Description
、ヒドロキシエチルデンプン(HES)および、ある程度、デキストランが挙げられる。これらの分子は、膠質の浸透圧(colloidal osmotic pressure)または膠質の膠質浸透圧(colloidal oncotic pressure)をもたらす。「膠質の浸透圧」および「膠質の膠質浸透圧」という用語は、本願中では互換可能に使用される。実際には、これは、血管系に投与された高膠質液が水を間質コンパートメントから血管系に引き出すことを意味する。これは、ECF間の分布を変え、間質コンパートメント内に存在する液体が少なくなる。したがって、高膠質溶液は、血管系における総容積を所与の総容積より多い量まで増加する。例えば、25%アルブミン溶液は、血管系の容積を所与の容積のほぼ5倍に増加する(Griffel et al., 1992)。血漿における正常なヒト膠質浸透圧は約28mmHgであり、高膠質溶液は、これより高い膠質浸透圧を提供しなければならない。膠質浸透圧が高ければ高いほど、より多くの水が間質から血管系に移る。

動物および動物の飼育
本実験には、平均体重が63kg(60〜72kgに分布)である16頭のスウェーデンの飼育ブタを使用した。動物はすべて、実験動物の管理と使用に関する指針(NIH Publication 85-23 revised 1985)に従って飼育をされた。実験前、すべての動物を一晩絶食させ、水は自由に取らせた。実験後、塩化カリウムの静注で心室細動を誘発することによって、すべての動物を安楽死させた。
すべての動物を、体重1kgあたり10mgのケタミンおよび体重1kgあたり0.2mgのキシラジンを筋肉投与で前投薬した。麻酔導入には、体重1kgあたり5mgのチオペンタールナトリウムおよび体重1kgあたり0.02mgのアトロピンを静注で使用した。気管切開および気管チューブの導入前に、パンクロニウムを静注で与えた。実験中、8gのケタミンおよび300mgの臭化パンクロニウムを5%グルコースに溶かし500mLにした混合物を1時間あたり35mLの連続注入で使用して麻酔を維持した。従量式換気を使用して、正常静脈状態(分時拍出量150〜200mL/kg、20呼吸/分、PEEP=5cm H2O、吸入酸素画分=0.5)を維持した。
ブタを無作為に、晶質群(1500mLの酢酸リンゲル液、n=8)または膠質群(1500mLの開示された溶液によるプライムECC(PrimeECC)液、その組成を下に示す)のいずれかに割り当てた。同一のメモの入った16の封筒を使用した。メモには晶質または膠質いずれか印付けられていた。動物を準備した後、30分間の安定化時間を開始し、その間の最後の15分をベースラインとして記録した。ECC(体外循環)を確立し、60分間維持した。次に、ECCを切り、動物をさらに120分間モニターした。
各実験を静動脈バイパスとして行い、1時間維持した。全手術は無菌状態の下で行った。胸骨正中切開を行った後、胸腺および心膜の前部を注意深く摘出し、心臓および大動脈を露出させた。全身ヘパリン化(300IU/kg)の後、右心房および大動脈弓にカニューレを挿入した。活性凝固時間(ACT)をヘパリンの間欠注射によって350秒より上に保った。すべてのかん流は正常体温(37℃)で行った。
血液サンプリング、薬物投与および圧力モニタリングのために、2つの中心静脈および2つの動脈ラインを頸部血管を通して設置した。胸骨正中切開を実施した後、肺動脈ラインを肺動脈の直接穿刺によって設置した。2つの静脈および動脈ラインを有した理由は、他のラインでの圧力モニタリングの妨害を最小限にして血液サンプリングを可能にするためであった。挿入前に、3つの圧力モニタリングカテーテルを右心房、内胸大動脈および肺動脈における気圧にそれぞれ調節した。血圧、MAP(平均動脈圧)、CVP(中心静脈圧)、PAP(肺動脈圧)、心拍数、ポンプ流、ポンプrpm、ポンプ圧および体温を、X線透視装置を用いて連続的に測定およびモニタリングした。尿排出量の測定のために、すべての動物で膀胱切開を行った。中心体温を鼻咽頭で測定した。また、2つの超音波血流プローブを右頸動脈および肺動脈のまわりに設置した。
結果はすべて、平均値±標準誤差(S.E.M.)として表される。単一時点(ベースライン)をスチューデントのt検定で対応のないデータについて解析し、データの全体的な解釈は濃度曲線下面積によって行った。
ベースラインにおいて、2群は測定した変数のいずれでも有意に異ならなかった。
本試験は、ECC中および後に生理的限界内に膠質浸透圧を保つという血流力学の結果を示している。血中赤血球容積は膠質群で有意に低く、膠質プライミング液によって供給された液体が血管系内にとどまったことを示し、その群ではSVRおよびMAPは有意に低かった。膠質浸透圧は晶質群では7mmHg大きく低下した一方で、膠質群では変化なくとどまるか、またはわずかに増加した。この違いは、バイパス中の静脈リザ−バーの液
体レベルを追跡することによって最も明瞭に観察することができた。膠質群では、そのレベルはすべての場合において増加し、追加の液体の必要性がなかった。しかし、晶質群では、そのレベルはすべての場合において減少し、静脈リザ−バー中の液体レベルを安全性の理由から設定された最小レベルより上に保ち、かん流が65mL/kg/分で流れ続けることができるように液体を加える必要があった。液体バランスは、2群で顕著に異なり、晶質群では+1900mLおよび膠質群では−18mLであった。膠質群では心拍出量およびMAPはECCの2時間後で有意に高く、その群での多量の血液容積を反映していた。上記のように、開示されたプライミング液を与えられた動物において過度の出血の兆候はなかった。従って、開示された溶液が、手術中または手術後のいずれも過度の出血を誘発することなく膠質浸透圧に関して意図したように機能することを本試験は示した。
材料および方法:
本試験は、1964年、フィンランド、ヘルシンキでの第18回世界医師会議で採択されたヒトを対象とする生物医学的研究に携わる医師に対する勧告に従って行われた。本試験について倫理委員会からの承認を得た。本試験に参加する患者は全員、書面での同意をした。本試験は、対照群およびプライムECC(商標)(試験)群の2群での、互いに平行に実施される前向きのランダム化ユーザー盲検としてデザインされ、実施された。ルンド(Lund)大学病院の臨床研究能力センター(Competence Centre
for Clinical Research)からの生物統計学者が、ランダム化を準備した。ランダム化リストは、試験溶液を包装した病院の薬局で保管した。盲検を維持するために、現地の薬局が試験溶液および対照溶液の両方を同一のバッグに作製した。手術日に、そのバッグをその薬局から人工心肺担当臨床工学技士に届けた。試験製品の日誌が薬局によって作成され、試験治療はランダム化番号が病院の記録でも確認された。各群に20名の患者が含まれ、合計40名の患者であった。晶質群(対照)の患者は、プライミング液としてマンニトール溶液および酢酸リンゲル液を与えられ、膠質(試験)群(プライムECC(商標))は、開示されたデキストランベースの高膠質溶液を与えられた。

使用したHLMは、HL20(Jostra AG、ヘッヒンゲン(Hechingen)、ドイツ)であった。酸素供給器(Quadrox+VKD 4201、Jostra AG、ヘッヒンゲン(Hechingen)、ドイツ)および動脈フィルター(Quart、Jostra AG、ヘッヒンゲン(Hechingen)、ドイツ)付きのハードシェル静脈/心臓切開術リザ−バーを、すべてのかん流に使用し、チューブ類も同社からのものであった。製造業者の取扱説明書に従って、かん流回路を組み立て、プライミングした。30000ダルトンの大きさをもつ半透過性の膜を使用する膠質浸透圧計(Wescor Inc、Logan、ユタ州、アメリカ合衆国)で測定した膠質浸透圧を除いて、血液パラメーターはラジオメーター(Radiometer)ABL725計測器で測定した。
・液体バランス測定を、麻酔調製中、CPBの開始前にベースラインとして記録し、次いでCPBを開始して30分、60分および120分後、およびCPB終了後の術後1日目に記録した。
・膠質浸透圧(COP)および血中赤血球容積の値を、麻酔調製中、CPBの開始前にベースラインとして記録し、次いでCPBを開始して30分、60分および120分後、およびCPB終了後の術後1日目に記録した。
2群間の比較のために、反復測定に対してボンフェローニの補正をしたスチューデントt検定を使用した。全データは平均値±標準偏差(SD)として提示した。
患者背景データについて群間で有意な差はなかった(表1)。
COPに関するベースライン値で2群間に統計学的な差はなかった。値は、プライムECC(商標)群で22mmHg±1であったのに対して、対照群で23mmHg±2であった。CPB30分および60分において、群間でCOPに有意差があり、CPBの30分においてプライムECC(商標)群では21mmHg±1に対して、対照群では14mmHg±1であり(p<0.0001)、CPBの60分においてCOPは、対照群では14mmHg±1およびプライムECC(商標)群では20mmHg±1であった(p0.0001)。CPBの120分後、統計学的な差は依然として見られ、プライムECC(商標)群では19mmHg±1であったのに対して対照群では16mmHg±1であっ
た(p<0.001)。術後1日目では、2群間でCOPに有意差はなかった(表3)。
予想された通り、血中赤血球容積は、CPBの間、膠質試験群で低かった。その差は、PBの120分後ではもはや大きくなく、CPB後1日目では差は全くなかった(表4)。
手術中に尿排出量または所与の晶質、膠質、SAGまたは血漿の量に関して2群間で有意差はなかった。しかし、対照群と比較してプライムECC(商標)群ではCPBバランスが有意に低く、プライムECC(商標)群では1817mL±167に対して対照群では2737mL±270であった(p<0.0001)。手術中の総液体バランスは、プライムECC(商標)群と比較して対照群で有意に高く、プライムECC(商標)群では3190mL±362に対して対照群では4067mL±294であった(p<0.01)。平均値を比較したとき、試験群において手術中の出血がより多いという有意ではない傾向があった。しかし、これは個々の患者データのためであった。出血を患者の体重と関連付けたとき、相関関係はなく、したがって投与量に依存的な出血はなかった(図8および表5)。
