本発明は、一般的に、アミロイド沈着を伴う疾患の治療及び/又は予防に関する。特に、本発明は、アミロイドーシスの治療に関する。

あるいは薬学的に許容できるその塩又はモノ若しくはジエステルが好ましい。上記の好ましい実施形態を含む、SAPを循環から枯渇させる化合物は、本明細書中以下SAP枯渇化合物と呼ぶ。
本発明の態様は、組織におけるアミロイドの沈着によって引き起こされる疾患の治療及び/又は予防に関し、そのような疾患はアミロイドーシスとして知られている。
本明細書における抗SAP抗体、SAP結合抗体及びSAPに特異的な抗体への言及は、完全に重なり合うものであり、特異的にSAPに結合し、循環中に存在する他の分子と実質的に交差反応しない抗体又は抗体由来の結合フラグメントを意味する。特に、本発明において抗体は、組織におけるアミロイド沈着物におけるアミロイド線維に結合しているSAPを標的にする。
本発明の化合物は、循環SAPの枯渇をもたらす化合物を含む。

あるいは薬学的に許容できるその塩又はモノ若しくはジエステルが好ましい。上で言及した好ましい実施形態を含む、SAPを循環から枯渇させる化合物は、本明細書中以下ではSAP枯渇化合物と呼ぶ。
適切には、本明細書で述べるSAP枯渇化合物と本明細書で述べる抗ヒトSAP抗体又はそのフラグメントは、治療上有効な量を含む医薬組成物として投与する。
SAP枯渇化合物は、薬学的に許容できる塩の形で投与することができる。
成分は、単独で投与することができるが、例えば、成分が意図する投与経路及び標準的な医薬実務を考慮して選択される適切な医薬用賦形剤、希釈剤又は担体との混合物である場合に医薬組成物として一般的に投与される。
適切且つ/又は好ましい製剤は、意図する投与経路、送達方式及び所望の用量に応じて、本開示及び製剤技術の一般的技術を考慮して決定することができる。
化合物は、当技術分野で知られている技術を用いて一つ又は複数の適切な担体、希釈剤又は賦形剤と混合することなどにより医薬組成物に製剤化することができる。
Fabフラグメント、scFv及びVHHを含む抗SAP抗体及び抗体フラグメントの産生のための様々な発現系が利用可能である。例えば、原核生物及び真核生物起源の発現システムは、抗体フラグメント及び抗体融合タンパク質の大規模産生に用いることができる。
本発明の他の実施形態において、アミロイドーシスの治療に有用な材料を含む製造品、すなわち「キット」を提供する。
さらなる態様において、本発明は、特にアミロイド沈着物の本質的に完全なクリアランスのためのアミロイドーシスの治療のための、本明細書で述べるようなSAP枯渇化合物と組み合わせて使用することができる一つ又は複数の物質を同定するためのアッセイ方法を提供する。
本発明は、特に示さない限り、当業者の能力の範囲内にある化学、分子生物学、微生物学、組換えDNA及び免疫学の従来の技術を用いている。そのような技術は、文献で説明されている。例えば、J. Sambrook、E. F. Fritsch及びT. Maniatis、1989年、Molecular Cloning: A Laboratory Manual、第2版、Books 1〜3、Cold Spring Harbor Laboratory Press、Ausbel F. M.ら(1995年及び定期増刊; Current Protocols in Molecular Biology、9、13及び16章、John Wiley & Sons、New York、N.Y.)、B. Roe、J. Crabtree及びA. 本試験では、すべての動物にアミロイド促進因子を注射した後、カゼインを反復注射して、持続性急性炎症を誘発し、それによりSAA産生の十分に持続した増加を誘発してAAアミロイド沈着を促進することによって、マウスにおけるAAアミロイドーシスを誘発した。マウスSAP遺伝子を欠失させ13、ヒトSAPトランスジーンを導入した14,15、純系C57BL/6動物の特別な系統を用いた。したがって、それらはマウスSAPを発現しないが、ヒトSAPをヒトに認められるより著しく高い濃度で発現する。本発明者は、すべてのマウスが、アミロイドを有さない無処置対照マウスと比較して放射性標識ヒトSAPトレーサーの著しく高い全身保持を示すことにより大量の全身性アミロイド沈着物を有していたことを確認した。次いで三つの非常に厳密に一致させた群に以下のように投与した:
群1、(R)-1-[6-[(R)-2-カルボキシ-ピロリジン-1-イル]-6-オキソ-ヘキサノイル]ピロリジン-2-カルボン酸(CPHPC)、すなわちSAP枯渇薬16,17及びヒツジ抗ヒトSAP抗体の単回用量;
群2、CPHPC及び無関係な抗血清からの対照ヒツジIgG;
群3、無処置。
マウスSAP遺伝子の欠失を有し、ヒトSAPの遺伝子を導入した成体雄(n=61)及び雌(n=32)純系C57BL/6マウスにおいて全身性AAアミロイドーシスを誘発した15。各マウスに静脈内注射によりアミロイド促進因子9を単回投与してから4日後に、0.1M NaHCO3を含む溶液中10重量/容積%カゼインを12日間にわたり1日10回皮下注射した13。カゼインの最終注射の7日後に、KIをすべてのマウスの飲料水に導入し、3日後に各マウスに標準的な用量の125I標識ヒトSAPを静脈内注射した6,18。同じコロニーからのものであるが、他の処置を受けなかった4匹の成体雄及び4匹の雌マウスにも対照としてKI及び125I-SAPを投与した。すべてのマウスについてトレーサーの注射の24、48、72、96及び168時間後に全身計数を行って、全身アミロイド負荷の指標としての放射能の保持率を測定した。すべての時点に対照と比較してすべての処置マウスで一貫してより多くの保持が認められ、それらのすべてが実質的な全身性アミロイド沈着物を有していたことが示された18。次いで、マウスを、各群の総数及び性別分布ができる限り同じに近い、三つの厳密に一致した群に割り付けた。125I-SAPの注射の10日後に群1及び2に飲料水に1mg/mlで混入したCPHPCを投与することを開始し、実験の終了までその処置を継続した。群3には試験のこの段階又は後の段階で処置を行わなかった。CPHPCの開始の5日後に群1の各動物にリン酸緩衝生理食塩水に溶解した溶液中7mg/mlの特異抗体を含む50mg/mlの単一特異性ヒツジ抗ヒトSAP抗血清の全IgG画分の1mlを含む腹腔内注射を行った。抗血清は、100%の純度のヒトSAPによるヒツジの免疫処置により産生させた。同時に群2のすべての動物にリン酸緩衝生理食塩水に溶解した溶液中50mg/mlの単一特異性ヒツジ抗ヒトオンコスタチンM受容体抗血清の全IgG画分の1mlを含む腹腔内注射を行った。ここで抗SAP試薬に対する対照として用いたこの抗血清は、標準的な免疫化学的及び免疫組織化学的試験で、ヒトSAP、マウス血漿又は正常マウス組織との反応性を示さなかった。腹腔内注射の28日後にすべてのマウスを末期麻酔下で放血により屠殺し、各マウスから肝臓及び脾臓を摘出した。各臓器を秤量し、三つの部分に分割し、そのうちの一つも秤量し、SAPの抽出に用い、一方、第2の部分は後の免疫組織化学的及び細胞化学的分析に備えて固定せずに急速凍結し、最後の部分は慣用の組織学的検査及びコンゴレッド染色によるアミロイドの推定19のために緩衝ホルマリンで固定した。すべてのマウスについてアミロイドーシスの誘導後であるが、125I-SAPの投与前の群への割付時に体重を測定した。実験の終了時に屠殺する前にすべてのマウスの体重を再び測定した。すべてのマウスから次の四つの時点に採血した:(1)群2及び3へのCPHPCの投与の開始直前;(2)群2及び3におけるIgG製剤の注射の前日;(3)IgG注射の14日後;(4)屠殺時。コンゴレッドで染色したすべての動物の脾臓及び肝臓の切片を、各マウスが受けた処置に対して盲検化した3人の専門観察者が独立に検査し、以前に報告したように存在するアミロイドの量についてスコア採点した。100〜104のスコアは、個々の臓器のいくつかの切片におけるアミロイドーシスの一つ又は二つの小斑点に対応する100から、約10,000倍多くのアミロイドを含む大量な広範囲の沈着物に対応する104までの、おおよその10を底とするログランクスケールを表す13。異なる観察者のスコア間のほぼ100%の一致があり、したがって、最も経験を積んだ観察者のスコアを本分析に用いた。血清及び臓器の臓器抽出液中のヒトSAPの濃度は、電気免疫測定法(electroimmunoassay)5により測定した。

最終カゼイン注射後60日目及び試験群への抗SAP抗体の投与後28日目のそれらのアミロイド負荷の推定のためにマウスをすべて屠殺した。最終カゼイン注射の10日後に測定した125I-SAPの保持率により示されたように、すべてのマウスが抗SAP又は対照の投与の前に実質的なアミロイド沈着物を有していた。群間に有意な差はなく、予想通り、試験の終了時までに少数の対照マウスにアミロイド沈着物のある程度の自然退縮が認められた。しかし、群2及び3の62匹のうちでアミロイドを全く有していなかった1匹の動物も存在していなかった。対照的に、抗SAP抗体並びにCPHPCを投与した31匹のマウスのうちの23匹は、脾臓又は肝臓に検出できるアミロイドを有していなかった。抗SAP投与動物で認められたアミロイド沈着物は、抗SAPを投与しなかった62匹のマウスのうちの50匹に存在した多数の沈着物より桁違いに少ない少数の顕微鏡斑点であった。CPHPC及び無関係の抗血清からの対照ヒツジIgG画分の投与を受けたマウス(群2)が投与を全く受けなかった群3対照と量が区別できない大量のアミロイドを有していたことから、抗SAP投与が群1におけるアミロイド沈着物のこの注目に値するクリアランスの原因であったことは明確に明らかである。
確立された全身性アミロイド沈着物を有する患者のCPHPC及び抗SAP抗体を用いた併用治療により、沈着物の迅速で、本質的に完全なクリアランスが安全且つ効果的にもたらされた。これは、以前には患者若しくは動物モデルにおいて、又は他の方法により全く達成されなかった。本発明は、すべての種類の後天的及び遺伝的な全身性及び局所性アミロイドーシスに、また、アルツハイマー病及び2型糖尿病を含むアミロイド沈着物に関連する他のすべての疾患に適用されるであろう。
全身性アミロイドーシスに罹患している患者は、アミロイドーシスの推定につながる診察及び慣用の調査の後の罹患組織の生検材料の専門家による組織化学的検査による明確な確認により診断される。放射性標識SAPシンチグラフィーは、英国NHS国立アミロイドーシスセンター(UK NHS National Amyloidosis Centre)及びRoyal Free病院のUniversity College Londonの医学部のアミロイドーシス及び急性期タンパク質センターにおいて実施されている。組織検査により、存在するアミロイドの特定の種類が同定され、心臓のエコー検査と組み合わせたスキャンにより、アミロイドが存在する部位が示され、その程度が定量される。臓器機能の慣用の臨床的調査により、アミロイドの沈着によって引き起こされる組織及び臓器の損傷の程度及び重症度、並びにアミロイドの沈着をもたらした可能性がある基礎にある一次疾患の存在及び重症度が確認される。
実験プロトコール及び方法
上の実施例1に記載したのと全く同じプロトコール及び試薬を用いた実験において、実施例1の場合と同じヒツジ抗ヒトSAP抗血清のIgG画分の次の用量をそれぞれ各群5匹のマウスの異なる群に投与した:50mg(実施例1と同じ用量);10mg;0.4mg;なし。これらの用量における抗SAP抗体の量は、それぞれ7mg、1.4mg、0.28、0.056mg及び0であった。
抗SAP抗体の最高用量を投与した2群において、脾臓及び肝臓における本質的にすべてのアミロイド沈着物が除去された。他の抗体投与群のいずれにおいてもアミロイド沈着物の減少はなく、抗体を投与しなかった対照群との差はなかった。
実施例1に記載したプロトコールで単回投与したこの特定のヒツジポリクローナル抗SAP抗体の最小有効用量は、0.28mgを超えており、1.4mgの用量が最大の有効性を示した。
実験プロトコール及び方法
上の実施例1に記載したのと全く同じプロトコール及び試薬を用いた実験において、それぞれ各群5匹のマウスの異なる群に、7mgの抗SAP抗体を含む50mgのIgG画分を含む当実験と全く同じ投与を受けさせ、抗体の投与後それぞれ1、2、3、4、7、10、14、21及び25日目に屠殺した。各群の屠殺した時に各動物から得た血漿サンプルを、実験の終了時の単一バッチでの生化学的分析の前に凍結保存した。死亡時に各マウスから脾臓及び肝臓を摘出し、実施例1に記載したように正確に分析のために処理した。アミロイド沈着の程度を測定するためのコンゴレッド染色に加えて、ヘマトキシリン及びエオシンで標準的方法で染色した固定組織切片を組織病理学的評価のために検査し、適切に処理した組織を十分に有効性が立証されている日常的に利用可能な試薬及び方法を用いて関連するタンパク質及び細胞マーカーについて免疫組織化学及び免疫細胞化学染色により分析した。一部の組織を標準的透過型電子顕微鏡によっても検査した。スライド標本のすべての検査、読取り及びスコア採点は、サンプルの同一性に対して盲検化した専門観察者が行った。対照組織は、全身性AAアミロイドーシスの全く同じ誘導を受けたが、抗SAP活性のない対照ヒツジIgGの投与を受けたマウスによって準備した。
抗SAP抗体注射の24時間後という最も早期の時点に、ヘマトキシリン及びエオシン染色により主としてマクロファージといくつかの顆粒球と同定された炎症性細胞によるアミロイド沈着物の大規模な浸潤が既にあった(図4)。この外観は、抗体を投与しなかった対照マウスにおける一般的に無細胞性のアミロイド沈着物と著しく異なっていた。1日目の組織の電子顕微鏡写真は、マクロファージ及び顆粒球とアミロイド沈着物との完全な咬合を示していた(図5)。次の数日間にわたり、圧倒的に単核で、アミロイドの漸進的に断片化し、縮小しつつある凝集塊を取り囲み、明らかに包み込んでいる増加しつつある数の多核巨細胞を含んでいたので、アミロイド沈着物の細胞浸潤は持続していた(図4)。細胞浸潤及びアミロイドの量は7〜10日目までに著しく減少し、15日目までにはアミロイド沈着物及び浸潤性細胞がほぼ消失した。21及び25日目の組織学的外観は、正常な非アミロイドーシス性組織とほとんど区別できなかった。
CPHPCにより循環SAPが枯渇したアミロイドーシスマウスへの抗SAP抗体の投与により、アミロイド沈着物の非常に速やかで強度な主としてマクロファージ浸潤が誘発された。これらの細胞の多くは融合して多核巨細胞を形成し、これらがアミロイドの島を取り囲み、取り込み、これに続いて、沈着物の速やか且つほぼ完全なクリアランスが起った。抗体投与の15日後までに、アミロイドは実質上消失し、組織の細胞集団は速やかに正常に戻った。投与の臨床的に明らかな有害作用はなく、各群のすべてのマウスから死亡時に採取した血漿サンプルは、腎臓又は肝臓機能、血漿脂質、総蛋白又はアルブミンの傷害、あるいは筋損傷の生化学的証拠を示さなかった。したがって、これらの動物における実質的な内臓アミロイド沈着物の本質的に完全なクリアランスは、臨床的に無影響であり、有害でなかった。
実施例1で詳細に述べた標準的プロトコールは、ヒトSAPの発現を可能にするためにマウスSAP遺伝子を欠失させ、ヒトSAPトランスジーンを挿入したマウスを用いるものである。これは、抗SAP抗体の投与の直前から開始し、実験の終了まで続ける飲料水中へのCPHPCの投与も用いた。そのようなCPHPCによる長期の治療の必要を探究するために、以前の実施例におけるようなマウスSAPノックアウトではなく、野生型バックグラウンド上にヒトSAPの遺伝子を導入したマウスに、非経口CPHPCの単回投与を用いた。SAPに無関係な遺伝子改変を有するマウスにおける試験で重要である他の手法において、アミロイドーシス野生型非トランスジェニックマウスは、単離された純粋なヒトSAPの単回非経口注射によりヒトSAPが「負荷された」アミロイド沈着物を有していた。
上の実施例1と全く同様な野生型バックグラウンドに基づくヒトSAPトランスジェニックマウスにおいてAAアミロイドーシスを誘導し、確認した。次いで、マウスに1mgのCPHPCを単回腹腔内注射し、続いて、5時間後に試験群(n=10)に以前の実施例で用いたのと同じヒツジ抗ヒトSAP血清の25mgのIgG画分を腹腔内注射した。対照マウス(n=8)に抗SAP活性を有さないヒツジIgGを投与した。16日後にコンゴレッド染色によりアミロイド負荷を推定するために、すべてのマウスを屠殺した。
野生型バックグラウンドに基づくヒトSAPトランスジェニックマウスにおけるアミロイド沈着物は、ヒトSAPトランスジェニックマウスノックアウトマウスにおけるすべての以前の実験で認められたように、抗SAP抗体を単回投与することにより効果的に除去された。
これらの結果は、アミロイド沈着物におけるヒトSAPの存在と循環中の有意な濃度のヒトSAPの非存在が、本発明による抗SAP抗体の治療効力を得るのに十分なものであることを示している。これらの十分な条件は、ヒトSAPトランスジェニック動物におけるCPHPCの単回非経口投与を用いることにより、又は野生型マウスにおけるアミロイド沈着物にヒトSAPの単回非経口投与によりヒトSAPを負荷することにより、最も容易に達成された。後者のモデルは、長期の同系交配する必要なしに遺伝改変マウスにおける抗SAP抗体の作用機序の分析を可能にする(下の実施例6を参照)ので、極めて有用である。
補体欠乏マウスと正常な補体十分な野生型動物との間の治療の効率を比較することにより、抗SAP抗体によるアミロイドのクリアランスにおける補体の役割を検討した。C1qの遺伝子の標的化欠失は、抗原-抗体複合体へのC1qの結合により開始される古典的補体経路の活性化を阻害するが、補体の主要な生物学的機能である走化性及びオプソニン作用を担う枢軸となる機能段階であるC3活性化は、第二経路及びレクチン経路を経て、並びに非補体セリンプロテイナーゼによる直接的なC3切断により依然として進行し得る。C3の遺伝子の標的化欠失により、これらの機能が完全に排除される。
補体欠乏マウスの2群(C3ノックアウト(n=14)及びC1qノックアウト(n=12))において実施例1で詳述したのと全く同様にAAアミロイドーシスを誘導し、確認した。次いで、すべてのマウスにヒトSAPを負荷し、その後アミロイド負荷の推定のために15日目に屠殺する前に、実施例5で述べたように各群の2匹を除くすべてに抗SAPを投与した。
以前に抗SAPを投与し、実施例1〜5で述べたすべての補体十分のマウスにおけるアミロイド沈着物の効果的なクリアランスと著しく異なり、各種類の2匹の補体欠乏対照マウスよりも高度に断片化された外観を有する傾向はあったが、補体欠乏マウスの両群に依然として大量のアミロイドが存在していた。脾臓アミロイドスコアは、C1q欠乏マウス(中央値104、範囲102〜104)においてC3欠乏マウス(中央値103、範囲102〜104)よりわずかに高かったが、これは統計的有意性に達しなかった。
C1q又はC3を欠くマウスにおいて、抗SAPの投与によりアミロイド沈着物は除去されなかった。したがって、抗SAPの治療効力は、非常に実質的に補体依存性であり、理論的にはそれらのFc(γ)受容体を介して食細胞に結合する(engage)可能性があるIgG抗体のみによって媒介されない。それにもかかわらず、補体欠乏マウスにおける残存アミロイド沈着物のより高度に断片化した外観は、抗体のみの少なくともある程度の効果を示唆するものである。また、C3欠乏動物と比較してC1q欠乏動物におけるクリアランスがより大きい傾向は、C3活性化は重要であり、一般的にIgG抗体により活性化されるC1q及び古典的経路が存在しない場合でさえ、ある程度の補体活性化が起っている可能性があることを示唆するものである。
IgG抗体による補体活性化は、Fc領域を含む完全なインタクト分子を必要とし、C1qの結合により開始される古典的経路を経て進行する。ある種の抗体-抗原系において、第二経路を経る効率がより低い補体活性化は、F(ab)2フラグメントにより媒介され得る。抗SAP抗体によるアミロイド除去の補体依存性を確認し、抗体のFc領域の潜在的な必要を検討するために、F(ab)2抗SAP抗体の効果を試験した。
実施例1で詳述したのと全く同様に野生型C57BL/6マウスにおいてAAアミロイドーシスを誘導し、確認した。実施例5で詳述したようにアミロイド沈着物にヒトSAPを負荷した後、マウスの群にヒツジポリクローナル抗ヒトSAP抗血清(n=8)、緩衝媒体のみ(n=10)又はpH4.0でペプシン消化により産生させたIgG画分のF(ab)2フラグメント(n=5)を投与した。注射した抗SAP抗体活性の用量は、F(ab)2を投与したマウス当たり7.28mgであり、全IgGを投与したマウス当たり7mg(通常のように総IgG 50mg)であった。コンゴレッド染色によるアミロイド負荷の推定のために14日後にすべてのマウスを屠殺した。
IgG抗SAP抗体を投与したマウスではアミロイド沈着物のクリアランスはほぼ完全であり、脾臓アミロイドスコアの中央値(範囲)は100(100〜103)であったのに対して、対照マウスではアミロイド沈着物は大量であり、脾臓アミロイドスコアの中央値(範囲)は105(104〜105)であった。F(ab)2を投与したマウスは、無処置対照より少ないアミロイドを有しており、スコアの中央値は102で、範囲は100〜104であったが、全IgG抗SAP抗体を投与したマウスより実質的に多かった。
全IgGと比較してF(ab)2フラグメントの分子量は小さいことから、この試験で用いたF(ab)2抗SAP抗体のモル用量は、IgG抗体より約1/3大きかった。しかし、アミロイドクリアランスに対する影響は実質的により小さく、抗SAP抗体の主要な作用はFc領域が必要であることが実証された。全IgGは、補体が十分であるマウスにおけるF(ab)2より補体欠乏動物において有効性が低かったので、これは、Fc(γ)受容体による細胞認識の直接関与のためではない。用いた高用量のF(ab)2は第二経路を経てある種の補体を活性化することができた可能がある。
本明細書で述べる組織学的及び組織化学的試験により、抗SAP抗体を投与したマウスにおけるアミロイド沈着物に浸潤し、取り囲み、食作用を示す細胞はマクロファージであることが示されている。それらが実際にアミロイドのクリアランスを担っていることを確認するために、すべてのマクロファージ活性をリポソームクロドロネートの投与により阻害したマウスにおける抗SAP投与の効果を試験した。試薬、実験プロトコール及びリポソームクロドロネートのマクロファージ機能に対する影響は、十分に確立されており、広範に文書化されている22。
上の実施例1で詳述したプロトコールを正確に用いて野生型マウスにおいてAAアミロイドーシスを誘導し、確認した後に、すべての動物に10mgの単離された純粋なヒトSAPを単回腹腔内投与して、それらのアミロイド沈着物にヒトSAPを負荷した。次いで、試験群(n=13)に直ちに、またその後2、7及び14日目に0.3mlのリポソームクロドロネートを腹腔内投与した。ヒトSAP注射後3日後に一つの対照群(n=12)及び試験群にヒツジ抗ヒトSAP抗血清の50mgのIgG1画分を単回腹腔内投与した。第二の対照群(n=12)には抗SAPを投与せず、また他の追加投与を行わなかった。試験及び抗体対照群に抗SAPを投与してから14日後にコンゴレッド染色によりアミロイド負荷を推定するために、すべてのマウスを屠殺した。
抗SAPの投与は、抗体を投与しなかった群と比較してアミロイド沈着物のほぼ完全なクリアランスをもたらし、脾臓アミロイドスコアの中央値(範囲)はそれぞれ100(0〜103)及び103(103〜104)であった。マクロファージの機能を完全に除去することが知られている療法においてリポソームクロドロネートの投与を受けたマウスにおいては、アミロイド沈着物のクリアランスはなく、アミロイド負荷スコアは105(104〜105)であった。
この実験の結果は、マクロファージ機能が抗ヒトSAP抗体によるアミロイド沈着物のクリアランスに不可欠であることを確認するものであった。
抗SAP抗体の投与は、ヒトSAPが、アミロイド沈着物中に存在するが、ヒトSAPトランスジェニックマウスへのCPHPCの投与により、又は野生型マウスへのヒトSAPの受動的投与により循環に存在しないAAアミロイドーシスマウスにおける内臓アミロイド沈着物の速やか且つほぼ完全なクリアランスを非常に再現性よく引き起こす。

Claims (13)
- 血清アミロイドP成分を循環から枯渇させる化合物を血清アミロイドP成分に特異的な抗体と組み合わせて含む医薬組成物であって、血清アミロイドP成分を循環から枯渇させる化合物が、(R)-1-[6-[(R)-2-カルボキシピロリジン-1-イル]-6-オキソヘキサノイル]ピロリジン-2-カルボン酸あるいは薬学的に許容できるその塩又はモノ若しくはジエステルである、医薬組成物。
- エタノールアミン及び/又はホスホエタノールアミン及び/又はβ-Dガラクトピラノースの4,6-ピルビン酸アセチル及び/又はカルシウム及び/又はIL-4及び/又はIL-3を含まない、請求項1に記載の医薬組成物。
- 前記血清アミロイドP成分に特異的な抗体が、単球からの線維細胞の形成の阻害に機能するSAPの部分を標的にしない、請求項1又は請求項2に記載の医薬組成物。
- アミロイド疾患の治療に使用するための、請求項1〜3のいずれかに記載の組成物。
- アミロイド疾患が全身性アミロイドーシス、限局性アミロイドーシス、アミロイド沈着に関連する疾患、アルツハイマー病及び2型糖尿病からなる群から選択される、請求項4に記載の組成物。
- 血清アミロイドP成分を循環から枯渇させる化合物を以前に投与された患者におけるアミロイド疾患の治療又は予防に使用するための、血清アミロイドP成分に特異的な抗体を含む組成物であって、血清アミロイドP成分を循環から枯渇させる化合物が、(R)-1-[6-[(R)-2-カルボキシピロリジン-1-イル]-6-オキソヘキサノイル]ピロリジン-2-カルボン酸あるいは薬学的に許容できるその塩又はモノ若しくはジエステルである、組成物。
- 血清アミロイドP成分に特異的な抗体をそれ以後に投与しようとする患者におけるアミロイド疾患の治療又は予防に使用するための、血清アミロイドP成分を循環から枯渇させる化合物を含む組成物であって、血清アミロイドP成分を循環から枯渇させる化合物が、(R)-1-[6-[(R)-2-カルボキシピロリジン-1-イル]-6-オキソヘキサノイル]ピロリジン-2-カルボン酸あるいは薬学的に許容できるその塩又はモノ若しくはジエステルである、組成物。
- アミロイドーシスの治療又は予防用の組成物の製造における、血清アミロイドP成分に特異的な抗体と組み合わせた、血清アミロイドP成分を循環から枯渇させる化合物の使用であって、血清アミロイドP成分を循環から枯渇させる化合物が、(R)-1-[6-[(R)-2-カルボキシピロリジン-1-イル]-6-オキソヘキサノイル]ピロリジン-2-カルボン酸あるいは薬学的に許容できるその塩又はモノ若しくはジエステルである、使用。
- 血清アミロイドP成分を循環から枯渇させる化合物及び血清アミロイドP成分に特異的な抗体を含む、アミロイドーシスの治療に使用するためのキットであって、血清アミロイドP成分を循環から枯渇させる化合物が、(R)-1-[6-[(R)-2-カルボキシピロリジン-1-イル]-6-オキソヘキサノイル]ピロリジン-2-カルボン酸あるいは薬学的に許容できるその塩又はモノ若しくはジエステルである、キット。
- 血清アミロイドP成分を循環から枯渇させる化合物及び血清アミロイドP成分に特異的な抗体の同時個別又は逐次投与のために製剤化されている、請求項9に記載のキット。
- 血清アミロイドP成分に特異的な抗体がヒト化抗体である、請求項1〜7のいずれか一項に記載の組成物。
- 血清アミロイドP成分に特異的な抗体がヒト化抗体である、請求項8に記載の使用。
- 血清アミロイドP成分に特異的な抗体がヒト化抗体である、請求項9又は請求項10に記載のキット。
