Description
本発明は、とりわけ癌、特に、PI3K-AKT-mTOR経路の上方調節を示す癌を治療する方法を提供する。一態様において、本方法はそれを必要とする被験者に、治療上有効な用量のアウリスタチン系薬物複合体とPI3K-AKT-mTOR経路インヒビターを投与するステップを含む。いくつかの実施形態において、アウリスタチン系薬物複合体とPI3K-AKT-mTOR経路インヒビターの投与は、癌の治療に相乗効果を与える。いくつかの実施形態において、アウリスタチン系薬物複合体とPI3K-AKT-mTOR経路インヒビターの投与は、腫瘍細胞の殺滅および/または腫瘍細胞の増殖阻害に相乗効果を与える。

本発明においては、抗体薬物複合体中のリガンドユニット(例えば、抗体)が癌細胞の表面上にある癌細胞抗原と特異的に結合する。
薬物または抗体薬物複合体が細胞に細胞分裂停止および/または細胞傷害性効果を及ぼすかどうかを確認する方法は公知である。一般に、抗体薬物複合体の細胞分裂停止活性を測定する方法は、抗体薬物複合体の標的タンパク質を発現する細胞を細胞培地に曝すステップ;前記細胞を約6時間〜約5日の間細胞を培養するステップ;および細胞生存率を測定するステップより成る。細胞に基づくin vitroアッセイを用いて、抗体薬物複合体の生存率(増殖)、細胞障害性、およびアポトーシス(カスパーゼ活性化)の誘導を測定することができる。
mTORは2つの型の複合体、raptorサブユニットを含有するmTORClとrictorサブユニットを含有するmTORC2で存在する。当技術分野では公知のように、「rictor」はヒト遺伝子遺伝子座5pl3.1を有する細胞増殖調節タンパク質を意味する。これらの複合体は異なるやり方で調節されかつ異なる基質スペクトルを有する。

本発明のさらなる理解を助けるために実施例を提供する。用いた特別な材料、プロトコルおよび条件は本発明のさらなる説明を意図しており、本発明の合理的な見方を限定すると解釈してはならない。
786-O細胞はRoswell Park Memorial Institute 培地(RPMI)1640+10%ウシ胎児血清(FBS)で培養した。Caki-1細胞はMcCoy's 5A培地+10%FBSで培養した。Caki-2細胞はMcCoy's 5A培地+2%FBSで培養した。786-O、Caki-1、およびCaki-2細胞(腎細胞癌細胞株)は全て37℃にて5%CO2で培養した。L540cy細胞はRPMI 1640+20%FBSで培養した。L428細胞はRPMI 1640+10%FBSで培養した。L540cyおよびL428細胞株はホジキンリンパ腫細胞株である。Karpas-299細胞はRPMI 1640+10%FBSで培養した。Karpas-299細胞株は未分化大細胞リンパ腫細胞(ALCL)株である。HT細胞はRPMI 1640+10%FBSで培養した。Raji 4RH細胞(クローン-11)はFrancisco J. Hernandez-Ilizaliturriから贈られたものであり、RPMI 1640+10%FBSで培養した。HTおよびRaji 4RH細胞株は非ホジキンリンパ腫細胞株である。Jeko-1細胞はRPMI-1640+10% FBSで培養した。Jeko-1細胞はマントル細胞リンパ腫細胞株である。
1ウエル当たり100μL増殖培地中の細胞を黒色-角型-クリア-平底96-ウエルプレート中にまいた。次いで低分子インヒビターと抗体薬物複合体の4X濃度の作業ストックを調製し、次いで2倍連続希釈物として用量設定し、10点の用量曲線を作った。単独薬物条件に対しては、50μL低分子インヒビターまたはADCまたは非複合アウリスタチンを各ウエルに、四重で50μLの培地と共に加え、最終体積200μL/ウエルとした。併用条件に対しては、50μL低分子インヒビターと50μLのADCまたは非複合アウリスタチン希釈液を細胞に四重で加え(各薬物の用量設定液に重ねて:高から低へ)、最終体積200μL/ウエルとした。処理した細胞を96時間、37℃にて、5%CO2にてインキュベートした。細胞傷害性を、100μL Cell Titer Glo(Promega)溶液で1時間インキュベートすることにより測定し、次いで発光をFusion HTプレートリーダー(Perkin Elmer)で測定した。データをExcel(Microsoft)およびGraphPad(Prism)で処理して用量応答曲線を作製し、各用量濃度において影響を受けた分率(fa)を計算した。次いでCalcuSyn(Biosoft)ソフトウエアを用い、Chou-Talalayのメジアン-効果式:

を使って、各薬物併用に対する相乗作用、加算性、または拮抗作用を決定した。ここで、(D)は薬物の用量である。(Dm)は効力を意味するメジアン-効果用量である。これはメジアン効果プロットのx-インターセプトにより決定する。(fa)は用量により影響される分率である。(fu)は影響されない分率、すなわち(1-fa)である。(m)は用量-効果曲線のシグモイド性(形状)を意味する指数である。これはメジアン-効果プロットのスロープにより決定する。線形相関係数「r」は0.90以上でなければならない。薬物濃度ユニットは任意である。
細胞を1細胞株当たり2枚の6ウエルプレート中に、4.5mL増殖培地中の2.5 x 105細胞/ウエルでまいた。テムシロリムス(Wyeth, Madison, New Jersey)の作業ストックを10X濃度にて作り、次いで2倍連続希釈物として用量設定し、10点の用量曲線を作った。上記in vitro併用研究で用いたのと同等の最終濃度で、テムシロリムス(0.5mL)を細胞に加えた。処理した細胞を24時間、37℃、5%CO2にて24時間インキュベートした。細胞ペレットを採集し、溶菌液を、PathScan ホスホ-S6リボソームタンパク質(Ser235/236)サンドイッチELISAキット(Cell Signaling, Beverly, Massachusetts)で提供される溶解バッファーにComplete Mini EDTA-free プロテアーゼインヒビター(Roche Diagnostics, Mannheim, Germany)を加えて調製した。タンパク質濃度を定量し、1サンプル当たり10μgの合計タンパク質抽出物をELISAプレート上に供給し、取扱説明書に従って処理した。プレートをHT Fusion(Perkin Elmer)を用いて読出した。ELISAを実施して、次の併用研究で用いた濃度のテムシロリムスの存在におけるホスホ-S6リン酸化の量を検出した。
PI3K-AKT-mTORシグナル伝達を測定するために用いた一次抗体は、抗ヒトホスホ-AKT(Ser473)(クローン:193H12)、抗ヒトAKT、抗ヒトホスホ-4E-BP1(Thr37/46)、抗ヒト4E-BP1、抗ヒトホスホ-S6リボソームタンパク質(Ser235/236)(クローン:2F9)、および抗ヒトS6リボソームタンパク質(クローン:54D2)(Cell Signaling; Beverly, Massachusetts)であった。用いた二次抗体は、Peroxidase-AffiniPure F(ab')2 Fragment Goat anti-Rabbit IgG、F(ab')2 Fragment Specific、およびPeroxidase-AffiniPure F(ab')2 Fragment Goat anti-Mouse IgG、Fcγ Fragment Specific (Jackson ImmunoResearch, West Grove, Pennsylvania)であった。ウェスタンブロットをSuperSignal West Picoミノール増強溶液(Thermo Scientific; Rockford, Illinois)化学発光基質を用いて現像し、そしてフィルムに曝露後、可視化した。
用量応答曲線を、RCC細胞株において、ADC h1F6-mcF、テムシロリムス、シロリムス、エベロリムス、h1F6-mcF+テムシロリムス、h1F6-mcF+シロリムス、およびh1F6-mcF+エベロリムスに対して作製した。RCC細胞株786-OおよびCaki-2において、低FBSの培地で増殖したCaki-2細胞株を除いて、テムシロリムスおよびシロリムス単独は強力な細胞障害性薬物でない。しかし、テムシロリムスとシロリムスは両方共、h1F6-mcFの細胞障害性を増強する。
CIを、上記のように、L540cy細胞株中のcAC10-vcE+テムシロリムス、MMAE+テムシロリムス、およびcAC10-vcE+エベロリムスについて計算した。CIを、上記のように、Karpas 299細胞株中のcAC10-vcE+テムシロリムス、MMAE+テムシロリムス、およびcAC10-vcE+エベロリムスについて計算した。図4はcAC10-vcEおよびMMAEがラパログ類、テムシロリムスおよびシロリムスと相乗作用を有することを示す。
B-様ホジキンリンパ腫においてin vivo併用治療を試験するために、マウス異種移植モデルを用いた。L428細胞をNOD-SCID-γマウス(The Jackson Laboratory)中に移植して腫瘍を作製した。腫瘍を平均90mm3まで増殖し、次いで7マウスのグループに分類した。マウスを治療しない、テムシロリムス(20mg/kg)で治療する、cAC10-vcE(1.0mg/kg)で治療する、またはcAC10-vcEとテムシロリムスで治療するの各グループであった。cAC10-vcEはq4dx4 ipで投与した。テムシロリムスはq4dx4 ipまたはq3dx6 ipで投与した。併用治療において、cAC10-vcEを投与した2日後にテムシロリムスを投与した。最初の投与後120日間、腫瘍体積を定期的に測定した。データを腫瘍3倍化の速度を示すKaplan-Meierプロットとしてプロットした。P値をLog-rank試験を用いて計算し、統計的に有意であることを確認した。図5に示しうるように、cAC10-vcE+テムシロリムスはいずれの単独治療よりも有意に優れた抗腫瘍活性を有した(Log-Rank 試験によるP-値=0.0011)

in vivoのB-様ホジキンリンパ腫の併用治療を試験するために、マウス異種移植モデルを用いた。L428細胞をNOD-SCID-γマウス(The Jackson Laboratory)中に移植して腫瘍を作製した。腫瘍を増殖して平均90mm3に増殖し、次いで10マウスのグループに分類した。マウスを治療しない、エベロリムス(15mg/kg)で治療する、cAC10(1mg/kg)で治療する、cAC10-vcE(1.0mg/kg)で治療する、cAC10とエベロリムスで治療する、またはcAC10-vcEとエベロリムスで治療するの各グループであった。cAC10-vcEおよびcAC10はq4dx4 ipで投与した。エベロリムスはq4dx4 ipまたはq1dx14 poで投与した。第61日にcAC10-vcEとエベロリムス併用治療アームは10/10完全な応答を有し、そしてcAC10-vcEおよびエベロリムス単独治療アームは完全な応答がなかった。
CIを、上記のように、HT細胞株中のhBU12-mcF+テムシロリムスおよびhBU12-mcF+シロリムスについて計算した。図6はhBU12-mcFがHT細胞株においてラパログ類、テムシロリムスおよびシロリムスと相乗作用を有することを示す。
CIを、786-O細胞、Caki-1細胞、Jeko-1細胞、およびRaji-4RH-11細胞中のh1F6-mcF+BEZ235;L540cy(HL)細胞中のcAC10-vcE+BEZ235およびcAC10-vcE+PP242;Karpas-299細胞中のcAC10-vcE+BEZ235;HT細胞中のhBU12-mcF+BEZ235;ならびにJeko-1細胞中のh1F6-mcF+PP242について、上記のように計算した。図8はNVP-BEZ235が多細胞株中のアウリスタチンADCと(ラパログ類、テムシロリムスおよびシロリムスの様に)相乗作用を有することを示す。RCCおよびNHL細胞株におけるさらに低いCI値(とりわけED90にて)は、さらに大きい相乗作用を示唆する。
CIを、Jeko-1細胞中のh1F6-mcF+MK-2206ならびにHT細胞中のhBU12-mcF+XL147について計算した。
ある化合物がPI3Kを阻害するかどうかを確認する例示のアッセイは、Knight et al (2006) Cell vol. 125 pp. 733-747に与えられ、これは参照によりその全てが本明細書に組み込まれる。概略を述べると、IC50値を、脂質キナーゼ活性に対する標準TLCアッセイまたはハイスループット膜捕獲アッセイのいずれかを用いて測定する。キナーゼ反応は、キナーゼ、インヒビター(2%DMSO最終濃度)、バッファー(25mM HEPES、pH 7.4、10mM MgCl2)、および新しく超音波処理したホスファチジルイノシトール(100μg/ml)を含有する反応混合物を調製することにより実施する。γ-32P-ATPの10μCiを含有するATPを最終濃度10または100μMまで加えることにより反応を開始する。反応は室温にて20分間進行させる。TLC分析については、反応を105μlの1N HClを、続いて160μlのCHCl3:MeOH(1:1)加えることにより終結させる。二層混合物を撹拌し、簡単に遠心分離し、そして有機相を、CHCl3をプレコートしたゲル添加ピペットを用いて新しいチューブに移す。この抽出物をTLCプレート上にスポットし、3〜4時間、n-プロパノール:1M酢酸の65:35溶液中で発色させる。TLCプレートを乾燥し、ホスホロイメージャースクリーン(Storm, Amerham)に曝し、そして定量する。各化合物に対して、キナーゼ活性を、典型的には、試験最高濃度(100μM)から2倍希釈の10〜12インヒビター濃度にて測定する。
Claims (19)
- PI3K-AKT-mTOR経路インヒビターとの併用療法で被験者の癌を治療するための医薬の製造のための、アウリスタチン系抗体薬物複合体の使用であって、該インヒビターがmTORに結合しかつラパログであり、該癌の細胞が、該複合体の抗体の結合時に内部移行可能な細胞表面抗原を発現し、該細胞においてPI3K-AKT-mTOR経路が構成的に活性化され、該抗体が抗CD30抗体もしくは抗CD19抗体であり、かつ該癌が血液学的に悪性の癌であるか、または該抗体が抗CD70抗体であり、かつ該癌が血液学的に悪性の癌もしくは腎癌であり、該抗体はアウリスタチンであるMMAEまたはMMAFと複合化されている、上記使用。
- アウリスタチン系抗体薬物複合体との併用療法で被験者の癌を治療するための医薬の製造のための、PI3K-AKT-mTOR経路インヒビターの使用であって、該インヒビターがmTORに結合しかつラパログであり、該癌の細胞が、該複合体の抗体の結合時に内部移行可能な細胞表面抗原を発現し、該細胞においてPI3K-AKT-mTOR経路が構成的に活性化され、該抗体が抗CD30抗体もしくは抗CD19抗体であり、かつ該癌が血液学的に悪性の癌であるか、または該抗体が抗CD70抗体であり、かつ該癌が血液学的に悪性の癌もしくは腎癌であり、該抗体はアウリスタチンであるMMAEまたはMMAFと複合化されている、上記使用。
- 併用療法で被験者の癌を治療するための医薬の製造のための、(a)mTORに結合しかつラパログであるPI3K-AKT-mTOR経路インヒビター、および(b)アウリスタチン系抗体薬物複合体を含む組成物の使用であって、該癌の細胞が、該複合体の抗体の結合時に内部移行可能な細胞表面抗原を発現し、該細胞においてPI3K-AKT-mTOR経路が構成的に活性化され、該抗体が抗CD30抗体もしくは抗CD19抗体であり、かつ該癌が血液学的に悪性の癌であるか、または該抗体が抗CD70抗体であり、かつ該癌が血液学的に悪性の癌もしくは腎癌であり、該抗体はアウリスタチンであるMMAEまたはMMAFと複合化されている、上記使用。
- 癌が血液学的に悪性の癌である、請求項1〜3のいずれか1項に記載の使用。
- 血液学的に悪性の癌がB細胞リンパ増殖性障害である、請求項4に記載の使用。
- 血液学的に悪性の癌がT細胞リンパ増殖性障害である、請求項4に記載の使用。
- 血液学的に悪性の癌がホジキンリンパ腫である、請求項4に記載の使用。
- 抗体が抗CD70抗体であり、癌が腎細胞癌である、請求項1〜3のいずれか1項に記載の使用。
- ラパログがシロリムス、テムシロリムス、エベロリムス、またはデフォロリムス(AP23573)である、請求項1〜8のいずれか1項に記載の使用。
- アウリスタチン系抗体薬物複合体が次式:
