Claims (15)
- カテーテル状要素を通じた患者の直腸からの排泄物の経肛門的ドレナージのための、及び/又は、カテーテル状要素を通じた流入流体の経肛門的投与のためのデバイスであって、チューブ状シャフト本体及びインフレータブルバルーンを備え、該インフレータブルバルーンは、制限された弾性変形性能の薄肉柔軟フィルム材料から製造されるとともに、ダンベル形状又は砂時計形状をもって製造時に予め形成され、より大きな管状半径(D1/2;D2/2)を持つ2つの端末バルーン部を有するとともに、該2つの端末バルーン部の間に配置されてそれに関してテーパー状とされた中間バルーン部が縮小された管状半径を有するとともに、遠位に隣接している半径方向に拡大された前記端末バルーン部が直腸内に配置されて近位に隣接している半径方向に拡大された前記端末バルーン部が体外で配置されるように、経肛門的に配置され、
前記2つの端末バルーン部の端部は先細になるとともに前記チューブ状シャフト本体のジャケット表面上に互いに軸方向に離れて固定されており、
前記チューブ状シャフト本体の腸側への軸方向偏位があるときに、前記チューブ状シャフト本体の端部を囲んでいる遠位又は近位のバルーン拡張部から腸を潜在的に傷つける方法で前記チューブ状シャフト本体の端部が出現できないように、前記チューブ状シャフト本体の端部を囲んでいるバルーン膜の付随する変形が、遠位方向への前記チューブ状シャフト本体の遠位端の移動を吸収及び/又は制限する、反対側に作用する力を発生する働きをするように、力のかかっていない状態で前記直腸内に配置された前記端末バルーン部が前記チューブ状シャフト本体の遠位領域を完全に囲むとともに隙間なく形成され、
前記チューブ状シャフト本体の遠位頂部が、偏位がなく止まっている状態で、対称軸(X)によって垂直に交差されるとともに前記直腸内に配置された前記端末バルーン部に遠位端で完全に接する平面(Z)の十分近位にあり、
前記チューブ状シャフト本体は、a)及びb)の弾性変形性能及び矯正性能を有する、
ことを特徴とする、前記デバイス
a)前記チューブ状シャフト本体は、強制的で偶発的な前記チューブ状シャフト本体の偏位に応じて、1又はそれ以上の軸方向の折れ曲がり又は折り畳みの形態で、軸方向に圧縮された状態に移行するが、近位方向から前記チューブ状シャフト本体に作用する力が除去されると、自発的に、最初の開口管腔の、軸方向に十分に真っ直ぐに矯正された状態に戻り;及び
b)前記チューブ状シャフト本体は、肛門括約筋によってもたらされる正常な緊張の下で半径方向に折り畳まれた状態に移行するが、括約筋の緊張が緩和すると、再び真っ直ぐに矯正されて元の直径寸法になる。 - 前記直腸内に配置された前記端末バルーン部及び前記体外に配置された前記端末バルーン部が、ほぼ同じ長手方向断面を有している、請求項1に記載のデバイス。
- シャフト偏位の多段階緩衝によって特徴づけられる請求項1又は2に記載のデバイス。
- 前記チューブ状シャフト本体と前記バルーンとの間の簡単な相対的遊びをもった許容差範囲、すなわち、最小の復元力Fr≒0で、中立の力のかかっていない状態から、最初の制限値G1までの値によって、偏位xがある場合:
x≦G1⇒Fr=0±ε
ここで、|ε|≦10gであることを特徴とする請求項1〜3の何れかに記載のデバイス。 - 前記直腸内に配置された前記端末バルーン部の変形による復元力、すなわち、前記バルーン膜の復元力Frで、中立の力のかかっていない状態から、最初の制限値G1及び第2制限値G2の間の値によって、偏位xがある場合:
G1≦x≦G2⇒Fr≒cH・(x−G1)
ここで、cHは圧力無しで膨らませたバルーン膜のバネ定数であることを特徴とする請求項1〜4の何れかに記載のデバイス。 - 前記チューブ状シャフト本体の変形(例えば軸方向の折れ曲がり又は座屈)による復元力、すなわち、自然な、力のかかっていない状態から、第2制限値G2を越える値によって、偏位xがある場合において、前記チューブ状シャフト本体の復元力Frは:
G2≦x⇒Fr=cK・(x−G2)
ここで、cKは前記チューブ状シャフト本体の最小のバネ定数であることを特徴とする請求項1〜5の何れかに記載のデバイス。 - 前記チューブ状シャフト本体がシースを備えることを特徴とする請求項1〜6の何れかに記載のデバイス。
- 前記シースが発泡エレメントの形態であることを特徴とする請求項7に記載のデバイス。
- 前記シースは、前記チューブ状シャフト本体の遠位端を3〜12mm超えて延びることを特徴とする請求項8に記載のデバイス。
- 前記シースは、分割されている、及び/または、断面積の変化を有する、及び/又は、凹所を有する、ことを特徴とする請求項8又は9に記載のデバイス。
- 前記シースは、前記バルーンによって、完全にまたは部分的に外側を囲まれていることを特徴とする請求項8〜10の何れかに記載のデバイス。
- 前記シースは、前記バルーンの内面に固定されていることを特徴とする請求項11に記載のデバイス。
- 所定の周囲領域、即ち、前記チューブ状シャフト本体の有する断面のテーパー及び/又は凹所、に沿った、前記チューブ状シャフト本体の折り畳みによって特徴づけられる請求項1〜12の何れかに記載のデバイス。
- 前記チューブ状シャフト本体は、外側から前記チューブ状シャフト本体に対して力がかかる括約筋緊張がある場合に、前記チューブ状シャフト本体が、前記ジャケットの半径方向の陥入の形態で半径方向に折り畳み、前記括約筋の緊張の緩和に際し、自発的に再び弾性的に矯正し、前記チューブ状シャフト本体のドレナージ管腔を自由にするように構成されていることを特徴とする請求項1〜13の何れかに記載のデバイス。
- 前記チューブ状シャフト本体及び/又は該チューブ状シャフト本体に適用される膜の弾性変形及び/又は矯正特性は、全ての側から前記チューブ状シャフト本体に生理的腹腔内圧値が作用する場合に、残っているドレナージ管腔が、最大ドレナージ管腔の85%より大きく、及び/又は、25mbar迄の充填圧力下では、前記本体の外側に自由に膨らんだバルーンを持つシャフトチューブのドレナージ管腔が、最大の狭窄領域で、自由に膨らんだ最初の寸法の50%以下に減らされ、及び/又は、45mbar迄の充填圧力下では、20%以下に減らされるように、調節されていることを特徴とする請求項1〜14の何れかに記載されたデバイス。
