Description
本出願は、米国特許法第119条に基づき、2010年4月15日にファイルされた米国仮特許出願第61/324503号および2010年9月10日にファイルされた同第61/381669号の優先権の利益を主張するものであり、それぞれの内容全体を本明細書に援用する。
クロストリジウム・ディフィシル(C.difficile)のトキシンAおよび/またはトキシンBに対する中和用モノクローナル抗体の作製
A.免疫原の調製
クロストリジウム・ディフィシル(C.difficile)のトキシンAおよび/またはトキシンB用の中和用モノクローナル抗体を、マウスをクロストリジウム・ディフィシル(C.difficile)トキシンAトキソイド(不活性形態のトキシン)および活性形態のトキシンAおよび/またはトキシンBで免疫して、作製した。また、動物をトキソイドAで免疫した後、活性形態のトキシンAおよび/またはトキシンBで免役して、マウスmAb(PA−38:抗トキシンA mAb、ATCC番号PTA−9888;PA−39:抗トキシンAおよびB mAb、ATCC番号PTA−9692;PA−41:抗トキシンB mAb ATCC番号PTA−9693;およびPA−50:抗トキシンA mAb、ATCC番号PTA−9694)を作製した。トキシンAトキソイド、トキシンA、トキシンB(List Biological Laboratories Inc., Campbell, CA)およびトキシンA(Techlab Inc., Blacksburg, VA)を、使用するまで4℃で保管した。トキシンおよびトキソイドには、一般に用いられているクロストリジウム・ディフィシル(C.difficile)の参考株である株VPI10463由来のものを用いた。必要な容量のトキソイドまたはトキシンにQuil Aアジュバント(Accurate Chemical, Westbury, NY)を加え、混合した。混合物を60分間の免疫で調製し、免疫の準備ができるまで氷上で保管した。融合前の最終ブーストのために、必要なトキシンをPBSで希釈し、免疫に使用するまで氷上で保管した。
30匹のメスのBALB/cマウス(Charles River Labs, Wilmington, MA)に、3週間間隔で活性トキシンAまたは活性トキシンBの用量を増してブースト免疫する前に、3週間間隔で、2免疫用量(PA−50の場合)または3免疫用量(PA−38、PA−39、PA−41の場合)のトキシンAトキソイド(10μg)を皮下注射した。PA−38では、1匹のマウスを3週間ごとにブースト免疫し、トキシンA(List Biological Laboratories Inc.)を合計で3回ブーストした。このとき、ブーストするたびに用量を500ngから2μgに増し、トキシンA(8μg)の最終ブーストは脾摘出の3日前とした。PA−39およびPA−41では、2匹のマウスをトキシンBで3週間ごとに3回または5回ブースト免疫し、ブーストするたびに用量を2μgから12.5μgに増し、トキシンB(20μg)の最終ブーストは脾摘出の3日前とした。PA−50では、1匹のマウスをトキシンA(Techlab Inc.)で3週間ごとにブースト免疫し、合計で4回ブーストした。このとき、ブーストするたびに用量を20ngから2.5μgに増し、トキシンA(10μg)の最終ブーストは脾摘出の3日前とした。免疫およびブースト用量のトキソイドおよびトキシンを、Quil A(10μg)などのアジュバントと一緒に投与した。活性形態のトキシンAまたはトキシンBでのブーストによって、保護抗体ができたであろう動物を同定した。免疫した動物からの血清を段階希釈し、後述するようにしてCHO−K1細胞に対するトキシンA細胞傷害作用の中和を試験した。中和抗体の力価が最も高かった動物を融合用に選択し、アジュバントなしでトキシンでブーストした。
ハイブリドーマ上清を、細胞に対するトキシンAまたはトキシンBの細胞傷害作用の中和能についてスクリーニングした。高スループットの方法を開発し、数千のハイブリドーマ上清を同時に処理した。細胞毒性(cytoxicity)アッセイでは、CHO−K1細胞(PA−38、PA−39、PA−41の場合)またはT−84細胞(PA−50の場合)のいずれかを使用した。Biomek FXロボットシステム(Beckman Coulter, Brea, CA)を用いて、細胞をアッセイプレート(96ウェル、乳白色の壁、透明な平底のプレート;Perkin Elmer, Waltham, MA)に加えた。アッセイプレートを37℃で4時間インキュベートし、細胞をウェルに付着させた。T−84のアッセイでは、トキシンAを240ng/mlまで希釈した。CHO−K1アッセイでは、トキシンAを2μg/mlまで希釈するか、トキシンBを6ng/mlまで希釈した。希釈後のトキシンを、Biosafety Cabinet(BSC)で試薬希釈プレート(96ウェルの丸底プレート;BD, Franklin Lakes, NJ)に手作業で加えた。ハイブリドーマ上清を手作業で回収し、Biomek FXシステムを用いて試薬希釈プレートのウェルに加えた。上清とトキシンの混合物を37℃で1時間インキュベートし、Biomek FXシステムを用いて細胞の入ったアッセイプレートに加えた。37℃で72時間インキュベートした後、20μL/ウェルのCellTiter−Blue(Promega, Madison, WI)を各ウェルに加えた。プレートをさらに4時間インキュベートした後、励起波長560nm、発光波長590nmでSpectraMax M5 Plate Reader(Molecular Devices, Sunnyvale, CA)を用いて読み取った。未処理培養とトキシン処理培養で細胞生存率を比較した。細胞生存率(%)を濃度に対してプロットした。
In vivoおよびin vitroでの作製方法を利用して、単離および/または精製された本発明のmAbを得た。マウスmAbのin vivoでの産生には、適切なハイブリドーマ細胞株をプリスタンで予備刺激したBALB/cマウスの腹膜腔に注射して、腹水液を調製した。硫酸アンモニウムでの沈殿後、プロテインAクロマトグラフィによってmAbを均一性>95%まで精製した。精製抗体をリン酸緩衝生理食塩水(PBS)に懸濁させた。
本発明の抗C difficileトキシンAおよび/またはトキシンB mAbの特異性およびトキシンAおよび/またはトキシンBに対する親和性
A.トキシンAおよび/またはトキシンBのmAbの特異性を判断するためのELISA
ELISAプレート(BD Biosciences)に50ng/ウェルのトキシンA(List Laboratories)または25ng/ウェルのトキシンB(List Laboratories)を4℃で一晩コートした。プレートをPBS−(カルシウムまたはマグネシウムなしのPBS、0.05%Tween 20)で洗浄した後、ウェルを200μlのブロッキング緩衝液(カルシウムまたはマグネシウムなしのPBS、0.1%Tween 20、2.5%無脂肪乳)で37℃にて1時間ブロックした。洗浄ステップを繰り返し、ハイブリドーマ上清または精製mAbを37℃で1時間加えた。プレートを洗浄し、ホースラディッシュペルオキシダーゼ(HRP)結合ヤギ抗マウスIgG−Fc(Jackson Immunoresearch, West Grove, PA)を用いて37℃で1時間インキュベートした。ABTSペルオキシダーゼストップ溶液(KPL)を用いて、プレートをABTSペルオキシダーゼ基質システム(KPL, Gaithersburg, MD)でデベロップし、SpectraMaxプレートリーダー(Molecular Devices)で405nmで読み取った。
Biacore 3000装置(GE Healthcare)を用いて、トキシンAおよび/またはトキシンBに対する本発明のmAbの結合特異性を判断した。MAbを、アミンカップリングの製造業者の指示に従って、約10,000レゾナンスユニット(RU)でCM5センサーチップ(GE Healthcare)に固定化した。特異性が無関係の(Southern Biotech)のアイソタイプマッチ抗体の参考表面を対照として用いた。HEPESベースのHPS−EP緩衝液(GE Healthcare)で25℃で結合実験を実施した。精製トキシンAまたはトキシンB(30nM;List Biological Laboratories)を対照フローセルと試験用フローセルに5μL/分の速度で通した。必要に応じて、追加のmAb(100nM)を5μL/分でフローセルに通し、多価または競合的結合について検討した。
Biacore分析も利用して、それぞれのトキシンに対する本発明のmAbの結合活性を判断した。Biacoreのマウス抗体キャプチャーキットで準備したCM5センサーチップを用いて、mAbをキャプチャーした。次に、トキシンをさまざまな濃度(0.4〜100nM、2倍増大)でフローセルに通した。すべてのトキシン濃度を二重に試験し、測定実施後に毎回、キットに指定された条件でチップ表面を再生した。トキシンに対するmAbのKDを算出するBia Evaluation Software 1:1(Langmuir)結合モデルを用いて、RUの変化を記録し、分析した。結合と解離データ、フィッティングを図3Aから図3Eに示す。
In vitro細胞ベースの中和アッセイ
CHO−K1細胞またはT−84細胞のいずれかを用いる細胞ベースの細胞毒性アッセイを用いて、標記の抗トキシンAおよび抗トキシンB mAbの中和活性を評価した。
アッセイプレート(96ウェル、乳白色の壁、透明な平底のプレート(Perkin Elmer))にCHO−K1細胞を播種した(50μL/ウェルに細胞2,000個)。処理の前に4時間、細胞を付着させた。等容量(35μL)の2μg/mLのトキシンA(List Biological Laboratories)と段階希釈mAbとを試薬希釈プレート(96ウェルの丸底プレート(Falcon))で37℃にて1時間混合した後、混合物50μlをプレートの各ウェルに加えた。72時間のインキュベーション後、20μL/ウェルのCellTiter−Blue(Promega)を各ウェルに加えた。プレートをさらに4時間インキュベートした後、励起波長560nm、発光波長590nmで、SpectraMax M5 Plate Reader(Molecular Devices)を用いて読み取った。未処理培養とトキシン処理培養で細胞生存率を比較した。細胞生存率(%)をmAbの濃度に対してプロットした。GraphPad Prismソフトウェアを用いて、阻害データを非線形回帰のシグモイド用量反応曲線にフィッティングし、細胞毒性(cytoxicity)を50%中和するのに必要なmAbの濃度(EC50)を算出した。図4に示すように、mAb PA−39は、CHO−K1細胞でのトキシンAの活性をEC50が93pMで完全に中和した。
CHO−K1細胞毒性アッセイを用いて、抗トキシンB特異的mAbの中和活性を評価した。抗トキシンA mAbの評価と同様に、96ウェルのプレートでCHO−K1(2,000個/ウェル)に加える前に、段階希釈mAbと一緒にトキシンB(8pg/mL、TechLab)を37℃で1時間インキュベートした。72時間後、20μL/ウェルのCellTiter−Blue(Promega)を各ウェルに加えた。プレートをさらに4時間インキュベートした後、励起波長560nm、発光波長590nmで、SpectraMax M5 Plate Reader(Molecular Devices)を用いて読み取った。CellTiter−Blueを用いて細胞生存度を判断し、処理培養と未処理培養とで細胞生存率を比較した。GraphPad Prismソフトウェアを用いて、阻害データを非線形回帰のシグモイド用量反応曲線にフィッティングし、細胞毒性(cytoxicity)を50%中和するのに必要なmAbの濃度(EC50)を算出した。図5に示すように、PA−41は、CHO−K1細胞でトキシンBの細胞毒性を中和する際に高い度合いの活性を示した(EC50が9.2pM)。
T−84細胞毒性アッセイを用いて、標記の抗トキシンA mAbの中和活性を評価した。アッセイプレート(96ウェル、乳白色の壁、透明な平底のプレート(Perkin Elmer))にT−84細胞を播種した(50μL/ウェルに細胞15,000個)。処理の前に4時間、細胞を付着させた。等容量(35μL)の240ng/mLのトキシンA(Techlab)と段階希釈mAbとを試薬希釈プレート(96ウェルの丸底プレート(Falcon))で37℃にて1時間混合した後、混合物50μlをアッセイプレートの各ウェルに加えた。72時間のインキュベーション後、20μL/ウェルのCellTiter−Blue(Promega)を各ウェルに加えた。プレートをさらに4時間インキュベートした後、励起波長560nm、発光波長590nmで、SpectraMax M5 Plate Reader(Molecular Devices)を用いて読み取った。未処理培養とトキシン処理培養で細胞生存率を比較した。GraphPad Prismソフトウェアを用いて、阻害データを非線形回帰のシグモイド用量反応曲線にフィッティングし、細胞毒性(cytoxicity)を50%中和するのに必要なmAbの濃度(EC50)を算出した。図6に示すように、mAb PA−38およびPA−50は、T−84細胞でトキシンAの活性を完全に中和し、EC50はそれぞれ175pMおよび146pMであった。T−84細胞アッセイで、mAb PA−39はトキシンAに対して最小活性を示し、PA−41は活性ではなかった。
本発明のmAbが、細胞受容体に対するトキシンAの結合をブロックする機能を、血球凝集アッセイで評価した。このアッセイでは、等容量(30μL/ウェル)のトキシンA(8μg/mL;TechLab)と段階希釈したmAbとを試薬希釈プレート(96ウェルの丸底プレート(Falcon))で4℃にて1時間混合した。ウサギ赤血球(RBC)(Colorado Serum Co., Denver, CO)をPBSで3回洗浄し、PBSに懸濁させた。1%RBC懸濁液60μLを、トキシンA−mAb混合物の入った96ウェルのプレートのウェルに加え、プレートを4℃で4時間インキュベートした。遊離トキシンAがRBCの血球凝集を引き起こす。したがって、トキシンAに結合する抗トキシンA mAbを加えると、血球凝集が防止されると思われる。ImageQuant 400機器を用いて、血球凝集の度合いを判断した。完全血球凝集が起これば、懸濁液中のRBCよりもシグナルが強くなった。GraphPad Prism非線形回帰シグモイド用量反応曲線フィッティングを用いて、阻害データからEC50値を算出した。図7に示すように、mAb PA−38(黒い正方形)およびmAb PA−50(黒い三角形)がRBCでトキシンA活性を完全に中和し、EC50はそれぞれ30nMおよび1.8nMであった。PA−38およびPA−50は、受容体に対するトキシンAの結合をブロックすることで、トキシンAを中和するように見える。アッセイで、mAb PA−39およびPA−41は、不活性であることがわかった。
96ウェルのMultiscreen Caco−2プレート(Millipore Billerica, MA)の上部チャンバーでCaco−2細胞を播種し(75μL/ウェルに25,000個)、250μLの培地を下部チャンバ−に加えた。3〜4日ごとに培地を定期的に交換しながら、細胞を10日間成長させた。10〜14日間のインキュベーション後、上皮細胞抵抗測定器(モデル:EVOMX、World Precision Instruments, Sarasota, FL)を用いて経上皮電気抵抗(TEER)を測定して隙間なく形成された単層の形成を確認した。単層の完全性を確立して判断した後、等容量(60μl)のトキシンA(50ng/ml)および段階希釈したmAbを37℃で1時間混合した後、アッセイプレートの上部チャンバーに加えた。プレートを18〜24時間インキュベートした後、抵抗測定器を用いてTEER値を測定した。未治療のウェルとトキシン処理したウェルで単層の完全性を比較した。図8に示すように、GraphPad Prismソフトウェアを用いて阻害データを非線形回帰シグモイド用量反応曲線にフィッティングし、50%中和(EC50)に必要なmAbの濃度を求めた。mAb PA−38およびPA−50は、トキシンAによるCaco−2単層の破壊を中和し、EC50がそれぞれ485pMおよび196pMであった。このアッセイで、他のmAbは不活性であることがわかった。
マウスにおける本発明の抗クロストリジウム・ディフィシル(C.difficile)トキシンAおよびトキシンB mAbのin vivoでの有効性の評価
in vivoでのマウスモデルを使用して、本明細書に記載のmAbが、動物で循環するクロストリジウム・ディフィシル(C.difficile)のトキシンを中和する機能を測定した。単独または組み合わせで投与したmAb PA−38、PA−39、PA−41またはPA−50のin vivoでの中和活性を、被検動物での組み合わせでの全身クロストリジウム・ディフィシル(C.difficile)のトキシンAおよびトキシンB(Techlab)の作用について試験した。
Golden Syrianハムスターにおけるクロストリジウム・ディフィシル(C.difficile)関連下痢症(CDAD)モデルにおける本発明の抗クロストリジウム・ディフィシル(C.difficile)トキシンAおよびトキシンB mAbの評価
ハムスターでのCDADモデルが、ヒトにおけるCDAD疾患のカギになる態様を再現する。抗生剤での治療時、正常な結腸細菌叢が根絶し、ハムスターがクロストリジウム・ディフィシル(C.difficile)の感染に対して易感受性となった。感染によって、重症の下痢症、偽膜性大腸炎および死に至る。ハムスターのCDADモデルを利用して、生きたクロストリジウム・ディフィシル(C.difficile)細菌からの攻撃に関連した動物の疾患および死を防ぐ本発明のmAbの考えられる有効性を評価した。これらの実験を、IACUCが承認したプロトコールで実施した。
生きたクロストリジウム・ディフィシル(C.difficile)微生物に感染したハムスターを用いてハムスターのモデルで有効性の研究を実施する前に、正常な未感染のハムスターで薬物動態研究を実施した。Golden Syrianハムスター(Harlan)に0.2mg/匹または1mg/匹の精製mAb PA−38またはmAb PA−41を腹腔内注射した。0.125日目、0.25日目、1日目、2日目、4日目、7日目、10日目、14日目、21日間目に、後眼窩または心臓の穿刺(全血)採血技術で、血液試料を回収した。血液試料を8000rpmで10分間遠心処理して、血清を得た。
有効性実験を実施して、本発明のマウスの抗トキシンAおよび抗トキシンB mAbが、ハムスターのクロストリジウム・ディフィシル(C.difficile)関連下痢症のin vivoモデルで感染動物の生存性に影響する能力を評価した。雄のGolden Syrianハムスター(約90g)(Crl:LVG(SYR))(Charles River Laboratories, Inc., Kingston, NY)を、単回皮下用量のクリンダマイシン(Sigma, St. Louis、5mg/mLのPBS溶液として配合)50mg/kgで前処理し、正常な結腸細菌叢を破壊した。翌日、関連の試験群のハムスターに経口用量(0.5mLを1×107CFU)のクロストリジウム・ディフィシル(C.difficile)(ATCC 43596株)懸濁液を与えた。株43596は、過去にハムスターモデルで抗体の中和に用いられていた。動物の体重を毎週計測し、健康状態と生存については毎日監視した。
ハムスターでの別の研究を実施し、組み合わせで投与したmAbの場合と比較して、感染動物に投与した本発明の個々のマウス mAbの有効性を評価した。この研究での処理群を表4に示す。
研究時に瀕死であることが明らかになった動物から血液を採取した。研究終了時、生きたままであった動物から血液を採取した。以下において特に明記しないかぎり、血液試料を処理して血清を集めた。以後に有り得る分析のために、処理済みの試料を<−70℃で冷凍した。
ハムスターでさらなる研究を実施して、本発明のヒト化抗トキシンAおよび抗トキシンB mAbのの組み合わせのin vivoでの有効性を、ヒト抗トキシンA対照のmAb、CDA−1、ヒト抗トキシンB対照のmAb、CDB−1の組み合わせでの場合と比較して、それぞれの抗体の組み合わせをクロストリジウム・ディフィシル(C.difficile)感染症の動物に投与した場合について評価した。この研究の処理群を、表5Aに示す。
クロストリジウム・ディフィシル(C.difficile)のトキシンAおよびトキシンBの領域に対するmAbの結合
本発明のmAbが結合したクロストリジウム・ディフィシル(C.difficile)のトキシンAおよびトキシンBのエピトープ領域を判断するための実験を実施した。クロストリジウム・ディフィシル(C.difficile)によって産生されるトキシンAおよびトキシンBはどちらも、約300kDaでかなりの配列と構造が相同である。どちらも、クロストリジウムの繰り返しオリゴペプチド(CROP)を含有するC末端受容体結合ドメインと、ポア形成を引き起こし、エンドソームの膜へのトキシンの挿入を媒介すると考えられている中央の疎水性のドメインと、N末端酵素ドメインを切断するタンパク質分解ドメインと、を有するため、グルコシルトランスフェラーゼが細胞質に入ることができる。クロストリジウム・ディフィシル(C.difficile)ならびに他のクロストリジウム・ディフィシル(C.difficile)タンパク質のトキシンをコードする核酸配列は、公開されており、National Center for Biotechnology Information(NCBI)データベース(すなわちwww.ncbi.nlm.nih.gov)でアクセスできる。たとえば、クロストリジウム・ディフィシル(C.difficile)株VPI 10463の場合、トキシンAおよびトキシンBをコードするDNA配列は、NCBI受託番号x92982に見られる;また、NCBI受託番号NC_009089、領域795842−803975は、クロストリジウム・ディフィシル(C.difficile)630染色体完全ゲノム配列由来のトキシンAのDNA配列を提供するものであり、かたやNCBI受託番号NC_009089、領域787393−794493は、クロストリジウム・ディフィシル(C.difficile)630染色体配列由来のトキシンBをコードするDNA配列を提供するものである。
クロストリジウム・ディフィシル(C.difficile)の全長トキシンBは、3つの主要ドメインすなわち、グルコシルトランスフェラーゼ(GT)活性を処理するN末端酵素ドメイン(63kDa)と、推定上のトランスロケーションドメイン(148kDa)の片側にあるC末端細胞受容体結合(59kDa)(図17Aおよび図17C)からなる。酵素カスパーゼ1を用いる全長トキシンBの酵素的な切断によって、いくつかのトキシンB断片を作製した(図17C)。トキシンBを37℃で96時間、カスパーゼ1(酵素/トキシン比約1U/μgトキシン)で処理した後、SDS−PAGEで検出した場合に断片(193および167kDa)を含有する2つのC末端と断片(103および63kDa)を含有する2つのN末端を含む4つの主要な断片を作製した(図17B)。26および14kDaなど、他のこれよりも小さな断片も作製されたように見えるが、3〜8%トリス酢酸ゲル分析では検出可能でなかった。
全長クロストリジウム・ディフィシル(C.difficile)のトキシンAは、分子量が310kDa(図20A)で、グルコシルトランスフェラーゼ(GT)活性を有するN末端酵素処理ドメイン(約63kDa)と、疎水性ドメイン(約144kDa)の片側にあるC末端CRBドメイン(約101kDa)の3つの主要ドメインを含有する。
抗クロストリジウム・ディフィシル(C.difficile)トキシンAおよびトキシンB mAb−作用機序の研究
A.作用機序研究に用いられるIn vitroでの細胞ベースのアッセイ
抗トキシンmAbの作用機序を評価するために、異なる濃度のトキシンAまたはトキシンBを用いて、in vitroでのアッセイを実施した。これらのアッセイでは、先の実施例3で説明したようにして、CHO−K1またはT−84細胞のいずれかを用いた。
抗クロストリジウム・ディフィシル(C.difficile)トキシンmAbの作用機序をさらに評価するために設計された実験で、各被検抗体(PA−39、PA−50、対照の抗トキシンA mAb CDA−1および抗トキシンAヤギポリクローナル抗体対照)を混合し、そのEC90値の100倍でトキシンAの高い細胞傷害性濃度で完全な中和を保証するようVero細胞に対するトキシンAのCC90濃度で1時間インキュベートした。次に、混合物をVero細胞と一緒に37℃で15分間インキュベートした。次に、細胞をPBSで洗浄し、固定し、透過処理した。被検抗体との結合と競合しない抗トキシンAホースラディッシュペルオキシダーゼ(HRP)標識抗体(PA−38)を用いて、内部移行したトキシンAに対するプローブとし、化学発光を用いて検出した(図31G)。このアッセイでは、結合して細胞に内部移行したトキシンAだけがプローブによって検出されるため、HRP化学発光反応による化学発光シグナルが得られる。化学発光検出では、可視光の生成につながる過酸化物の存在下、酵素を用いてHRP酵素とその基質との間の反応を触媒する(すなわち、過酸化物によるルミノールの触媒酸化)。酸化したルミノールは、基底状態状態まで減衰する際に光を発する。基質がHRPによって触媒されたら、ルミノメーター(Analyst GT)で光シグナルを定量化する。
抗トキシンA mAb
抗トキシンA抗体の中和活性および作用機序の評価に用いた細胞毒性アッセイでは、この実施例のセクションAで説明したように、濃度を増しながら細胞にトキシンAを加えた。抗トキシンA mAb PA−39、PA−50および対照のmAb CDA−1によるトキシンAの中和を評価したこれらの実験の結果を、図31B〜図31Dと以下の表Aに示す。
抗トキシンB抗体の中和効力および作用機序の評価に用いた細胞毒性アッセイでは、この実施例のセクションAで説明したように、濃度を増しながら細胞にトキシンBを加えた。抗トキシンB mAb PA−41および対照のmAb CDB−1によるトキシンBの中和を評価した効力実験の結果を、図31Eおよび図31Fと以下の表Bに示す。表のデータからわかるように、トキシン効力アッセイでのPA−41のin vitro活性には、培養に加えるトキシンBの量を増やすと、EC50および最大阻害率の両方にシフトが認められることから、阻害PA−41の混合−競合的機序があることがわかる。
高病原性分離株または株を含むクロストリジウム・ディフィシル(C.difficile)分離株または株のパネルに対する本発明のmAbの試験
本発明のクロストリジウム・ディフィシル(C.difficile)抗トキシンAおよび抗B mAbが、クロストリジウム・ディフィシル(C.difficile)の広い範囲にわたる関連分離株からのトキシンを中和する機能を評価するために、高病原性BI/NAP1/027分離株を含む20の毒素産生性臨床クロストリジウム・ディフィシル(C.difficile)分離株または株の集合に対してmAbの中和活性を試験した。クロストリジウム・ディフィシル(C.difficile)は、トキシンAおよびBをコードする遺伝子においてかなりの株間異種性を呈するため、標記のmAb、特にmAb PA−50およびPA−41によるトキシン中和の幅を判断するためにこれらの研究を実施した。
キメラmAbの生成
親マウスmAbの可変領域とヒトIgG1の定常領域とを含むキメラモノクローナル抗体を作製してキャラクタライズした。正しい抗トキシン活性と結合特異性を有するマウス可変領域をクローニングし、ヒト定常領域でのマウス可変領域の構築によって各クローンmAbの結合特性および中和特性が有意に変化することはないことを保証するために、キメラmAbを作製した。キメラmAbは一般に、親マウスmAbと同一の結合活性を呈する。
マウスmAbのヒト化とトキシン中和効力についてのin vitroでの本発明のヒト化mAbの試験
従来技術において周知の方法で、ヒト化mAbを作製した。さまざまなヒト化mAbの例と説明が、たとえば、Zenapax(65,66)、Synagis(67〜69)、Herceptin(70〜72)、Mylotarg(73、74)、Xolair(75〜77)、Raptiva(78〜80)、Avastin(81、82)、Tysabri(83)に含まれている。ヒトのmAb活性が悪影響をおよぼさないようにして、免疫原を最小限にするのに効果的なヒト化モノクローナル抗体を作製可能である(84〜87)。好ましくは、ヒト化mAbは、親マウスmAbの2倍以内のトキシン中和活性を示す。さらに、ヒト化mAbは、クロストリジウム・ディフィシル(C.difficile)感染症のハムスターモデルで強力な有効性を最適に示す。
確立された方法を使用して、抗体遺伝子をクローニングした。(88)簡単に説明すると、1×107ハイブリドーマ細胞から、TRIzol試薬(Invitrogen)を用いて、製造業者が提案したプロトコールに従って全RNAを精製した。オリゴ−dTプライマーを使うSuperScript II Reverse Transcriptase(Invitrogen)を用いて、5μgの全RNAを逆転写した。得られたcDNAをRNAse Hで処理してRNAテンプレートを除去し、QIAquick PCR精製キット(Qiagen)を用いてこれを精製して、遊離ヌクレオチドおよびプライマーを除去した。次に、製造業者が提案したプロトコールに従って、dGTPの存在下、酵素末端トランスフェラーゼ(NEB)を用いて、グアニジンヌクレオチドの尾をcDNAの3’末端に加えた。得られた尾状のcDNAを、重鎖または軽鎖の定常領域にアニールした1つのプライマーと、cDNAのグアノシン尾にアニールした1つのユニバーサルプライマーとを用いて、PCR処理した。重鎖および軽鎖の両方に、ユニバーサルプライマー5’TATATCTAGAATTCCCCCCCCCCCCCCCCC3’配列番号11を用いた。軽鎖を増幅するために、プライマー5’TATAGAGCTCAAGCTTGGATGGTGGGAAGATGGATACAGTTGGTGC3’(配列番号12)を用いた。また、重鎖についてはプライマー5’TATAGAGCTCAAGCTTCCAGTGGATAGAC(CAT)GATGGGG(GC)TGT(TC)GTTTTGGC3’(配列番号13)を用いて増幅した。ここで、括弧内の配列は、塩基の縮重を示す。得られたPCR増幅DNAを、QIAquick PCR精製キット(Qiagen)を用いて精製し、配列決定した。PCR反応を実施し、三重に配列決定して、約500塩基対のDNA断片の増幅時に誤差が含まれないようにした。
ヒト化mAbの配列を作製するために、CDR構造に重要なフレームワークアミノ酸残基を、まず同定した。並行して、マウスVHおよびVLとそれぞれ高い相同性を有するヒトVHおよびVL配列を既知のヒト免疫グロブリン配列から選択した。マウスmAbのCDR配列を、必要であればCDRの構造を維持するのに重要なフレームワークアミノ酸残基と一緒に、選択したヒトフレームワーク配列にグラフトした。また、得られるヒト化mAbの潜在的な免疫原を抑えるために、対応するV領域サブグループで非定型であることが見出されるヒトフレームワークアミノ酸残基を、一般的な残基で置換した。これらのヒト化VHおよびVL領域をpCONγ1およびpCONκ(Lonza Biologics, Berkshire, UK)などであるが是に限定されるものではない発現ベクターに、それぞれクローニングした。これらのベクターは、ヒト免疫グロブリン重鎖および軽鎖遺伝子の定常領域をコードする。Effecteneシステム(Qiagen, Valencia, CA)を使用して、293F細胞を一過的にこれらの発現ベクターでトランスフェクトした。分泌されたヒト化mAbを含む細胞上清をトランスフェクションの7日後に回収し、プロテインAクロマトグラフィを用いて精製した。
安定したCHO細胞/細胞株を作製することで、in vitroでの細胞アッセイと、ゴールデンSyrianハムスターのクロストリジウム・ディフィシル(C.difficile)関連下痢症(CDAD)モデルの両方で試験するだけの十分な量のmAbを産生できるようになる。一例として、安定したトランスフェクトCHO細胞を作製するためのグルタミンシンテターゼ選択および増幅システム(GS)を用いて、Lonza Biologics(Berkshire、UK)由来のCHO K1 SV細胞を使用することが可能である。Lonza GSシステムでは一般に、相当に大量のヒト化mAbを産生可能なCHO細胞株を高産生率で得られる。
分子クローニングしたヒト化mAbを上述したように単離し、キャラクタライズした(以下のセクションEを参照のこと)。各ヒト化抗体の軽(L)鎖定常領域(CL)がカッパ(κ)クラスのものであり、各ヒト化抗体の重(H)鎖定常領域(CH)はIgG1アイソタイプのものである。独特の可変(V)領域を含むヒト化mAbが、クロストリジウム・ディフィシル(C.difficile)のトキシンAまたはトキシンBの活性を結合および中和することがあきらかになった。ヒト化mAbのL鎖およびH鎖のV領域は、一般にジスルフィド結合でリンクした2つのH鎖ポリペプチドと2つのL鎖ポリペプチドからなる完全免疫グロブリン(Ig)または抗体分子の一部をなすことがあり、あるいは、抗体の離散した一部または断片、特に、トキシンAおよび/またはトキシンBに結合および/またはトキシン活性を中和する抗体部分または断片のこともある。好適なV領域含有免疫グロブリン断片または部分の非限定的な例として、F(ab)断片、F(ab’)断片またはF(ab’)2断片があげられる。
in vitroにてクロストリジウム・ディフィシル(C.difficile)のトキシンの中和実験を実施し、ヒト化mAbの機能的活性を親のマウスmAbの機能的活性と比較した。図29に示すように、ヒト化PA−41(hPA−41)mAbは、EC50が9pMであるマウスPA−41 mAb(mPA−41)の場合と比較して、トキシンBの細胞毒性を強力に中和した(EC50が6pM)。同様に、hPA−39には図30、hPA−50には図31に示すように、CHO−K1細胞またはT−84細胞を用いてマウス親mAbと比較すると、トキシンAの中和にあたって、ヒト化PA−39 mAb(hPA−39)およびヒト化PA−50 mAb(hPA−50)は等しく強力であることが明らかになった。これらの結果から、親のマウスmAbのヒト化に成功し、ヒト化mAbが機能的かつ効果的であることがわかる。
本発明のマウス抗トキシン mAbのFab断片の生成
A.Fab断片の調製物
マウスIgG1 FabおよびF(ab’)2調製キット(Pierce)とキットに同梱されている試薬を用いて、製造業者の指示に従ってFab断片化を実施した。すべてのmAbすなわちPA−39、PA−41、PA−50で、断片化には同一のプロトコールを用いた。簡単に説明すると、固定化したフィチンスラリー(750μl)を消化緩衝液(75mMシステイン、pH5.6)で洗浄した後、約3mgのmAbを加え、コンスタントに転倒回転しながら混合物を37℃で4時間インキュベートした。消化が終わったら、スラリーを遠心処理し、消化産物を回収した。スラリーをプロテインA結合緩衝液で3回洗浄し、洗浄材料を加えて消化を完了させた。NAbプロテインAカラムをプロテインA結合緩衝液で平衡化し、消化抗体試料を加えた。カラムおよび試料を室温にて10分間インキュベートした。カラムを1000gで1分間遠心処理し、Fab断片を含む通過画分を回収した。カラムをプロテインA結合緩衝液で3回洗浄した。通過画分を回収し、緩衝液をPBSに交換して、濃縮した。
Novexゲルシステム(Invitrogen)を用いてSDS−PAGEで試料を分析し、特に明記しないかぎり、以下に列挙する試薬はすべてInvitrogenから入手した。試料をNuPage試料緩衝液と混合し、DTTで還元した。還元試料と非還元試料を100℃で10分間インキュベートした。試料(4μg)を4〜12%のBis Tris NuPageゲルにロードした後、MOPSランニング緩衝液を用いて180Vで60分間、電気泳動を実施した。電気泳動後、固定液(40%メタノール、10%酢酸)と一緒に20分間、ゲルをインキュベートし、水ですすぎ、コンスタントに回転しながらSimply Blue Stainで一晩染色した。
In vitroクロストリジウム・ディフィシル(C.difficile)のトキシン中和実験を実施して、結合部位数を基準にFab(▲)の機能的活性を完全mAb(■)の機能的活性と比較した。PA−39のFabは、完全PA−39よりもCHO−K1細胞でトキシンA細胞毒性を強く中和した(それぞれEC50が880pM、EC50が200 pM)(図41A)。PA−41のFabは、CHO−K1細胞でのトキシンB活性の中和にあたって完全PA−41と等しく強力なであることがわかった(それぞれEC50が88pM、EC50が80pM)(図41B)。T−84細胞でのトキシンAの中和にあたっての完全PA−50 mAbのEC50値100pMであったのに対し、PA−50のFabは、EC50値が1.8nMであった(図41C)。
ヒト組織標本のヒト化抗クロストリジウム・ディフィシル(C.difficile)トキシンmAbの免疫組織化学的分析
特定抗原の発現を研究する際の免疫組織化学検査(IHC)の値は、正常な組織と腫瘍組織における微小解剖の詳細と不均一性を評価できるようにするものである。IHCは、個々の細胞タイプにタンパク質を直接局在化できるので、他の分析方法よりも好都合である。正常な組織と腫瘍組織の遺伝子発現差を検出可能であり、同時に細胞数と組成の変化も把握できる。この技術の制約には、研究対象となる分子の低レベルの発現がゆえに偽陰性の結果が出る可能性があることと、同様のエピトープまたは他の抗原が共通のエピトープへの抗体結合がゆえに偽陽性の結果(交差反応性)が出ることである。これらの制約に対処するために、本研究は、陽性のトキシン特異的注射マウス対照標本で強く特異的な染色を示した各抗体で可能な限り最も低い濃度で実施した。
非ヒト霊長類におけるヒト化抗クロストリジウム・ディフィシル(C.difficile)トキシンmAbの薬物動態分析
精製ヒト化mAb PA−41またはmAb PA−50を用いて未感作ではないカニクイザルでの薬物動態(PK)研究を実施した。この研究では、雄の未感作ではないカニクイザル(Macaca fascicularis)に1mg/kg/匹または5mg/kg/匹の精製ヒト化mAb PA−41またはmAb PA−50を静脈内注射した。Institutional Animal Care and Use Committee(IACUC)のポリシーと手順に沿って研究を実施した。

Claims (39)
- クロストリジウム・ディフィシル(C.difficile)のトキシンAに特異的に結合する、単離された抗体またはその抗原結合性断片であって、ATCC受託番号PTA−9694で寄託されたハイブリドーマ細胞株によって産生されるモノクローナル抗体PA−50もしくはその抗原結合性断片、モノクローナル抗体PA−50のキメラ形態もしくはその抗原結合性断片、またはモノクローナル抗体PA−50のヒト化形態もしくはその抗原結合性断片を含む、前記単離された抗体またはその抗原結合性断片。
- 抗体が、ATCC受託番号PTA−9694で寄託されたハイブリドーマ細胞株によって産生されるモノクローナル抗体PA−50である、請求項1に記載の抗体またはその抗原結合性断片。
- クロストリジウム・ディフィシル(C.difficile)のトキシンAに特異的に結合する、単離された抗体またはその抗原結合性断片であって、ATCC受託番号PTA−9694で寄託されたハイブリドーマ細胞株によって産生されるモノクローナル抗体のVHのCDR1、CDR2、およびCDR3、ならびにATCC受託番号PTA−9694で寄託されたハイブリドーマ細胞株によって産生されるモノクローナル抗体のVLのCDR1、CDR2、およびCDR3を含む、前記単離された抗体またはその抗原結合性断片。
- クロストリジウム・ディフィシル(C.difficile)のトキシンAに特異的に結合する、単離された抗体またはその抗原結合性断片であって、VHが、配列番号5または配列番号6のCDR1、CDR2、およびCDR3を含み、VLが、配列番号7のCDR1、CDR2、およびCDR3を含む、前記単離された抗体またはその抗原結合性断片。
- クロストリジウム・ディフィシル(C.difficile)のトキシンAに特異的に結合する、単離された抗体またはその抗原結合性断片であって、VHが、配列番号5のアミノ酸残基31〜35を含むCDR1、配列番号5のアミノ酸残基50〜65を含むCDR2、および配列番号5のアミノ酸残基95〜102を含むCDR3を含み、VLが、配列番号7のアミノ酸残基24〜33を含むCDR1、配列番号7のアミノ酸残基49〜55を含むCDR2、および配列番号7のアミノ酸残基88〜94を含むCDR3を含む、前記単離された抗体またはその抗原結合性断片。
- L鎖のV領域が、配列番号7に記載のアミノ酸配列を含み、および/またはH鎖のV領域が、配列番号5または配列番号6から選択されるアミノ酸配列を含み;または
L鎖のV領域が、配列番号16に記載のアミノ酸配列を含み、および/またはH鎖のV領域が、配列番号14に記載のアミノ酸配列を含む、
請求項1〜5のいずれか一項に記載の抗体またはその抗原結合性断片。 - キメラまたはヒト化形態である、請求項2〜6のいずれか一項に記載の単離されたモノクローナル抗体またはその抗原結合性断片。
- CH領域が、IgG1、IgG2a、IgG2b、IgG3、IgG4、IgA、IgE、またはIgMから選択される、および/またはCL領域が、κまたはλアイソタイプから選択される、請求項1〜7のいずれか一項に記載の抗体またはその抗原結合性断片。
- CH領域がIgG1である、請求項8に記載の抗体またはその抗原結合性断片。
- CL領域が、κまたはλアイソタイプから選択される、請求項1〜9のいずれか一項に記載の抗体またはその抗原結合性断片。
- 抗原結合性断片が、単鎖抗体、Fab断片、F(ab’) 2 断片、またはFv断片から選択される、請求項1〜10のいずれか一項に記載の抗体またはその抗原結合性断片。
- EC 50 値が2.6 −12 M〜7.7 −11 Mであることを理由に判断して、クロストリジウム・ディフィシル(C.difficile)の高病原性株のトキシンAを中和する、請求項1〜11のいずれか一項に記載の抗体またはその抗原結合性断片。
- 二重特異性抗体またはその抗原結合性断片であって、
(i)請求項1〜12のいずれか一項に記載のクロストリジウム・ディフィシル(C.difficile)のトキシンAに特異的に結合する抗体の抗原結合性断片を含む第一抗原結合領域;および
(ii)クロストリジウム・ディフィシル(C.difficile)のトキシンBに特異的に結合する抗体の抗原結合性断片を含む第二抗原結合領域
を含む、前記二重特異性抗体またはその抗原結合性断片。 - 請求項13に記載の二重特異性抗体またはその抗原結合性断片であって、第二抗原結合領域が、
(a)クロストリジウム・ディフィシル(C.difficile)のトキシンBに特異的に結合し、ATCC受託番号PTA−9693で寄託されたハイブリドーマ細胞株によって産生されるモノクローナル抗体の可変重鎖(VH)のCDR1、CDR2、およびCDR3、ならびに可変軽鎖(VL)のCDR1、CDR2、およびCDR3を含む抗体の抗原結合性断片;または
(b)クロストリジウム・ディフィシル(C.difficile)のトキシンBに特異的に結合し、ATCC受託番号PTA−9692で寄託されたハイブリドーマ細胞株によって産生されるモノクローナル抗体のVHのCDR1、CDR2、およびCDR3、ならびにVLのCDR1、CDR2、およびCDR3を含む抗体の抗原結合性断片
を含む、前記二重特異性抗体またはその抗原結合性断片。 - ATCC受託番号PTA−9694で寄託された、ハイブリドーマ細胞株。
- 請求項1〜12のいずれか一項に記載の抗体またはその抗原結合性断片をコードする、単離された核酸。
- 配列番号15または配列番号17に記載の配列を有する、請求項16に記載の単離された核酸。
- 請求項16または17に記載の核酸を含む、発現ベクター。
- 請求項16または17に記載の核酸または請求項18に記載のベクターを発現するために操作された、宿主細胞。
- 請求項1〜12のいずれか一項に記載の抗体またはその抗原結合性断片、請求項13または14に記載の二重特異性抗体、請求項16または17に記載の核酸、請求項18に記載の発現ベクター、および/または請求項19に記載の宿主細胞、ならびに取扱説明書を含む、キット。
- 請求項1〜12のいずれか一項に記載の抗体またはその抗原結合性断片、請求項13または14に記載の二重特異性抗体、請求項16または17に記載の核酸、請求項18に記載の発現ベクター、および/または請求項19に記載の宿主細胞を含む、組成物。
- 薬学的に許容されるキャリア、希釈剤、賦形剤、または溶媒をさらに含む、請求項21に記載の組成物。
- (i)請求項1〜12のいずれか一項に記載の抗体またはその抗原結合性断片、および(ii)クロストリジウム・ディフィシル(C.difficile)のトキシンBに特異的に結合する抗体または抗原結合性断片を含む、組成物。
- 請求項23に記載の組成物であって、クロストリジウム・ディフィシル(C.difficile)のトキシンBに特異的に結合する抗体またはその抗原結合性断片が、
(a)ATCC受託番号PTA−9693で寄託されたハイブリドーマ細胞株によって産生されるモノクローナル抗体の可変重鎖(VH)のCDR1、CDR2、およびCDR3、ならびに可変軽鎖(VL)のCDR1、CDR2、およびCDR3;または
(b)ATCC受託番号PTA−9692で寄託されたハイブリドーマ細胞株によって産生されるモノクローナル抗体のVHのCDR1、CDR2、およびCDR3、ならびにVLのCDR1、CDR2、およびCDR3
を含む、前記組成物。 - 少なくとも1つの追加の治療剤をさらに含む、請求項21〜24のいずれか一項に記載の組成物。
- 少なくとも1つの追加の治療剤が、メトロニダゾール、バンコマイシン、フィダキソマイシン、ニタゾキサニド、リファキシミン、ラモプラニン、またはこれらの組み合わせである、請求項25に記載の組成物。

- 被検体におけるクロストリジウム・ディフィシル(C.difficile)感染症、クロストリジウム・ディフィシル(C.difficile)関連疾患、またはクロストリジウム・ディフィシル(C.difficile)関連下痢症(CDAD)の治療に使用するための、請求項13または14に記載の二重特異性抗体。
- 被検体におけるクロストリジウム・ディフィシル(C.difficile)感染症、クロストリジウム・ディフィシル(C.difficile)関連疾患、またはクロストリジウム・ディフィシル(C.difficile)関連下痢症(CDAD)の治療に使用するための、請求項16または17に記載の核酸。
- 被検体におけるクロストリジウム・ディフィシル(C.difficile)感染症、クロストリジウム・ディフィシル(C.difficile)関連疾患、またはクロストリジウム・ディフィシル(C.difficile)関連下痢症(CDAD)の治療に使用するための、請求項18に記載のベクター。
- 被検体におけるクロストリジウム・ディフィシル(C.difficile)感染症、クロストリジウム・ディフィシル(C.difficile)関連疾患、またはクロストリジウム・ディフィシル(C.difficile)関連下痢症(CDAD)の治療に使用するための、請求項19に記載の宿主細胞。
- 被検体におけるクロストリジウム・ディフィシル(C.difficile)感染症、クロストリジウム・ディフィシル(C.difficile)関連疾患、またはクロストリジウム・ディフィシル(C.difficile)関連下痢症(CDAD)の治療に使用するための、請求項21〜26のいずれか一項に記載の組成物。
- 被検体におけるクロストリジウム・ディフィシル(C.difficile)感染症、クロストリジウム・ディフィシル(C.difficile)関連疾患、またはクロストリジウム・ディフィシル(C.difficile)関連下痢症(CDAD)の治療のための医薬剤の調製における、請求項1〜12のいずれか一項に記載の抗体またはその抗原結合性断片の使用。
- 被検体におけるクロストリジウム・ディフィシル(C.difficile)感染症、クロストリジウム・ディフィシル(C.difficile)関連疾患、またはクロストリジウム・ディフィシル(C.difficile)関連下痢症(CDAD)の治療のための医薬剤の調製における、請求項13または14に記載の二重特異性抗体の使用。
- 被検体におけるクロストリジウム・ディフィシル(C.difficile)感染症、クロストリジウム・ディフィシル(C.difficile)関連疾患、またはクロストリジウム・ディフィシル(C.difficile)関連下痢症(CDAD)の治療のための医薬剤の調製における、請求項16または17に記載の核酸の使用。
- 被検体におけるクロストリジウム・ディフィシル(C.difficile)感染症、クロストリジウム・ディフィシル(C.difficile)関連疾患、またはクロストリジウム・ディフィシル(C.difficile)関連下痢症(CDAD)の治療のための医薬剤の調製における、請求項18に記載のベクターの使用。
- 被検体におけるクロストリジウム・ディフィシル(C.difficile)感染症、クロストリジウム・ディフィシル(C.difficile)関連疾患、またはクロストリジウム・ディフィシル(C.difficile)関連下痢症(CDAD)の治療のための医薬剤の調製における、請求項19に記載の宿主細胞の使用。
- 被検体におけるクロストリジウム・ディフィシル(C.difficile)感染症、クロストリジウム・ディフィシル(C.difficile)関連疾患、またはクロストリジウム・ディフィシル(C.difficile)関連下痢症(CDAD)の治療のための医薬剤の調製における、請求項21〜26のいずれか一項に記載の組成物の使用。
- クロストリジウム・ディフィシル(C.difficile)のトキシンAおよびトキシンBによる、細胞に対する毒性を阻害または中和する、ex vivoでの方法であって、前記細胞を、有効クロストリジウム・ディフィシル(C.difficile)トキシンA阻害もしくは中和用量の請求項1〜12のいずれか一項に記載の抗体または抗原結合性断片、請求項13または14に記載の二重特異性抗体、または請求項21〜26のいずれか一項に記載の組成物、および有効クロストリジウム・ディフィシル(C.difficile)トキシンB阻害もしくは中和用量の抗体またはその抗原結合性断片に曝露することを含む、前記方法。
