Description
バルーンからの薬剤の早期喪失は、種々の方法により取り組まれている大きな問題である。これらのうちの幾つかは、例えば、保護チューブ、保護スリーブまたは保護エンベロープを使用する機械的な方法である。一例として、特許文献1、特許文献2および特許文献3では、バルーンが膨む前に引っ込む種々の保護シースが開示され、また特許文献4では、バルーン拡張時に破れるカバーが提案されている。特許文献5では、小さな空洞を有している構造にされたバルーン膜が開示され、特許文献6では、コーティングの付着を強化するために粗面化されたバルーン膜が用いられている。または、特許文献7のバルーンの薬剤含有層を保護するパウチおよび特許文献8の折りたたまれたバルーンの折り目の下に薬剤を選択的に配置する方法において、種々のアプローチが取られている。これらの方法は、有効ではあるが、製造の複雑性および費用を増大させたり、機器の操作をより困難としたり、または、機器の径を増大してしまう(機器は、狭窄している病変中を容易に通過するために、可能な限り小さく保たなければならない)という欠点を有している。幾つかの実施形態において、保護膜または孔の空いた膜は、組織への均一な薬剤送達を妨害し、または患者を危険にさらすことさえある。
一般的に、潤滑剤により、固体とダイ壁との間の摩擦が低下して、錠剤の形成および取り出しが、確実に起こり得る。タルクまたはシリカのような普通の鉱物や、例えば植物ステアリン、ステアリン酸マグネシウムまたはステアリン酸などの脂肪は、医薬品において最も頻繁に使用される潤滑剤である。潤滑剤は、特定の加工特性を改善するために、錠剤およびカプセル製剤に少量添加される薬剤である。潤滑剤と識別される、以下のような3つの役割がある。
1.真の潤滑剤的な役割:突出し時に、錠剤表面とダイ壁との間の界面の摩擦を低減させ、かつ打ち抜き型上の摩耗を低下させる。
2.付着防止の役割:パンチ面への粘着を防止するかまたはカプセル化の場合は、機械の供与器(machine dosators)、突き固め用ピン(tamping pin)等への粘着を防止する。
3.流動促進剤的な役割:粒子間の摩擦を低下させることによる生産フローの向上。
1.親水性−流動促進または付着防止特性を有さない、一般的に質の悪い潤滑剤
2.親油性−今日、最も広く使用されている潤滑剤は、親油性ジャンルのものである。親油性の潤滑剤は、一般的に良質な潤滑剤であり、通常、比較的低い濃度で効果がある。また、多数の親油性の潤滑剤が付着防止および流動促進特性の両方を具備している。これため、親油性の潤滑剤は、親水性化合物よりも頻繁に使用されている。最も一般的な潤滑剤としては、ステアリン酸マグネシウムがある。

Claims (16)
- 医療機器であって、前記医療機器の少なくとも表面の一部が、少なくとも1つの薬剤と、前記薬剤100重量%に対して0.1〜500重量%の割合の少なくとも1つの親油性の潤滑剤とを担持し、
前記少なくとも1つの薬剤が、パクリタキセル、シロリムス、エベロリムス、ゾタロリムス、バイオリムス、およびテムシロリムスから選択され、
前記少なくとも1つの親油性の潤滑剤がC6−C30のモノカルボン酸塩であり、更にこのC6−C30のモノカルボン酸塩がマグネシウム、カルシウム、亜鉛若しくはアンモニウム塩、または一価の有機塩基の塩であり、
前記少なくとも1つの薬剤および前記少なくとも1つの潤滑剤が単一の同じコーティング層中に存在しているか、または、前記少なくとも1つの薬剤により被覆した前記医療機器が前記少なくとも1つの潤滑剤の別の層によりさらに被覆されている医療機器。 - 前記C6−C30のモノカルボン酸塩が、ステアリン酸マグネシウム、ステアリン酸カルシウム、ステアリン酸亜鉛、パルミチン酸マグネシウム、パルミチン酸カルシウム、パルミチン酸亜鉛、ミリスチン酸マグネシウム、ミリスチン酸カルシウム、ラウリン酸マグネシウム、ラウリン酸カルシウム、カプリン酸マグネシウム、カプリン酸カルシウム、カプリル酸マグネシウム、カプリル酸カルシウム、オレイン酸マグネシウム、オレイン酸カルシウム、パルミトレイン酸マグネシウムまたはパルミトレイン酸カルシウムから選択される請求項1に記載の医療機器。
- 前記C6−C30のモノカルボン酸塩が、ステアリン酸、パルミチン酸、ラウリン酸、カプリン酸、カプリル酸、オレイン酸、パルミトレイン酸、ステアリンアルコール、パルミチンアルコール、ラウリンアルコール、酢酸マグネシウムおよび酢酸カルシウムから選択される1つ以上の成分と混合されている請求項1または2に記載の医療機器。
- 前記機器が、短時間の使用、好ましくは画像誘導インターベンションの間の使用のための血管形成術用のバルーンカテーテルである請求項1〜3のいずれか一項に記載の医療機器。
- 前記少なくとも1つの親油性の潤滑剤が、少なくとも1つの薬剤100重量%に対して、0.1〜500重量%の割合で含有されている請求項1〜4のいずれか一項に記載の医療機器。
- 前記少なくとも1つの薬剤が、前記少なくとも1つの親油性の潤滑剤よりも親油性を有する請求項1〜5のいずれか一項に記載の医療機器。
- 前記少なくとも1つの薬剤がシロリムスである請求項1〜6のいずれか一項に記載の医療機器。
- 前記少なくとも1つの薬剤が、天然、半合成または合成のものである請求項1〜7のいずれか一項に記載の医療機器。
- 親油性の潤滑剤の装填量が、被覆される機器表面の1mm2当たり10μgまでである請求項1〜8のいずれか一項に記載の医療機器。
- 請求項1〜9のいずれか一項に記載の医療機器を、調製する方法であって、
前記少なくとも1つの薬剤および前記少なくとも1つの親油性の潤滑剤で医療機器を被覆することを含み、
前記少なくとも1つの薬剤および前記少なくとも1つの親油性の潤滑剤が、テトラヒドロフランもしくは25%(v/v)を超えるテトラヒドロフランを含む溶媒混合物中に両方一緒に溶解している、または
前記少なくとも1つの親油性の潤滑剤のために、任意選択的に、アセトン、酢酸、水およびアルコール類から選択された異なる溶媒または溶媒混合物にそれぞれ別々に溶解している調製方法。 - 請求項1〜9のいずれか一項に記載の医療機器を、調製する方法であって、
前記少なくとも1つの薬剤および前記少なくとも1つの親油性の潤滑剤で医療機器を被覆することを含み、
前記少なくとも1つの薬剤および前記少なくとも1つの親油性の潤滑剤が、アセトンもしくは25%(v/v)を超えるアセトンを含む溶媒混合物中に両方一緒に溶解している、または
前記少なくとも1つの親油性の潤滑剤のために、任意選択的に、テトラヒドロフラン、酢酸、水およびアルコール類から選択された異なる溶媒または溶媒混合物にそれぞれ別々に溶解している調製方法。 - 請求項1〜9のいずれか一項に記載の医療機器を、調製する方法であって、
前記少なくとも1つの薬剤および前記少なくとも1つの親油性の潤滑剤で医療機器を被覆することを含み、
前記少なくとも1つの薬剤および前記少なくとも1つの親油性の潤滑剤が、イソプロパノールもしくは25%(v/v)を超えるイソプロパノールを含む溶媒混合物中に両方一緒に溶解している、または
前記少なくとも1つの親油性の潤滑剤のために、任意選択的に、アセトン、テトラヒドロフラン、酢酸、水および他のアルコール類から選択された異なる溶媒または溶媒混合物にそれぞれ別々に溶解している調製方法。 - 請求項1〜9のいずれか一項に記載の医療機器を、調製する方法であって、
前記少なくとも1つの薬剤および前記少なくとも1つの親油性の潤滑剤で医療機器を被覆することを含み、
前記少なくとも1つの薬剤および前記少なくとも1つの親油性の潤滑剤が、溶媒または溶媒混合物中に両方一緒に溶解している、または、任意選択的に前記少なくとも1つの薬剤のために異なる溶媒に溶解しており、
前記溶媒または溶媒混合物が、0.01〜25%(v/v)を超える酢酸またはギ酸をさらに含有している調製方法。 - 前記薬剤および潤滑剤が均一に混ざらないように、最初に前記少なくとも1つの薬剤で被覆され、その後前記少なくとも1つの親油性の潤滑剤で順次被覆した請求項1〜9のいずれか一項に記載の医療機器。
- 前記少なくとも1つの薬剤と、前記少なくとも1つの潤滑剤と、前記少なくとも1つの追加の賦形剤とにより被覆した請求項1〜9のいずれか一項に記載の医療機器。
- 前記追加の賦形剤が抗酸化剤である請求項15に記載の医療機器。
