Description
本発明は、自己免疫疾患の診断方法、ペプチド、及び自己免疫疾患の処置に関する。
1型糖尿病(T1D)の生理病理学は、調節性T細胞(Treg細胞)ホメオスタシス及びそれ故に免疫寛容を損なうインターロイキン−2(IL−2)経路における複数の欠陥に関係する。
本発明者らは、中和能力を有する抗IL−2自己抗体(IL−2AAb)がT1Dに関連することを発見した。本発明者らは更に、T1Dだけではなく、全身性エリテマトーデス、関節リウマチ、シェーグレン症候群及び自己免疫性筋炎患者においても、IL−2自己抗体が高頻度で存在することを発見した。
自己免疫疾患を有する患者、特にT1D、全身性エリテマトーデス及び関節リウマチ患者は、高頻度のIL−2AAbを示す。したがって、hIL−2自己抗体は、このような疾患のバイオマーカーである。

− 例えば前記技術のために確立されたカットオフよりもELISAによって得られたAUの値が高い、抗IL−2抗体が存在する場合には、患者に投与すべきIL−2の量を、所望の免疫応答に応じて適合すべきである。

使用したヒト血清及び血漿サンプルを以下に説明する。
hIL−2AAbの血清力価をELISAによって評価した。マイクロタイター96ウェルプレート(Medisorp, Nunc)を、105IU/ml hIL−2を含有する100μl/ウェルのカーボネートコーティング緩衝液(「IL−2コーティングウェル」)又は緩衝液のみ(「非コーティングウェル」、ブランク)と共に4℃で一晩インキュベーションした。PBS/2%BSAで2時間ブロッキングした後、プレートを、連続希釈血清サンプル50μlと共に室温で2時間インキュベーションした。PBS/0.1%Tween20で十分に洗浄した後、HRP結合抗ヒトIgG(1:2,000;Dako)を各ウェルに追加し、プレートを室温で1時間維持した。前述のようにTMB基質を用いて、ペルオキシダーゼ活性を測定した。HRP結合ヤギ抗ラットIgで明らかにした2倍連続希釈のラット抗ヒトIL−2(クローンMQ1-17H12、eBioscience)を使用して、標準曲線を作成した。ブランクの控除後に得られたO.D.値を使用して、各サンプルの任意単位を計算した。ヒト競合アッセイには、hIL−2AAb−健常ドナー又はhIL−2AAb+患者由来の血清(1/100、1/200又は1/300希釈)を、漸増濃度のhIL−2と共に室温で1時間プレインキュベーションした。次いで、サンプルをELISAプレートに追加し、プレートを上記のように処理した。
異なる患者群における抗hIL−2 IgGの血清力価及び異なる患者群におけるhIL−2A陽性患者の割合の結果を図1a及び1bに示す(実施例の図5a、b)。左のグラフの破線は、陽性の閾値を示す。記号は個々の被験者を表し、横方向のバーは中央値である。**P<0.01;***P<0.001(フィッシャーの直接確率検定)。hIL−2AAbを定量するELISA試験には、本発明者らは、95%の特異性で健常ドナーとT1D被験者とを識別することを可能にする、陽性の閾値を24.3AUの値に固定した。患者としてコホート1、2及び3のT1D被験者(n=75);並びにコントロールとしてこれらのコホートの健常ドナー(n=103)を使用して、95%信頼区間のROC曲線を用いて、このカットオフを計算した(図1c〜d)。
異なるコホート間の抗hIL−2陽性被験者の割合の結果(右のグラフ)は、1型糖尿病では、IL−2自己抗体が高頻度で存在していることを証明している。

35%エタノールで活性化した後、96ウェルPVDFプレート(MAIP4510, Millipore)を、70μL/ウェルの5μg/mL mIL−2(Peprotech)によって4℃で一晩コーティングした。PBSで洗浄した後、プレートを、無タンパク質ブロッキング緩衝液(Thermo)によって室温で1時間ブロッキングし、次いで、完全RPMI培地によって室温で30分間ブロッキングした。10〜18週齢の雌性B6又はNODマウス由来の連続希釈脾臓細胞又は骨髄細胞(完全RPMI培地中、1ウェル当たり細胞5×104〜4×105個)をELISPOTプレートに追加した。一連の実験では、10〜18週齢の雌性B6又はNODマウス由来の脾臓細胞を、10μg/mL CpG−ODN 1018を含む完全RPMI培地中、細胞1×106個/mLで6日間培養して、記憶B細胞を増殖させた。次いで、連続希釈CpG前活性化脾細胞(1ウェル当たり細胞5×104〜4×105個)をELISPOTプレートに追加した。18時間培養した後、プレートをPBS/0.25%Tween−20で3回洗浄し、PBSで3回洗浄し、次いで、PBS/2%BSAで希釈したアルカリホスファターゼ−抗マウス−IgG(1:1,000;Sigma-Aldrich)と共に室温で2時間インキュベーションした。次いで、プレートを洗浄し、100μL/ウェルの基質(Bio-Rad)を追加して、ホスファターゼ活性を測定した。15分間インキュベーションした後、水道水で十分に洗浄することによって、反応をブロックした。AIDカメラを用いて、スポットをカウントした。
上記結果は、B細胞による抗IL−2抗体の産生を検出するために、ヒトでは、B細胞Elispotなどの技術を使用し得ることを証明している。
血清サンプルは、健常ドナー(HD、n=249)、T1D(プールした3つのコホートにおいて、n=75)、多発性硬化症(MS;n=33)、シェーグレン症候群(SJO;n=22)、抗JO1陽性多発性筋炎(JO1;n=16)、関節リウマチ(RA;n=33)、全身性エリテマトーデス(SLE;n=20)、慢性炎症性脱髄性多発神経障害(CIPD;n=51)及びガン(ガン;n=128)患者から得た。(a)異なるコホートにおける抗hIL−2 IgGの血清力価。破線は、陽性の閾値(実施例1と同じ値に設定)を示す。(b)異なるコホート間の抗hIL−2陽性被験者の割合。記号は個々の被験者を表し、横方向のバーは中央値である。*P<0.05;***P<0.001(フィッシャーの直接確率検定)。
SLE、RA、SJO及びJO−1を有する患者は高頻度のhIL−2Aを示すので、上記結果は、hIL−2Aがこのような疾患のバイオマーカーであることを証明している。
DMSO、mIL−2ペプチド(これは、最初のスクリーニングにおいて陽性応答をもたらした)(3及び10μmol/Lの各ペプチド)、P31ペプチド(3及び10μmol/L)又はaCD3−CD28コーティングビーズ(1ビーズ:1細胞の比)で72時間刺激した後、10〜18週齢の雌性B6(n=3)又は前糖尿病NOD(n=7)の脾臓細胞によるIFN−g産生を、CBAによって培養上清中で定量した。記号は、個々のマウスを表す。データは、2回の独立した実験の累積である。
NODマウスは、(2つの異なるペプチドに対して反応性の)IL−2特異的T細胞を示すので、上記結果は、(IL−2ペプチドによる再刺激後にIFNg CBAによって検出される)IL−2特異的T細胞をT1Dのバイオマーカーとして使用し得ることを証明している。
以下のヒトIL−2ペプチドのライブラリーを調製した:21。
T1D患者は、(1つのペプチドに対して特異的ではなく多くに対して反応性の)IL−2特異的T細胞を示すので、上記結果は、(IL−2ペプチドによる再刺激後にIFNg ELISPOTによって検出される)IL−2特異的T細胞をT1Dのバイオマーカーとして使用し得ることを証明している。
マウスを後眼窩洞から採血し、IL−2/IL−2AAb免疫複合体の血清力価をELISAによって定量した。マイクロタイター96ウェルプレート(Medisorp, Nunc)を、0.5μg/mLポリクローナル抗mIL−2(PeproTech)を含有する100μl/ウェルのカーボネートコーティング緩衝液(pH9.6)と共に4℃で一晩インキュベーションした。PBS/2%BSAで2時間ブロッキングした後、プレートを、2連の連続希釈血清50μlと共に室温で2時間インキュベーションした。PBS/0.1%Tween20で十分に洗浄した後、ビオチン標識抗マウスIgG(1:5,000;Southern Biotech)を各ウェルに追加し、プレートを室温で1時間維持した。続いて、プレートを、西洋ワサビペルオキシダーゼ(HRP)結合ストレプトアビジン(1:2,000;Invitrogen)と共に30分間インキュベーションし、続いて、TMB基質(eBioscience又はBD Biosciences)と共に10分間インキュベーションした。反応を酸でブロックし、DTX 880 Multimode Detector (Beckman Coulter)を用いて、吸光度を450nmで読み取った。
血清サンプルは、異なるマウス系統(全てが年齢適合の雌)から得た:B6、野生型NOD、NOD.Idd3B6、Il2−半接合NOD:NOD.Idd3NOD/NOD−IL−2null(NOD.Il2+/−)。異なるマウス系統におけるIL−2/IL−2AAb複合体の血清力価(b)。記号は個々のマウスを表し、横方向のバーは中央値である。ns、非有意。*P<0.05;**P<0.01;***P<0.001(ノンパラメトリックマンホイットニー検定)。
NODは、(ELISAによって検出されたように)高い力価のIL−2/IL−2A免疫複合体を示すので、上記結果は、(本実施例でELISAによって検出されたような)IL−2/IL−2A免疫複合体をT1Dのバイオマーカーとして使用し得ることを証明している。
リコンビナントmIL−2をカルボキシル化ビーズ(Bio-Rad Laboratories)に共有結合させた。最初に、ビーズを、製造業者の説明書にしたがって、N−ヒドロキシスクシンイミド(Thermo Fisher)の存在下、1−エチル−3−[3−ジメチルアミノプロピル]カルボジイミドヒドロクロリドによって活性化して、アミン反応性中間体を形成した。活性化ビーズを、回転下、反応混合物中で、10μg/mL mIL−2と共に室温で2時間インキュベーションした。次いで、ビーズを、製造業者の説明書にしたがってブロッキング及び保存した。市販の抗mIL−2モノクローナル抗体(クローンJES6-1A12、eBioscience)、ビオチン−抗ラットIg(BD Biosciences)及び次いでPE−ストレプトアビジン(Invitrogen)を使用して、カップリングを確認した。mIL−2結合ビーズを、暗所において、水平シェーカー上の96ウェルプレート中で、B6マウス又はNODマウス由来の連続希釈血清と共に室温で2時間インキュベーションした。ビーズをPBS/0.05%Tween−20で2回洗浄し、ビオチン標識抗マウスIgG抗体(1:250;Southern Biotech)と共に1時間インキュベーションし、洗浄し、PE−ストレプトアビジン(1:125;Invitrogen)と共に30分間インキュベーションし、再度洗浄し、PBS/0.05%Tween−20 100μLに再懸濁した。次いで、ビーズをLSRIIフローサイトメーター(BD Biosciences)によって分析し、データをFlowJoソフトウェアによって分析した。mIL−2AAb競合アッセイには、B6マウス又はNODマウス由来の血清(1/10希釈)を、漸増濃度のmIL−2と共に室温で2時間プレインキュベーションした。次いで、mIL−2コーティングビーズを追加し、上記のように、多重粒子ベースのフローサイトメトリーによって処理した。
多重粒子ベースのフローサイトメトリーは、特定のIL−2AAbの検出を可能にするので、上記結果は、(多重粒子ベースのフローサイトメーター、競合多重粒子ベースのフローサイトメトリーによって検出される)IL−2AAbをT1Dのバイオマーカーとして使用し得ることを証明している。
B6マウス又はNODマウス由来の熱不活性化(56℃で30分間)連続希釈血清を用いて又は用いずに、mIL−2不含、1ng/mL mIL−2含有又は3IU/mL hIL−2含有の完全RPMI培地(Gibco)中、96ウェルプレートにおいて、CTLL−2細胞(ATCC、マイコプラズマを含まない)を培養した(細胞104個/ウェル)。48時間後、培養物を[3H]−チミジン(1μCi/ウェル)で18時間パルスし、液体シンチレーションによってカウントした。
1ng/mL mIL−2及び異なる濃度のB6(黒丸)又はNOD(白丸)血清と共に3日間培養したCTLL−2細胞の増殖。増殖は、コントロール(マウス血清なしで1ng/mL mIL−2と共に3日間培養したCTLL−2)に対する割合として表されている。記号及び曲線は個々のマウスを表し、横方向のバーは中央値である。データは、少なくとも2回の独立した実験の累積である。
CTLL−2ベースの中和アッセイは、中和IL−2AAbの検出を可能にするので、上記結果は、(in vitro中和アッセイによって検出される)中和IL−2AAbをT1Dのバイオマーカーとして使用し得ることを証明している。
12アミノ酸が重複し、(シグナルペプチドを含む)mIL−2配列全体をカバーする15merのペプチドライブラリーを作製した(GL-Biochem)。ペプチド(10mmol/L)を使用まで−20℃のDMSO中で保存した。10〜18週齢の雌性NODマウス由来の脾細胞を、DMSO(ネガティブコントロール)、aCD3−CD28ビーズ(ポジティブコントロール、1ビーズ:1細胞の比、Life Technologies)又はペプチド(10μmol/L)を含有するX-Vivo 15無血清培地(Lonza)中、3連で培養した(細胞4×105個/150μL/ウェル)。96時間後、培養物を[3H]−チミジン(1μCi/ウェル)で18時間パルスし、液体シンチレーションによってカウントした。
DMSO、mIL−2ペプチド(これは、最初のスクリーニングにおいて陽性応答をもたらした)(10μmol/L)又はaCD3−CD28コーティングビーズ(1ビーズ:1細胞の比)による96時間の刺激後、10〜18週齢の雌性前糖尿病NOD(n=3)脾細胞による増殖(cpm)をチミジン取り込みによって定量した。
NODマウスは、(2つの異なるペプチドに対して反応性の)IL−2特異的T細胞を示すので、上記結果は、(IL−2ペプチドによる再刺激後にチミジン増殖アッセイによって検出される)IL−2特異的T細胞をT1Dのバイオマーカーとして使用し得ることを証明している。
hIL−2AAb−健常ドナー又はahIL−2AAb+T1D患者由来の熱不活性化(56℃で30分間)血清(1/10希釈)を用いて又は用いずに、1IU/mL hIL−2を含有する無SVF RPMI培地(Gibco, France)中、75μL/ウェルで、96ウェルプレートにおいて、健常ドナー由来のPBMCを培養した。5分間の刺激後、培養物を、225μL/ウェルのPBS/2%ホルムアルデヒドによって室温で10分間固定した。PBS/0.2%BSAで洗浄した後、細胞を、100μL/ウェルの氷冷メタノールによって氷上で10分間透過処理した。次いで、細胞をPBS/0.2%BSAで洗浄し、抗CD3 PE−Cy7(クローンUCHT1;1:200;Beckman-Coulter)、抗CD4 PerCP(クローンRPA-T4 1:100;Ozyme)、抗CD25 PE(クローンM-A251;1:5;BD Biosciences及びクローン3G10;1:10;Miltenyi)、抗Foxp3 Alexa488(クローン236A/E7;1:20;eBiosciences)及び抗pSTAT5 Alexa647(クローン47/Stat5(pY694);1:20;BD Biosciences)によって4℃で45分間染色した。細胞を、LSRII又はFortessaフローサイトメーターによって取得し、FlowJoソフトウェアを用いて分析した。
上記結果は、抗IL−2自己抗体がin vitro中和活性を有し得ることを証明している。
ISA(SEPPIC, Paris)50%と、KLH(100μg/用量)にカップリングされたヒトIL2由来の合成ペプチドLTRMLTFKFYMPKKAの水溶液50%とから構成される油中水型エマルジョンから、ワクチン調製物を製造した。
Claims (11)
- 患者が自己免疫疾患を有するか又は自己免疫疾患を有する若しくは発症するリスクがあるかどうかを決定するための、或いは、自己免疫疾患の重症度を評価する又は自己免疫疾患の転帰を予測するためのin vitro方法であって、患者から得られた生物学的サンプルにおける免疫抗IL−2応答を検出又は定量する工程を含み、ここで
自己免疫疾患は、全身性エリテマトーデス(SLE)、関節リウマチ(RA)、シェーングレン症候群(SJO)及び自己免疫性多発性筋炎(JO1)からなる群より選択される、in vitro方法。 - −抗IL−2自己抗体を産生するB細胞;
−抗IL−2抗体;
−IL−2特異的T細胞
の1つ以上の検出又は定量によって、免疫抗IL2応答が証明される、請求項1に記載のin vitro方法。 - 抗IL−2自己抗体を産生するB細胞又は抗IL−2抗体の検出又は定量によって、免疫抗IL2応答が証明され、該自己抗体が中和抗IL−2自己抗体である、請求項1又は2に記載のin vitro方法。
- 患者が自己免疫疾患を有するか、又は自己免疫疾患を有する若しくは発症するリスクがあるかどうかを決定するための方法である、請求項1〜3のいずれか一項に記載のin vitro方法。
- 自己免疫疾患の重症度を評価するための方法である、請求項1〜4のいずれか一項に記載のin vitro方法。
- 自己免疫疾患の転帰を予測するための方法である、請求項1〜5のいずれか一項に記載のin vitro方法。
- 治療目的で外因性投与されたIL−2に対する応答を予測/調整するための方法である、請求項1〜6のいずれか一項に記載のin vitro方法。
- 1型糖尿病(T1D)、全身性エリテマトーデス、関節リウマチ、シェーグレン症候群及び自己免疫性多発性筋炎患者の抗IL2抗体又はIL−2特異的T細胞によって特異的に認識されるペプチドであって、アミノ酸配列が、
LTRMLTFKFYMPKKA(配列番号1)、又は
EFLNRWITFSQSIIS(配列番号2)
からなる、ペプチド。 - 生物学的サンプルにおけるT細胞媒介性免疫応答を検出するためのin vitro方法であって、請求項8に記載のペプチドを検出に使用する、in vitro方法。
