Description
図1は、例示的な外科用ステープル留め及び切断器具(10)を表し、この器具は、ハンドル組立体(20)と、シャフト組立体(30)と、エンドエフェクタ(40)と、を含む。エンドエフェクタ(40)及びシャフト組立体(30)の遠位部分は、外科的処置を行うために、図1に描写されるような非関節動作状態で、トロカールカニューレを通って患者内の手術部位まで挿入するように寸法決めされている。単なる例示として、患者の腹部内に、患者の2本の肋骨の間に、又はその他の部位に、かかるトロカールを挿入してもよい。一部の状況では、器具(10)は、トロカールなしで使用される。例えば、エンドエフェクタ(40)及びシャフト組立体(30)の遠位部分を、開胸術又は他の種類の切開によって直接挿入することができる。本明細書では、「近位」及び「遠位」といった用語は、器具(10)のハンドル組立体(20)を握っている臨床医を基準として使用されていることを理解されたい。したがって、エンドエフェクタ(40)は、より近位にあるハンドル組立体(20)に対して遠位にある。便宜上、また説明を明確にするため、本明細書では「垂直」及び「水平」といった空間的な用語が、図面に対して使用されている点も更に認識されるであろう。しかしながら、外科器具は、多くの配向及び位置で使用されるものであり、これらの用語は、限定的かつ絶対的なものであることを意図するものではない。
図1に示すように、本例のハンドル組立体(20)は、ピストル把持部(22)と、閉鎖トリガ(24)と、発射トリガ(26)と、を含む。各トリガ(24、26)は、以下により詳細に記載されるように、ピストル把持部(22)に向かって、かつそれから離れるように選択的に枢動可能である。ハンドル組立体(20)は、取り外し式電池パック(28)を更に含む。これらの構成要素についても、以下でより詳細に説明する。勿論、ハンドル組立体(20)は、上記したもののいずれかに加えて又はその代わりに様々な他の構成要素、特徴、及び動作性を有することができる。ハンドル組立体(20)の他の好適な構成は、本明細書の教示を考慮することで当業者には明らかであろう。
図1〜図3にも示されているように、本例のエンドエフェクタ(40)は、下部ジョー(50)及び枢動可能なアンビル(60)を含む。アンビル(60)は、下部ジョー(50)の対応する湾曲スロット(54)に配置されている一対の一体的な、外側に延在するピン(66)を含む。アンビル(60)は、開放位置(図2に示す)と閉鎖位置(図1に示す)との間で、下部ジョー(50)に向かって、また下部ジョー(50)から離れるように枢動可能である。「枢動可能」という用語(及び「枢動」を基体とした類義語)の使用は、必ずしも固定軸を中心とした枢動運動を必要とすると理解されるべきではない。例えば、本例において、アンビル(60)は、ピン(66)により画定される軸を中心に枢動し、このピンは、アンビル(60)が下部ジョー(50)に向かって動くと、下部ジョー(50)の湾曲スロット(54)に沿って摺動する。かかる形態では、枢動軸がスロット(54)によって画定された経路に沿って並進する一方で、アンビル(60)はその軸を中心として同時に枢動する。追加的にあるいは代替的に、まず枢動軸がスロット(54)に沿って摺動し、次いで枢動軸がスロット(54)に沿ってある一定の距離を摺動した後に、アンビル(60)が枢動軸を中心として枢動してもよい。そのような摺動/並進枢動運動は、「枢動」、「枢動する」「枢動の」、「枢動可能な」、「枢動している」などの用語内に包含されることを理解されたい。当然のことながら、一部の形態は、固定されたままである、かつスロット又はチャネルなどの内側を並進しない、軸を中心としたアンビル(60)の枢動運動を提供してもよい。
本例において、アンビル(60)は、閉鎖リング(36)をエンドエフェクタ(40)に対して遠位側に前進させることによって、下部ジョー(50)に向かって駆動される。閉鎖リング(36)は、カム作用を介してアンビル(60)と協働し、エンドエフェクタ(40)に対する閉鎖リング(36)の遠位への並進に反応してアンビル(60)を下部ジョー(50)に向かって駆動する。同様に、閉鎖リング(36)は、アンビル(60)と協働し、エンドエフェクタ(40)に対する閉鎖リング(36)の近位側への並進に反応してアンビル(60)を下部ジョー(50)から離れて開放することができる。単に例として、閉鎖リング(36)とアンビル(60)とは、その開示内容が参照により本明細書に組み込まれる、2014年8月28日公開の米国特許公開第2014/0239036号、名称「Jaw Closure Feature for End Effector of Surgical Instrument」の教示の少なくとも一部、及び/又はその開示内容が参照により本明細書に組み込まれる、2014年6月25日出願の米国特許出願第14/314,108号の教示の少なくとも一部に従って相互作用し得る。
いくつかの事例では、ステープル(90)によってもたらされる組織の機械的締結を補強するため、エンドエフェクタ(40)に支持具材料を備え付けることが望ましくなり得る。かかる支持具は、適用されたステープル(90)が組織から引き抜かれるのを防止し得、またステープル(90)の適用部位又はその付近の組織が裂けるリスクを別の方法で低減し得る。ステープル(90)のラインに構造的支持及び一体性もたらすことに加え、又はそれに代わるものとして、支持具は、空隙若しくは間隙の充填、治療薬の投与、及び/又は別の効果などのその他様々な種類の効果をもたらし得る。場合によっては、支持具は、ステープルカートリッジ(70)のデッキ(73)上に提供され得る。いくつかの別の場合では、支持具は、ステープルカートリッジ(70)に面するアンビル(60)の表面上に提供され得る。第1支持具がステープルカートリッジ(70)のデッキ(73)上に提供され得る一方で、第2支持具が同じエンドエフェクタ(40)のアンビル(60)上に提供され得ることも理解されよう。支持具が取り得る様々な形態の例を、以下により詳細に記載する。支持具がステープルカートリッジ(70)又はアンビル(60)に固定され得る様々な方法も、以下により詳細に記載される。

場合によっては、外科手術中に支持具組立体(100、110)を備える複数の連続するステープル(90)のラインを組織上に配置することが望ましい場合がある。こうした作業は、オペレータが、エンドエフェクタ(40)を患者から取り除き(例えばトロカールを介して)、使用済みのステープルカートリッジ(70)を取り除き、ステープルカートリッジ(70)を交換して、トロカールを介してエンドエフェクタ(40)を患者内に再挿入することを必要とする場合がある。エンドエフェクタ(40)を患者に再挿入する前に、オペレータは新しい支持具組立体(100)をアンビル(60)に装着してもよい。更に、交換品のステープルカートリッジ(70)は、新しい支持具組立体(110)を含んでもよい。一部のこうした場合では、エンドエフェクタ(40)が作動される度に、ナイフ部材(80)が1つ又は2つの新しい支持具組立体(100、110)を切断することが必要となる場合がある。このように、複数の連続したステープル及び支持具組立体のラインを配置することは、ナイフ部材(80)に対して圧迫及び摩耗を生じさせ、オペレータの疲労をもたらす場合がある。したがって、エンドエフェクタ(40)を作動させる間にナイフ部材(80)によって切断する必要のある構造体を低減又は排除することにより、エンドエフェクタ(40)を作動させるのに必要な力の量を低減させ、ナイフ部材(80)に対する圧迫及び摩耗を低減させることが望ましい場合がある。エンドエフェクタ(40)の作動中にナイフ部材(80)が支持具組立体(110)を切断してしまうことを防ぐいくつかの代表的な機構を以下で説明する。
図7は、例示的な代替的ステープルカートリッジ(270)に取り外し可能に結合された例示的な代替的支持具組立体(210)を示す。図示した実施例では、支持具組立体(210)は、カートリッジ(270)に接着接合されるのではなく、カートリッジ(270)に取り外し可能にかつ機械的に結合される。支持具組立体(210)は一対の対向する支持具本体(212)を含む。支持具本体(212)は、上記の支持具本体(100、112)と実質的に類似するように構成され得る。エンドエフェクタ(40)を作動させると、一連のステープル(90)が同様に、支持具組立体(210)を組織の層(T1、T2)に対して捕捉して保持することにより、支持具組立体(210)を図6に示すものと同様の方法で組織(T1、T2)に固定することを理解されたい。いくつかの実施例では、一連のステープル(90)が同様に支持具組立体(100、210)を組織の層(T1、T2)に対して捕捉して保持することにより、支持具組立体(100、210)を図6に示すものと同様の方法で組織(T1、T2)に固定するように、支持具組立体(210)をアンビル(60)上で支持具組立体(100)と共に用いてもよい。
図10〜図14は、例示的な代替的ステープルカートリッジ(370)と取り外し可能に結合された支持具本体(312)を備える例示的な代替的支持具組立体(310)を示す。図示した実施例では、支持具組立体(310)は、カートリッジ(370)に接着接合されるのではなく、カートリッジ(370)に取り外し可能にかつ機械的に結合される。支持具本体(312)は、上記の支持具(112)と実質的に類似するように構成され得る。エンドエフェクタ(40)を作動させると、一連のステープル(90)が同様に、支持具組立体(310)を組織の層(T1、T2)に対して捕捉して保持することにより、支持具組立体(310)を図6に示すように組織(T1、T2)に固定することを理解されたい。いくつかの実施例では、一連のステープル(90)が同様に支持具組立体(100、310)を組織の層(T1、T2)に対して捕捉して保持することにより、支持具組立体(100、310)を図6に示すものと同様の方法で組織(T1、T2)に固定するように、支持具組立体(310)をアンビル(60)上で支持具組立体(100)と共に用いてもよい。
図31及び図34〜図36は、エンドエフェクタ(40)の下部ジョー(50)に組み込まれた例示的な代替的ステープルカートリッジ(970)を示す。ステープルカートリッジ(970)は、以下の違いを除いてステープルカートリッジ(70)と実質的に同様に動作するように構成される。具体的には、ステープルカートリッジ(970)は、ステープル開口部(945)の近位に位置決めされた一対のアパーチャ(990)を含む。下記でより詳細に論じるとおり、支持具組立体(910)を取り外し可能かつ機械的に結合するために、アパーチャ(990)は支持具組立体(910)の取付機構(980)を受容するように構成される。
図56は、斜めの織布メッシュ(1515)によって接続された一対の支持具本体(1512)を含む例示的な代替的支持具組立体(1510)を示す。支持具組立体(1510)の一部分のみが示されるが、支持具組立体(1510)は、外科用器具(例えば器具(10))の好適なステープルカートリッジ(例えばステープルカートリッジ(70))に組み込まれてもよく、また図5A〜図6に示すものなどの外科用ステープル留め手順の工程で用いられてもよいことが理解されよう。更に、支持具組立体(1510)の構造は、本明細書に記載される様々な支持具組立体のいずれかに容易に組み込むことができる。
図66〜図67Bは、例示的な代替的支持具組立体(2310)を示す。図66で最も良く示されるように、支持具組立体(2310)は、上記のステープルカートリッジ(70)と実質的に同様に動作するように構成されたステープルカートリッジ(2370)上に配設される。カートリッジ(2370)はエンドエフェクタ(40)の下部ジョー(50)に取り外し可能に結合される。本実施例では、ステープルカートリッジ(2370)は、2列のステープル空洞(2345)の代わりに、チャネル(2372)の各側部上の3組の空洞(2345)内に3列のステープル(90)を含む。更に、カートリッジ(2370)は、ステープルデッキ(2373)に沿って延在する細長いトラフ(2379)を含む。トラフ(2379)は、カートリッジ(2370)の長さに沿って長手方向に延在し、空洞(2345)の最外列と空洞(2345)の中央列との間に横方向に位置決めされている。トラフ(2379)は部分的に円形の断面プロファイルを有する。しかしながら、その他の実施例では、本明細書の教示を考慮すれば当業者には明らかになるように、トラフ(2379)は任意の他の好適な形状を有してもよい。

カートリッジは、デッキ下のカートリッジ内に配設されたスレッド部材を更に備え、スレッド部材はデッキ下を長手方向に横断するように構成されている、実施例1〜3のいずれか1つ又は2つ以上に記載の装置。

ステープルカートリッジを備える装置であって、ステープルカートリッジは、(i)複数のステープルと、(ii)デッキであって、デッキは複数の開口部を画定し、複数の開口部の各開口部は、複数のステープルのうちの対応するステープルと関連付けられており、これにより各ステープルは、複数の開口部のうちの対応する開口部を通過するように構成されている、デッキと、(iii)デッキ上に配設された支持具組立体であって、支持具組立体は複数の開口部の全部未満を被覆し、その結果、ステープルの第1の部分は、ステープルの第1の部分が対応する開口部を通過すると、支持具の一部を捕捉するように構成され、また、その結果、ステープルの第2の部分は、ステープルの第2の部分が対応する開口部を通過したときに、支持具の一部を捕捉しないように構成されている、支持具組立体と、を備える、装置。
デッキはチャネルを画定し、複数の開口部は、内側で長手方向に延在する開口部の列と、外側で長手方向に延在する開口部の列と、を画定し、外側で長手方向に延在する開口部の列は、内側で長手方向に延在する開口部の列よりもチャネルから離れて横方向に位置決めされ、支持具組立体は外側で長手方向に延在する開口部の列を被覆しない、実施例16に記載の装置。
デッキは遠位部分及び近位部分を画定し、支持具組立体は、近位部分では開口部を被覆するが、遠位部分では開口部を被覆しない、実施例16〜17のいずれか1つ又は2つ以上に記載の装置。
外科用支持具組立体を備える装置であって、支持具組立体は、(i)第1の支持具本体と、(ii)第1の支持具本体から離間配置された第2の支持具本体であって、それにより第1の支持具本体と第2の支持具本体との間の間隙を画定する、第2の支持具本体と、(iii)第1の支持具本体を第2の支持具本体に接続する接続構造体であって、接続構造体の第1の部分は、第1の支持具本体及び第2の支持具本体のうちの一方又は両方と一致し、接続構造体の第2の部分は間隙と一致し、第2の部分は第1の部分よりも密度が低い、接続構造体と、を備える、装置。
接続構造体は複数のフィラメントを含む、実施例19に記載の装置。

(1) ステープルカートリッジを備える装置であって、前記ステープルカートリッジは、
(i)複数のステープルと、
(ii)デッキであって、前記デッキは複数の開口部を画定し、前記複数の開口部の各開口部が、前記複数のステープルのうちの対応するステープルと関連付けられ、これにより各ステープルが、前記複数の開口部のうちの対応する開口部を通過するように構成され、前記デッキが第1の機械的結合機構を更に備える、デッキと、
(iii)支持具組立体であって、
(A)支持具本体と、
(B)第2の機械的結合機構であって、前記第1の機械的結合機構と係合して、前記支持具本体を前記デッキに解除自在に結合するように構成されている、第2の機械的結合機構と、を備える、支持具組立体と、を備える、装置。
(2) 前記第1の機械的結合機構が前記デッキ内にアパーチャを備え、前記第2の機械的結合機構が前記アパーチャ内に受容されるように構成されている、実施態様1に記載の装置。
(3) 前記第2の機械的結合機構が、前記支持具本体に結合されたタブを備える、実施態様2に記載の装置。
(4) 前記カートリッジが、前記デッキ下の前記カートリッジ内に配設されたスレッド部材を更に備え、前記スレッド部材が前記デッキ下を長手方向に横断するように構成されている、実施態様1に記載の装置。
(5) 第1の機械的結合機構が前記スレッド部材を備える、実施態様4に記載の装置。
(7) 前記デッキが外側部分及び内側部分を画定し、前記第1の機械的結合機構が前記デッキの前記内側部分を更に備える、実施態様5に記載の装置。
(8) 前記第2の機械的結合機構が、前記アパーチャ内に誘導されることに応答して、前記スレッド、及び前記デッキの前記内側部分と係合するように構成されている、実施態様7に記載の装置。
(9) 前記第1の機械的結合機構が前記デッキに接合されている、実施態様1に記載の装置。
(10) 前記第2の機械的結合機構が、前記支持具本体内にスロットを備える、実施態様1に記載の装置。
(12) 前記支持具本体が長手方向軸を画定し、前記スロットが前記スロットに対して平行に位置決めされている、実施態様10に記載の装置。
(13) 前記支持具本体が、
(1)前記デッキの一方の側部上における第1の個別の支持具部分と、
(2)前記デッキのもう一方の側部上における第2の個別の支持具部分と、を備える、実施態様1に記載の装置。
(14) 前記第2の機械的結合機構が前記支持具本体から延在し、前記第2の機械的結合機構が前記支持具本体とは異なる材料を含む、実施態様1に記載の装置。
(15) (a)エンドエフェクタであって、
(i)アンビルと、
(ii)下部ジョーと、を備え、前記アンビルが、前記アンビルと前記下部ジョーとの間に組織を捕捉するために、前記下部ジョーに向かって枢動可能であり、前記ステープルカートリッジが前記下部ジョーに固定されている、エンドエフェクタと、
(b)前記シャフト組立体及び前記エンドエフェクタと連通するステープル留め及び切断機構であって、前記ステープル留め及び切断機構が、前記アンビルと前記下部ジョーとの間にクランプされた組織を切断及びステープル留めするように構成されている、ステープル留め及び切断機構と、を更に備える、実施態様1に記載の装置。
(i)複数のステープルと、
(ii)デッキであって、前記デッキが複数の開口部を画定し、前記複数の開口部の各開口部が、前記複数のステープルのうちの対応するステープルと関連付けられており、これにより各ステープルが、前記複数の開口部のうちの対応する開口部を通過するように構成されている、デッキと、
(iii)前記デッキ上に配設された支持具組立体であって、前記支持具組立体が前記複数の開口部の全部未満を被覆し、その結果、前記ステープルの第1の部分は、前記ステープルの前記第1の部分が対応する開口部を通過すると、前記支持具の一部を捕捉するように構成され、また、その結果、前記ステープルの第2の部分は、前記ステープルの前記第2の部分が対応する開口部を通過したときに、前記支持具の一部を捕捉しないように構成されている、支持具組立体と、を備える、装置。
(17) 前記デッキがチャネルを画定し、前記複数の開口部が、内側で長手方向に延在する開口部の列と、外側で長手方向に延在する開口部の列と、を画定し、前記外側で長手方向に延在する開口部の列が、前記内側で長手方向に延在する開口部の列よりも前記チャネルから離れて横方向に位置決めされ、前記支持具組立体が前記外側で長手方向に延在する開口部の列を被覆しない、実施態様16に記載の装置。
(18) 前記デッキが遠位部分及び近位部分を画定し、前記支持具組立体が、前記近位部分では前記開口部を被覆するが、前記遠位部分では前記開口部を被覆しない、実施態様16に記載の装置。
(19) 外科用支持具組立体を備える装置であって、前記支持具組立体が、
(i)第1の支持具本体と、
(ii)前記第1の支持具本体から離間配置された第2の支持具本体であって、それにより前記第1の支持具本体と前記第2の支持具本体との間の間隙を画定する、第2の支持具本体と、
(iii)前記第1の支持具本体を前記第2の支持具本体に接続する接続構造体であって、前記接続構造体の第1の部分は、前記第1の支持具本体及び前記第2の支持具本体のうちの一方又は両方と一致し、前記接続構造体の第2の部分は前記間隙と一致し、前記第2の部分が前記第1の部分よりも密度が低い、接続構造体と、を備える、装置。
(20) 前記接続構造体が複数のフィラメントを含む、実施態様19に記載の装置。
Claims (10)
- ステープルカートリッジを備える装置であって、前記ステープルカートリッジは、
(i)複数のステープルと、
(ii)デッキであって、前記デッキは複数の開口部を画定し、前記複数の開口部の各開口部が、前記複数のステープルのうちの対応するステープルと関連付けられ、これにより各ステープルが、前記複数の開口部のうちの対応する開口部を通過するように構成され、前記デッキが第1の機械的結合機構を更に備える、デッキと、
(iii)支持具組立体であって、
(A)支持具本体と、
(B)第2の機械的結合機構であって、前記第1の機械的結合機構と係合して、前記支持具本体を前記デッキに解除自在に結合するように構成されている、第2の機械的結合機構と、を備える、支持具組立体と、を備え、
前記ステープルカートリッジが、前記デッキ下の前記ステープルカートリッジ内に配設されたスレッド部材を更に備え、前記スレッド部材が前記デッキ下を長手方向に横断するように構成されており、
前記第1の機械的結合機構が前記スレッド部材を備え、
前記デッキが外側部分及び内側部分を画定し、前記第1の機械的結合機構が前記デッキの前記内側部分を更に備える、装置。 - 前記第1の機械的結合機構が前記デッキ内にアパーチャを備え、前記第2の機械的結合機構が前記アパーチャ内に受容されるように構成されている、請求項1に記載の装置。
- 前記第2の機械的結合機構が、前記支持具本体に結合されたタブを備える、請求項2に記載の装置。

- 前記第2の機械的結合機構が、前記アパーチャ内に誘導されることに応答して、前記スレッド、及び前記デッキの前記内側部分と係合するように構成されている、請求項1に記載の装置。
- 前記第1の機械的結合機構が前記デッキに接合されている、請求項1に記載の装置。
- 前記第2の機械的結合機構が、前記支持具本体内にスロットを備える、請求項1に記載の装置。
- 前記支持具本体が、
(1)前記デッキの一方の側部上における第1の個別の支持具部分と、
