本開示の一態様は、アセトアミノフェン-誘導性毒性を診断するためのAPAP-タンパク質付加物を提供する。アセトアミノフェン-誘導性毒性は、対象体の体内における反応的な代謝物であるN-アセチル-p-ベンゾキノンイミン(NAPQI)へのアセトアミノフェンの変換が介在する。NAPQIは、主に肝臓においてAPAP-タンパク質付加物を形成するタンパク質またはペプチドのシステイン基に共有結合し、そしてアセトアミノフェンを代謝できる他の組織においてより低い程度に共有結合する。これらのAPAP-タンパク質付加物は、APAP環のアセトアミド基のメタ位かつフェノール基のオルト位と共有結合するシステインの硫黄基を有し、3-(システイン-S-イル)APAP(3-Cys-A)-タンパク質付加物とも呼ばれる。NAPQIは、肝臓の天然の抗酸化剤グルタチオンを激減させ、直接的に肝臓の細胞を損傷させ、肝不全および循環系へのAPAP-タンパク質付加物の放出の原因となる。
他の態様において、本開示は、対象体においてアセトアミノフェン-誘導性毒性を検出する方法を包含する。本開示の方法は、対象体から生体試料を得ること、および一定量の試料をインビトロで処理して、1以上のAPAP-タンパク質付加物を検出することを含み、ここで各APAP-タンパク質付加物は、NAPQIで修飾されたタンパク質を含み、ここで該タンパク質は、ベタイン-ホモシステイン-S-メチルトランスフェラーゼ1、細胞質型アスパラギン酸アミノトランスフェラーゼ、1,4-α-グルカン-分枝酵素、ホルムイミドイルトランスフェラーゼ-シクロデアミナーゼまたはジストロフィンからなる群より選択される。
本明細書で用いる用語「対象体」は、アセトアミノフェンを投与され得る生物体を意味する。適切な対象体としては、ヒト、家畜動物、コンパニオン・アニマル、実験動物および動物園の動物が挙げられるが、これらに限定されない。一実施態様において、対象体は、げっ歯動物、例えばマウス、ラット、モルモットなどであり得る。別の一実施態様において、対象体は、家畜動物であり得る。適切な家畜動物の制限されない例としては、ブタ、ウシ、ウマ、ヤギ、ヒツジ、ラマおよびアルパカが挙げられ得る。またさらに別の一実施態様において、対象体は、コンパニオン・アニマルであり得る。コンパニオン・アニマルの制限されない例としては、ペット、例えばイヌ(dog)、ネコ(cat)、ウサギおよびトリが挙げられ得る。またさらに別の一実施態様において、対象体は、動物園の動物であり得る。本明細書で用いる「動物園の動物」は、動物園で見られ得る動物を意味する。該動物としては、ヒト以外の霊長動物、大型ネコ、オオカミおよびクマが挙げられ得る。特定の実施態様において、動物は、実験動物である。実験動物の制限されない例としては、げっ歯動物、イヌ科(canine)、ネコ科(feline)およびヒト以外の霊長動物が挙げられ得る。ある特定の実施態様において、動物は、げっ歯動物である。げっ歯動物の制限されない例としては、マウス、ラット、モルモットなどが挙げられ得る。好ましい実施態様において、対象体は、ヒトである。対象体は、新生児、青年、成体、中年または高年を含む、任意の年齢であり得る。
本開示の方法は、対象体から生体試料を得ることを部分的に含む。本明細書で用いる用語「生体試料」は、対象体から得た試料を意味する。アセトアミノフェン-タンパク質付加物を含む任意の生体試料が適切である。当該技術分野において多数のタイプの生体試料が知られている。適切な生体試料としては、毛髪、組織試料または体液が挙げられ得るが、これらに限定されない。いくつかの実施態様において、生体試料は、組織試料、例えば組織生検である。組織生検は、肝臓組織の生検であり得る。生検組織を固定し、パラフィンまたはプラスチックに包埋し、切断し得るか、または生検組織を凍結し、低温切片化し得る。別法として、生検組織を個々の細胞または外植片に処理し得るか、またはホモジェネート、細胞抽出物、膜画分またはタンパク質抽出物に処理し得る。他の実施態様において、試料は体液であり得る。適切な体液の制限されない例としては、血液、血漿、血清、尿、唾液、精液、汗、涙液、粘液、痰、組織溶解物または他の排泄物(例えば糞便)が挙げられる。特定の実施態様において、体液は尿である。別の特定の実施態様において、体液は血漿である。また別の特定の実施態様において、体液は血清である。さらにまた別の特定の実施態様において、体液は唾液である。異なる実施態様において、生体試料は毛髪である。当該液体は、「そのまま(as is)」用いられ得るか、細胞成分は液体から単離され得るか、またはタンパク質画分は標準方法を用いて当該液体から単離され得る。
本開示の方法は、対象体からの試料中の1以上のAPAP-タンパク質付加物を検出することを含む。本明細書で用いる用語「APAP-タンパク質付加物を検出すること」は、APAP-タンパク質付加物の存在を検出するか、または対象体からの試料中のAPAP-タンパク質付加物の存在および濃度もしくは量を検出するために、用いられ得る。特定の実施態様において、本開示の方法は、1以上のAPAP-タンパク質付加物を検出することを含み、ここで各APAP-タンパク質付加物は、NAPQIで修飾されたタンパク質を含み、ここで該タンパク質は、ベタイン-ホモシステイン-S-メチルトランスフェラーゼ1、細胞質型アスパラギン酸アミノトランスフェラーゼ、1,4-α-グルカン-分枝酵素、ホルムイミドイルトランスフェラーゼ-シクロデアミナーゼまたはジストロフィンからなる群より選択される。
ある態様において、本発明は、対象体から得た生体試料中の測定されたアセトアミノフェン-タンパク質付加物の量に基づいて対象体を分類する方法を提供する。当該方法は、一般的に(i)対象体から生体試料を得て、試料中のアセトアミノフェン-タンパク質付加物の量を測定すること、(ii)試料中のアセトアミノフェン-タンパク質付加物の量を基準値と比較すること、および(iii)試料中の測定されたアセトアミノフェン-タンパク質付加物の量に基づいて、高量または低量のアセトアミノフェン-タンパク質付加物の量を有するとして対象体を分類することを含む。上記方法において、1以上のアセトアミノフェン-タンパク質付加物を測定し得る。例えば、1、2、3、4、5、6、7、8、9または10個のアセトアミノフェン-タンパク質付加物を測定し得る。対象体から生体試料を得て試料中のアセトアミノフェン-タンパク質付加物の量を測定するための方法は、上に詳細に記載されている。好ましい実施態様において、生体試料は、血液、血漿、血清、尿および唾液からなる群より選択される生体液である。
他に定義されない限り、本明細書で用いるすべての技術的および科学的用語は、当業者に普通に解されるのと同じ意味を有する。すべての特許、出願、公表された出願および他の刊行物は、出典明示によりその全体として本明細書の一部とする。本明細書中の用語について複数の定義がある場合、他に断らない限りこのセクション中のものを優先する。
アセトアミノフェン(APAP)は、今日世界中で疼痛および発熱の処置のために用いられる最も一般的な薬物であり、米国において急性肝不全の主要原因でもある。APAP毒性の初期は、よく特徴付けられており、化学上反応的な代謝物であって細胞タンパク質に共有結合するN-アセチル-p-ベンゾキノンイミン(NAPQI)への親薬物の生体内変化を含む。肝臓のグルタチオン(GSH)のシステイニルチオールに結合することによりNAPQIを無毒化する。有毒なAPAP曝露において、GSHの蓄えは消費され、細胞タンパク質のシステイニルチオールに結合するNAPQIの量が増加し、様々なAPAP-タンパク質付加物を生成する。毒性は、肝細胞の溶解および血清への修飾タンパク質の放出で生じる。ヒトにおいてNAPQIにより修飾されたタンパク質は、これまで報告されていなかった。
特定のNAPQI修飾タンパク質、例えば実施例1に記載のタンパク質が特異的に検出され得るかを試験するために、合成的に製造したNAPQIとオバルブミン(OA)およびウシ血清アルブミン(BSA)タンパク質を反応させ、MS/MSを用いるプロテオミクス評価を実施した。MS/MSプロテオーム解析は、NAPQI(149)の分子量により増加されたタンパク質の分子量を決定でき、特定の1または複数の修飾部位を決定した。MS/MSプロテオーム解析はまた、2以上のNAPQI(149n、nはタンパク質に付加したNAPQIの数である)の分子量により増加されたタンパク質の分子量を決定できた。さらに、発明者により開発された抗付加抗体によって特異的に修飾された付加タンパク質(OA-APAPおよびBSA-APAP)を認識し、それ故にイムノアッセイによって定量できた。
実施例1に記載するNAPQI付加タンパク質の同定は、アセトアミノフェン毒性のための特定のイムノアッセイの開発を可能にし得る。本開示は、1または複数の付加タンパク質を捕捉および単離するためのタンパク質特異的抗体の使用、ならびにタンパク質の付加を検出するためのアセトアミノフェン-システイン付加物(付加物全体)に特異的な第2のイムノアッセイの利用に基づき得る。
Claims (10)
- 単離アセトアミノフェン(APAP)-タンパク質付加物であって、該APAP-タンパク質付加物は、N-アセチル-p-ベンゾキノンイミン(NAPQI)が共有結合したタンパク質を含み、該タンパク質は、ベタイン-ホモシステイン-S-メチルトランスフェラーゼ1、細胞質型アスパラギン酸アミノトランスフェラーゼ、1,4-α-グルカン-分枝酵素、ホルムイミドイルトランスフェラーゼ-シクロデアミナーゼまたはジストロフィンからなる群より選択される、APAP-タンパク質付加物。
- 生体試料におけるアセトアミノフェン-タンパク質付加物の量を測定するための方法であって、
1以上のAPAP-タンパク質付加物を検出することにより対象体由来の生体試料中のアセトアミノフェン-タンパク質付加物の量を測定すること、ここで各APAP-タンパク質付加物は、NAPQIが共有結合したタンパク質を含み、該タンパク質は、ベタイン-ホモシステイン-S-メチルトランスフェラーゼ1、細胞質型アスパラギン酸アミノトランスフェラーゼ、1,4-α-グルカン-分枝酵素、ホルムイミドイルトランスフェラーゼ-シクロデアミナーゼまたはジストロフィンからなる群より選択される
を含む方法。 - 生体試料が、血液、血漿、血清、尿、唾液および毛髪からなる群より選択される生体液である、請求項2に記載の方法。
- 対象体におけるアセトアミノフェン-誘導性毒性を検出する方法であって、
a) 1以上のAPAP-タンパク質付加物を検出することにより対象体由来の生体試料中のアセトアミノフェン-タンパク質付加物の量を測定すること、ここで各APAP-タンパク質付加物は、NAPQIが共有結合したタンパク質を含み、該タンパク質は、ベタイン-ホモシステイン-S-メチルトランスフェラーゼ1、細胞質型アスパラギン酸アミノトランスフェラーゼ、1,4-α-グルカン-分枝酵素、ホルムイミドイルトランスフェラーゼ-シクロデアミナーゼまたはジストロフィンからなる群より選択される;および
b) 試料中のアセトアミノフェン-タンパク質付加物の量を基準値と比較すること、ここで、基準値と比較してより大きい試料中のアセトアミノフェン-タンパク質付加物の量は、対象体におけるアセトアミノフェン-誘導性毒性を示す
を含む方法。 - アセトアミノフェン-誘導性毒性が、アセトアミノフェンの過剰摂取に直接的または間接的に関連する、請求項4に記載の方法。
- アセトアミノフェン-誘導性毒性が、肝毒性である、請求項4に記載の方法。
- 生体試料が、血液、血漿、血清、尿、唾液および毛髪からなる群より選択される生体液である、請求項4に記載の方法。
- 生体試料が、未知の病因の肝毒性を有する対象体からである、請求項4に記載の方法。
- 対象体における肝毒性がアセトアミノフェン-誘導性毒性によるものかを決定する方法であって、
a) 1以上のAPAP-タンパク質付加物を検出することにより肝毒性の症状を有する対象体由来の生体試料中のアセトアミノフェン-タンパク質付加物の量を測定すること、ここで各APAP-タンパク質付加物は、NAPQIが共有結合したタンパク質を含み、該タンパク質は、ベタイン-ホモシステイン-S-メチルトランスフェラーゼ1、細胞質型アスパラギン酸アミノトランスフェラーゼ、1,4-α-グルカン-分枝酵素、ホルムイミドイルトランスフェラーゼ-シクロデアミナーゼまたはジストロフィンからなる群より選択される;および
b) アセトアミノフェン-タンパク質付加物が存在するかを決定すること、ここで、アセトアミノフェン-タンパク質付加物が存在しない場合、対象体における肝毒性はアセトアミノフェン-誘導性毒性によるものではなく、そして1以上のアセトアミノフェン-タンパク質付加物が存在する場合、試料中の1以上のアセトアミノフェン-タンパク質付加物の量を基準値と比較し、基準値と比較してより大量の試料中のアセトアミノフェン-タンパク質付加物は、対象体における肝毒性がアセトアミノフェン-誘導性毒性によるものであることを示す
を含む方法。 - 対象体におけるアセトアミノフェン-誘導性毒性を検出する方法であって、
a) 対象体におけるAPAP-タンパク質付加物のプロファイルを決定するために対象体由来の生体試料中のアセトアミノフェン-タンパク質付加物の量を測定すること、ここで該プロファイルは、対象体からの試料における1以上のAPAP-タンパク質付加物のアイデンティティおよび濃度を含み、各APAP-タンパク質付加物は、NAPQIが共有結合したタンパク質を含み、該タンパク質は、ベタイン-ホモシステイン-S-メチルトランスフェラーゼ1、細胞質型アスパラギン酸アミノトランスフェラーゼ、1,4-α-グルカン-分枝酵素、ホルムイミドイルトランスフェラーゼ-シクロデアミナーゼまたはジストロフィンからなる群より選択される;
b) アセトアミノフェン毒性と相関する1以上のAPAP-タンパク質付加物の存在および濃度を含むデータベースと(b)で決定したプロファイルを比較すること;
c) 1以上のAPAP-タンパク質付加物のアイデンティティおよび濃度が試料中の1以上のAPAP-タンパク質付加物のアイデンティティおよび濃度とマッチするデータベースのマッチするエントリを特定すること;および
d) マッチするエントリの特定のアセトアミノフェン毒性を含むアセトアミノフェン毒性を決定すること
を含む方法。
