先天性免疫系
先天性免疫反応は、組織と外部環境との間のバリア、例えば皮膚、経口胃粘膜、気道などに関連する進化的に保存された防護システムである。侵入する病原体の速やかな認識および根絶ならびに細胞損傷への応答を提供することは、しばしば、炎症反応に関連し、および適応免疫の活性化に重要な貢献をしている。病原体および/または損傷関連分子(RAMPまたはDAMP)の、例えばトール様受容体(TLR)などのパターン認識受容体への結合によって、先天性防御は誘引される。パターン認識受容体は、多くの細胞型中または細胞型上で見出され、循環性コンパートメント及び組織常在性のコンパートメントの両方中に全身を通じて分布しており、および非特異的な抗菌分子、サイトカイン、ケモカイン、ならびにホスト防御タンパク質およびペプチドの遊離、そして高度に組織化された様式での免疫細胞(好中球、マクロファージ、単球)の補充をもたらす早期の「危険」信号を提供するように機能する(Janeway, 2002、Beutler, 2003、Beutler, 2004、Athman, 2004、Tosi, 2005、Doyle, 2006、Foster, 2007、Matzinger, 2002)。さらに、先天性免疫系は、消化管中の共生微生物叢への耐性の産出ならびに消化管修復および免疫防御に直接関与する(Santaolalla, 2011、Molloy 2012)。
粘膜炎は抗癌療法によって与えられる粘膜への損傷に関する臨床用語である。これは任意の粘膜領域で起こり得るが、最も一般的には、口、そしてそれに続いて小腸に関連している。多くの粘膜炎に対する尺度が臨床的に使用されているが、最も一般的に使用されている2つの評価システムは、NCI尺度およびWHO尺度である。
急性放射線症候群(ARS)は、全身(またはその大部分)が高い用量の放射線を、典型的には短い期間にわたって、受けた場合に発生する重篤な疾患である。1940年代における広島および長崎の原子爆弾の多くの生存者ならびに1986年のチェルノブイリ原子力発電所事故後に最初に応じた消防士の多くが、ARSに罹った(CDC, 2013)。
・放射線量が高かった(胸部X線検査などの医療行為からの用量はARSを起こすには低すぎる)、
・放射線が透過した(すなわち、臓器に到達することができた)、
・個体の全身またはその大部分が線量を受けた、および
・放射線を、短期間で、通常数分以内で受けた。
ウイルス、細菌、真菌、寄生生物などのさまざまな微生物が、疾患を起こし得る。微生物細胞は、天然において単独で生きることが不可能である、多細胞生物の一部としてのみ存在する動物および植物の細胞からは異なっている。微生物細胞は、病原性または非病原性であり、これは、部分的には、微生物および宿主の状態に依存する。例えば、免疫不全宿主においては、通常は無害な細菌が病原体となり得る。宿主細胞への侵入は、細胞内環境内で複製を行う細菌性病原体の生存に重要である。細胞外サイトで複製を行う生物においては、宿主細胞への細菌進入の重要性はよく規定されていない。
先天性防衛レギュレータ(IDRs)は、細胞内シグナル伝達事象と相互作用し、および先天性防御反応を調節する。IDRsとの初期作業の多くは炎症を制御しながらの感染との戦いにおけるそれらの役割に焦点が当てられるのに対し、化学療法または放射線により誘導される粘膜炎及び創傷治癒の動物モデルにおける最近の結果は、IDRsが、病原体以上により広い範囲の損傷誘導剤への応答のあいだ有益であり得ることを示唆している。IDRsは、PAMPsまたはDAMPsによって活性化された際、身体の先天性防御応答を選択的に改変することによって感染を、関連する炎症応答を誘起することなく処置および予防する(Matzinger 2002)。同じメカニズムが、DAMPsの認識の下流の信号伝達が影響を受けている粘膜炎及び創傷治癒モデルにおいて見られるプラスの効果の下にもある。RIVPA(配列番号5)は、ヒトにおける安全性、ならびに、分割照射により誘導されたおよび化学療法により誘導された口腔粘膜炎の動物モデル、消化管への化学療法により誘導された損傷のモデル、および、免疫応答性及び易感染性宿主における局所性及び全身性のグラム陽性及びグラム陰性感染症のモデルにおける有効性を実証した。

を個体に投与する工程を含む方法を提供することである。
a)7個のアミノ酸までのアミノ酸配列を含むペプチドであって、前記ペプチドは、X1X2X3Pであるアミノ酸配列(配列番号:56)を含み、ここで、

を個体に投与する工程を含む方法を提供することである。
RIVPA(配列番号:5)の剤形は、注射用の水性、無菌処理された、滅菌溶液である。各バイアルには、5mLの60mg/mL溶液(300mgのRIVPA(配列番号:5))を含む。RIVPA(配列番号:5)は、注入のための水中に処方され、そして、pHが目標値6.0に調整される。処方は賦形剤を含まず、そして約300mOsm/kgの重量モル浸透圧濃度を有する。
RIVPA(配列番号:5)製剤は、受容個体の体重及び指定された用量レベルに従ってmg/kgベースで決定される、注入のために適切な濃度まで滅菌生理食塩水中に希釈される。希釈されたRIVPA(配列番号:5)は、3日に1度、4分以上で、25mLの静脈内(IV)注入として投与えるされる。
ペプチド合成
表1記載のペプチドは、固相ペプチド合成技術を用いて合成された。
合成完了後、ペプチドは、97.5%トリフルオロ酢酸(TFA)及び2.5%の水を含有する開裂反応混液を用いてレジンから切断された。レジンは1時間半の間、開裂反応混液中に浸された。溶液はその後ブフナー漏斗を用いて重力ろ過され、ろ液は50mLの遠心チューブ中に回収された。ペプチドは、冷ジエチルエーテルを用いて沈殿させることにより単離された。遠心分離およびジエチルエーテルをデカントした後、粗ペプチドは再度ジエチルエーテルで洗浄され、その後真空デシケーター中で2時間乾燥させされた。ペプチドは、その後、脱イオン水(10mL)中に溶解され、−80℃で凍結及び凍結乾燥された。そして乾燥ペプチドはHPLC精製の準備が完了した。
RIVPA(配列番号:5)及び他のIDRは、炎症カスケードによって起きる重篤な損傷を低減し、及び疾患の消散を促進して、組織損傷に対する先天性防御応答を調節する。IDRのこの特性は、マウスでの化学療法誘導性の口腔及びGI粘膜炎において、ハムスターでの放射線誘導性の口腔粘膜炎において、およびマウスでのDSS誘導性の大腸炎において、示されてきた。これらのモデルの各々において、初期の損傷が、損傷の重篤度を上昇させる先天的防御シグナル伝達カスケードを誘起すると考えられている(Marks, 2011、Sonis, 2010)。RIVPA(配列番号:5)及び他のIDRは、傷害の結果として生じる重篤度を低下させ、および重篤な組織損傷の期間を減少させて、シグナル伝達カスケードを相殺する。
RIVPA(配列番号:5)及び他のIDRは、炎症カスケードによって起きる重篤な損傷を低減し、及び疾患の消散を促進して、組織損傷に対する先天性防御応答を調節する。上述のように、IDRsは、口腔粘膜炎モデル(図1、図2)において放射線損傷に対する応答を緩和することができる。別のモデルでは、放射線誘導性の粘膜炎(25Gyが0日目にマウスの鼻に照射された)の予防が評価され、RIVPA(配列番号:5)(2日毎にIV投与される25mg/kg用量を5回)は、疾患の進行にはなんらの顕著な影響を及ぼさなかった。マウス舌の進行性の薄化は、0、2、4、6、8、10日目に、単位面積当たりおよび単位長さ当たりの、基底層および基底層上アポトーシス性、有糸分裂期の、および総計の上皮細胞の数の組織病理学的な分析によって評価した。使用される放射線線量(25Gy)が、舌上皮の進行性薄化が観察されるが、明白な粘膜炎が発生しないように選択されたことに留意するべきである。この結果は、RIVPA(配列番号:5)の増殖性の欠如を示しており、そしてRIVPA(配列番号:5)の効果が、関連する経路が明白な組織損傷または病原体の侵入によって刺激された場合にのみ、観測可能であることを示唆している。
直接的にGI粘膜表面を保護するための、予防的または治療的に投与された、IV RIVPA(配列番号:5)の能力が、DSS誘導大腸炎モデルにおいて確認された。このモデルでは、DSSが研究の0日目から5日目までにC57BL/6雄マウスの飲水中での3%DSS溶液として投与した。大腸炎は、7、14および21日目にビデオ内視鏡検査によりモニターされた。RIVPA(配列番号:5)(25mg/kg)が、0日目から18日目まで3日毎(Q3d d0〜18)、3日目から18日目まで3日毎(Q3d d3〜18)、6日目から18日目まで3日毎(Q3d d6〜18)に投与された。この研究の結果を図3に示されている。14日目までに、すべてのRIVPA(配列番号:5)の治療レジメンは、内視鏡的な大腸炎の重症度スコアにおいて統計的に有意な減少を示した。しかし、7日目のスコアにおける減少は、その時までにRIVPA(配列番号:5)の少なくとも2回用量を受けていたグループ(すなわち、Q3d d6〜18ではなく、Q3 d0〜18及びQ3d d3〜18)のみで観察された。21日目に、すべての3つの処置群は、同様の様式で応答しているように見えた。21日目の結腸の組織病理学は、いくつかのRIVPA(配列番号:5)処理群では、統計的に有意に減少された浮腫や壊死を有していたが、他のRIVPA(配列番号:5)処理群では類似の応答を有しているものの、統計的な有意差には達しなかったことを示していた。統計分析は、t検定を用いて実施され、アスタリスクは対照からの統計学的に有意な差異(P<0.05)を示す。
RIVPA(配列番号:5)は、さまざまなマウス感染モデルにおいて、抗生物質処置の存在下または非存在下、細菌性負荷を低減し、および生存率を改善し、そしてこの効果は、25mg/kg IVおよびそれより高い用量レベルで一貫しており、および5日間までの持続的な薬力学的効果である。RIVPA(配列番号:5)の効果は、通常及び免疫低下マウスの両方において抗生物質処置に対して相補的である。RIVPA(配列番号:5)の効果は、グラム陽性(黄色ブドウ球菌及びMRSA)及びグラム陰性(クレブシエラ、大腸菌及びB.pseudomallei)感染によって起きる疾患に対して示された。
RIVPA(配列番号:5)は、バンコマイシン処置との組み合わせ及び独立した処置の両方で試験された。
・RIVPA(配列番号:5)の効果は、マウスにおいて1mg/kg〜50mg/kgの間で用量依存性であり、25mg/kg以上の用量レベルは一貫した有効性を示した(表2、図9及び図10)。この用量レベルはまた、損傷モデルにおいて得られるより多くの慢性疾患環境においても有効であった(図1〜図6)。
・RIVPA(配列番号:5)の毎日の投与は不要であり、および、1日おき、または2日おきの投与で十分であった。損傷モデルにおいて得られるより慢性の疾患環境において、2日おきの投与が最適であることがさらに確認された(データは示さず)。
・RIVPA(配列番号:5)は、MRSA菌血症モデルにおいて感染の開始後24時間までに投与でき、そして依然として、延命効果をもたらすことができる(データは示さず)。したがって、その作用は迅速である。
・用量レベルに依存して、一回量のRIVPA(配列番号:5)を、感染開始の5日前までに投与でき、そして依然として、延命効果をもたらすことができ(データは示さず)、これは、マウス循環からの迅速(数分以内)な薬物動態(PK)クリアランスにもかかわらず、RIVPA(配列番号:5)の薬力学的影響の持続性を反映している。
・RIVPA(配列番号:5)処置によってもたらされる延命効果は、少なくとも21日間持続され得る(図9)。
RIVPA(配列番号:5)は、全身性(IV)また局所的(IP)のどちらかで投与された場合、グラム陰性腹膜感染モデルにおいて生存率を上昇させた。特に、全身性投与は局所投与と同等かより良好であった。RIVPA(配列番号:5)処置(24mg/kg)は、肺炎桿菌 (カタログ番号43816、ATCC)を2.8×105 CFU(図13 パネルA)または5.3×102 CFU(図13 パネルB)のいずれで播種された雌Balb/cマウス(N=8/群)に対して、IP(感染の24時間前または感染後4時間)またはIV(感染後4時間)のいずれかで投与され、そして生存が24時間以上にわたってモニターされた。この状況におけるRRIVPA(SEQ ID NO.5)の保護効果は、図13に示されている。生存エンドポイントは、細菌をより多く播種された動物において示されている(パネルA)。より低い播種量を受けたすべての動物は、すべてのグループで生存し(パネルB)、そして感染後24時間の臨床徴候(例えば、立毛、運動減少、丸めた腹部)について評価された。これらは臨床スコアとしてまとめられた。
全身性で投与されたRIVPA(配列番号:5)はまた、皮膚損傷及び感染の場合に有効であり、MRSA皮膚感染モデルにおける皮膚治癒を促進する。感染は、各マウスの背側の領域から毛が取り除かれた後、1日で開始された。RIVPA(配列番号:5)(25mg/kg IVまたは100mg/kg SC)が感染の4時間前及び示されているように感染後のさまざまな時点で投与された。経口リネゾリドは比較物として使用され、そして12.5mg/kgで毎日投与された。1日(−1時間)目に、各マウスはイソフルランを用いて麻酔され、そして剃毛された背部皮膚が7回の連続した外科テープの適用および除去によって損傷された。この病変は、直ちに、10μLの細菌懸濁液の局所投与によって、合計でマウス当たり7.6log10 CFUの曝露に付して、感染された。有効性は、細菌への曝露後48時間(図14 パネルA)及び96時間(図14 パネルB)での皮膚のパンチ生検材料における細菌負荷の測定によって、及び、感染後48時間(図14、パネルC)及び96時間(図14、パネルD)での、盲検の、公認の病理学者による皮膚のデジタル画像の巨視的評価によって評価された。特に、いかなる単離された細菌の局在化(すなわち、皮膚表面上または組織内)は測定されなかったが、リネゾリドもRIVPA(配列番号:5)も、対照群と比較して、48または96時間の生検材料において細菌負荷を減少させなかった。それにもかかわらず、創傷治癒が明白に起こった。それぞれの処置群における細菌負荷の平均値が、不等分散を仮定したエクセル上での平均値のt−検定比較を使用して、時間が一致している生理食塩水の対照の値と統計学的に比較された。0.05以下のp値を示した比較が、統計学的に有差であると考慮された。
でにより、2つのパイロットおよび2つの最終的な反復用量毒性試験が、静脈内(IV;ゆっくりとしたボーラス投与)への投与経路を用いて、マウス及びカニクイザルでRIVPA(配列番号:5)を用いて行われた。すべての試験は、LAB Research Inc., Canadaで実施された。
非GLP薬理試験において、RIVPA(配列番号:5)は、CD−1マウスにおける白血球減少の誘導後の血液細胞集団の循環の回復を変化させなかった。白血球減少は、2回のCpのIP注入(1日目における150mg/kg及び4日目における100mg/kg)によって誘導され、4日目までに良好に確立された白血球減少症をもたらし、これは約10日目まで持続した。生理食塩水またはRIVPA(配列番号:5)(20または50mg/kg)が、5、7、9および11日目にIV投与された。1グループ当たり6匹の動物が、6、8、10、12、および14日目のそれぞれに犠牲死され、そして、全血球数および差分が評価された。溶媒対照群と比較した場合、全白血球及び分画白血球数のレベルまたはダイナミクスのどちらも、回復の過程のあいだに変化はなかった(図15)。
RIVPA(配列番号:5)を用いた臨床治験は、フェーズ1試験で得られた。本試験の主な目的は、健康なボランティアにおける、IV投与後の、RIVPA(配列番号:5)注入可能溶液の単回および反復投与用量漸増用法におけるIV用量の最大耐量(MTD)を決定することであった。本試験の第2の目的は、RIVPA(配列番号:5)の単一および反復漸増投与後のRIVPA(配列番号:5)の用量制限毒性(DLT)、安全性、PKおよび薬力学(PD)プロファイルのの評価が含んでいた。試験は、2つのフェーズに分割された:単回投与用量漸増試験(SAD)フェーズおよび反復投与用量漸増試験(MAD)。
RIVPA(配列番号:5)の単回IV用量は、最大試験値(8mg/kg)まで良好な耐容性を示し、そして毎日のIV用量は最大試験値(6.5mg/kg、7日間)まで良好な耐容性を示した。用量制限毒性(DLT)は存在せず、そして、いずれの治験のフェーズにおいてもMTDには達しなかった。試験中、死亡例はなく、臨床的に、顕著な、重篤な、深刻な有害事象(AEs)の報告はなかった。安全性の懸念、または平均値もしくはベースラインからの変化における有意な差異は、薬剤処置群およびプラセボ対照被験者のあいだでのバイタルサイン測定、臨床検査結果または心電図(ECG)結果について認められなかった。
RIVPA(配列番号:5)を受けた被験者におけるTEAEの発生率は、用量応答性でなく、および事象は、プラセボを受けた被験者と比較してRIVPA(配列番号:5)をうけた被験者に臨床的に有意に高い割合でも発生しなかった。最も頻繁に報告されたTEAEs(RIVPA(配列番号:5)を受けた複数の被験者で観察され、プラセボ被験者よりも高い割合(%)で観察されたる)は、例えば血管穿刺部位血腫、血管穿刺部位反応および血管穿刺部位の痛みなどの、試験処置手順に関連した事象(全身障害および投与局所様態)であった。すべての血管穿刺に関連する事象は軽度であり、QIによる治療処置には無関係であることが判明した。2番目に頻繁に報告されたTEAEsは、神経系障害であり、具体的には頭痛およびめまいであったが、これらの事象は軽度から中度にすぎなかった。他のすべてのTEAEsは、全ての試験された用量レベル(最大3用量レベル)において1人の被験者によってのみ報告された。TEAEsの特性または期間における臨床的に顕著な傾向は、任意の研究コホートにおいて示されなかった。
TEAEsの最も高い発生率は、(4.5及び6.5mg/kg/日)で観察された。2つの最高用量レベルにおいても、「関連している可能性のある」事象の発生率はまた、2つの最高用量レベルにおいて高かった。しかし、サンプルサイズの小ささ(それぞれのコホートにおいて4被験者が動的治療を受けた)のため、結果が間違いなく用量応答性を表すかどうかを結論付けることはできなかった。TEAEsの大部分は治験処置に関連せず、および重症度において軽微であり、1件の事象のみが中程度と報告された。RIVPA(配列番号:5)を受けた被験者において最も頻繁に報告されたTEAEsは、例えば血管穿刺部位血腫、血管穿刺部位反応および血管穿刺部位の痛みなどの、全身障害及び投与局所様態(すなわち、手順に関連した事象)であった。すべての血管穿刺に関連する事象は軽度であり、治療処置には無関係であることが判明した。アラニンアミノトランスフェラーゼ(ALT)の増加及び背部痛が、RIVPA(配列番号:5)を受けた3人の被験者(15.0%)によって報告され、そしてこれらの事象は、プラセボを受けた被験者では唯1人の被験者(10.0%)によってのみ観察された。
ヒト被験者への静脈内投与後、および動物試験における発見と一致して、RIVPA(配列番号:5)は数分以内に循環から除去された。健康なボランティアによるフェーズ1試験の単回投与量試験フェーズにおいて、RIVPA(配列番号:5)は、迅速に除去され、4分間のIV注入の開始後9分以内に血漿レベルは最大濃度(Cmax)の10%未満までに減少した。急速な低下に続き、より遅い排出フェーズが観察された。最大濃度の平均時間(Tmax)は、0.15mg/kgから8mg/kgの範囲の用量の注入の開始後、約4分〜約4.8分の間であった。最大血漿濃度及び総曝露レベルは、用量比例性であり、および循環からのRIVPA(配列番号:5)のクリアランスは迅速であって、マウスおよびNHPの実験と一致していた。
フェーズ1のあいだに健康なヒトボランティア試験から得られた血液サンプルを用いたex vivo研究において、いくつかのサイトカイン及びケモカイン分析物が、全血のLPSを用いたin vitro刺激の4時間後に定量された。分析物のレベルにおける個体間変動は、時間における、またはRIVPA(配列番号:5)もしくはプラセボの投与に対する応答におけるいかなる変動よりも大きく、したがってデータは、個々の被験者それぞれの全ての応答を標準化するために、個々の投与前の分析物レベルを使用して自己正規化された(アクティビティー率)。RIVPA(配列番号:5)の分析物アクティビティー比率(ARs)に及ぼす影響は、時間を通じて一定でなく、また、線形な用量応答性でもなかった。それにもかかわらず、0.15〜2mg/kgの用量範囲において、「抗炎症状態」(すなわち、各個体からの血液のLPS刺激後の、より低いTNFαおよびIL−1βレベルを伴った、より高い抗炎症性TNF RIIおよびIL−1raレベル)の増加の証拠が見られた。
粘膜炎
粘膜炎は先天性防御系の異常調節にリンクされており、粘膜内層にさらに損傷を与え、および明白な粘膜炎をもたらす一連の炎症作用を引き起こす(Sonis, 2004)。具体的には、化学療法または放射線治療は基底にある内皮及び上皮への損傷を誘起する一方、結果として生じる「DAMPS」に対する先天性防御システムの応答は、この損傷を悪化させる炎症カスケードをもたらす。深刻な口腔粘膜炎に罹りやすい動物およびヒトにおける遺伝子発現を評価している最近の研究は、疾患における先天性防御システムの役割を支持した(Sonis, 2010)。さらに、下部消化管の粘膜炎がまた、同様の機構に起因していた(Bowen, 2008)。
急性放射線被曝は、上皮(皮膚)、骨髄(造血症候群)および消化管(GI)への損傷に関係付けられる。さらに、放射線被曝が増大するにつれ、死亡率は急激に増加し、治療学的介入の可能性を制限する。急性放射線曝露後の早期死亡率(<2週間)は、胃腸管への損傷に関連付けられる。急性放射線はリーベルキューン腺小窩の底部内の幹細胞への直接的な損傷を引き起こし、アポトーシス機構を介して、有糸分裂停止および死をもたらす(Potten, 1997a、Potten, 1997b)。この損傷の回復および/または長期の二次的結果は、GIの微生物叢及び先天性免疫修復応答の両方に関連されることが示されている(Crawford, 2005、Garg, 2010)。正常及び癌化組織の放射線生物学における進行中の研究は、放射線への先天性免疫システムの応答に顕著な役割を示した(Schaue and McBride, 2010、Lauber, 2012、Burnette, 2012)。さらに、先天性の防御システムを標的とするアゴニスト(すなわち、TLR−9アゴニスト)は、GI ARSの設定で放射線防護性であることが示されている(Saha, 2012)。

- 放射線または化学療法薬剤の有害量に曝露された個体における口腔粘膜炎の治療薬であって、
配列番号:92で示されるアミノ酸配列を含むペプチド、またはその薬学的な塩、ならびに、薬学的に許容可能な担体、希釈剤、または賦形剤
を含む治療薬。 - 前記ペプチドの有効量が少なくとも1.5mg/kgである請求項1記載の治療薬。
- 前記治療薬が、局所的に活性なコルチコステロイドまたはその代謝物をさらに含む請求項1又は2に記載の治療薬。
- 前記局所的に活性なコルチコステロイドがジプロピオン酸ベクロメタゾンである請求項3記載の治療薬。
- 前記代謝物が17−モノプロピオン酸ベクロメタゾンである請求項3記載の治療薬。
- 有効量の抗生物質を更に含む請求項1乃至5の何れか1項に記載の治療薬。
- 配列番号:92で示されるアミノ酸配列、またはその薬学的な塩を含む単離されたペプチド。
- RIV(mp2)A−NH2のアミノ酸配列からなる単離ペプチドであって、
ここで、mp2=4-アミノ−1−メチル−1H−ピロール−2−カルボン酸
- 個体において化学療法誘導性の口腔粘膜炎及び大腸炎の重症度を軽減するための薬剤であって、有効量の以下:
配列番号:92で示されるアミノ酸配列を含むペプチド、またはその薬学的な塩、ならびに、薬学的に許容可能な担体、希釈剤、または賦形剤を含む薬剤。
