Description
(1) 組織層を接合するための線形外科用ステープラーであって、
a)対向する第2の顎部に接続された第1の顎部に設置された使い捨てカートリッジであって、
組織切除スロットによって分離されたアレイ内に複数の展開可能なステープルを収容する、カートリッジと、
b)薬剤を放出する複数の吸収性マイクロニードルであって、鋭い組織貫通遠位端と近位端とを有する細長いロッドから構成され、
前記カートリッジの組織に面する表面上又は内に解放可能に配置され、前記マイクロニードルの前記近位端は前記カートリッジ上に支持されている、マイクロニードルと、を備える、ステープラー。
(2) 前記マイクロニードルが、前記カートリッジの周縁に、前記ステープルの前記アレイの外側かつ前記組織切除スロットに遠位に、位置付けられている、実施態様1に記載のステープラー。
(3) 前記マイクロニードルが、その前記遠位端にかかりを更に含み、前記マイクロニードルが、前記組織に面する表面に実質的に垂直である、実施態様1に記載のステープラー。
(4) 前記マイクロニードルが、前記組織に面する表面上に配置された取り付けストリップによって解放可能に支持されている、実施態様1に記載のステープラー。
(5) 前記取り付けストリップが、圧縮可能な発泡体層であり、前記マイクロニードルが、前記圧縮可能な発泡体層内に埋め込まれている、実施態様4に記載のステープラー。
(7) 前記マイクロニードルが、前記組織層の接合後、前記組織層内に残されるように構成されている、実施態様1に記載のステープラー。
(8) 前記薬剤が、薬物、酵素、成長因子、抗炎症薬、血管拡張剤、ペプチド、タンパク質、栄養素、賦形剤、細胞、又はこれらの組み合わせを含む、実施態様1に記載のステープラー。
(9) 前記薬剤が、約2時間〜約4週間、より好ましくは約4時間〜約5日間、最も好ましくは約4時間〜約3日間の期間にわたって放出されている、実施態様1に記載のステープラー。
(10) 前記カートリッジが少なくとも10本のマイクロニードルを含み、前記マイクロニードルは、直径50マイクロメートル〜約1000マイクロメートル、及び長さ約100マイクロメートル〜約2500マイクロメートルである、実施態様1に記載のステープラー。
a)前記接続された第1の顎部と前記第2の顎部との間に少なくとも2つの組織層を位置付けることと、
b)前記接続された第1の顎部と前記第2の顎部とを持ち寄せることによって、前記顎部間の前記組織層を圧縮することと、
c)それによって、前記マイクロニードルを前記組織層の中へ展開させることと、
d)前記複数のステープルを前記組織層の中へ同時に又は連続的に展開させることによって、前記組織層を閉鎖する又は接合することと、
e)前記顎部を開放し、前記カートリッジと共に前記線形ステープラーを前記組織層との接触から取り外し、前記薬剤が前記マイクロニードルに近接する及び/又は隣接する前記組織層の中へ経時的に放出されるように、前記マイクロニードルが前記組織層内に留まることを可能にすることと、を含む、方法。
(12) 前記マイクロニードルが、前記カートリッジの周縁に、前記ステープルの前記アレイの外側かつ前記組織切除スロットに遠位に、位置付けられている、実施態様11に記載の方法。
(13) 前記マイクロニードルが、その前記遠位端にかかりを更に含み、前記マイクロニードルが、前記組織に面する表面に実質的に垂直である、実施態様11に記載の方法。
(14) 前記マイクロニードルが、前記組織に面する表面上に配置された取り付けストリップによって解放可能に支持されている、実施態様11に記載の方法。
(15) 前記取り付けストリップが、圧縮可能な発泡体層であり、前記マイクロニードルが、前記圧縮可能な発泡体層内に埋め込まれている、実施態様14に記載の方法。
(17) 前記顎部を開放し、前記カートリッジと共に前記線形ステープラーが前記組織層との接触から取り外されると、前記マイクロニードルが前記カートリッジから分離するように構成され、前記マイクロニードルが、前記組織層の接合後、前記組織層内に留まり、前記マイクロニードルが、経時的に前記組織層内で完全に溶解する又は吸収される、実施態様11に記載の方法。
(18) 前記薬剤が、薬物、酵素、成長因子、抗炎症薬、血管拡張剤、ペプチド、タンパク質、栄養素、賦形剤、細胞、又はこれらの組み合わせを含む、実施態様11に記載の方法。
(19) 前記薬剤が、約2時間〜約4週間、より好ましくは約4時間〜約5日間、最も好ましくは約4時間〜約3日間の期間にわたって放出されている、実施態様11に記載の方法。

- 組織層を接合するための線形外科用ステープラーであって、
a)対向する第2の顎部に接続された第1の顎部に設置された使い捨てカートリッジであって、
組織切除スロットによって分離されたアレイ内に複数の展開可能なステープルを収容する、カートリッジと、
b)薬剤を放出する複数の吸収性マイクロニードルであって、鋭い組織貫通遠位端と近位端とを有する細長いロッドから構成され、
前記カートリッジの組織に面する表面上又は内に解放可能に配置され、前記マイクロニードルの前記近位端は前記カートリッジ上に支持されている、マイクロニードルと、を備える、ステープラー。 - 前記マイクロニードルが、前記カートリッジの周縁に、前記ステープルの前記アレイの外側かつ前記組織切除スロットに遠位に、位置付けられている、請求項1に記載のステープラー。

- 前記薬剤が、薬物、酵素、成長因子、抗炎症薬、血管拡張剤、ペプチド、タンパク質、栄養素、賦形剤、細胞、又はこれらの組み合わせを含む、請求項1に記載のステープラー。
- 前記薬剤が、2時間〜4週間の期間にわたって放出されている、請求項1に記載のステープラー。
