ヘルパーT細胞の活性を低下させること、および/またはCD8+T細胞の活性を増大させることを含みうるが、これらに限定されない。
本明細書で同義で用いる、「対象」、「個体」および「患者」という用語は、診断、予後診断または処置が望ましい、あらゆる対象、好ましくは哺乳類対象、最も好ましくはヒト対象を意味するものとする。哺乳類対象は、例えば、ヒト、非ヒト霊長類、イヌ、ネコ、マウス、ラット、モルモットなどを含む、ヒト、家畜、飼育動物、スポーツ動物および動物園の動物を含む。
本明細書で示した様々な異なる「活性物質」は、全身投与または局所投与を含む、任意の適切な経路を経て対象に投与することができる。「全身投与」は、活性物質が例えば、腸、非経口、吸入または経皮経路を経て循環系に入るように活性物質を投与することを意味する。腸投与経路は、消化管を含み、制限なく、経口、舌下、口腔内および直腸送達を含む。非経口投与経路は、消化管以外の経路を含み、制限なく、静脈内、筋肉内、腹腔内、脊髄内および皮下を含む。好ましくは非経口投与を用いる。より好ましくは、静脈内非経口投与を用いる。「局所投与」は、医薬組成物をその作用が望ましい場所(例えば、B細胞性リンパ腫の部位または近く)に、例えば、直接腫瘍内注射により、直接投与することを意味する。用いられる特定の活性物質の性質および処置される特定のB細胞性リンパ腫の性質を考慮に入れて、適切な投与経路を選択することは、当業者の技術の範囲内にある。
本明細書で開示した活性物質、組成物または医薬組成物の「有効量」は、上の「処置」の定義で述べた1つもしくは複数のアウトカムを達成するのに、または達成することに寄与するのに十分な量である。個々の場合における適切な「有効」量は、用量漸増試験などの、当技術分野で公知の標準的技術を用いて決定することができ、活性物質の性質、所望の投与経路、所望の投与頻度、処置される特定のB細胞性リンパ腫、対象、年齢、性別および/または体重等のような因子を考慮に入れて決定することができる。さらに、「有効量」は、用いられる任意の他の処置との関連で決定することができる。例えば、本明細書で述べた活性物質を他の処置法または他の薬剤と併用して投与または使用すべきある状況においては、有効量は、そのようなさらなる処置法が用いられない場合よりも低い可能性がある。
いくつかの実施形態において、本発明は、対象が本明細書で示した組成物または方法のいずれかを用いる処置の候補であるかどうかを判定する方法を提供する。いくつかの実施形態において、そのような方法は、対象のB細胞性リンパ腫またはB細胞性リンパ腫細胞における、HVEMの発現もしくは活性の低下もしくは非存在、またはHVEM突然変異の存在を判定すること、または測定すること、または検出することを含み、それにより、対象のB細胞性リンパ腫またはB細胞性リンパ腫細胞がHVEMの発現もしくは活性の低下もしくは非存在、またはHVEM突然変異の存在を示す場合、対象は、処置の候補でありうる。同様に、他の実施形態において、そのような方法は、対象のB細胞性リンパ腫またはB細胞性リンパ腫細胞におけるBTLAの発現、もしくは高レベルの発現を判定すること、または測定すること、または検出することを含み、それにより、対象のB細胞性リンパ腫またはB細胞性リンパ腫細胞が検出可能なレベルのBTLAを発現する(すなわち、BTLA+である)または高レベルのBTLAを発現する(すなわち、BTLAhiである)場合、対象は、処置の候補でありうる。さらに、他の実施形態において、そのような方法は、これらの2つのアプローチの組合せ、すなわち、対象のB細胞性リンパ腫またはB細胞性リンパ腫細胞における(a)HVEMの発現もしくは活性の低下もしくは非存在、またはHVEM突然変異の存在、および(b)BTLAの発現、もしくは高レベルの発現の両方を判定すること、または測定すること、または検出することを含む。
本発明のいくつかの実施形態は、組成物、例えば、医薬組成物を含む。「組成物」という用語は、本明細書で述べた少なくとも1つの「活性物質」、ならびに希釈剤、緩衝液、生理食塩水(リン酸緩衝生理食塩水など)、細胞培養培地および同類のものなどの、1つまたは複数のさらなる成分を含む組成物を意味する。そのような「組成物」が「医薬組成物」である場合、1つまたは複数のさらなる成分は、希釈剤、緩衝液、生理食塩水(リン酸緩衝生理食塩水など)、担体、安定化剤、分散剤、懸濁化剤および同類のものなどの、生存対象への送達に適する成分でなければならない。
この実施例に提示する結果は、HVEM−BTLA相互作用がB細胞性リンパ腫における腫瘍抑制機能を有することを示しており、また重要なことに、可溶性HVEMエクトドメインタンパク質の投与がインビボでこれらの作用を逆転させ、リンパ腫の増殖を阻止することを示している。
大部分のヒトリンパ腫は、胚中心(GC)B細胞から生じる。これらは、重大な健康上の課題をもたらし続けているびまん性大細胞型B細胞性リンパ腫(DLBCL)および濾胞性リンパ腫(FL)を含む。最近のゲノム研究で、リンパ腫の病因に対する新たな重要な洞察が得られ、再発性ゲノム病変が分類された(Challa-Malladi et al., 2011; Cheung et al., 2010; Lohr et al., 2012; Morin et al., 2011; Okosun et al., 2014; Oricchio et al., 2011; Pasqualucci et al., 2014)。さらに、胚中心(GC)微小環境は、リンパ腫の発生における重要な因子として議論された(Ame-Thomas et al., 2007; Amin et al., 2015; Mourcin et al., 2012; Pangault et al., 2010)。しかし、GC微小環境をGCリンパ腫の発生に関連付ける明確な機序は、不明である。
特に断らない限り、実施例1における「solHVEM」への言及は、配列番号8のLeu39〜Val202可溶性HVEMエクトドメインポリペプチド(配列番号7のヌクレオチド配列によりコードされる)を指す。
ヒトFLsの大規模集合(n=141)において、HVEM突然変異が標本の28%(n=40)に存在し、これらの3分の1(35%)が同型接合突然変異であったことが見出された(図1A〜C)(Cheung et al., 2010;Launay et al., 2012; Lohr et al., 2012; Ross et al., 2007)。HVEM突然変異は、受容体のエクトドメインを標的とし、ミスセンス(65%)、ノンセンス(32.5%)およびフレームシフト突然変異(2.5%)を含む。さらに、HVEMは、B細胞悪性腫瘍にわたって一般的に失われている領域である、染色体1p36欠失の最小共通領域に局在する(Cheung et al., 2010; Fitzgibbon et al., 2007)。2つの別個の一連のアレイCGHデータ(MSKCC:n=64(Oricchio et al., 2011);UNMCコホート:n=198(Bouska et al., 2014))のメタ解析により、HVEM遺伝子座の喪失が緩慢性FL標本(n=262)の34%および形質転換FLs(n=67)の37%において起こることが示されている(図1D〜F、図8AおよびB)。GISTIC(Genomic Identification of Significant Targets in Cancer)解析により、これらの病変の22〜24%が緩慢性および形質転換標本の両方における同型接合性喪失であることが示されている(図1Eおよび図8A)。したがって、ゲノムにおける証拠は、FLの発生時にHVEM受容体遺伝子に対する強力な選択を示すものである。
FLの発生におけるHVEM不活性化の役割を解明するために、ヒトBCL2陽性FLsの遺伝学および病理学の重要な側面を反復するvavPBcl2モデル(Egle et al., 2004; Oricchio et al., 2011)を用いた。手短に述べると、胎児肝臓から単離されたvavPBcl2造血前駆細胞(HPCs)にHVEMに対する短ヘアピンRNAs(shRNAs)を発現するレトロウイルスまたは空ベクター対照を導入した。次いで、これらの細胞を致死照射マウスに移植し、リンパ腫の発生についてレシピエントをモニターした(図2A)。HVEMのノックダウン(赤、n=19)は、対照(青、n=11)と比較してリンパ腫の発生の有意な加速および浸透度の増加をもたらした。対照動物の90%が100日間を超える期間にわたり腫瘍がないままであったのに対して、shHVEMの導入を受けた動物では10%のみであった(p<0.01)(図2B)。これらの結果は、HVEMに対する第2のshRNAについて確認された。HVEM mRNAのノックダウンも確認され、HVEM表面発現の減少がFACSにより認められた(図2CおよびD、図9B)。B細胞コンパートメントにおけるHVEMノックダウンがリンパ腫の発生の原因であったかどうかを試験するために、GFPレポーターと共発現するshHVEMの発現を、派生造血コンパートメント内への最初のHPC感染から追跡した。HPCsの初期の導入効率は、約15%であり、富化は、80%以上がshHVEMおよびGFPを発現した、FACS選別リンパ腫B細胞においてのみ認められた(図2E)。したがって、これらの試験により、HVEMの喪失がインビボでのB細胞の自律拡大およびリンパ腫の発生をもたらすことが示されている。
HVEMおよびBTLAの喪失は、マウスリンパ腫におけるBCRの活性化をもたらす(図2Gおよび3F)。BCRシグナルの活性化は、CD79に結合するBTLAの能力に関連する直接的な効果でありうるか、あるいは局所サイトカインレベルの変化に続発するものでありうる(Vendel et al., 2009)。HVEMがシグナル伝達に対する直接的な、細胞自律的、かつBTLA依存的効果を有するかどうかを直接的に試験するために、単離リンパ腫B細胞を、HVEMの結合特性を保持している精製可溶性HVEMエクトドメインタンパク質断片(solHVEM:Leu39〜Val202)で処理した(Cheung et al., 2005; del Rio et al., 2010)。手短に述べると、BCL1マウスリンパ腫細胞におけるBCRシグナル伝達経路を、solHVEM(10μg/ml)または薬理学的BTK阻害剤イブルチニブ(10nM)の存在下または非存在下でIgMで刺激し、BCR経路の活性化の指標としてのBTKのリン酸化をフローサイトメトリーにより測定した。solHVEMの添加は、薬理学的阻害剤と同様にBTKのリン酸化および活性化を阻止した(図4A)。solHVEMがBCRシグナル伝達を阻止する能力には、BTLAを必要とし、BTLAのノックダウンがBCL1細胞におけるBTKの阻害を妨げた(図4B)。同様な観察を初代ヒトFL B細胞において行った。BTLA発現を、精製ヒトFL B細胞の10個の試料にわたってFACSにより解析し、試料をBTLAhiおよびBTLAlo群に分けた(図4C)。solHVEM(10μg/ml)の存在下または非存在下で抗IgGによりB細胞を刺激したところ、SYKおよびERKの阻害がBTLAhi細胞において認められたのに対して、solHVEMは、BTLAlo細胞においてほとんど効果を有さなかった(図4D、図11)。10個すべての初代ヒトFL B細胞の累積的解析により、SYKリン酸化を阻止するsolHVEMの能力とBTLA表面発現との間の有意な相関関係が確認された(r=0.697、p=0.03)(図4E)。
ヒトFLsにおいて、悪性B細胞は、悪性B細胞の支えとなるリンパ基質と混合している(Mourcin et al., 2012)。これらの非造血性リンパ腫成分は、特定のCD21L陽性濾胞性樹状細胞(FDCs)およびトランスグルタミナーゼ陽性線維芽細胞性網状細胞(FRCs)に含まれる(図12A)。マウスリンパ腫において、我々は、免疫処置の非存在下で腫瘍支質の活性化を認め、これは、HVEM欠損リンパ腫において有意により顕著であった(図5A)。顕微鏡画像の定量的解析により、対照腫瘍と比較してHVEM欠損腫瘍における濾胞内のCD21/CD35陽性FDCネットワークの有意な(p<0.05)増加が示された(それぞれについてn=3)(図5B)。同様に、濾胞周囲部におけるI型コラーゲン密度は、HVEM欠損リンパ腫において有意に(p<0.05)増加し、ERTR7陽性FRCネットワークの細胞拡大の非存在下におけるFRCsの活性化が示唆される(図5B、図12Bおよび12C)。これらの顕微鏡所見と一致して、対照と比較してHVEM欠損腫瘍におけるFDCおよびFRC由来のサイトカインCXCL13およびCCL19の発現の有意な増大が見出された(n=5、p<0.01)(Mueller and Germain, 2009)(図5Dおよび5E)。
支質由来サイトカインCXCL13は、CXCR5陽性濾胞性ヘルパーT細胞(TFH)に対する主な化学誘引物質である(Crotty, 2014)。HVEM欠損リンパ腫におけるCXCL13の産生の増加(図5D)と一致して、TFH細胞の存在量の有意な増加が、対照腫瘍と比較してHVEM欠損腫瘍において認められた(それぞれについてn=3;p<0.01)(図6Aおよび6B)。TFH細胞数のこの増加は、TFH由来のサイトカインの発現の増加を随伴する。具体的には、IL21、IL4、ならびに支質活性化サイトカインTNFa、LTaおよびLTbの発現の増大が、対照リンパ腫と比べてHVEM欠損リンパ腫からのFACS精製CD3+T細胞において認められた(各遺伝子型のn=5、p<0.01)(図6C〜6E)。さらに、TFH細胞によるIL21およびIL4の産生の増加が、HVEM欠損リンパ腫からのFACS精製リンパ腫B細胞上のIL21およびIL4受容体の発現の上昇(p<0.01)(図13Aおよび13B)と一致していたことが認められた。ヒトTFH細胞は、BTLA受容体の高レベルの発現を特徴としており(図13C)、HVEMがこれらの腫瘍浸潤性T細胞に直接影響を及ぼすかどうかを試験するために実験を行った。TFH細胞に対するHVEMの直接的な影響を試験するために、精製ヒトTFH細胞を単離し、抗CD3および抗CD28による刺激の存在下または非存在下で前のようにsolHVEMにより処理した。solHVEMは、TFH細胞の数または生存率に影響を及ぼさず、LTaおよびLTbの発現の低下が認められたが、TNFa、IL21またはCXCL13については認められなかった(図6F〜H、13D〜F)。したがって、HVEM欠損リンパ腫は、支質の活性化に寄与し、IL4およびIL21の産生によってB細胞の増殖を支持するより多数のTFH細胞を動員する。
solHVEMタンパク質がBCR経路の活性化をBTLA依存的に阻害し、リンパ腫B細胞およびTFH細胞における異常なサイトカインの産生を少なくとも一部逆転させることができることが本明細書で示された。これらの所見に基づいて、solHVEMがリンパ腫に対して単剤活性を有するかどうかを試験するために実験を行った。最初に、BTLA受容体の発現をヒトおよびマウスリンパ腫(主にDLBCL)細胞株のパネルにわたって特徴付けた。ヒトFL試料および初代FL B細胞における所見(図1および4)と一致して、細胞株がBTLAhi(DOHH2、SU−DHL6、マウスMYC/BCL2)およびBTLAlo(Granta、Su−DHL10)(図14C)群に分類されることが見出された。SolHVEM(10μg/ml)は、BTLAhiであり、HVEMの同型接合性欠失を有する(示さず)DOHH2細胞におけるBTK、SYKおよびERKの活性化を容易に阻止した(図7A〜7C−図7Aおよび7Bのデータは、SolHVEM Pro37〜Val202、すなわち、配列番号6を用いて得られた)。全パネルにわたって、SolHVEMは、すべてのBTLAhiリンパ腫細胞における有意な増殖阻害をもたらしたのに対して、BTLAlo細胞は、全体的なより高いベースライン増殖速度を示し、SolHVEMよる影響を受けなかった(図7D)。次に、SolHVEMが確立されたBTLAhiリンパ腫に対するインビボでの効果を有するかどうかを試験するために実験を行った。手短に述べると、BTLA(BTLAhi)を発現する攻撃的なMYC/BCL2二重陽性マウスリンパ腫細胞をJ:Nuヌードマウスの側腹部に移植し、移植した腫瘍が約50mm3の容積に達したならば、マウスを20μgのsolHVEMまたはビヒクル(PBS)により3日ごとに処置した。SolHVEMタンパク質による処置により、さらなる腫瘍増殖が予防されたのに対して、ビヒクル処置腫瘍は、急速に拡大した(媒ビヒクル体およびSolHVEMについてn=4;p<0.01)(図7Eおよび7F、図14Dおよび14E)。SolHVEMの効果は、単に細胞増殖抑制性ではなく、タネル染色は、多大なアポトーシスを示し、免疫ブロットは、インビボでのERK阻害を示している(図7Gおよび7H)。したがって、SolHVEMは、リンパ腫に対するインビボでの有意な単剤活性を有する。同様の結果がアミノ酸Pro37〜Val202(配列番号6)の細胞外領域からなる異なる可溶性HVEM分子によりインビトロおよびインビボの両方で得られた。これらの結果は、実施例2で要約する。
リンパ腫におけるHVEM−BTLA腫瘍抑制因子軸の二重機能
GCは、大部分のヒトB細胞性リンパ腫の起源であり、本明細書に提示するデータは、それらの病因についての新たな洞察を提供する。HVEM−BTLA相互作用がGC B細胞性リンパ腫の75%において分断されていることが今や示され、これにより、それがリンパ腫の発生に対する重要なバリアーであることがわかる。HVEM受容体遺伝子は、リンパ腫におけるとりわけ最も高頻度の遺伝的標的であり、体細胞突然変異および染色体欠失は、FLsおよびDLBCLsを含むGCリンパ腫の大部分における完全な不活性化をもたらす。BTLAは、B細胞において発現する唯一のHVEM相互作用性受容体であり、野生型HVEMを保持しているリンパ腫は、BTLA受容体遺伝子の発現をサイレンシングする可能性がある。しかし、BTLAは、突然変異または欠失の標的ではない。それどころかBTLAは、KMT2D(MLL2)ヒストンメチルトランスフェラーゼの標的であり、リンパ腫におけるKMT2Dの不活性化は、BTLAの発現の低下に寄与しうる(Ortega-Molina et al., 2015)。
HVEMは、FLおよびDLBCLにおける最も高頻度で突然変異する遺伝子の1つである。したがって、HVEM欠損リンパ腫に適した処置戦略は、極めて有益であることになる。特に、腫瘍抑制性のHVEMおよびBTLA受容体間の相互作用は、細胞表面上で起こり、したがって、直接的に利用できる。本試験において、可溶性HVEMエクトドメインは、BTLAに結合し、インビボでのBCRシグナル伝達、サイトカイン産生および腫瘍増殖に対する有意な単剤効果をもたらすことができた。solHVEMのこれらの処置効果は、BTLAの発現に依存するものであり、BTLA欠損リンパ腫を処置するためには代替戦略が必要でありうることがわかり、またBTLAの発現がsolHVEM応答の予測因子でありうることが示唆される。本明細書に提示する結果は、GCリンパ腫におけるHVEMの腫瘍抑制機能を少なくとも一部、回復させることを目的とする処置戦略の概念実証を示している。HVEMまたはBTLA活性化抗体に基づく増強されたリガンドおよび腫瘍特異的なHVEMの送達のための改善されたビヒクルもGCリンパ腫における腫瘍抑制機能効果をもたらしうる。
統計的方法
細胞型間またはマウス遺伝子型間の比較のための標本の大きさは、ミード(Mead)の勧告(Festing,2002)に準拠した。標本は、それらのあらかじめ判定した型(すなわち、マウス遺伝子型)に従ってそれらの実験群に割り当てたため、無作為化はなかった。研究者らは、実験群に対して盲検化しなかった。データを図2Bおよび3Aに示す実験においては、リンパ腫を発現したマウスのみを考慮し、リンパ腫を発現しなかったマウスは、検閲し、カプラン・マイヤー曲線にチェックマークで示した。定量的PCRデータは、独立した生物学的反復実験から得られた(n値は、対応する図の説明に示した)。正規分布および等分散性は、大部分のデータにおいて確認された。したがって、すべての標本について正規性および等分散性を推定した。Studentのt検定(両側、対応のない)を用いて、統計的有意性を推定した。マウス実験における生存率は、カプラン・マイヤー曲線を用いて表示し、有意性は、対数順位検定により推定した。ヒトFL組織生検におけるHVEMおよびBTLA発現の間の関連解析については、カイ二乗検定を用いた。
症例は、以前に記載された通りに解析した(Li et al., 2014; Yildiz et al., 2015)。手短に述べると、HVEMの全コーディングエクソンおよび隣接イントロン配列を増幅し、シークエンスするためのプライマーは、プライマー3プログラム(http://primer3.ut.ee/)を用いて設計し、述べた直接シークエンス法を用いて配列情報を得た。突然変異は、非増幅リンパ腫細胞由来DNAおよび選別細胞からの対のCD3細胞由来のDNAを鋳型として用いて体細胞において獲得されたことが確認された。
専用の多重プライマーパネル(Qiagen Gene Read Panel)を用いて、HVEMの全コーディングエクソンを増幅した。PCR産物をプールし、バーコードアダプターを用いて作製したライブラリーをシークエンシングした。シークエンシングは、HiSeq2000シークエンサーを用いて行った。配列変異体のバイオインフォーマティクスノミネーションは、専用アルゴリズムを用いて実施した。FastqファイルをQiagen GeneReadデータポータル(http://ngsdataanalysis.sabiosciences.com)にアップロードして、読取りからのプライマー領域をトリミングし、bowtie26を用いてヒトゲノム(ビルドhg19)に対してアライメントした。アライメントしたbamファイルをQiagenポータルから個別にダウンロードし、デフォルトパラメーターを用いて変異体を呼び出すためにVarScan(2.3.6)に提出した。SnpEff(3.4B)を用いて、変異体に遺伝子名により注釈を付け、アミノ酸配列に対する影響を予測し、dbNSFP(2.1)を用いて、標準ツール(Sift、Polyphen、MutationTaster)に基づく予測される機能上の影響に注釈を付けた。1000ゲノムフェーズ2(1000 Genomes phase 2)に見出された変異体は、除外した。UM Bioinformatics Coreにより開発された専用ツールである、Jacquardを用いて、すべての試料VCFsを試料別の変異体の単一マトリックスに集約した。VAF>15%を有するすべての配列変異体をサンガーシークエンシング法を用いてストックTおよび対N DNAにおいてバリデートした。
新鮮凍結またはOCT包埋組織からのDNAを単離し、以前に記載された通りに処理した(Bouska et al., 2014; Oricchio et al., 2011, Bouska, 2014 #43)。手短に述べると、標識およびハイブリダイゼーションは、Agilent Technologiesにより実施されたプロトコールに従って行った。データは、受託番号GSE40989のもとにGEOで入手可能である。197例の濾胞性リンパ腫患者からなる第2のデータセット(UNMCデータセット)からのコピー数データは、(Bouska et al., 2014)に記載されているGeneChipヒトマッピング250K Nsp SNPアレイ(Affymetrix)を用いて得られた。有意に増幅され、欠失した領域を同定するために、GISTICアルゴリズム(Beroukhim et al., 2010)を実装したGistic 2.0Rパッケージを用いた。GISTICは、Bioconductor製のDNAcopy Rパッケージ(Olshen et al., 2004)を用いて各データセットについて得られたセグメント化コピー数データについて実行した。
レトロウイルス導入HPCsへの養子移植に適合した、vavPBcl2マウスモデルを用いた(Egle et al., 2004およびWendel et al., 2004を参照)。手短に述べると、vavPBcl2トランスジェニック胎児肝臓細胞を胎生期14.5日目(E14.5)にvavPBcl2異型接合動物から単離した。HPCsを特別に適応させた増殖培地中で4日間インビトロで増殖させ、目的のshRNAsの発現を導くMSCVベクターをレトロウイルスにより導入した。HPCsを致死照射野生型レシピエントに移植し、疾患の発症を触診により週1回モニターした。データは、統計的有意性に関する対数順位(Mantel−Cox)検定を用いてカプラン・マイヤーフォーマットで解析した。
免疫組織化学(IHC)は、FLと診断された186例の患者からの199個のホルマリン固定パラフィン包埋(FFPE)組織標本の1.5mm二連コアを含む組織マイクロアレイ(TMA)に適用した(Kridel et al., 2015)。4μm切片を切断し、HVEMに対するマウスモノクローナル抗体(希釈度1:50;クローン2G6−2C7;Abnova、Walnut、CA)およびCD272/BTLAに対するウサギポリクローナル抗体(希釈度1:100;Epitomics、カタログ#S2379;Toronto、ON)を用いてVentana Bench Mark XTプラットフォーム(Ventana、AZ)上でIHCを実施した。スライドは、陽性腫瘍細胞の百分率(10%増分で)および染色強度(0=陰性、1=弱、2=中、3=強)について2人の血液病理学者によって評価された。Nikon Eclipse E600顕微鏡に接続されたNikon DS−Fi1カメラを用いて代表的な画像を取得した。組織学および免疫化学解析のために脾臓を採取した。切片を以前に記載された通りにHE、PNA、BCL6、タネル、Ki67で染色した(Oricchio et al., 2011)。Ki67陽性細胞をMetamorphソフトウエアを用いて定量した。
BD LSR Fortessa(Becton Dickinson、Franklin Lakes、NJ)で機械的解離を実施した後に細胞懸濁液のフローサイトメトリー解析を実施した。各遺伝子型の代表的な腫瘍の腫瘍細胞懸濁液を記載された通りに染色した(Wendel et al., 2004)。染色に用いた以下の抗体をBD Biosciencesから入手した:CD8(クローンRA3−6B2)、CD4(クローン1D3)、FAS(クローンJo2)、TおよびB細胞活性化抗原(GL7)、IgG(A85−1)、IgM(R6−60.2)、CD3(クローン17A2)、CXCR5(クローンRF8B2)、またはebiosciencesから:PD−1(クローンJH3)、CD44(クローンIM7)、CD62L(クローンMEL14)、またはBiolegendから:HVEM(クローンHMHV−1B18)、BTLA(クローン6A6)。
BおよびT細胞は、ビーズ細胞分離を用いてマウスの脾臓から単離した。製造業者の指示に従って、Pan T細胞単離キットまたはB細胞単離キット(Miltenyl Biotec)を用いて全細胞溶解物を分離にかけ、単離した。
マウス脾臓およびヒトリンパ節をOCT(Tissue−Tek OCT Compound)で急速冷凍した。20μm切片を4%PFAで室温で15分間固定した。切片をブロッキング溶液(PBS、10%BSA、10%ロバ血清、0.1%サポニン)とともに1時間インキュベートし、次いで以下の一次抗体とともに加湿チャンバー内で4℃で一夜インキュベートした:マウス脾臓についてCD21/CD35(ラットIgG2b、希釈度1/50、BD Biosciences)およびコラーゲンI(ウサギポリクローナル、希釈度1/100、Abcam);ならびにヒトリンパ節についてCD21L(マウスIgM、希釈度1/100、Dako)、トランスグルタミナーゼ−2(マウスIgG1、希釈度1/50、Abcam)およびCD20(ポリクローナルウサギ、希釈度1/50、Abcam)。洗浄後、スライド標本を対応する二次抗体(Jackson ImmunoResearch)とともにインキュベートし、最後にSytoxBlueを含むMowiol退色防止用封入剤(希釈度1/500、Invitrogen)にマウントし、SP8(Leica Microsystems)で共焦点顕微鏡法により解析した。ImageJソフトウエアを画像解析に用いた。
対象は、治験審査委員会承認およびインフォームドコンセントの手続きのもとに募集した。試料は、濾胞性リンパ腫(FL)を有する患者から得られたリンパ節(LN)および日常的扁桃摘出術を受けていた小児から採取した扁桃腺を含んでいた。組織を断片に切断し、注射器および針を用いてフラッシュした。扁桃腺TFHは、記載された(Mourcin et al., 2012){Pangault, 2010 #2203}ように、FACSAria(Becton Dickinson)を用いて98%を超える純度を有するCD3陽性CD4陽性CXCR5hiICOShiCD25陰性細胞として選別した。初代FL B細胞は、B細胞単離キットII(Miltenyi Biotech)を用いて精製した。染色に用いた抗体は、Miltenyi CD3(クローンBW264/56)、Beckman Coulter CD4(クローン13B8.2)、eBiosciences(クローンJ105)およびBD Biosciences CD25(クローンM−A251)、CXCR5(RF8B2)およびBTLA(クローンJ168−540)であった。
精製TFHを、solHVEM(10μg/mL)の存在下または非存在下で抗CD3(0.6μg/mL)および抗CD28(0.6μg/mL、Pelicluster Sanquin)MAbsを含むかまたは含まないIMDM 10%FCS中で培養した。3日間の培養後、計数ビーズ(Flow Count、Beckman Coulter)およびTopro−3染色(Invitrogen)を用いてフローサイトメトリーにより生存TFHの数を評価した。CXCL13は、製造業者の指示に従ってELISA(R&D Systems)により培養上清において定量した。
solHVEM(10μg/mL)を含むかまたは含まないH2O2(1mM)の存在下でFITCコンジュゲートヤギ抗ヒトIgG(Invitrogen、10mg/mL)を用いて精製IgG陽性FL B細胞を刺激した。4%の最終濃度のPFAを室温で15分間加えることによって反応を停止させた。洗浄し、PBS−1%BSAで再脱水する前に固定細胞を80%メタノールで−20℃で20分間暗中透過処理した。Alexa 647コンジュゲート抗pSyk(クローン17A/p−ZAP70)、抗pBLNK(クローンj117−1278)または抗pERK1/2(クローン20A、BD Biosciences)を用いてリンタンパク質活性化を定量し、抗IgG FITC AbおよびPEコンジュゲート抗カッパAb(Southern Biotech)を用いてコンジュゲートされたクローン重鎖および軽鎖を発現するB細胞について解析した。
ホスホ−BTK、ホスホ−Syk染色のために、5μg/mLのsHVEM(R&D Systems)または10ng/mLのイブルチニブ(ChemieTek PCI−32765)で細胞を37℃で60分間前処理した。等容積のホルムアルデヒドを細胞に直接加えることによって細胞を固定した。細胞を室温で10分間インキュベートし、PBSで2回洗浄し、残存細胞を1mLの氷冷メタノール(100%)で氷上で30分間透過処理した。次いで細胞を2回洗浄し、ホスホ−BTK(Bd Biosciences クローンN35−88)およびホスホ−Syk(Bd Biosciences クローン17 A/P−Zap70)で染色し、BD LSRFortessaで解析した。
潜在的に腫瘍性リンパ組織から、また対照としての正常マウス脾臓からRNAを調製した。μ重鎖転写物からの発現VDJ領域を次世代法により配列決定した。この戦略は、5’RACE PCR、パイロシークエンシングならびにIMGT(the International ImMunoGeneTics information website(www.imgt.org))で利用可能なIMGT/High−V−QuestmRNAおよび関連ツールを用いる最も高頻度のクローン型の定量による正確なレパートリー解析を組み合わせたものであった。RACE−PCRは、μCH1エクソン内の逆プライマーハイブリダイゼーションから開始した。
リンパ腫細胞株DoHH2、Ly−10、Granta、Su−DHL−6を10%ウシ胎児血清、1%L−グルタミンおよび1%ペニシリン/ストレプトマイシンを含むRPMI 1640中で維持した。マウスリンパ腫細胞株myc−bcl2を10%ウシ胎児血清、1%L−グルタミンおよび1%ペニシリン/ストレプトマイシンを含むIMDM−DMEM(50:50)中で維持した。細胞株を5×105/mLで播種し、5μg/mlのsHVEMで処理した。24時間後に細胞数を処理後合計72時間にわたり血球計を用いて計数した。
移植および処置試験は、以前に記載された(Schatz et al., 2011)ように創出した。要約すると、Matrigel(BD)と混合した100万個のmyc−bcl2マウスリンパ腫細胞の皮下注射をJ:Nu Nude(Foxn1 nu/Foxn1 nu)マウスの右および左側腹部に行った。腫瘍が75mm3に達したならば、PBSで希釈した20μgのsHVEM(右側腹部)またはビヒクル対照(左側腹部)の腫瘍内注射によりマウスを3日ごとに処置した。3日ごとに腫瘍サイズを測定し、記録した。動物を屠殺し、腫瘍を切除した後に腫瘍を秤量した。
免疫ブロットは、以前に記載された通りに全細胞溶解物または上清を用いて実施した(Wendel et al., 2004)。手短に述べると、30μgのタンパク質/試料をSDS−PAGEゲル上で分離し、Immobilon−P膜(Millipore)に転移させた。抗体は、pSyk(Cell Signaling Technologies #2712)、Syk(Cell Signaling Technologies #2710)、pBTK(Cell Signaling Technologies # 5082)、BTK(Cell Signaling Technologies # 3533)、pERK(Cell Signaling Technologies #9102)、ERK(Cell Signaling Technologies #4370)およびチューブリン(Sigma−Aldrich)に対する抗体であった。増強化学発光を検出のために用いた(ECL;GE Healthcare)。
上記の実施例1で述べたいくつかの実験は、(全長HVEMアミノ酸配列(配列番号2)のアミノ酸L39〜V202からなる、配列番号8に示す配列を有する)L39〜V202可溶性HVEMポリペプチドの使用を含む。他の可溶性HVEMタンパク質配列を用いることによっても同等の結果が得られた。この実施例に示す結果は、Pro37〜Val202可溶性HVEMポリペプチド(配列番号5のヌクレオチド配列によりコードされ、配列番号2の全長HVEMアミノ酸配列のアミノ酸Pro37〜Val202からなる、配列番号6に示すアミノ酸配列を有する)を用いて得られた。特に断らない限り、実施例2または図15〜24における「solHVEM」への言及は、配列番号6のPro37〜Val202可溶性HVEMエクトドメインポリペプチド(配列番号5のヌクレオチド配列によりコードされる)を指す。
CD19+B細胞性悪性腫瘍が、操作されたキメラ抗原受容体(CAR)T細胞の再導入を含む免疫調節療法に対して感受性であることが最近明らかになった(Brentjens, Riviere et al. 2011, Kalos, Levine et al. 2011, Kochenderfer, Dudley et al. 2012, Brentjens, Davila et al. 2013)。これらのT細胞は、非主要組織適合性腫瘍認識および根絶の能力がある腫瘍標的T細胞の発生を可能にするCARを発現する。さらに、これらのT細胞は、CD19+腫瘍を有するマウスの生存を向上させるIL12などのさらなる因子を分泌するように操作することができる(Pegram, Purdon et al. 2015)。本明細書で述べたように、このスキームは、可溶性TNFRSF14/HVEMポリペプチドを用いてFLなどのCD19+B細胞性悪性腫瘍の処置を可能にするように今回修正した。このアプローチの概略図を図25に示す。
可溶性HVEMエクトドメインポリペプチドは、B細胞性リンパ腫を特異的に標的とする薬剤のような、適切な腫瘍標的薬剤に連結させることができる。例えば、本実施例において、可溶性HVEMエクトドメインポリペプチドを抗CD20抗体リツキシマブに共有結合させ、次いでB細胞性リンパ腫を有する対象に投与する。同様な標的アプローチがFLにおける他の細胞外腫瘍抑制因子を用いて功を奏することが既に示された(Oricchio, Nanjangud et al. 2011, Oricchio and Wendel 2012)。特に、この種のアプローチは、現行の療法と比べてオフターゲット効果の低減を含む長所を有し、非常に低い用量の可溶性HVEMエクトドメインポリペプチドの使用の可能性を有する。

- 前記CARが、濾胞性リンパ腫細胞上の細胞表面抗原に結合する、請求項1に記載の核酸分子。
- 前記CARが、DLBCLリンパ腫細胞上の細胞表面抗原に結合する、請求項1に記載の核酸分子。
- 前記細胞表面抗原がCD19、CD20、CD22、CD30、IgkおよびROR1からなる群から選択される、請求項2に記載の核酸分子。
- 前記細胞表面抗原がCD19である、請求項2に記載の核酸分子。
- 前記可溶性HVEMエクトドメインポリペプチドが、BTLA結合、BTLA活性化、BTLA+B細胞性リンパ腫細胞の増殖の阻害、BTLA+B細胞性リンパ腫の増殖の阻害、CD8+T細胞の活性の刺激、B細胞性リンパ腫細胞におけるB細胞受容体の活性化の阻害、濾胞性ヘルパーT(TFH)細胞によるIL−21の分泌の阻害、B細胞性リンパ腫細胞によるIL−21の分泌の阻害、BCR経路活性化の阻害、ならびにBTLA+B細胞性リンパ腫細胞におけるBTK、SYKおよび/またはERK活性化の阻害からなる群から選択される1つまたは複数の活性を有する、請求項1から6のいずれか一項に記載の核酸分子。
