Description
本発明は、癌免疫療法の新規かつ改良された方法、およびこの方法にて用いるためのキットに関する。具体的には、本発明は癌患者の前哨リンパ節から得られるリンパ球を用意し、それをインビトロにて拡張させる方法に関する。
癌は、多くは種々の型の悪性腫瘍を表すのに用いられる語であり、ヒトの主たる死亡原因の一つである。現在のところ、癌は世界の死亡全体のおよそ23%を占め、より多くの命を奪うのは心血管疾患だけである。個々の人生において、癌は、どの器官のどの組織、いずれの年齢においても発生し、その形質転換した細胞は回りの組織に侵入するか、または体内のいくつかの部位に転移する。現在では、内因的因子とは別に、喫煙、食品に含まれる、あるいは工業プロセスで放出される特定の化合物への暴露などの特定の活性化物質および環境条件もまた、特定の型の癌および腫瘍の発生の原因となり、その危険性を増大させている。
癌と診断されれば、治療方法を決定することが最優先事項となる。過去においては、ローカルおよびレジオナル療法の両方が用いられ、具体的には、外科手術および/または放射線療法が、通常、全身性療法、すなわち、抗腫瘍薬の投与と組み合わせて用いられた。
光線力学療法においては、光感作化合物およびレーザーを用いて腫瘍壊死を生じさせるものである。腫瘍を特定する光感作化合物を患者に全身投与し、つづいてレーザーを用いて活性化させる。感作剤に適当な波長の光を吸収させて励起状態とし、その感作剤と処理される組織内にある酸素分子とを相互作用させて細胞毒性の一重項酸素を発生させ、それを介して細胞障害性を得る。
原理上、哺乳動物の免疫系は、体を保護するための2つの一般的機構として、非特異的または先天的免疫応答、および特異的または後天的免疫応答を有する。外来の侵入物質と非特異的に戦う先天的応答とは異なり、クローン選択によりなされる、特異的応答は特定の物質(抗原)に応じて調整される。後天的免疫は、特異性細胞、エフェクター分子としての抗体を産生するB細胞(体液免疫応答を提供する)により媒介され、細胞そのものを介してその効能を媒介するT−細胞(細胞媒介免疫性)により媒介される。
これらの機序を癌の治療に活用する医学的試みにおいて、これまで2つの一般的方法が腫瘍免疫療法にて用いられてきた。第一の方法によれば、インターロイキン−2、腫瘍壊死因子またはインターフェロンなどの物質を腫瘍患者に投与することにより先天的免疫応答が刺激される。しかし、この方法は完全に成功であるとは言い切れず、その有効濃度で投与物質が示す高い毒性のため、該方法は強い副作用を示した。
この欠点を克服するために、インビボにて腫瘍に浸潤させたリンパ球の集団、いわゆる腫瘍浸潤リンパ球を単離して拡張させることを含む、種々の操作が提案された。この方法はまだ、腫瘍発生のかなり後の段階でのみ存在し、数が少ない、特定の型のリンパ球を必要とするという欠点がある。さらには、腫瘍浸潤リンパ球は腫瘍からの物質の免疫抑制剤に供され、最終的にかかるリンパ球の単離が実施において常に容易な操作であるとは限らない。
したがって、本発明の問題は従来技術の上記した欠点を克服し、免疫療法の新規かつ改善された方法を提供することである。
この問題は、前哨リンパ節からリンパ球を得、こうして得られたリンパ球をインビトロにて活性化させ、拡張させる行程を含む、癌の再発を治療および/または予防する方法により解決される。
図1は前哨リンパ節の術中の同定結果を示す。
図2はリンパ球の特徴化から由来の結果を示す。
図3は転移についての非特異的活性化および細胞内FACS分析の結果を示す。
図4は、抗原ペプチドの提示およびT細胞の活性化が起こる際に、腫瘍からの抗原物質がどのようにしてリンパ管により排液性リンパ節に輸送され、トレーサー色素により同定されるかの仮定となるスキームを示す。
トレーサー物質として、例えば、パテントブルー色素、リンファズリンブルー(lymphazurine blue)または99Tc標識アルブミンを用いることができる。
次の工程では、残りの前哨リンパ節に存するリンパ球を集め、インビトロ培養に移す。このことは、例えば、コラゲナーゼと一緒にまたはなしでリンパ節がその内容物を放出するようにリンパ節を押圧することで達成することができる。
こうして得られた細胞を、その後、インビトロ培養に供する。
不要な細胞を該培養から除去する場合、および/またはT−細胞の特定の部分集合、例えば、CD4、CD8、CD69、CD62Lを選択する場合には、選択される細胞に独特な表面マーカーと関連付けられる抗体などの、ポジティブまたはネガティブ選択技法を用いることができる。かかる技法の一例が、選択される細胞上に存在する表面マーカーに対して方向付けられるモノクローナル抗体をキャリアに結合させるところの、磁性免疫粘着法である。細胞の選択はまた、滅菌条件下、フローサイトメトリーにより補助される細胞分類により行うこともできる。
リンパ球は、培養の間、拡張用刺激剤に、所望により付加的な活性化シグナルに暴露される。
リンパ球の拡張は、細胞を抗−CD3抗体と接触させること、または細胞を蛋白キナーゼC活性化物質(ホルボールエステル)とカルシウムイオノホールとの併用において接触させることによりなされうる。T−細胞表面上でアクセサリー分子を刺激するために、そのアクセサリー分子と結合するリガンドを利用してもよい。例えば、T細胞は、T−細胞の増殖を刺激するのに適する条件下、抗−CD3抗体および抗−CD28抗体を用いて活性化されうる。
本発明の方法によれば、既にインビボにて感作がなされており、腫瘍抗原に対する特異性を有するリンパ球が使用されているため、クローン拡張が既に起こっているから、原則として、付加的な活性化/感作は不要である。
リンパ球の培養が所望の程度まで拡張され、刺激された場合に、リンパ球を集め、所望により患者の健康に有害な物質を一掃してもよく、そしてそれを患者に戻す。この操作は約1時間から6時間の間に静脈内注入により行うことができ、その投与される単球の数は増殖工程の間に生成される細胞の数に依存するにすぎない。
本発明の方法は従来技術に比べて明らかな利点を提供する。リンパ球は前哨リンパ節から集められるから、具体的には既に腫瘍抗原に拮抗する活性および特異性を有するリンパ球が拡張される。この特異性は、そのリンパ球を自己腫瘍エキスの存在下で培養することにより、インビトロにて強化され、クローン選択および拡張を促進することできる。原発腫瘍に対して特異的な反応性を有するT−細胞は前哨リンパ節にて同定することができ、これらの細胞は細胞免疫療法にて後の使用のために特異的に拡張させることができる。
次に実施例を用いて本発明を説明するが、これらは何ら本発明を限定するものではなく、単なる例示にすぎない。
細胞採集と調製
手術前の遠隔転移またはリンパ節病変の徴候のない5人の結腸癌の患者をこの実験に利用した(図1)。この実験は倫理委員会により承認されており、患者とのインフォームド・コンセントがなされた。
検査を容易にするために腹膜癒着の分離を介して結腸腫瘍部位を移動させた。1mlのパテントブルー色素(Guerbet, Paris)を腫瘍の回りに表面的に注射した。顕微鏡を用いて5分以内に前哨リンパ節としての1ないし3個の青色に変色した腸間膜リンパ節を同定し、それらを縫合でマークした。
前哨および非前哨リンパ節を半分に切断した。厚みが1mmより薄いスライスをフローサイトメトリーおよび増殖分析用にそのリンパ節の中央部および周辺部より切断した。残りのリンパ節を慣用的な組織病理学検査に付した。組織を組織病理学的にデューク段階A−C(11)として分類した(表1)。
原発腫瘍の一片(浸潤マージンの部分を含む)をフローサイトメトリー分析用および抗原の供給源として摘出した。
前哨リンパ節の術中同定および病理学的分類
腫瘍の周辺にパテントブルーを注射することで手術中に1ないし3個の前哨リンパ節を検出した(図1)。摘出した試験片を肉眼で解剖して、18と29個の間にあるリンパ節(平均22個)を同定して組織病理学的評価に組み込んだ(図2A、2B左パネル)。患者2および3(表1)は前哨リンパ節までの転移拡張を有し、組織病理学的にデュークCに分類された(図2B左パネル)。他の3人の患者は、腫瘍が腸の筋肉壁を介して増殖している(図2A左パネル)にも拘わらず、前哨リンパ節まで、または他のリンパ節までの転移拡張の徴候を示さなかった。それらをデュークBに分類した。
腫瘍、前哨および非前哨リンパ節からの細胞をサイトケラチン−20、上皮癌のマーカーで細胞内染色してからFACSにより検査した。本発明者らは細胞のサポニンでの透過が腫瘍中の大部分のサイトケラチン−20陽性細胞の検出を可能とすることを見出した(図3B)。興味深いことに、本発明者らは微小転移巣の前哨リンパ節中の少量のサイトケラチン−20陽性細胞も検出することができた(図3B、図2B左パネル)。
Claims (20)
- (a)患者より摘出した転移していない前哨リンパ節から得られるリンパ球を準備し;
(b)IL−2によって該リンパ球をインビトロにて刺激し、該リンパ球をインビトロにて増殖させて、工程(b)が工程(a)で準備されているリンパ球を付加的な活性化シグナルに曝露することをさらに含み、該活性化シグナルがリンパ球を自己腫瘍エキスに曝露することを含む、リンパ球の増殖方法。 - 活性化が抗原提示細胞の存在下にてなされる、請求項1記載の方法。
- 抗原提示細胞がB−細胞、マクロファージ、単球、樹枝状細胞、ランゲルハンス細胞を含む、請求項2記載の方法。
- リンパ球がヒトまたは動物の起源から由来される、請求項1ないし3のいずれか一項に記載の方法。
- リンパ球がコラゲナーゼと一緒にまたはなしで前哨リンパ節より供給される、請求項1ないし4のいずれか一項に記載の方法。
- T−細胞の部分集合が増殖について選択される、請求項1ないし5のいずれか一項に記載の方法。
- T−細胞がCD4である、請求項6記載の方法。
- リンパ球が血清および/または抗生物質を含んでいてもよい培地にて培養される、請求項1ないし7のいずれか一項に記載の方法。
- 培地が最小必須培地またはRPMI培地1640である、請求項8記載の方法。
- T−細胞の増殖速度を定期的にモニター観察する、請求項1ないし9のいずれか一項に記載の方法。
- さらにT−細胞の再活性化および再刺激も含む、請求項1ないし10のいずれか一項に記載の方法。
- T−細胞の径が小さくなると、T−細胞が再び活性化かつ刺激される、請求項11記載の方法。
- 再活性化および再刺激を数回繰り返し、CD4またはCD8T−細胞の集団をプロモートする、請求項11ないし12のいずれか一項に記載の方法。
- 工程(b)がサイトカインの添加を含み、新たな培地を細胞培養物に添加する場合に、培養を行う最初の日からサイトカインの添加を行い、一日おきに添加する、請求項1ないし13のいずれか一項に記載の方法。
- IL−2が100U/mLの最終濃度で利用される、請求項14記載の方法。
- 抗原提示細胞がMHC分子と併用して用いられる、請求項2または3記載の方法。
- 癌の治療にて用いられる請求項1ないし16のいずれか一項に記載のインビトロにて増殖させたリンパ球を含み、その増殖に供されるリンパ球が前哨リンパ節を起源とする、組成物。
- 癌の再発を治療および/または防止する、請求項17記載の組成物。
- 前哨リンパ節が癌に罹患している患者から由来する、請求項17または18記載の組成物。
- リンパ球がヒトまたは動物起源より由来する、請求項17ないし19のいずれか一項に記載の組成物。
