本開示において、心臓刺激を使用して、心房内圧および拡張を増大させることができ、それにより血圧(Blood Pressure:BP)を下げてもよい。心臓刺激によって、心房の心房収縮により生じる心房内圧が、心房の受動内圧上昇と時間が重なり合うことにより、刺激により生じる心房の心房内圧が、心房収縮により生じる心房内圧と受動内圧上昇との組み合わせによって、刺激がない場合の心房の心房内圧よりも高くなるように心臓を刺激することによって、心房内圧の増加を達成してもよい。

a. 本願発明者は、刺激が維持されている間、血圧は、ある時間ののち血圧が増加する適応パターン(そのいくらかはしばしば、5分未満の、または1分未満でさえある短い時間のうちに起きる)を示すことがあり、(場合によっては少なくとも圧反射ゆえ)刺激前の血圧値近く、または、より高くにさえ達することを発見した。適応は、少なくとも一部が、総末梢抵抗(total peripheral resistance)の増加などの、心臓血管系の特性の変化
に起因していてもよい。本願発明者はさらに、刺激の終了により、刺激前の値に、または、より高い値にさえ、血圧が速やかに戻る結果になること、およびその後、心臓が、それほど刺激されなかった心臓に類似した程度で、血圧低減刺激信号に反応するようになることを発見した。くわえて、複数のBPR刺激設定を備える異なる刺激パターンが、異なる血圧適応パターンにつながることが発見された。
式中、(mmHgでの)Pは収縮期血圧を表しており、Pi(mmHg)は、BPR刺激の開始時の第1の平均の低減された血圧であり、DP(mmHg)は、新たな定常状態水準への最初の下落後の、圧力の増加の量を表す定数であり、k(秒)は応答時間定数であり、eは、自然対数の底(base)である数学定数であり、t(秒)は時間である。
式中、Piは115mmHgであることが分かった。DPは23mmHgであった。Kは15.5秒であった。

ご覧のように、Piは、かつDPも、第1の刺激パターンの対応する値(図3Aのa-a’)と比較可能であった。しかし、第2のパターンのkは、第1の刺激パターンの時定数の2倍近くであった。この時間間隔において、適応は図3Aにおいてよりも、より遅い比率で起きたが、パターンが刺激パルスの間で切り替わったときに、血圧は、図3Aにおいてよりも、急上昇した。この結果は、交替する刺激設定を有する刺激パターンの使用が適応を低減することを証明している。
式中、Piは109.7mmHgであることが分かった。DPは22.3mmHgであった。Kは45.4秒であった。ご覧のように、血圧の最初の低減は図3Aに示すもの(Pi=115または109.5)と比較可能であったが、適応時定数(k)はもっと高かった(45.4秒対15.5秒)。これは、図3Aよりも約3倍大きな期間の間、低い血圧が維持されたことを意味している。

図9は、いくつかの実施形態に係る、血圧を低減するためのシステム700を模式的に示している。システム700は、1つの装置であってもよく、または、好ましくは有線または無線通信により関連付けられた、複数の装置を備えていてもよい。装置は、ハウジング内に配置された、かつ/または、電気的におよび/もしくはワイヤでハウジングに接続された、複数の部品を有していてもよい。図9に示すように、心臓701が、1つ以上の刺激電極702により、システム700に接続されている。刺激電極は、患者の心臓の少なくとも1つのチャンバを、刺激パルスで刺激するよう構成されていてもよい。

心房の受動内圧上昇と時間が重なり合う前記心房の心房収縮により生じる心房内圧を提供しない第2の刺激パルスとを異なる比で有する複数の刺激パターンを交互に行うことをさらに含む、請求項33に記載の方法。

興奮の相対的タイミングが約2msの房室遅延に対応するように、前記心臓の前記心房および前記心臓の心室のうちの少なくとも1つをペーシングすることをさらに含む、請求項23に記載の方法。

興奮の相対的タイミングが約30ms〜約0msの房室遅延に対応するように、前記心臓の前記心房および前記心臓の心室のうちの少なくとも1つをペーシングすることをさらに含む、請求項23に記載の方法。
Claims (13)
- 患者の血圧を下げるためのシステムであって、
1つまたは複数の刺激パターンの刺激パルスを患者の心臓の少なくとも1つの心臓チャンバに伝達するように構成された刺激回路と、
前記少なくとも1つの心臓チャンバへの前記1つまたは複数の刺激パターンの刺激パルスの伝達を実行するように構成された少なくとも1つのコントローラとを備え、
前記刺激パルスのうちの少なくとも1つは、第1の刺激設定を有し、前記刺激パルスのうちの少なくとも1つは、前記第1の刺激設定とは異なる第2の刺激設定を有し、
前記第1の刺激設定および前記第2の刺激設定のうちの少なくとも1つは、房性刺激を低減または防止するように構成され、
房性刺激を低減または防止するように構成された刺激設定を有する刺激パルスは、必要に応じて伝達される、システム。 - 前記少なくとも1つのコントローラは、24時間の一部の間だけ、房性刺激を低減または防止するように構成された刺激設定を有する前記刺激パルスを伝達するように構成されている、請求項1に記載のシステム。
- 前記24時間の一部は夜間またはその一部である、請求項2に記載のシステム。
- 前記24時間の一部は昼間またはその一部である、請求項2に記載のシステム。
- 前記少なくとも1つのコントローラは、心拍数が所定の閾値未満のときだけ、房性刺激を低減または防止するように構成された刺激設定を有する前記刺激パルスを伝達するように構成されている、請求項1に記載のシステム。
- 前記所定の閾値は絶対値である、請求項5に記載のシステム。
- 前記絶対値は90bpmである、請求項6に記載のシステム。
- 前記所定の閾値は前記患者の平均心拍数に関連する値に設定される、請求項5に記載のシステム。
- 前記所定の閾値は、平均心拍数を30回超える心拍数および前記心拍数の第80百分位数超のうちの少なくとも1つである、請求項8に記載のシステム。
- 前記少なくとも1つのコントローラは、前記患者が安静にしているかまたは低い活動レベルであるときだけ、房性刺激を低減または防止するように構成された刺激設定を有する前記刺激パルスを伝達するように構成されている、請求項1に記載のシステム。
- 前記システムは、動作、姿勢、呼吸数、および心拍数のうちの少なくとも1つを検知することによって、前記患者が安静にしているかまたは低い活動レベルであるかを判断する、請求項10に記載のシステム。
- 前記少なくとも1つのコントローラは、測定された血圧パラメータに基づいて前記1つまたは複数の刺激パターンを選択するように構成されている、請求項1に記載のシステム。
- 前記少なくとも1つのコントローラは、圧反射が検知された場合に前記1つまたは複数の刺激パターンを変更するように構成されている、請求項12に記載のシステム。
