（技術分野）

（関連技術の背景）
関節運動のために手術用器具の本体部分の遠位端に取着される、関節運動のためのツールアセンブリを含む手術用器具が、公知である。一般に、そのような手術用器具は関節運動制御機構を含み、この関節運動制御機構は、操作者が、手術用器具の本体部分に対してツールアセンブリを遠隔操作で関節運動させることを可能にする。このことは、操作者がより容易に組織にアクセスすること、手術すること、および／または処理することを可能にする。

現在公知の装置は、組織をクランプし、組織内に外科用ファスナーを配備および形成するために、一般に１０ポンド〜６０ポンドの手動のための手の力を必要とし得、そのことが、反復使用を経て外科医の手を疲労させ得る。手術用ファスナーを組織の中に埋め込む気体駆動の空気式ステープラーが当該技術分野において公知である。これらの器具のうちの特定の器具は、トリガー機構に接続する加圧気体供給源を利用する。トリガー機構は、押下されると、単に加圧気体を解放して、組織をクランプし、組織の中にファスナーを埋め込むだけである。

本明細書で開示されている手術用器具の実施形態が、ここで図面を参照して説明され、図面においては、同様の参照番号はいくつかの図面の各々において同一の要素または対応する要素を示す。本明細書で使用される場合、用語「遠位」は、ユーザから遠い電動手術用器具の部分またはその構成要素を指し、一方、用語「近位」は、ユーザに近い電動手術用器具の部分またはその構成要素を指す。
本開示に従う電動手術用器具、例えば、手術用ステープラーは、参照番号１０として図面に示される。最初に、図１を参照すると、電動手術用器具１０は、筺体１１０と、内視鏡部分１４０であって、内視鏡部分１４０を通って延びる第一の長手軸Ａ−Ａを画定する内視鏡部分１４０と、関節運動ツールアセンブリ（例えば、エンドエフェクタ１６０）であって、関節運動ツールアセンブリを通って延びる第二の長手軸Ｂ−Ｂを画定する関節運動ツールアセンブリを備える。内視鏡部分１４０は、筺体１１０から遠位に延び、エンドエフェクタ１６０は、内視鏡部分１４０の遠位部分に隣接して配置される。一つの実施形態において、筺体１１０の構成要素は、微粒子の侵入および／または流体の浸入に対して密封され、殺菌処理による構成要素の損傷を防止することに役立つ。
本発明の実施形態に従うと、エンドエフェクタ１６０は、一つ以上の外科用ファスナー（例えば、カートリッジアセンブリ１６４）を有する第一の顎部材と外科用ファスナー（例えば、アンビルアセンブリ１６２）を配備および形成するアンビル部分を含む第二の対向する顎部材とを含む。特定の実施形態において、ステープルは、カートリッジアセンブリ１６４内に収容され、直線状の列のステープルを同時的または連続的な態様で身体組織に適用するように構成される。アンビルアセンブリ１６２およびカートリッジアセンブリ１６４の一方または両方は、開放位置と近接またはクランプ位置との間で、相互に移動可能であり、開放位置においては、アンビルアセンブリ１６２は、カートリッジアセンブリ１６４から間隔をあけており、近接またはクランプ位置においては、アンビルアセンブリ１６２は、カートリッジアセンブリ１６４と並列して整列している。
エンドエフェクタ１６０は、取着部分１６６に取り付けられ、この取着部分１６６は本体部分１６８に旋回可能に取り付けられることがさらに想定される。本体部分１６８は、電動手術用器具１０の内視鏡部分１４０と一体的であり得るか、または、器具１０に取り外し可能に取り付けられ得ることにより、取替え可能で使い捨て可能な装填ユニット（ＤＬＵ）または一回使用の装填ユニット（ＳＵＬＵ）（例えば、装填ユニット１６９）を提供する。特定の実施形態においては、再使用可能部分は、その後の外科処置における殺菌および再使用に対して構成されてもよい。
装填ユニット１６９は、差し込み接続を介して内視鏡部分１４０に接続可能であり得る。装填ユニット１６９は、装填ユニット１６９の取着部分１６６に接続される関節運動連結部を有し、この関節運動連結部は、連結ロッド２２０（図４）に接続されることにより、以下にさらに詳述されるように、連結ロッド２２０が第一の長手軸Ａ−Ａに沿って遠位−近位方向に並進する際に、エンドエフェクタ１６０が関節運動させられることが想定される。エンドエフェクタ１６０を内視鏡部分１４０に接続して関節運動を可能にする他の手段、例えば、可撓性チューブ、または複数の旋回可能部材を備えるチューブが使用され得る。
装填ユニット１６９は、様々なエンドエフェクタ、例えば、血管密封デバイス、直線ステープリングデバイス、円形ステープリングデバイス、カッター、把持器等を組み込み得るか、または組み込むように構成され得る。そのようなエンドエフェクタは、電動手術用器具１０の内視鏡部分１４０に結合され得る。中間の可撓性シャフトがハンドル部分１１２と装填ユニットとの間に含まれ得る。可撓性シャフトを組み込むことは、身体の特定の領域に、そして／またはその内部にアクセスすることを容易にし得ることが想定される。
図１および２を参照すると、本開示の実施形態に従う筺体１１０の拡大図が図示されている。図示される実施形態において、筺体１１０はハンドル部分１１２を含み、このハンドル部分１１２はその上に主駆動スイッチ１１４が配置されている。スイッチ１１４は、トグルスイッチとして共に形成される第一および第二のスイッチ１１４ａおよび１１４ｂを含み得る。ハンドル部分１１２は、ハンドル軸Ｈ−Ｈを画定し、ユーザの指１０によって把握されるように構成される。ハンドル部分１１２は、十分な手の平のグリップによる梃子作用を提供する人間工学的な形状を有し、この梃子作用は、ハンドル部分１１２が操作中にユーザの手から引き出されることを防ぐ。各スイッチ１１４ａおよび１１４ｂが、ユーザの一本の指または複数の指によるその押下を容易にするように、ハンドル部分１１２上の好適な場所に配置されるように示される。
さらに、そして図１および２を参照すると、スイッチ１１４ａ、１１４ｂは、電源３００（図４）に接続された駆動モータ２００の開始および／または停止の動作に対して使用され得る。一つの実施形態において、スイッチ１１４ａは、第一の方向に駆動モータ２００を起動させて、発射ロッド（図示されず）を遠位方向に前進させ、それによって、アンビルアセンブリ１６２とカートリッジアセンブリ１６４とを接近させるように構成される。逆に、スイッチ１１４ｂは、発射ロッドを後退させて、駆動モータ２００を逆方向に起動させることにより、アンビルアセンブリ１６２とカートリッジアセンブリ１６４とを開放させるように構成され得る。この後退モードは、機械的なロックアウトを始動させて、装填ユニット１６９によるステープリングおよび切断の更なる進行を防ぐ。トグルは、スイッチ１１４ａを起動する第一の位置と、スイッチ１１４ｂを起動する第二の位置と、第一の位置と第二の位置との間の中立の位置とを有する。同様の後退機構が、Ｍｉｌｌｉｍａｎらによる共有に係る米国特許第７，３０３，１０７号に開示され、その開示全体が本明細書に参照として援用される。
筺体１１０、特にハンドル部分１１２は、スイッチシールド１１７ａおよび１１７ｂを含む。スイッチシールド１１７ａおよび１１７ｂは、それぞれ、スイッチ１１４ａの底面部分およびスイッチ１１４ｂの上面部分を囲む、リブ様の形状を有し得る。スイッチシールド１１７ａおよび１１７ｂは、スイッチ１１４の偶発的な起動を防ぐ。さらに、スイッチ１１４ａおよび１１４ｂは、起動に対して、圧力の増加を必要とする高触覚のフィードバックを有する。
一つの実施形態において、スイッチ１１４ａおよび１１４ｂは、駆動モータ２００の速度および発射ロッドを非線形態様で制御する多重速度（例えば、２つ以上）、増分または可変速度スイッチとして構成され得る。例えば、スイッチ１１４ａ、１１４ｂは感圧性であり得る。このタイプの制御インタフェースは、比較的遅く、正確なモードから比較的速い動作まで、駆動構成要素の速度割合での漸増を可能にする。
スイッチ１１４ａおよび１１４ｂは、非線形速度制御回路に結合され、この非線形速度制御回路は、電圧調整回路、可変抵抗回路、またはマイクロエレクトロニクスのパルス幅変調回路として実装され得る。スイッチ１１４ａおよび１１ｂは、可変制御デバイス（例えば、抵抗制御（ｒｈｅｏｓｔａｔｉｃ）デバイス、多重位置スイッチ回路、直線および／または回転可変変位変換器、直線および／または回転電位差計、光学式エンコーダ、強磁性センサ、およびホール効果センサ）を変位または作動させることによって制御回路と接続し得る。これにより、スイッチ１１４ａおよび１１ｂは、例えば、スイッチ１１４ａおよび１１４ｂの押下に基づいて、使用中の制御回路のタイプに応じて、増分的にまたは漸増的に駆動モータ２００の速度を連続的に増加させるような、多重速度モードで駆動モータ２００を動作させることが可能となる。
図２〜４は、関節運動機構１７０を図示しており、これは、関節運動筺体１７２と、電動関節運動スイッチ１７４ａおよび１７４ｂと、電源３００に結合される関節運動モータ１３２と手動関節運動ノブ１７６とを含む。スイッチ１７４ａおよび１７４ｂは、パドル型の形状を有する。関節運動スイッチ１７４ａおよび１７４ｂは、エンドエフェクタ１６０の電動関節運動を提供し、手動関節運動ノブ１７６は、手動の関節運動を提供する。電動関節運動スイッチ１７４ａおよび１７４ｂの係合および／または並進、または、手動関節運動ノブ１７６の旋回は、関節運動モータ１３２を起動させ、このことが、次いで、図４に示されるように、関節運動機構１７０の関節運動ギア２３３を作動させる。関節運動ギア２３３は、次に、連結ロッド２２０に動作可能に結合される。さらに具体的には、関節運動機構１７０は、連結ロッド２２０を長手軸Ａ−Ａに沿って遠位方向または近位方向に移動させ、このことがエンドエフェクタ１６０に関節運動を付与する。関節運動機構１７０の作動は、エンドエフェクタ１６０をその第一の位置（この位置において、長手軸Ｂ−Ｂは長手軸Ａ−Ａと実質的に整列している）から、ある位置（この位置において、長手軸Ｂ−Ｂは、長手軸Ａ−Ａに関してどちらかの方向に、長手軸Ａ−Ａに対してある角度で配置されている）に向かって移動させる。電動関節運動スイッチ１７４ａおよび１７４ｂは、また、クランプ機構として同様の非線形速度制御を組み込みし得、このクランプ機構はスイッチ１１４ａおよび１１４ｂによって制御される。
図２および３を参照すると、筺体１１０は、スイッチシールド１６９を含み、このスイッチシールド１６９は、翼様の形状を有し、スイッチ１７４ａおよび１７４ｂの上を筺体１１０の上部面から延びている。スイッチシールド１６９は、スイッチ１７４ａおよび１７４ｂの偶発的な起動を防ぎ、関節運動機構１７０を起動するためにはユーザにシールド１６９の下に到達することを要求する。
関節運動筺体１７２、電動関節運動スイッチ１７４ａおよび１７４ｂ、手動関節運動ノブ１７６ならびにエンドエフェクタ１６０へ関節運動を提供することについてのさらなる詳細は、Ｍａｒｃｚｙｋに対する共有に係る米国特許第７，４３１，１８８号に詳述されており、その内容は、全体が参照として本明細書に援用される。筺体１１０内に配置し得る、リミットスイッチ、近接センサ（例えば、光学式および／または強磁性）、直線可変変位変換器およびシャフトエンコーダの任意の組み合わせが、エンドエフェクタ１６０の関節運動角度ならびに連結ロッド２２０の位置を制御および／または記録するために利用され得ることが想定される。
図４に示されるように、器具１０は、また、モータ２００に電気的に結合される制御装置４００と、器具１０内に配置される様々なセンサとを含む。センサは器具１０の様々な動作パラメータ（例えば、直線速度、回転速度、関節運動位置、温度、バッテリ充電等）を検出し、これらの動作パラメータは、その後、制御装置４００に報告される。従って、制御装置４００は、その後、計測された動作パラメータに応答し得る（例えば、モータ２００の速度を調整する、関節運動角度を制御する、電力供給を遮断する、エラー状態を報告する等）。
制御装置４００は、スイッチ１１４ならびにスイッチ１７４ａおよび１７４ｂに結合されて、モータ２００および１３２をそれぞれ制御する。スイッチ１７４ａおよび１７４ｂは、関節運動機構１７０を複数のモードで動作させるように構成され得る。別の実施形態において、スイッチ１７４ａおよび１７４ｂは、筺体１１０内の一つ以上のガイドレール上に摺動可能に配置され得る。スイッチ１７４ａおよび１７４ｂは、機械的におよび／または電気的に連結され得ることにより、関節運動機構１７０が、直列のスイッチ１７４ａおよび１７４ｂを切り替えることによって起動される。
スイッチ１７４ａおよび１７４ｂは、ピボット（例えば、共通または別個のピボット）の周りに配置され得、このピボットの周りでのその回転によって作動させられ得る。エンドエフェクタ１６０の関節運動は、所望の関節運動方向に対応する方向にスイッチ１７４ａおよび１７４ｂを係合および移動させることによって達成され得る。さらに具体的には、エンドエフェクタ１６０を時計回りの方向に関節運動させるために、スイッチ１７４ａおよび１７４ｂは、また、時計回りの方向に移動させられ、すなわち、スイッチ１７４ａは近位方向に移動させられ、スイッチ１７４ｂは、遠位方向に移動させられる。エンドエフェクタ１６０を反時計回りの方向に関節運動させるために、スイッチ１７４ａおよび１７４ｂは、また、反時計回りの方向に移動させられ、すなわち、スイッチ１７４ａは、遠位方向に移動させられ、１７４ｂは、近位方向に移動させられる。この構成は、スイッチ１７４ａおよび１７４ｂの動きとエンドエフェクタ１６９との間の自然な関連を提供し、その結果、スイッチ１７４ａおよび１７４ｂのうちの一つを近位方向に引くことは、エンドエフェクタ１６９を対応する側に向かって引き、スイッチ１７４ａおよび１７４ｂのうちの一つを遠位方向に引くことは、エンドエフェクタ１６９を対応する側から引く。
実施形態において、スイッチは同一の方向、例えば、スイッチ１７４ａおよび１７４ｂを近位方向に押してエンドエフェクタ１６９を一方向に関節運動させ、スイッチ１７４ａおよび１７４ｂを遠位方向に引いてエンドエフェクタ１６９を反対方向に（例えば、反時計回りに）関節運動させるように、係合され得る。
実施形態において、スイッチ１７４ａおよび１７４ｂは独立して移動可能であり、それにより、関節運動機構１７０は、スイッチ１７４ａおよび１７４ｂのうちのどちらかを切り替えることによって起動させられ得る。実施形態のどちらか一方において、スイッチ１７４ａおよび１７４ｂは起動スイッチを遅延させ得、それにより、スイッチ１７４ａおよび１７４ｂは、関節運動機構１７０を起動するために、所定期間の間（例えば、約１秒から約５秒まで）、連続的に係合されなければならい。このことはスイッチ１７４ａおよび１７４ｂを制御装置４００に結合することによって達成され得、制御装置４００は、係合期間を追跡する。一旦、係合期間が所定期間を超過すると、制御装置４００は、関節運動機構１７０を起動させる。

スイッチ１７４ａおよび１７４ｂは、また、最初の関節運動コマンドが処理された後に、制御装置４００に信号を提供するように構成される。このことは、制御装置４００が、以前に発行されたコマンドを中断し、その後に、関節運動機構１７０に関節運動を逆転するように信号を送ることを可能にする。この機能は、スイッチ１７４ａおよび１７４ｂの両方が、所定期間の間、係合されていなければ、制御装置４００によって逆転のコマンドが送信されないように、上述のデュアル起動構成と組み合わされ得る。さらに具体的には、所定期間の間の第一の配向（例えば、スイッチ１７４ａを引きつつスイッチ１７４ｂを押すこと）におけるスイッチ１７４ａおよび１７４ｂの係合は、第一の方向にエンドエフェクタ１６０を関節運動させるように、制御装置４００に信号を送る。所定期間の間の第二の配向（例えば、スイッチ１７４ａを押しつつスイッチ１７４ｂを引くこと）におけるスイッチ１７４ａおよび１７４ｂの係合は、第一の方向とは反対の第二の方向にエンドエフェクタ１６０を関節運動させるように、制御装置４００に信号を送る。
図２および４に関して、器具１０は、また、同時起動関節運動コマンドを中断するように構成される停止スイッチ１１５を含む。スイッチ１１５は、また、制御装置４００に結合され、スイッチ１１５の係合は、現在の関節運動コマンドを中断する停止コマンドをモータ１３２に発行するように、制御装置４００に信号を送る。一つの実施形態において、スイッチ１１５は、また、制御装置４００が関節運動機構１７０を起動解除させる前に、スイッチ１１５が、所定期間の間、係合されなければならないように、遅延起動スイッチングに対して構成され得る。
関節運動プロセスの手動による起動解除および中断に加えて、自動インターロックがまた、利用されることも想定される。図４に関して、器具１０は、制御装置４００に電気的に結合されるロックアウトセンサ２９０を含む。ロックアウトセンサ２９０は、連結ロッド２２０に近接して筺体１１０内に配置され、連結ロッド２２０の長手方向位置を決定するように構成される。ロックアウトセンサ２９０は、連結ロッド２２０の直線変位を決定することに適した任意のタイプの直線位置センサ（例えば、電位差計、近接センサ（例えば、光学式および／または強磁性）、直線可変変位変換器、シャフトエンコーダ、ホール効果センサ等）である。ロックアウトセンサ２９０は、エンドエフェクタ１６０が係合されているとき、すなわち、エンドエフェクタ１６０の第一および第二の対向する顎部材が組織を把持しているときを、連結ロッド２２０の変位を測定することによって決定する。一旦、連結ロッド２２０が所定の地点を越えて遠位に移動させられると、顎部材は係合され、組織はその間に把持される。従って、顎部材が係合されると、ロックアウトセンサ２９０が連結ロッド２２０の遠位変位を測定し、制御装置４００に信号を送り、その信号に応答して、制御装置４００は、スイッチ１７４ａおよび１７４ｂからのいかなる関節運動コマンドも無視することによって関節運動機構１７０の起動を防ぐ。関節運動機構１７０の偶発的な係合は、器具１０に損傷を与え、さらに重要なことに、患者に傷害を生じさせ得る。制御装置４００と組み合わされてロックアウトセンサ２９０を使用することは、関節運動機構１７０のそのような偶発的な誤用を防ぐ。

を含む、電動手術用器具。 - 前記第一のスイッチおよび前記第二のスイッチは、所望の関節運動の方向に対応する方向に係合される、請求項６に記載の電動手術用器具。
- 前記関節運動機構は、停止スイッチをさらに含み、該停止スイッチは、該関節運動機構を使用不能にするように構成される、請求項６に記載の電動手術用器具。
- 前記制御装置は、第一のコマンドに応答して前記関節運動機構を起動させることと、第二のコマンドに応答して該関節運動機構の該起動を中断させることとを行うように構成される、請求項６に記載の電動手術用器具。
- 前記関節運動機構は、ロックアウトセンサをさらに含み、該ロックアウトセンサは、前記制御装置に電気的に結合され、前記関節運動ツールアセンブリの組織との係合を決定するように構成され、該ロックアウトセンサは、該関節運動機構の起動を防ぐ、請求項６に記載の電動手術用器具。
- 関節運動ツールアセンブリを関節運動させるように構成された関節運動機構であって、該関節運動機構は、
関節運動ツールアセンブリに動作可能に結合されたモータと、

該制御装置に結合された第一のスイッチおよび第二のスイッチであって、該第一のスイッチおよび該第二のスイッチは、該関節運動機構の両側に配置され、該第一のスイッチおよび該第二のスイッチは、第一の方向および第二の方向を有し、該制御装置は、該第一のスイッチおよび第二のスイッチが、所定期間の間、反対方向に同時に係合されていることに応答してのみ、該関節運動機構を起動させる、第一のスイッチおよび第二のスイッチと
を備えている、関節運動機構。 - 前記第一のスイッチおよび第二のスイッチは、所望の関節運動の方向に対応する方向で係合される、請求項１１に記載の関節運動機構。
- 前記関節運動機構を使用不能にするように構成される停止スイッチをさらに備えている、請求項１１に記載の関節運動機構。
- 前記制御装置は、第一のコマンドに応答して前記関節運動機構を起動させることと、第二のコマンドに応答して該関節運動機構の該起動を中断することとを行うように構成される、請求項１１に記載の関節運動機構。
- ロックアウトセンサをさらに含み、該ロックアウトセンサは、前記制御装置に電気的に結合され、前記関節運動ツールアセンブリの組織との係合を決定するように構成され、該ロックアウトセンサは、前記関節運動機構の起動を防ぐ、請求項１１に記載の電動手術用器具。
