Description
別の一実施形態では前述の埋込型医療装置において、プロセッサーはさらに、P波の検出に応じて初期検出を偽陽性検出として識別し、初期検出が偽陽性検出として識別されたことに応じて調節カウントをインクリメントし、調節カウントに応じて適応閾値を判定するよう構成される。
別の一実施形態では前述の埋込型医療装置において、埋込型医療装置は埋込型心臓モニターであるか、または埋込型心臓除細動器である。
本明細書に記載の方法は、埋込型医療装置または外部医療装置におけるソフトウェア、ハードウェアまたはファームウェアにおいて具現化され得る。そのような装置の例として心臓EGM/ECGモニタリング能力および関連するEGM/ECG感知電極を有する埋込型モニタリング装置が挙げられ、電極は心臓内電極、心外膜電極、または皮下電極であってもよい。
AF証拠=不規則性証拠−原初カウント−PAC証拠
によって定義され得、式中、「不規則性証拠」は、ローレンツプロット区域の原点を中心に定義されるゼロセグメント外での占有状態のヒストグラムビンの数を指す。正常洞調律または高度に規則的な心房頻脈の間、連続するRRI間での差が比較的小さく、一貫していることから、ほぼすべてのポイントがゼロセグメントに該当する。したがって、ゼロセグメント外での占有状態ヒストグラムビンの数が多いと、これはAFの肯定的証拠である。

検出された心房不整脈事象の持続時間が持続時間閾値よりも長いか否かを判定するステップと、
検出された心房不整脈事象の持続時間が持続時間閾値よりも長いことに応じて調節カウントをゼロに設定するステップと
をさらに含む、方法。
感知された心臓信号に関連するRR間隔変動を判定するステップと、
RR間隔変動に応じて心房不整脈スコアを判定するステップと、
スコアが心房不整脈閾値を超えるか否かを判定するステップと、
正常洞カウントをインクリメントするステップと、
正常洞カウントが正常洞カウント閾値を超えるか否かを判定するステップと、
正常洞カウントが正常洞カウント閾値を超えていることに応じて調節カウントをゼロに設定するステップと

形態1または2に記載の埋込型医療装置において、前記プロセッサーがさらに、P波の検出に応じて初期検出を偽陽性検出として識別し、前記初期検出が偽陽性検出として識別されたことに応じて調節カウントをインクリメントし、前記調節カウントに応じて前記適応閾値を判定するように構成された、装置。

形態3に記載の埋込型医療装置において、前記プロセッサーがさらに、前記適応閾値を、前記調節カウントと、前記心房不整脈事象の不適切な判定を不合格とするための望ましい攻撃性水準に相当する調整係数との比率に等しくなるように設定するように構成された、装置。

形態3に記載の埋込型医療装置において、前記プロセッサーが、前記適応閾値が最大適応閾値を超えるか否かを判定し、前記適応閾値が前記最大適応閾値を超えていないことに応じて前記心房不整脈閾値に前記適応閾値を加算するように構成された、装置。

形態7に記載の埋込型医療装置において、前記プロセッサーが、感知された心臓信号に関連するRR間隔変動を判定し、前記RR間隔変動に応じて心房不整脈スコアを判定し、前記スコアが前記心房不整脈閾値を超えるか否かを判定し、正常洞カウントをインクリメントし、前記正常洞カウントが正常洞カウント閾値を超えるか否かを判定し、前記正常洞カウントが前記正常洞カウント閾値を超えていることに応じて前記調節カウントをゼロに設定するように構成された、装置。

Claims (5)
- 心房不整脈事象を判定するための埋込型医療装置であって、
回路機構を内蔵するハウジングを含む心臓用感知装置と、
心臓信号を感知する前記回路機構に電気的に連結された電極と、
心房不整脈閾値に応じて心房不整脈事象の初期検出を生成し、前記初期検出中におけるP波発生の有無を判定し、P波の検出に応じて適応閾値を判定し、前記適応閾値に応じて前記心房不整脈閾値を調節し、調整後の心房不整脈閾値を使用して後続の心房不整脈事象の初期検出を生成するように構成されたプロセッサーと
を含み、
前記プロセッサーがさらに、P波の検出に応じて初期検出を偽陽性検出として識別し、前記初期検出が偽陽性検出として識別されたことに応じて調節カウントをインクリメントし、前記調節カウントに応じて前記適応閾値を判定するように構成された、装置。 - 請求項1に記載の埋込型医療装置において、前記心房不整脈事象が心房細動事象または心房頻脈事象を含む、装置。
- 請求項1に記載の埋込型医療装置において、前記プロセッサーがさらに、前記適応閾値を、前記調節カウントと、前記心房不整脈事象の不適切な判定を不合格とするための望ましい攻撃性水準に相当する調整係数との比率に等しくなるように設定するように構成された、装置。
- 請求項1から3のいずれか一項に記載の埋込型医療装置において、前記埋込型医療装置が埋込型心臓モニターであるか、または埋込型心臓除細動器である、装置。
- 請求項4に記載の埋込型医療装置において、前記埋込型医療装置が第1および第2の皮下電極を備えた埋込型心臓モニターである、装置。
