Description
加齢黄斑変性(AMD)、糖尿病網膜症、糖尿病黄斑浮腫(DME)、後部ぶどう膜炎、脈絡膜新生血管(CNV)、及び嚢胞様黄斑浮腫(CME)は、視覚を脅かす眼の奥の疾患である。加齢黄斑変性及び糖尿病網膜症は、米国などにおいて、視力障害の重大な原因となっており;一般に、これらの病気は、網膜を傷つける血管新生(眼中の望ましくない血管成長)に起因し、最終的に失明を引起こす恐れがある。後部ぶどう膜炎は、慢性炎症性の疾患であり、米国において、失明の約10パーセントの原因となっている。
本明細書中に開示される一発明は、原位置で形成される架橋ヒドロゲルであり、該ヒドロゲルは、治療薬を放出し、これを使用して眼の奥の疾患を治療することができる。この実施態様において、水性のポリマー前駆体と薬剤とを生体外にて、流動性の濃度/粘度で組合せて、かつ小口径の針を通過し、硝子体内又は結膜下の経路を経由して眼中に注入し、ここで、該前駆体が、時間をかけて該薬剤を放出する架橋ヒドロゲルを形成する。該ヒドロゲルを処方して、眼の中又は眼の周囲の組織に接着し、薬剤放出の効果及び安定性を増強する、炎症を引起こすことなく生体適合性の成分に分解する、かつ所定の位置で架橋することができる。このように、形成される形状安定性のヒドロゲルは、薬剤を効果的に送達でき、かつ優位には、適切に制御されたサイズ、形状、及び表面積を有する。すでに形成された材料の代わりに、可溶性又は流動性の前駆体を使用することができるので、テノン嚢下注射用の小口径針又は鈍端(blunt tip)カニューレを使用して該材料を注入することができる。
生体適合性材料はまた、効果的に自己除去(self-removing)する、又は除去される場合に、自己除去する部分だけを残すようにさせることができる。いくつかの実施態様は、柔らかく、可動性の、架橋された生体材料で作られるインプラントであり、該インプラントの回収が必要である場合に、小開口部を通して引き抜くか、又は別の方法で排除することができる程の強度である。
水性溶液中の前駆体から原位置で作られる、局所的形成ヒドロゲルは、目薬送達のための薬剤又は他の治療薬の貯蔵所として機能することができる。こうした貯蔵所は、例えば、眼の表面に局所的に、結膜と強膜の中、及び/又はその間に経強膜的に、眼内に注入して、又は眼周囲に形成してなど、必要に応じて形成することができる。
様々な深刻な眼疾患が存在し、投与計画による治療を必要とする。本明細書中に、組織上の原位置で形成し、薬剤を送達することができるヒドロゲルについて記載する。原位置とは、その使用を意図する部位で材料を形成することを意味する。従って、患者において、例えば、制御された放出のための薬剤貯蔵所として、ヒドロゲルの使用を意図するその部位で、該ヒドロゲルを形成することができる。
該前駆体は、反応を誘発されて、架橋ヒドロゲルを形成することができるものである。一般に、該前駆体は、重合可能であり、かつ多くの場合、常にではないが、重合可能な前駆体である架橋剤を含む。従って、重合可能な前駆体は、官能基を有する前駆体であり、互いに反応して繰返し単位から作られるポリマーを形成する。
このように、いくつかの前駆体は、連鎖成長重合によって反応し、付加重合とも呼ばれ、かつ二重又は三重結合を組込んでいるモノマーが、互いに結合することを含む。こうした不飽和のモノマーは、余剰の内部結合を有し、該結合は、切断し、かつ他のモノマーと結合して、繰返し鎖を形成することが可能である。モノマーは、少なくとも1つの基が他の基と反応してポリマーを形成する、重合可能な分子である。マクロモノマーは、ポリマー又はオリゴマーであり、多くの場合、その末端に、少なくとも1つの反応基を有し、該反応基が該マクロモノマーを、モノマーとして機能できるようにする。各マクロモノマー分子は、該反応基の反応によってポリマーに付加される。このように、2つ以上のモノマー又は他の官能基を有するマクロモノマーは、共有結合性の架橋を形成する傾向がある。付加重合は、例えば、ポリプロピレン又はポリ塩化ビニルの製造に関与する。付加重合の1種に、リビング重合がある。
モノマーは、ポリマー又は小分子とし得る。ポリマーは、多数の小分子(モノマー)を規則正しく組合せて形成される有機分子であり、少なくとも2つのモノマーから形成されるポリマー及びオリゴマーを含み、本明細書中の本用語は、約20未満のモノマーの繰返し単位を有するポリマーを意味する。一般に、小分子は、約2000ダルトン未満の分子を意味する。
合成前駆体を使用できる。合成とは、天然には見られない、又は通常、人体には見られない分子を意味する。いくつかの合成ポリマーは、天然に存在するアミノ酸を含まない、又はアミノ酸配列を含まない。いくつかの合成分子は、天然には見られない、又は通常、人体には見られない、例えば、ジ-、トリ-、又はテトラ-リジンなどのポリペプチドである。いくつかの合成分子はアミノ酸残基を有するが、隣接する1、2、又は3つだけであり、アミノ酸又はそれらの集団は、非天然のポリマー又は群によって分離されている。
いくつかの前駆体は、開始剤を用いて反応する。開始剤基は、ラジカル重合反応を開始することが可能な化学基である。例えば、それは、分離成分、又は前駆体上の垂れ下がった基(pendent group)として存在することができる。開始剤基には、熱開始剤、光学活性可能な開始剤、及び酸化-還元(レドックス)系が含まれる。長波長のUV及び可視光に光学活性化可能な開始剤に含まれるのは、例えば、エチルエオシン基、2,2-ジメトキシ-2-フェニルアセトフェノン基、他のアセトフェノン誘導体、チオキサントン基、ベンゾフェノン基、及びカンファーキノン基である。熱反応性開始剤の例に含まれるのは、4,4'アゾビス(4-シアノペンタン酸)基、及び過酸化ベンゾイル基の類似体である。Wako Chemicals USA社(リッチモンド、Va.)から入手可能なV-044など、いくつかの市販されている低温ラジカル開始剤を使用して、体温でラジカル架橋反応を開始して、前述のモノマーとともにヒドロゲル被覆を形成することができる。
該前駆体は、官能基を有し、互いに反応して、原位置で材料を形成する。一般に、該官能基は、重合若しくは求電子剤-求核剤反応で互いに反応するための反応中心を有するか、又は他の重合反応に関与するように形成されている。本明細書中の前駆体の節で、重合反応の様々な態様を論じている。
従って、いくつかの実施態様において、前駆体は、重合可能な基を有し、重合分野で使用される、光開始又はレドックス系によって活性化され、又は例えば、求電子性官能基が、カルボジイミダゾール、塩化スルホニル、クロロカルボナート、n-ヒドロキシスクシンイミジルエステル、スクシンイミジルエステル、若しくはスルファスクシンイミジル(sulfasuccinimidyl)エステルであるか、又は米国特許第5,410,016号若しくは第6,149,931号に記載されたものである(これらは、本明細書にて明確に開示されたものと矛盾しない限りにおいて、その範囲が本明細書に参照により組込まれている。)。該求核性官能基は、例えば、アミン、ヒドロキシル、カルボキシル及びチオールとし得る。
求電子剤の別の部類は、例えば、米国特許第6,958,212号に記載されるようなアシルであり、該文献は、特に、反応しているポリマーに対するマイケル付加のスキームについて記載している。
概して、前駆体と、遅延架橋用の化学種を有する流動性の組成物とを組合せて、共有結合性の架橋材料を原位置で調製することができ、該材料は、適当な期間をかけて放出される治療薬を含む。概して、該架橋反応は、生理的条件下、水性溶液中で生じる。好ましくは、該架橋反応は、重合の熱を発しないか、又は重合の開始若しくは誘発に外部のエネルギー供給源を必要としない。一般に、光化学的開始は、例えば、眼への損害を回避するために避けられる。注入される材料の場合、その粘性を制御して、該材料が小口径のカテーテル又は針を通って導入されるようにする。眼の周囲に適用する材料の場合、カテーテル/針などを通って任意に送達され、更に、粘性を制御して、ゲルを形成するまで、前駆体を所定の位置に保持して、該前駆体が意図する使用部位から流出しないようにする。
膨潤を低減させるための別の実施態様は、より膨潤しない傾向がある、より低親水性材料を用いて、親水度を制御することであり;例えば、PEOなどの高親水性の材料と、PPOなどのあまり親水性ではない材料とを、又はアルキルなどの疎水性基さえも、組合わすことができる。
該ヒドロゲルの固体パーセントは、機械的特性及び生体適合性に影響を与え、かつ競合する要件間のバランスを反映し得る。一般に、例えば、約2.5%〜約25%の比較的低い固形物含有量は、最も有用である傾向があり、例えば、約2.5%〜約10%、約5%〜約15%、又は約15%未満を含む。
哺乳類の眼の構造は、3つの主要な層又は膜:線維膜、血管膜、及び神経膜に分けることができる。眼球線維膜としても公知の該線維膜は、角膜及び強膜からなる眼球の外層である。該強膜は、眼の支持壁であり、かつ眼にその白色の大部分を与える。該強膜は、角膜(眼の透明な前部)から眼の後側の視神経まで伸びている。該強膜は、繊維性、弾性、かつ保護性の組織であり、しっかりと詰まったコラーゲン原線維からなり、約70%の水を含有している。
該線維膜を覆っているのが、結膜である。該結膜は、強膜(眼の白色部分)を覆い、かつ眼瞼内部の内側を覆う膜である。該結膜は、涙腺よりも少量の涙ではあるが、粘液及び涙を産生して眼を潤すのを助ける。典型的に、該結膜は、下記の3つの部分に分けられる:(a) 眼瞼(palpebral)又は眼瞼(tarsal)結膜は、眼瞼の内側を覆っている結膜であり;該眼瞼結膜は、上円蓋及び下円蓋で折り返して、眼球結膜となる、(b) 結膜円蓋:眼瞼の内部及び眼球が接する結膜である、(c)眼球(bulbar)及び眼球(ocular)結膜:眼球を覆っている結膜であり、強膜を覆う。結膜のこの領域は、しっかりと結合し、かつ眼球運動とともに動く。
眼球血管膜としても公知の血管膜は、中層であり、虹彩、毛様体、及び脈絡膜を含む。該脈絡膜は血管を含み、網膜細胞に酸素を供給し、呼吸による老廃物を取除く。
また、哺乳類の眼は、2つの主要部分:前部及び後部に分けることができる。該前部は、前及び後眼房からなる。該前眼房は、虹彩の前面、及び角膜内皮の後部に位置し、かつ瞳孔、虹彩、毛様体、及び水性の体液を含む。該後眼房は、虹彩の後部及び硝子体の表面の前部に位置し、ここで、水性環境中の前部及び後部の嚢間に、水晶体及び毛様体小帯が位置する。
眼に入った光は、角膜を通過し、2つの房水の1つ目に入る。眼の全屈折力のおよそ2/3が、固定の曲率を有する角膜によるものである。該房水は、角膜と眼の水晶体とを繋ぐ透明な物質であり、角膜の凸形状の維持を助け(水晶体での光の収束に必要である。)、かつ角膜内皮に栄養分を与える。
図5は、眼10の又はその付近の特定の送達位置を示す。一領域は、眼10の表面上の点で示している領域60の局所である。別の領域は、番号62で示されるような硝子体内領域、又は番号64で示されるような経強膜的な領域である。使用において、例えば、注射器66、カテーテル(示さず)、又は他の機器を使用して、針68を任意に通過させて、70のように硝子体内的に、又は72のように眼周囲的に、のいずれかで、ヒドロゲル又はヒドロゲル前駆体を、眼の中に送達する。薬剤又は他の治療薬が、眼内の部位に放出される。眼の奥の疾患の場合、薬剤は、眼周囲又は硝子体内の経路を経由して、標的付近の領域74に向かい、ここで、それらは生物特性で作用し治療を実現する。
本明細書中に記載の材料を使用して、薬剤又は他の治療薬(例えば、造影剤若しくはマーカーなど)を、眼又は近くの組織に送達することができる。病状のいくつかは、眼の奥の疾患である。用語、眼の奥の疾患は、当業者には理解されるが、一般に、脈管構造、及び網膜、黄斑、又は脈絡膜の統合性に影響を与える、後部の眼疾患を意味し、視力障害、視界の消失(loss of sight)、又は失明を引起こす。該後部の病状は、年齢、外傷、外科的処置、及び遺伝性の因子に起因する。いくつかの眼の奥の疾患は;加齢黄斑変性(AMD)、嚢胞様黄斑浮腫(CME)、糖尿病黄斑浮腫(DME)、後部ぶどう膜炎、及び糖尿病網膜症である。いくつかの眼の奥の疾患は、黄斑変性症又は糖尿病網膜症などの、望ましくない、血管新生又は血管増殖に起因する。これら及び他の症状のための薬剤治療の選択肢を、本明細書の別の場所に更に記載する。
適用の一方法は、前駆体と他の材料(例えば、治療薬、増粘化剤(viscosifying agent)、促進剤、開始剤など)との混合物を、針、カニューレ、カテーテル、又は中空ワイヤーを通して、眼の中又は近隣部位に適用することである。該混合物を、例えば、手動制御の注射器又は機械制御の注射器(例えば、シリンジポンプ)を使用して送達できる。或いは、二重式注射器(dual syringe)、若しくは多筒式注射器(multiple-barreled syringe)、又はマルチルーメン系(multi-lumen system)を使用して、該部位で又は付近で、前駆体を混合することができる。
試験を行った1つの系は、薬剤を希釈剤に混合すること、かつ200マイクロリッターの薬剤/希釈剤を1mlの注射器に吸引することを含む。トリリジンからなる、約66mgの前駆体粉末を、別の1mlの注射器に注入した。2つのシリンジを、メス-メスのルアー継手を介して取り付け、該溶液を、乾燥前駆体が完全に溶解するまで、注射器間で前後に動かした。多腕の求電子性前駆体の水200μl中の溶液を、第3の1mlの注射器に吸入した。別のメス-メスルアー継手を使用して、再構成したPEG/薬剤の溶液と求電子性前駆体とを混合した。該溶液を、少なくとも約10回、前後に迅速に注入して(inject)、良好な混合を行うようにした。該溶液を1の注射器に吸入し、次いで、更なる使用に利用した。
眼の奥の疾患は、例えば、局所、全身、眼内、及び結膜下の送達経路を利用して、薬剤で治療することができる。全身及び局所的な薬剤送達様式は、後部の疾患を治療するレベルの治療薬を送達するのに、不十分である。これらの薬剤送達方法は、眼内及び全身系の本来の解剖学的障壁によって、拡散及び薬剤の希釈という問題に直面し、重大な患者の副作用(1日複数回の投与のため)、乏しい生体利用効率、及びコンプライアンス問題を引起こす。結膜下、眼球後方、又はテノン下の空間を使用する、眼部のヒドロゲルインプラントの特定の薬剤送達は、局所及び全身的な経路と比較して、より安全で、かつ強化された網膜への薬剤送達系を提供する可能性を有する。
三次元の統合性を有するインプラントの1つの利点は、細胞浸潤に抵抗する傾向があり、局所的に投与された薬剤が、貪食され、かつ早々に該部位から除去されるのを防ぐことができることである。その代わりに、送達されるまで、それは所定の位置にとどまる。対照として、微小粒子、リポソーム、又はペグ化タンパク質は、生物学的効果を発揮するより前に、網内系によって、体内から迅速に除去される傾向がある。
後部の眼疾患において、治療量の薬剤の網膜への送達は、課題が残っている。抗VEGF剤の硝子体腔への硝子体内注射は、黄斑変性症のような、慢性的な加齢に関わる疾患を抑え、場合によっては、逆転させるという見込みを示したが、これらの技術及び方法は、危険及び副作用を伴わないわけではない。硝子体腔への治療薬の硝子体内投与は、白内障、眼内炎、及び網膜剥離を引起こす恐れがある。この形態の治療は、12ヶ月の期間、毎月抗VEGF剤の眼内注入を受けることを、患者に要求し、このため、感染症、硝子体の芯(wick)及び網膜の剥離の危険が増大する。原位置でのヒドロゲルの生分解性インプラントに向けられた実施態様は、眼の奥の疾患の効果的な代替治療を提供し、かつ硝子体内注射の反復に関連する共通の副作用を低減させることが期待される。硝子体内の生分解性薬剤送達インプラント系の実施態様を、下記に要約する。
他の節でより詳細に記載するように、原位置のヒドロゲル薬剤送達インプラントの薬剤貯蔵所は、例えば、約1〜約3ヶ月の範囲で、制御された長期の薬剤放出のために設計することができ;かつ例えば、加齢黄斑変性、糖尿病網膜症、糖尿病黄斑浮腫、及び嚢胞様黄斑を含む、後部の疾患の治療に、任意に向けることができる。該デバイスは、様々な症状に対して、薬剤最大積載量の様々な種類の治療薬を運ぶことができ、該治療薬のいくつかに含まれるのは、例えば、ステロイド、抗生物質、NSAIDS及び/又は抗血管新生薬、並びにそれらの組み合わせである。
該ヒドロゲルを、結膜が存在する、又は存在なしに、強膜組織上で形成することができる。該ヒドロゲルは、強膜又は強膜付近の他の組織と接着して、対象とする組織を通して、薬剤の拡散を促進する、又は必要に応じて、治療薬を向かわせるための安定した貯蔵所を提供することができる。いくつかの実施態様において、眼の結膜を、除去、浸軟、切断、又は細片化せずに、該ヒドロゲルの埋め込み、又は注射のために、組織を強膜から持ち上げて、強膜の特定領域に接近することができる。ヒドロゲルは、原位置で形成され、表面領域に層を形成し、かつ接着する。該結膜がまだ存在するか、又は接触するのに十分な機械的完全性を維持する場合、該結膜を、該組織と接触させてもよい。いくつかの実施態様において、該ヒドロゲルは、少なくとも50%、75%、80%、90%、又は99% w/wの水溶性の前駆体(水若しくは溶媒、又はヒドロゲル成分の重量を無視するために、親水性前駆体の重量を測定し、全前駆体の重量で割って算出する。)を含み、該ヒドロゲルの非接着特性を強化する。いくつかの実施態様において、そのような親水性前駆体は、実質的に、PEOsを含む。いくつかの実施態様において、組織接着を低減する薬剤は、細胞有糸分裂、細胞遊走、又はマクロファージ遊走若しくは活性化によって媒介され、該薬剤に含まれるのは、例えば、抗炎症剤、細胞分裂抑制剤、抗生物質、PACLITAXEL、MITOMYCIN、又はタキソールである。
いくつかの態様において、該ヒドロゲルの原位置での形成は、該ヒドロゲルを所定の位置で、ゲル化又は架橋させて、針又はカニューレを取り去るときに、針の管を通って逆流しないようにし、かつ切り口部を通過して外眼に広がらないようにする。このように、形成され、形状が安定したヒドロゲルは、有効に薬剤を送達でき、かつ優位には、適切に制御された、サイズ、形状、及び表面積を有することができる。すでに形成された材料の代わりに、可溶性又は流動性の前駆体を使用することができるので、小口径の針を使用して、材料を注入することができる。対照的に、導入時に、迅速かつ堅固に架橋されない別の材料では、切り口から逆流する傾向がある。更に、共有結合性の架橋を行わない材料は、材料が継続的に再編成し、かつ材料の一部又は全部が流れ出るため、塑性変性又は浸出を起こしやすい。
原位置でのヒドロゲルによる治療において、接着性は、重要な役割を果たすことができる。例えば、強膜組織に接着したヒドロゲルは、該強膜と接触する、良好な表面積を有し、薬剤及び他の試薬の強膜への拡散を促進することができる。対照的に、接着できないと、拡散障壁が生じるか、又は該薬剤貯蔵所と眼との間に液体の侵入を許すため、薬剤が洗い流されてしまう。一方、眼周囲のヒドロゲルが周囲の組織に接着する、又は組織を成長させてヒドロゲルに接着する場合、薬剤の送達は損なわれ得る。従って、強膜(ヒドロゲルの前方表面)にしっかりと接着するが、反対側の表面(被膜の表面の後方)の組織、又は表面(より複雑な形状について)には接着しない、ヒドロゲル貯蔵所が有用であろう。原位置で調製される材料は、ゲル化及び/又は架橋の時間中、該材料の他の表面に組織がない、又は実質的に、接触する組織がないような、強膜上の原位置で物質を形成させることによって、これらの対立する要求を調和することができる。すでに説明したように、いくつかの実施態様は、例えば、強膜及び/又は結膜などの特定部位に接着するヒドロゲルを提供することに関する。
ヒドロゲルを形成することのいくつかの実施態様は、前駆体を混合することを含み、該前駆体は、例えば、患者の組織上の表面に適用後、実質的に、架橋して、生分解性のヒドロゲル貯蔵所を形成する。本発明を特定の操作理論に制限することなく、組織表面と接触後、架橋する反応性の前駆体種は、三次元構造を形成し、被覆された組織と機械的に連結するとされる。この連結は、接着、密接な接触、及び組織の被覆領域の、実質的に連続した被覆に寄与する。更に、強い求電子性官能基を用いる形成は、組織上で求核性基と反応させ、共有結合性の架橋を形成する傾向があるが、但し、求電子剤が適当な濃度で存在し、かつ求核剤が適当なpHで存在する。
接着の別の態様は、該インプラントが対象とする使用部位から、移動するのを防ぐことである。これは、患者の快適性を増大させ、刺激を低減させ、かつ該インプラント内の治療薬に影響を与える、涙を流す又は体液の流れという生体反応を低減させる。また、該インプラントが対象とする部位に作用し続けるという確信とともに、例えば、特定の組織間又は組織上に、該インプラントを正確に配置することができる。
接着は、薬剤送達に有用である。いくつかの実施態様において、例えば、図5のような、特定の領域を接着の標的をする。例えば、薬剤を伴う材料を、強膜及び/又は結膜に接着させることができる。又は、該材料を、眼の内部の表面、眼の前方部分の表面に接着させることができる。いくつかの場合において、該材料は、眼の内部の表面、かつ黄斑の1〜10mm以内に接着することが目標とされる(例えば、10、9、8、7、6、5、4mm未満、又は少なくとも1〜10mm、又は2mm、約25mm以下の距離など、全ての範囲及び値が、明記されている範囲内であることを企図することが、当業者には、直ちに理解されよう。)。別法として、又は例として、該材料の配置に使用することができる、そのような標的は、眼の内側へ向かう位置であり、ここで、該材料が、網膜まで眼を通過する光路に入り込まない。
該ヒドロゲルを使用して、ステロイド、非ステロイド性の抗炎症剤(NSAIDS)、眼圧下降剤、抗生物質、又はその他を含む、各種の薬剤の種類を送達することができる。該ヒドロゲルを使用して、薬剤及び治療薬、例えば、抗炎症剤(例えば、ジクロフェナック)、鎮痛剤(例えば、ブピバカイン)、カルシウムチャネル遮断薬(例えば、ニフェジピン)、抗生物質(例えば、シプロフロキサシン)、細胞周期阻害剤(例えば、シプロフロキサシン)、タンパク質(例えば、インスリン)を送達することができる。該ヒドロゲルからの放出速度は、薬剤及びヒドロゲルの特性によって決まり、該特性は、薬剤の大きさ、相対的疎水性、ヒドロゲルの密度、ヒドロゲルの固体含有量、及び例えば、微小粒子などの他の薬剤送達モチーフを含む因子を有する。
いくつかの実施態様において、架橋剤及び架橋可能なポリマーが反応して、架橋ポリマー網又はゲルを作製する場合、該治療薬又は試薬は、分離相に存在している。この相分離は、NHSエステルとアミノ基との間の反応など、化学的架橋反応における生物活性物質の関与を防ぐ。「分離相」が、油(水中油乳濁液)、生分解性の媒体などである場合に、この分離相はまた、架橋物質又はゲルからの活性試薬の放出速度を調節するのに役立つ。 活性試薬が存在し得る生分解性の媒体に含まれるのは、微小粒子、微小球、微小ビーズ、マイクロペレット(micropellet)などの封入媒体であり、ここで、該活性試薬は、生侵食性(bioerodable)又は生分解性のポリマーに封入され、該ポリマーは、例えば、下記のポリマー及び共重合体:ポリ(無水物)、ポリ(ヒドロキシ酸)、ポリ(ラクトン)、ポリ(炭酸トリメチレン)、ポリ(グリコール酸)、ポリ(乳酸)、ポリ(グリコール酸)-コ-ポリ(グリコール酸)、ポリ(オルト炭酸)、ポリ(カプロラクトン)、架橋した生分解性のヒドロゲル、フィブリン糊又はフィブリン組織接着剤などの架橋生分解性のヒドロゲル網、分子の収容及び捕捉、シクロデキストリン類似体、モレキュラーシーブなどである。ポリマー及び(ポリ(ラクトン)とポリ(ヒドロキシ酸)との共重合体から製造される微小球は、生分解性の封入媒体として、特に好ましい。
本発明の特定の実施態様は、ヒドロゲルを用いて比較的低分子量の治療種の放出を制御するための、組成物及び方法を提供することによって達成される。まず、治療薬を、1つ以上の比較的疎水性の速度調節剤とともに、分散又は溶解させて、混合物を形成する。該混合物を、粒子又は微小粒子状に形成することができ、次いで、これを、生体吸収性のヒドロゲルマトリックス内に捕捉して、制御された方法で水溶性の治療薬を放出する。別法として、該微小粒子は、ヒドロゲルの架橋の間、原位置で形成することができる。
本発明の実施態様は、複合性のヒドロゲルベースのマトリックス、及び捕捉された治療用化合物を有する微小球を形成するための、組成物及び方法を含む。一実施態様において、生物活性試薬が、疎水性(疎水性微小領域とも呼ばれる)を有する微小粒子に捕捉され、捕捉された試薬の漏れ出しを抑制する。いくつかの場合において、該複合性材料は、2相の分散を有し、どちらの相も吸収性であるが、混和しない。例えば、該連続相を、親水性のネットワーク(架橋された、又はされていないヒドロゲルなど)とし、一方、分散相を、疎水性(油、脂肪酸、ワックス、フッ化炭素、若しくは他の合成物、又は概して、本明細書中で、「油」又は「疎水性」相と呼ばれる、天然の水不混和性の相)とし得る。
インビボで投与される場合、疎水性微小領域は、それら自身で、分解され又は早急に除去され、これが、捕捉された試薬を含有する微小滴又は微小粒子を直接用いて、持続型の放出を達成するのを困難にさせる。該ゲルマトリックスは、疎水性微小領域を急速な排除から保護するが、該微小滴又は微小粒子の能力を損なうことなく、それらの内容物を徐々に放出する。例えば、ゲルマトリックス又は微小領域中に、可視化剤を含むことができる。
また、分解可能な、生物活性分子の共有結合で架橋されるヒドロゲル網による、本明細書中に開示される系を用いて、制御された薬剤送達速度を得ることができる。該共有結合の性質は、数時間〜数週間又はそれ以上で、放出速度を調整できるように制御し得る。一連の加水分解時間の結合から製造される複合体を使用することによって、制御型放出特性を、より長時間に延長することができる。
一般に、該ヒドロゲルは、水分解性基の分解によって、インビトロにおいて過剰の水に溶けるほどの、水分解性である。この試験は、インビボでの加水分解による溶解の予測となり、その進行は、細胞又はプロテアーゼによる分解とは対照的である。該ヒドロゲルは、選択した薬剤、治療する疾患、所要の放出期間、及び選択した特定の薬剤の放出特性に応じて、数日、数週間又は数ヶ月かけて吸収可能とするように選択され得る。しかし、いくつかの実施態様では、具体的に、30〜120日とされ、その理由は、より長い期間は、投与計画の使用者制御をより少なくさせるからであり、該薬剤が目的の効果を発揮しない場合、重要な因子となり得る。
該生分解性の結合は、水分解性又は酵素分解性とし得る。例示的な水分解性、生分解性の結合に含まれるのは、ポリマー、グリコリドの共重合体及びオリゴマー、dl-ラクチド、1-ラクチド、ジオキサノン、エステル、カルボナート及び炭酸トリメチレンである。例示的な酵素的生分解性の結合に含まれるのは、メタロプロテイナーゼ及びコラゲナーゼによって切断可能な、ペプチド性の結合である。生分解性の結合の例に含まれるのは、ポリ(ヒドロキシ酸)、ポリ(オルト炭酸)、ポリ(無水物)、ポリ(ラクトン)、ポリ(アミノ酸)、ポリ(炭酸)、及びポリ(ホスホン酸)である。
該生体適合性の架橋ポリマーが、生分解性又は吸収性であることが望ましい場合、官能基間に生分解性の結合が存在する、1つ以上の前駆体を使用することができる。該生分解性の結合は、1つ以上の該前駆体の水溶性の母核としても、任意に機能することができる。それぞれの方法について、生分解性の結合を選択して、得られる生分解性の、生体適合性の架橋ポリマーは、所望の期間内に分解、又は所望の期間内に吸収されるようにすることができる。
該ヒドロゲルとともに可視化剤を使用する。それは、ヒトの眼で検出可能な波長の光を、反射する又は発光して、ヒドロゲルを適用する使用者が、該ゲルを観察できるようにする。
好ましい生体適合性の可視化剤は、FD&C BLUE #1、FD&C BLUE #2、及びメチレンブルーである。より大きな濃度を、可視化剤の溶解度の限界まで、潜在的に使用することができるが、好ましくは、これらの試薬は、最終的な求電子性-求核性反応前駆体種の融合体が、0.05mg/mlより大きい濃度、かつ好ましくは、少なくとも0.1〜12mg/mlの濃度、かつより好ましくは、0.1〜4.0mg/mlの範囲で存在する。これらの濃度範囲は、架橋時間(反応性前駆体種がゲル化する時間で測定される。)に干渉するすることなく、ヒドロゲルに色を与えることができる。
加えて、機械支援の造影剤、例えば、蛍光化合物、X線画像装置で画像処理するためのX線造影剤(例えば、ヨウ素化合物)、超音波造影剤、又はMRI造影剤(例えば、ガドリニウム含有化合物)を使用することができる。
本明細書において、混合された前駆体を有する組成物は、手動で小口径の針を通して導入するのに適した粘度で調製される。小口径の針は、針の直径が27ゲージ以下、例えば、28、29、30、31、32、又は33ゲージの直径を有する。更に、血管内の分野で使用される、中空管ワイヤーを使用して、該物質を送達することができ、内径及び/又は外径が、小口径の針と等しいか、又は小さいものが含まれる。従って、約1〜約100,000センチポアズの粘度を使用することができる(例えば、約10〜約10,000センチポアズ、約5未満〜約10,000センチポアズ、約100未満若しくは約500センチポアズ、又は約1〜約100センチポアズなど、全ての範囲及び値が、明記されている範囲内であることを企図することが、当業者には、直ちに理解されよう。)。該粘性は、例えば、適切な前駆体の選択、固形物濃度の調整、及び反応速によって制御することができる。一般に、前駆体の濃度が低いほど、親水性が増加し、分子量が低いほど、低い粘性となる。
増粘剤(viscosity enhancer)を前駆体とともに、使用することができる。一般に、該増粘剤は、該前駆体と反応して、共有結合を形成することはない。一般に、前駆体は、望ましくない副反応に関与する場合があり得る、そのような結合を含まないことが理解されると同時に、これらは、該ヒドロゲルに対して影響を与えないようにして、前駆体が、このような反応を「含まない」ようにする。例えば、該前駆体が、求電子-求核反応によって反応する場合に、低レベルの望ましくない副反応がいくつか存在する場合であっても、該増粘剤は、該前駆体の官能基と共有結合を形成することができる、求電子剤又は求核剤を含まないことができる。一般に、増粘剤は、少なくとも20,000、又は約10,000〜約500,000ダルトンの分子量を有する親水性ポリマーである(例えば、少なくとも約100,000又は200,000など、全ての値及び範囲が、明記されている値の間で記載されていることが、当業者には、直ちに理解されよう。)。例えば、約5%〜約25% w/wの濃度を、使用することができる。例えば、PEG(例えば、M.W. 100,000〜250,000)が有用である。増粘剤は、求電子剤及び/又は求核剤を含まないことができる。増粘剤は、ヒドロキシル、カルボキシル、アミン、又はチオールなどの、1つ以上の官能基の対価(fee)とし得る。増粘剤は、前駆体に対して、1つ以上の本明細書記載の生分解性結合を含むことができる。増粘剤は、前駆体が架橋してゲルを形成する前に、前駆体が組織部位から流出するのを防ぐのに役立つことができる。
合成前駆体を使用して作製した、特定の重合可能なヒドロゲルが、医療分野で公知であり、例えば、FOCALSEAL(Genzyme社)、COSEAL(Angiotech Pharmaceuticals社)、及びDURASEAL(Confluent Surgical社)などの製品に使用され、例えば、米国特許第6,656,200号;第5,874,500号;第5,543,441号;第5,514,379号;第5,410,016号;第5,162,430号;第5,324,775号;第5,752,974号;及び第5,550,187号;に記載されている(これら文献のそれぞれは、本明細書にて明確に開示されたものと矛盾しない限りにおいて、その範囲が本明細書に参照により組込まれている。)。これらの物質のどれも、眼の中又は眼の周囲での使用に適さないようである。これらは、制御された方法で注射するには、重合が速すぎるというのが、1つの理由である。また、COSEAL及びDURASEALは、非常に高いpHを有しており、眼の組織に有害となる恐れがある(pH9より高い。)。別の理由は、明らかに、それらは、大きく膨潤するということである。COSEAL及びDURASEALの膨潤は、フィブリン組織接着剤と比較して、インビトロモデルを使用して、測定されている(Campbellらの文献, 「吸収性外科用充填剤の評価:インビトロ試験(Evaluation of Absorbable Surgical Sealants: In vitro Testing)」, 2005)3日にわたる試験で、COSEALは、平均約558重量%膨潤し、DURASEALは、平均約98重量%増加し、かつフィブリン組織接着剤は、約3%膨潤した。全ての軸に沿って均一に膨張したとすると、単一の軸におけるパーセントの増加は、COSEAL、DURASEAL、及びフィブリン組織接着剤に対して、それぞれ87%、26%、及び1%と計算される。FOCALSEALは、300%を上回る膨潤が公知である。かつ活性化するために、外部の光が必要であり、そのため、特に、そのような照射に敏感な眼の中又は周囲の、注入可能な薬剤送達貯蔵所としては、あまり適したものではない。フィブリン組織接着剤は、タンパク質性の接着剤であり、接着性、密封性、及びCOSEAL、DURASEAL、及び本明細書中に記載される他のヒドロゲルより劣る機械的特性を有する。更に、典型的には、それは、潜在的に異物が混入した、生物供給源由来であり、水による分解とは異なる機構で、身体から除去され、かつ通常、保管する間は、冷凍を要する。
眼に薬剤を送達するためのいくつかの系は、外用の点眼薬に頼っている。例えば、白内障及び硝子体網膜の手術後、数日の間、数時間毎に抗生物質を投与する必要がある。加えて、非ステロイド性の抗炎症剤(NSAIDS)などの他の薬剤もまた、頻繁に施す必要がある。多くの場合、これら点眼薬のいくつかは、例えば、RESTASIS(Allergan社)もまた、投与と関連する、刺すようなかつ焼けるような感覚を有する。RESTASISは、ドライアイに適応され、一日に数回、患者によって使用されなければならない。同様に、他の眼の疾患、例えば、嚢胞様黄斑浮腫、糖尿病黄斑浮腫(DME)、及び糖尿病網膜症などの治療も、ステロイド又はNSAID剤の投与が必要である。黄斑変性症などの、いくつかの血管増殖性疾患は、VEGF阻害剤の硝子体内注射を用いて治療される。VEGF阻害剤は、LUCENTIS及びAVASTIN(Genentech社)及びMACUGEN(OSI社)などの薬剤を含む。そのような薬剤を、本明細書中の該ヒドロゲルの系を用いて、反復投与の措置を避けて;例えば、毎日、毎週、又は毎月の薬剤を新たに適用させずに、又は薬剤を投与するのに外用の点眼薬を用いることなく、送達することができる。

Surmodics社は、硝子体内用、除去可能なインプラント製品を有する。それは、外科的に配置され、治療薬は、トリアムシノロンアセトニドである。名目上の送達期限は、約2年である。症状は、DMEである。現在、第I相試験について進行している。
ヒドロゲル作製のためのキット又は系を製造することができる。該キットは、医療として許容し得る条件を使用して製造され、かつ無菌性、純度、及び医薬として許容し得る製造方法を有する前駆体を含む。該キットは、必要に応じて塗布器、及び取扱説明書を含むことができる。治療薬は、予め混合したもの、又は混合に利用できるものを含むことができる。溶媒/溶液を、一式で若しくは別々に提供することができ、又は該成分を、溶媒を用いて予め混合することができる。該キットは、混合及び/又は送達のための、注射器及び/又は針を含むことができる。
いくつかの実施態様において、該キットは、少なくとも1つの前駆体及び塗布器を有する。いくつかの実施態様において、生分解可能な、ポリマーの、合成ヒドロゲルは、それぞれの腕の各末端にNHS-エステルを有する、8本腕の分子量15,000のポリエチレングリコール(PEG)と、トリリジン(一級アミンの求核剤を有する。)との、リン酸及び他の緩衝溶液中での反応によって形成される。可視化剤(例えば、FD&C Blue #1)を、該密封剤に組込むことができる。
いくつかの実施態様において、前駆体、及び原位置でのヒドロゲル形成に必要な他の材料を、治療薬とともに有するキットを提供することができ、該構成成分には、本明細書中に記載のものが含まれる。従って、いくつかの態様において、該ヒドロゲルの特徴を選択して、該ヒドロゲルを、わずかにしか膨潤しない、細い針を通って送達される、配置の後にゲル化するように水性、低粘性の調製品になるようにさせることができる。該ヒドロゲルは、炎症性又は血管新生性がなく、生体適合性の前駆体に依っており、かつ柔軟、親水性、かつ配置された空間に適合する。該ヒドロゲルは、容易に除去する、又は自ら取除くことができ、かつ生分解性とし得る、又は分散せずに容易に到達可能な領域への送達に適合することができる。単一の容器で全ての前駆体を組合せる選択によって、混合及び使用が容易であるようにさせることができる。該ヒドロゲルは、安全な、全て合成の材料で作製することができる。該ヒドロゲルの形成は、組織に接着するように作製することができる。分解及び/又は送達速度を制御して、記載される時間に適合させることができる。該ヒドロゲルは、架橋されているため、沈殿したように形状が安定しているので、針の管又は送達のために作り出された他の穴から流れ出ることはない。該ヒドロゲルの貯蔵所は、点眼薬と比べて優位である。97%を超える、外用で投与された点眼薬が、涙管によって排除され、結局眼を浸透することにはならない。反復投与が回避されることで、患者コンプライアンスを向上することができる。
該ヒドロゲル又は他の架橋材料の1つの中のいくつかの試薬は、眼周囲の経路を通って、体循環で失われ得るが、一方、非常に大きなインプラントのサイズは、治療薬濃度を保持し、かつヒトの強膜の表面積が約17cm2であることを踏まえ、該強膜を横断して、かつ眼の奥への向かう、薬剤の適切な経強膜的な拡散を可能にするように、大きなインプラントを適応させる、能力を有する。また、該ヒドロゲルは、薬剤の局在化に有用である。対照として、薬剤の懸濁液又は微小粒子を、硝子体内に注入する場合、それらは視野を移動し、かつ視覚を妨げる恐れがある。
2つの前駆体及び希釈剤を調製した。第1の前駆体は、各腕の末端にグルタル酸スクシンイミジルを有する、8本腕のポリエチレングリコールであり、約15,000の分子量を有する。該前駆体は、粉末で提供され、0.11% w/wの濃度の染料(FD&A Blue)と配合した。第2の前駆体は、0.2Mのリン酸ナトリウム緩衝液(pH8)中のトリリジンであった。該第1の前駆体のための希釈剤を、0.01Mのリン酸ナトリウム(pH4.8)に調製した。
薬剤(下記実施例に示される)を、希釈剤中の薬剤に混合し、かつ約200μlの薬剤/希釈剤を、1mlの注射器に吸入した。66mgの第1の前駆体の粉末を、別の1mlの注射器に入れた。2つの注射器を、メス-メスのルアー継手を介して取り付け、かつ該溶液を、該粉末が完全に溶解するまで、前後に動かした。溶液中の第2の前駆体を、第3の注射器に吸い込んだ(200μl)。別のメス-メスのルアー継手を用いて、該第1及び第2の前駆体を十分に混合した。該混合溶液を注射器の1つに吸い込んで、内容物を受け取る、長さ4インチのシリコンチューブを取り付けた。適当な反応時間を与えた後、該チューブを所望の長さに切り取り、かつ内部のゲルをマンドレルで押出した。得られたヒドロゲルのプラグは、概して、直径0.125インチ、及び厚さ約6.4 mmであった。
特に明記しない限り、薬剤放出特性の分析は、高圧液体クロマトグラフィー(HPLC)を用いて確認した。ディスクを溶液で保持し、かつ該溶液を定期的に採取し、HPLCで試験して、溶液中の薬剤濃度を測定した。全薬剤導入を、該ディスクを水溶液、又はオクタノールのようなアルコールの存在下、高pHで溶解させ、ディスク中の薬剤含有量を測定することによって測定した。特に明記しない限り、薬剤導入は、5%(薬剤の重量/内容物を含むヒドロゲルの全重量)であった。
ジクロフェナクナトリウムは、約1113mg/Lの水溶解度を有する。該ジクロフェナクナトリウムは、抗炎症剤である。該ジクロフェナクナトリウムは、実施例1のようにヒドロゲルに導入され、かつ図9に示すように放出された。約8時間で、実質的に、完全な薬剤の放出が、観察された。
ブピバカインは、約86mg/Lの水溶解度を有する。該ブピバカインは、鎮痛剤であり、HCl塩から遊離塩基に変換され、水溶解度が低減した。該ジクロフェナクナトリウムは、実施例1のようにヒドロゲルに導入され、かつ図10に示すように放出された。徐放型のゼロ次放出が、6日間にわたって観察された。
ニフェジピンは、約1mg/L未満の水溶解度を有する。該ニフェジピンは、カルシウムチャネル遮断薬及び降圧薬であり、心臓への血流を増加させて、狭心症を軽減する。該ニフェジピンは、実施例1のようにヒドロゲルに導入され、かつ図11に示すように放出された。徐放型のゼロ次放出が、45日間にわたって観察された。
シプロフロキサシンは、約160mg/Lの水溶解度を有する。該シプロフロキサシンは、抗生物質である。該ニフェジピンは、実施例1のようにヒドロゲルに導入され、かつ図12に示すように放出された。図12は、pH9.0(四角)又はpH7.4(丸)での試験を示す。3日までに、50%の薬剤が放出された。
メフェナム酸は、疼痛を治療するための、NSAIDである。該メフェナム酸は、実施例1のようにヒドロゲルに導入され、かつ図13に示すように放出された。図13は、pH9.0(四角)又はpH7.4(丸)での試験を示す。後の時間のその成分への更なる分解が、更なる量の薬剤を放出するが、それは、約15日を通して放出された。
インドメタシンは、疼痛の治療をするための別のNSAIDである。該インドメタシンは、実施例1のようにヒドロゲルに導入され、かつ図14に示すように放出された。図14は、pH9.0(四角)又はpH7.4(丸)での試験を示す。該図は、約6日間の放出特性を示す。更なる放出を、観察したが、定量化されなかった。
トリアムシノロンは、極めて小さな水への溶解度を有し、通常、注射、吸入、又は局所的なクリームによって、経口で投与される合成副腎皮質ステロイドである。該トリアムシノロンは、導入量が5%の代わりに約4%であった以外は、実施例1のようにヒドロゲルに導入され、かつ図15に示すように放出された。図15は、約1週間までの試験を示す。
デキサメタゾンは、グルココルチコイド系のステロイドホルモンである。該デキサメタゾンは、抗炎症剤及び免疫抑制剤として作用する。該デキサメタゾンは、実施例1のようにヒドロゲルに導入され、かつ図16に示すように放出された。該図は、約6日間の放出特性を示す。更なる放出を、観察したが、定量化されなかった。
Claims (10)
- 治療薬を眼に送達するための合成生体適合性ポリマーヒドロゲルであって、
第1の合成前駆体が、第2の合成前駆体と共有結合により架橋して、該生体適合性のヒドロゲルを形成し、
該ヒドロゲル中の治療薬は、5重量%以下の濃度で導入されるものであって、該濃度は、内容物を含む該ヒドロゲルの全重量あたりの治療薬の重量比であり、かつ、該治療薬は、少なくとも2日の期間中、該ヒドロゲルから放出され、
ここで、該第1の前駆体又は第2の前駆体が、水分解性基を含み、
該ヒドロゲルが、24時間の生理溶液への曝露時に、形成時のヒドロゲルの重量と比べて、該ヒドロゲルの重量が50%以下の増加を有し、かつ
該ヒドロゲルが、その水分解性基の分解によって、インビトロにおいて過剰の水に溶けるほどの、水分解性である、前記ヒドロゲル。 - 前記ポリマーの親水性部分が、ポリ(エチレン)グリコールの繰返しを含む、請求項1記載のヒドロゲル。
- 前記第1の前駆体が架橋前に求核性官能基を含み、かつ前記第2の前駆体が架橋前に求電子性官能基を含み、かつ該求核性官能基が該求電子性官能基と反応して、該前駆体を共有結合的に架橋する、請求項1又は2記載のヒドロゲル。
- 前記ヒドロゲルが、前記前駆体上の官能基のラジカル重合によって形成される、共有結合を含む、請求項1又は2記載のヒドロゲル。
- 前記ヒドロゲル内に捕捉された粒子を含み、前記治療薬が放出される期間中、該治療薬の放出速度を遅くさせる相を更に含む、請求項1〜4のいずれかに記載のヒドロゲル。
- 前記期間が2日〜2年の範囲内である、請求項1〜4のいずれかに記載のヒドロゲル。
- 患者の眼を侵す眼病を治療するための医薬品として使用するための、請求項1記載のヒドロゲル。
- 前記疾患が、滲出型黄斑変性、萎縮型黄斑変性、糖尿病黄斑浮腫、嚢胞様黄斑浮腫、及び糖尿病網膜症からなる群から選択される眼の奥の疾患である、請求項7記載のヒドロゲル。
- 少なくとも0.5 mlの請求項1記載のヒドロゲルを作製するための成分を含むキット。
- 30ゲージ以下の針を含む塗布器をさらに含む、請求項9記載のキット。
