Description
本発明は、一般に医療装置の分野に関係する。特に本発明は術後創傷補助装置及び処置に関するものであり、特に閉鎖創傷での出血制御及び創傷滲出物の処理に関係がある装置及び処置に関する。それには、滲出物を排出することと、閉鎖創傷の創傷腔を囲む皮下組織の止血を促進することが含まれる。
止血は、創傷の出血工程を停止させる複雑な工程である。それは、損傷血管を修復する多段階工程を言い、血管収縮と、血小板凝集による止血と、及び血小板血栓を凝血塊へと安定化させるフィブリンタンパク繊維メッシュの形成で終了する一連の酵素反応とが含まれる。
従って、本発明の実施形態は、好ましくは、添付の特許請求の範囲に従う医療装置及び方法を単独又は任意の組合せで提供することにより、技術的な1又は複数の欠乏、不利な点又は問題(例えば先ほど確認したもの)を緩和するか、軽減するか又は解決しようとする。
上記処置は、
流体連通チャネルを備える医療排出装置を提供することと、
術後閉鎖創傷の創傷腔に流体連通チャネルの遠位端を経皮的に配置して、創傷腔外に流体連通チャネルの近位端を配置することと、
従って、流体連通チャネルの遠位端から近位端へ滲出物を排出すること、;
細長い外筒と、外筒に対して相対的に長軸方向へ移動可能な膨張可能且つ収縮可能バルーンと、バルーンと流体連通する、細長い外筒に沿った中空の膨張収縮筒状部材と、を備える止血促進医療装置を提供することと、
細長い外筒の近位端を創傷腔の外側に配置し、収縮したバルーンを外筒に備える細長い外筒の遠位端を創傷腔に経皮的に配置することと、
その創傷腔においてバルーンを外筒の遠位端の外へ放出することと、
筒状部材を通じて所望の圧力までバルーンを膨張させて、止血促進時間の間、止血を促進するために創傷腔を囲む組織と少なくとも部分的に接触することと、
バルーンを収縮させることと、
外筒の遠位端の中へ収縮したバルーンを再装填することと、
創傷腔の外側に外筒の遠位端を引き抜くことと、を備える。
本発明の実施形態が可能にするこれら並びに他の態様、特徴及び利点は、本発明の実施形態に関する以下の説明、添付図面の参照から明白且つ明瞭である。

Claims (14)
- 閉鎖創傷(20)の止血促進用医療装置(1)であって、
前記装置は、
流体連通チャネルと、使用中は前記創傷(20)の外側に配置される近位端(16)と、創傷腔(22)内に経皮的に送達されてその中に配置される遠位端(18)と、を備える細長い外筒(10)と、
前記細長いチャネルの中に含まれる中空の膨張収縮筒状部材(12)と、
前記筒状部材(12)と流体連通する膨張可能且つ収縮可能バルーン(15)と、を具備し、
前記バルーン(15)は、前記外筒(10)に対して相対的に長軸方向へ移動可能であり、
前記バルーン(15)は、
収縮時には、前記外筒(10)内部の前記遠位端(18)付近に配置され、
膨張時には、前記外筒(10)外側で前記遠位端(18)の遠位側に配置されて、止血促進のために前記創傷(20)の組織(21)と接触し、
前記バルーン(15)は、収縮可能であって、前記創傷(20)から撤去するために前記外筒(10)へ格納可能であり、
前記バルーン(15)は、前記バルーン内部の液体から放出される一酸化窒素(NO)に対して透過性であり、
前記バルーン(15)は、前記液体によって膨張されると前記液体で満たされる、装置。 - 前記外筒(10)は、少なくとも一部が、液体に対して不浸透性であるのと同時に前記組織(21)に対して非接着性である外表面を備える、請求項1に記載の装置。
- 前記バルーン(15)は、流体との接触によって膨潤しない膜を備える、請求項1又は2に記載の装置。
- 前記バルーン(15)は、凝固血液、血小板又は他の創傷組織の接着を阻害する表面を備える、請求項1から3のいずれかに記載の装置。
- 前記バルーン(15)は、抗生物質、凝固促進剤又は血小板付着阻害剤を含む医薬物質を備える表面コーティングを備える、請求項1から4いずれかに記載の装置。
- 前記バルーン(15)は、ポリテトラフルオロエチレン(PTFE)又は微孔質ポリテトラフルオロエチレン(PTFE)でできている、請求項1から5のいずれかに記載の装置。
- 前記バルーン(15)は、前記創腔における前記バルーンの開存性を補助するための前記バルーン膨張時用の第二膨張構成と、前記バルーン収縮時用の第一折畳構成と、を備える双安定性補助ステント(17)に配置されている、請求項1から6のいずれかに記載の装置。
- 所望の圧力を前記組織に加えられるように所望のサイズの前記バルーンにする目的で、膨張時に前記バルーンの内圧を調節するための圧力センサーを備える、請求項1から7のいずれかに記載の装置。
- 前記バルーンの抜け検出用圧力センサー(18)を更に備える、請求項8に記載の装置。
- 前記バルーンを膨張させている液体を通じて、前記バルーン壁(15)から前記周辺組織(21)に伝播する超音波音波を発生させる超音波発生機(19)を更に備える、請求項1から9のいずれかに記載の装置。
- 前記流体連通チャネル(5)は、前記外筒(10)と一体的に形成され、
前記バルーンが前記外筒の外側に配置されると、少なくとも一時的な止血促進特性を有する医療排出装置が提供される、請求項1から10のいずれかに記載の装置。 - 前記創傷由来の創傷浸出液(23)用の流体連通チャネル(5)を更に備え、
前記チャネル(5)は、前記創傷腔(22)に配置される遠位端(50)を備える、請求項1から10のいずれかに記載の装置。 - 排出を促進するために、前記創傷における前記流体連通チャネルの前記遠位端に対して、前記創傷腔のガス圧よりも低いガス圧を提供するためのガス圧源を更に備える、請求項1から12のいずれかに記載の装置。
- 前記バルーンは、柔軟性がある、請求項1から13のいずれかに記載の装置。
