Description
a.本願発明者は、刺激が維持されている間、血圧は、ある時間ののち血圧が増加する適応パターン(そのいくらかはしばしば、5分未満の、または1分未満でさえある短い時間のうちに起きる)を示すことがあり、(場合によっては少なくとも圧反射ゆえ)刺激前の血圧値近く、または、より高くにさえ達することを発見した。適応は、少なくとも一部が、総末梢抵抗(total peripheral resistance)の増加などの、心臓血管系の特性の変化に起因していてもよい。本願発明者はさらに、刺激の終了により、刺激前の値に、または、より高い値にさえ、血圧が速やかに戻る結果になること、およびその後、心臓が、それほど刺激されなかった心臓に類似した程度で、血圧低減刺激信号に反応するようになることを発見した。くわえて、複数のBPR刺激設定を備える異なる刺激パターンが、異なる血圧適応パターンにつながることが発見された。
式中、(mmHgでの)Pは収縮期血圧を表しており、Pi(mmHg)は、BPR刺激の開始時の第1の平均の低減された血圧であり、DP(mmHg)は、新たな定常状態水準への最初の下落後の、圧力の増加の量を表す定数であり、k(秒)は応答時間定数であり、eは、自然対数の底(base)である数学定数であり、t(秒)は時間である。
式中、Piは115mmHgであることが分かった。DPは23 mmHgであった。Kは15.5秒であった。
式中、Piは190mmHgであることが分かった。DPは-35mmHgであった。Kは4.946秒であった。
ご覧のように、Piは、かつDPも、第1の刺激パターンの対応する値(図3Aのa-a’)と比較可能であった。しかし、第2のパターンのkは、第1の刺激パターンの時定数の2倍近くであった。この時間間隔において、適応は図3Aにおいてよりも、より遅い比率で起きたが、パターンが刺激パルスの間で切り替わったときに、血圧は、図3Aにおいてよりも、急上昇した。この結果は、交替する刺激設定を有する刺激パターンの使用が適応を低減することを証明している。
式中、Piは109.7mmHgであることが分かった。DPは22.3 mmHgであった。Kは45.4秒であった。ご覧のように、血圧の最初の低減は図3Aに示すもの(Pi=115または109.5)と比較可能であったが、適応時定数(k)はもっと高かった(45.4秒対15.5秒)。これは、図3Aよりも約3倍大きな期間の間、低い血圧が維持されたことを意味している。
刺激パターンを設定および/または選択するための方法600を、図8に模式的に示す。方法600は、BPR刺激を行うための装置の埋め込み中に、および/または、装置作動パラメータを調整するために周期的に、および/または、作動中に継続的に、行われてもよい。方法600は、以下に説明するシステム700により行われてもよい。したがって、システム700は、方法600のステップを行うよう構成されていてもよい。同様に、方法600は、システム700が行うよう構成されたステップを含んでいてもよい。例えば、方法600は、システム700について以下に説明する機能を含んでいてもよい。くわえて、方法600は、以下に説明する装置50により行われてもよい。方法600は、装置50が行うよう構成されたステップを含んでいてもよい。
図9は、いくつかの実施形態に係る、血圧を低減するためのシステム700を模式的に示している。システム700は、1つの装置であってもよく、または、好ましくは有線または無線通信により関連付けられた、複数の装置を備えていてもよい。装置は、ハウジング内に配置された、かつ/または、電気的におよび/もしくはワイヤでハウジングに接続された、複数の部品を有していてもよい。図9に示すように、心臓701が、1つ以上の刺激電極702により、システム700に接続されている。刺激電極は、患者の心臓の少なくとも1つのチャンバを、刺激パルスで刺激するよう構成されていてもよい。
いくつかの実施形態は、血圧は、心房興奮の少なくとも一部の間に、少なくとも1つのAV弁の閉鎖を引き起こすにより低減することができるという、発明者の理解に由来している。これは心室の充満に対する心房の収縮の貢献を低減し、または防止しさえし、その結果、閉鎖の終わりに心臓充満を低減し、その結果、血圧を低減することになる。
血圧を低減するための方法40を図13に模式的に示す。方法40は、以下に説明する、図14の装置50により行われてもよい。したがって、装置50は、方法40のいずれかの、またはすべてのステップを行うよう構成されていてもよい。同様に、方法40は、装置50が行うよう構成されたいずれのステップを含んでいてもよい。例えば、方法40は、装置50に関して上述した機能のいずれかを含んでいてもよい。方法40は、方法600からのいずれのステップを含んでいてもよい。同様に、方法600は、方法40からのいずれのステップを含んでいてもよい。方法40は、システム700が行うよう構成されたいずれのステップを含んでいてもよい。システム700は、方法40のいずれかの、またはすべてのステップを行うよう構成されていてもよい。
ここで、一実施形態に係る装置50を模式的に示す、図14を参照する。装置50は、本明細書中で説明するいくつかの修正をした状態で、当該技術で本質的に公知の仕方で、構成されていてもよく、かつ、心臓ペースメーカーに類似の部品を有していてもよい。好ましくは、装置は埋め込み可能とされている。好ましくは、装置は、心臓の追加のおよび/または代替的な電気的治療(例えば除細動)を提供できる部品を備えている。装置50は、好ましくは本明細書中で説明するいくつかの修正をした状態で、埋め込み可能なペースメーカーについて当該技術で本質的に公知な仕方で、患者の体における埋め込み用に構成されていてもよい。装置50は、システム700の部品を含んでいてもよく、システム700は、装置50の部品を含んでいてもよい。
くわえてまたは代替的に、装置50は、少なくとも1つの埋め込みの人工弁562の作動を直接制御するよう構成されていてもよい。ここで、本発明の実施形態に係る人工弁60を模式的に示す図15に注目する。この例に示す弁60は、人工弁に関する当該技術で本質的に公知のように、二葉とされている。以下の例は二葉弁に関するものであるが、実施形態は、例えばケージ型ボール弁およびディスク弁などの他の人工弁で実施されてもよい。
全体として、開示された方法およびシステムの実施形態のいくつかは、少なくとも1つの心室の充満を低減し、その結果、血圧を低減するための異なる手法を提供する。血圧を低減するための以前の機械的な方法と異なり、本明細書に記載した実施形態のいくつかは、少なくとも1つの対応する心房内で圧力を増加させることなく、この目標を達成することができる。心房性ナトリウム利尿ホルモン(atrial natriuretic hormone)または心房性ナトリウム利尿ペプチド(atrial natriuretic peptide)の分泌を誘発する心房圧の増加なしに、血圧の低減を機械的に制御することができる。この開示された実施形態は、心拍数への望ましくない影響を防止することができ、かつ、正規の(canon)心房波の可能性を低減することができる。

前記少なくとも1つの刺激設定は、前記AV弁が開いているときに心房を最大の収縮とならせるよう構成されている、予備請求項84に記載のシステム。

心房および心室の少なくとも一方の興奮率を検知するためのセンサと、
心房および心室の少なくとも一方へ刺激パルスを伝達するよう構成された刺激回路と、
前記刺激回路に結合されたプロセッサ回路であって、次の心房興奮のタイミングが、前記検知された興奮率に基づき予測され、かつ、少なくとも1つの心室が、前記予測された次の心房興奮の約50ms前〜約10ms後の間の時間に刺激される、作動モードで作動するよう構成されている、プロセッサ回路とを備える、システム。
心房および心室の少なくとも一方の前記興奮率を検知するための前記センサは、心房興奮を検知するための電極を備えている、予備請求項110に記載のシステム。

次の心房興奮の予測が、間隔および周期的な変動の変化率を含む、先行する検知された興奮の関数に基づいている、予備請求項110および予備請求項111のいずれか1項に記載のシステム。

前記刺激パルスは、前記検知された興奮率よりも高い率で伝達され、前記刺激パルスは、前記心房の刺激の約50ms前〜約70ms後の間の時間に心室を刺激するよう構成されている、システム。
前記作動モードは、前記心室を刺激して、前記少なくとも1つの心房の収縮の終了前に前記心室を収縮させることを含む、予備請求項33に記載の装置。
[予備請求項120]
前記作動モードは、前記心室を刺激して、前記少なくとも1つの心房の収縮の前記終了前に前記心室を収縮させて、それにより、前記少なくとも1つの心房の収縮の少なくとも一部の間に前記AV弁が閉じられるようにすることを含む、予備請求項34に記載の装置。

Claims (6)
- 治療前の心室充満量を有する高血圧の患者の心室充満量を低減するためのシステムであって、
少なくとも1つの心臓チャンバに刺激パルスを伝達するよう構成された刺激回路と、
少なくとも1つの心臓チャンバに1つ以上の刺激パルスの刺激パターンの伝達を実施するよう構成された、少なくとも1つの制御器とを備え、
前記刺激パルスの少なくとも1つは、心房興奮の開始後、0msを超えてから70msまでの間に心室興奮を開始するよう構成された設定を有し、それにより、前記心室充満量を前記治療前の心室充満量から低減させ、該患者の血圧を低減させる、システム。 - 前記刺激回路は、少なくとも1つの心房および少なくとも1つの心室の両方に刺激パルスを伝達するよう構成されている、請求項1に記載のシステム。
- 心房または心室のいずれか少なくとも1つの興奮率を検知するためのセンサを備え、
前記少なくとも1つの制御器は、前記検知に基づき前記患者の心拍数を検知し、かつ、前記検知された興奮率よりも高いレートで、前記刺激パルスを伝達するよう構成されている、請求項2に記載のシステム。 - 前記刺激パルスの前記1つ以上の刺激パターンの伝達は、前記心房に関連する房室弁が閉じられたときに、前記少なくとも1つの心臓チャンバに房室収縮の40%と房室収縮の100%との間で刺激を引き起こすことを含む、請求項1〜請求項3のいずれか1項に記載のシステム。
- 前記刺激パルスの前記1つ以上の刺激パターンの伝達は、心室収縮中に心房収縮の50%〜95%が引き起こされるように、前記少なくとも1つの心臓チャンバをペーシングすることを含む、請求項1〜請求項4のいずれか1項に記載のシステム。
- 前記刺激パルスの前記1つ以上の刺激パターンの伝達は、心房収縮が、心室収縮の50ms前〜5ms前に始まるように、前記少なくとも1つの心臓チャンバをペーシングすることを含む、請求項1〜請求項5のいずれか1項に記載のシステム。
