Description
一態様では、図13に示されるように、マイクロニードル100は、組織に面する表面160に遠位の端部で任意選択のかかり101を有する。別の態様では、マイクロニードル100は圧縮可能な組織厚さコンペンセーター104によって組織に支持されるので、マイクロニードル100はかかり101を有しない。
(1) 組織の吻合接合用の円形外科用ステープラーであって、
a)対向するアンビルに接続されたステープル留め頭部であって、複数の展開可能なステープルを同心のアレイ内に収容する、ステープル留め頭部と、
b)薬剤を放出する複数の吸収性マイクロニードルであって、鋭い組織貫通遠位端と近位端とを有する細長いロッドから構成され、前記ステープル留め頭部又は前記アンビルの上又は内に解放可能に配置されている、マイクロニードルと、を備える、ステープラー。
(2) 前記マイクロニードルが、前記ステープル留め頭部の組織に面する表面上に、前記ステープル留め頭部の周縁にかつ前記展開可能なステープルの前記同心のアレイの外側に配置されている、実施態様1に記載のステープラー。
(3) 前記マイクロニードルが、その前記遠位端にかかりを更に含み、前記マイクロニードルが、前記組織に面する表面に実質的に垂直である、実施態様2に記載のステープラー。
(4) 前記マイクロニードルが、前記組織に面する表面上に配置された取り付けストリップによって解放可能に支持されている、実施態様2に記載のステープラー。
(5) 前記取り付けストリップが、圧縮可能な発泡体層であり、前記マイクロニードルが、前記圧縮可能な発泡体層内に埋め込まれている、実施態様4に記載のステープラー。
(7) 前記マイクロニードルが、前記組織層の接合後、前記組織層内に残されるように構成されている、実施態様2に記載のステープラー。
(8) 前記薬剤が、薬物、酵素、成長因子、抗炎症薬、血管拡張剤、ペプチド、タンパク質、栄養素、賦形剤、細胞、又はこれらの組み合わせを含む、実施態様1に記載のステープラー。

(10) 前記ステープル留め頭部が少なくとも10本のマイクロニードルを含み、前記マイクロニードルは、直径50マイクロメートル〜約1000マイクロメートル、及び長さ約100マイクロメートル〜約2500マイクロメートルである、実施態様2に記載のステープラー。
前記ステープラーは、対向するアンビルに接続可能なステープル留め頭部を有し、
前記ステープル留め頭部は、複数の展開可能なステープルを同心のアレイ内に収容し、
前記ステープラーは、前記ステープル留め頭部又は前記アンビルの上又は内に解放可能に配置された、薬剤を放出する複数の吸収性マイクロニードルを更に含み、
前記マイクロニードルは、鋭い組織貫通遠位端と近位端とを有する細長いロッドから構成され、
前記方法は、
a)前記ステープル留め頭部を第1の管状組織内に軸方向に位置付け、前記アンビルを第2の管状組織内に軸方向に位置付けて、前記ステープル留め頭部を前記アンビルと接続することと、
b)前記ステープル留め頭部を前記アンビルと接近させることによって、前記第1の組織及び前記第2の組織を前記ステープル留め頭部と前記アンビルとの間で圧縮することと、
c)前記マイクロニードルを前記第1の組織及び前記第2の組織の中へ展開させることと、
d)前記複数のステープルを前記組織層の中へ同時に又は連続的に展開させることによって、前記第1の組織及び前記第2の組織を閉鎖する又は接合することと、

(12) 前記マイクロニードルが、前記ステープル留め頭部の組織に面する表面上に、前記ステープル留め頭部の周縁にかつ前記展開可能なステープルの前記同心のアレイの外側に配置されている、実施態様11に記載の方法。
(13) 前記マイクロニードルが、その前記遠位端にかかりを更に含み、前記マイクロニードルが、前記組織に面する表面に実質的に垂直である、実施態様11に記載の方法。
(14) 前記マイクロニードルが、前記組織に面する表面上に配置された取り付けストリップによって解放可能に支持されている、実施態様11に記載の方法。
(15) 前記取り付けストリップが、圧縮可能な発泡体層であり、前記マイクロニードルが、前記圧縮可能な発泡体層内に埋め込まれている、実施態様14に記載の方法。
(17) 前記マイクロニードルが、前記組織層の接合後、前記組織層内に残されるように構成されている、実施態様11に記載の方法。
(18) 前記薬剤が、薬物、酵素、成長因子、抗炎症薬、血管拡張剤、ペプチド、タンパク質、栄養素、賦形剤、細胞、又はこれらの組み合わせを含む、実施態様11に記載の方法。
(19) 前記薬剤が、約2時間〜約4週間、より好ましくは約4時間〜約5日間、最も好ましくは約4時間〜約3日間の期間にわたって放出されている、実施態様11に記載の方法。
(20) 前記ステープル留め頭部が少なくとも10本のマイクロニードルを含み、前記マイクロニードルは、直径50マイクロメートル〜約1000マイクロメートル、及び長さ約100マイクロメートル〜約2500マイクロメートルである、実施態様11に記載の方法。
Claims (10)
- 組織の吻合接合用の円形外科用ステープラーであって、
a)対向するアンビルに接続されたステープル留め頭部であって、複数の展開可能なステープルを同心のアレイ内に収容する、ステープル留め頭部と、
b)薬剤を放出する複数の吸収性マイクロニードルであって、鋭い組織貫通遠位端と近位端とを有する細長いロッドから構成され、前記ステープル留め頭部又は前記アンビルの上又は内に解放可能に配置されている、マイクロニードルと、を備える、ステープラー。 - 前記マイクロニードルが、前記ステープル留め頭部の組織に面する表面上に、前記ステープル留め頭部の周縁にかつ前記展開可能なステープルの前記同心のアレイの外側に配置されている、請求項1に記載のステープラー。
- 前記マイクロニードルが、その前記遠位端にかかりを更に含み、前記マイクロニードルが、前記組織に面する表面に実質的に垂直である、請求項2に記載のステープラー。
- 前記マイクロニードルが、前記組織に面する表面上に配置された取り付けストリップによって解放可能に支持されている、請求項2に記載のステープラー。
- 前記取り付けストリップが、圧縮可能な発泡体層であり、前記マイクロニードルが、前記圧縮可能な発泡体層内に埋め込まれている、請求項4に記載のステープラー。
- 前記マイクロニードルが、前記組織に面する表面上に解放可能に配置されたバットレス又は組織厚さコンペンセーターによって支持されている、請求項2に記載のステープラー。
- 前記マイクロニードルが、前記組織層の接合後、前記組織層内に残されるように構成されている、請求項2に記載のステープラー。
- 前記薬剤が、薬物、酵素、成長因子、抗炎症薬、血管拡張剤、ペプチド、タンパク質、栄養素、賦形剤、細胞、又はこれらの組み合わせを含む、請求項1に記載のステープラー。
- 前記薬剤が、約2時間〜約4週間、より好ましくは約4時間〜約5日間、最も好ましくは約4時間〜約3日間の期間にわたって放出されている、請求項1に記載のステープラー。
- 前記ステープル留め頭部が少なくとも10本のマイクロニードルを含み、前記マイクロニードルは、直径50マイクロメートル〜約1000マイクロメートル、及び長さ約100マイクロメートル〜約2500マイクロメートルである、請求項2に記載のステープラー。
