Description
本出願は、2008年4月14日出願の米国特許仮出願第61/044,779号の優先権を主張するものであり、その全内容を参照により本明細書に組み込む。
1)ICG結合血液が組織中に存在する量。
2)ICG結合血液が組織中に存在する時間。
3)ICG結合血液が組織中を移動する速度。
4)ボーラス注入後、解剖学的領域が発光する順序。
時間積分蛍光を経過時間で除算して、平均蛍光を計算すること。
蛍光の増大/流出率を求めること。
ピーク蛍光に達する経過時間を求めること。
吻合開通、ならびに動脈流および静脈流の確認。これは、潜在的に皮弁不全をなくす結果を向上させ得るものであり、皮弁不全は、動脈流が低下し、灌流(perfusion)が不十分となり、静脈還流もやはり低下して、鬱血が生じる結果として生じる可能性がある。
1)画像シーケンス中の全画像の全ピクセルについて、積算強度を時間窓にわたって計算する。
2)積算強度を、選択したラベルの領域(例えば、5×5ピクセル正方行列)にわたって平均する。
3)平均強度を、全画像の積算強度の最大値に正規化する。
4)正規化した平均を、伝達関数の最大値が100%を表すようにスケーリングする。
Claims (14)
- 被検体の組織の組織灌流を評価する装置の作動制御方法であって、
蛍光応答を検出する手段が、インドシアニングリーン(ICG)による、前記組織からの蛍光応答を検出するステップと、
時間的な画像シーケンスを取得する手段が、ある期間にわたって前記蛍光応答の時間的な画像シーケンスを取得するステップと、
処理する手段が、前記組織の前記時間的な画像シーケンスにおける各ピクセルを独立に処理し、複数の画像に基づいて各ピクセルについての算出値を算出するステップであって、前記算出値が、
前記時間的な画像シーケンスの各フレーム中に取得された各ピクセル強度を、ICG蛍光の観察の開始からの経過時間で除算し、前記除算されたピクセル強度を加算することにより算出された時間積分強度値、
各ピクセルについての強度の増大率値、
各ピクセルにおけるピーク蛍光値、又は
それぞれのピクセルが他のピクセルに対してピーク蛍光強度に達する順序を表す最大蛍光までの経過時間値、である、ステップと、
複数の前記算出値を表示する手段が、複数の前記算出値を表示するステップであって、前記複数の前記算出値は、前記組織における組織灌流を表し、前記複数の前記算出値は、画像、被検体の解剖学的画像上の半透明オーバーレイ、等高線図、起伏図、またはそれらの組み合わせとして表示される、ステップと、を含む方法。 - 前記組織は、穿通枝皮弁を含む請求項1に記載の方法。
- 前記各ピクセルについての強度の増大率値を算出することが、各ピクセルについて、経過時間に対するピクセル強度の傾きを算出することを含む、請求項1又は2に記載の方法。
- (i)前記表示された複数の前記算出値の視覚差を強調する、(ii)前記表示された複数の前記算出値を、異なる灌流特性を異なる色で表す画像に変換する、又は(i)と(ii)の組み合わせを達するために、前記処理する手段が、前記複数の前記算出値のうち各ピクセルについての前記算出値にコントラスト伝達関数を適用するステップをさらに含む、請求項1乃至3のいずれか1項に記載の方法。
- 前記コントラスト伝達関数は、線形コントラスト伝達関数または非線形コントラスト伝達関数である、請求項4に記載の方法。
- 前記非線形コントラスト伝達関数は、異なる傾きの領域を有する関数であり、前記異なる傾きと、前記異なる傾き間の移行とを、組織灌流を評価するために請求項1乃至4のいずれか1項に記載の方法を実行する間に調節する、請求項5に記載の方法。
- (i)前記表示する手段が、標的領域内の組織灌流の量的表現を表示するステップ、又は(ii)前記表示する手段が、前記被検体の2つ以上の解剖学的位置の標的領域内の組織灌流の量的表現を同時に表示するステップをさらに含む、請求項1乃至6のいずれか1項に記載の方法。
- インドシアニングリーン(ICG)のボーラスが血流に加えられた被検体の組織の組織灌流を評価するためのシステムであって、
前記ICGによる、前記組織からの蛍光応答を検出する検出手段と、
ある期間にわたって前記蛍光応答の時間的な画像シーケンスを取得する取得手段と、
前記組織の前記時間的な画像シーケンスにおける各ピクセルを独立に処理し、複数の画像に基づいて各ピクセルについての算出値を算出する処理手段であって、前記算出値が、
前記時間的な画像シーケンスの各フレーム中に取得された各ピクセル強度を、ICG蛍光の観察の開始からの経過時間で除算し、前記除算されたピクセル強度を加算することにより算出された時間積分強度値、
各ピクセルについての強度の増大率値、
各ピクセルにおけるピーク蛍光値、又は
それぞれのピクセルが他のピクセルに対してピーク蛍光強度に達する順序を表す最大蛍光までの経過時間値、である、処理手段と、
複数の前記算出値を表示する表示手段と、を含み、
前記複数の前記算出値は、前記組織における組織灌流を表し、画像、被検体の解剖学的画像上の半透明オーバーレイ、等高線図、起伏図、またはそれらの組み合わせとして表示される、システム。 - 前記組織は、穿通枝皮弁を含む請求項8に記載のシステム。
- 前記各ピクセルについての強度の増大率値を算出することが、各ピクセルについて、経過時間に対するピクセル強度の傾きを算出することを含む、請求項8又は9に記載のシステム。
- (i)前記表示された複数の前記算出値の視覚差を強調する、(ii)前記表示された複数の前記算出値を、異なる灌流特性を異なる色で表す画像に変換する、又は(i)と(ii)の組み合わせを達するために、前記処理手段が、前記複数の前記算出値のうち各ピクセルについての前記算出値にコントラスト伝達関数を適用する、請求項8乃至10のいずれか1項に記載のシステム。
- 前記コントラスト伝達関数は、線形コントラスト伝達関数または非線形コントラスト伝達関数である、請求項11に記載のシステム。
- 前記非線形コントラスト伝達関数は、異なる傾きの領域を有する関数であり、前記異なる傾きと、前記異なる傾き間の移行とを、請求項8乃至11のいずれか1項に記載の組織灌流を評価するためのシステムを使用する間に調節する、請求項12に記載のシステム。
- 前記表示手段は、(i)標的領域内の組織灌流の量的表現を表示する、又は(ii)前記被検体の2つ以上の解剖学的位置の標的領域内の組織灌流の量的表現を同時に表示するようにさらに構成されている、請求項8乃至13のいずれか1項に記載のシステム。
