外科用ステープルは、様々な医療処置（例えば、手術）の間に利用され得る高度に専門的なステープルである。皮膚損傷の縫合、腸の吻合、および肺の一部の摘出は、外科用ステープルが利用され得る様々なタイプの医療処置のほんの一部分である。
臨床医は、医療環境における慣習的な縫合方法に対する一般的な代替案として、外科用ステープラと、それに関連する外科用ステープルとに関心を向けてきた。処置の中に外科用ステープラを組み込むことは、臨床医が迅速に、かつ容易に組織を縫合することを可能にする。

（概要）
本開示は、ステープル適用後、組織圧縮の量に比例する量で跳ね返るように構成され、かつ、そうするような寸法にされているバックスパンを有するステープルを有利に提供し、それによって、特定の処置における組織の引き裂きのリスクを有利に緩和し、低減する。従って、組織に適用されるステープルの全体の剛性が低減される。
本開示は、ステープル適用後、組織圧縮の量に比例する量で跳ね返るように構成され、かつ、そうするような寸法にされているバックスパンを有するステープルを有利に提供し、それによって、特定の処置における組織の引き裂きのリスクを有利に緩和し、低減する。従って、組織に適用されるステープルの全体の剛性が低減される。

（摘要）
外科用ステープルは、第一の変形可能な脚部と、第二の変形可能な脚部と、第一の横の部分および第二の横の部分を有するバックスパンとを備え、第一および第二の横の部分は、それらの間にギャップを作り出すために、間隔をおいて配置されている。バックスパンは、第一および第二の横の部分から遠位方向に、かつギャップの遠位方向に間隔をおいて配置されている第三の横の部分をさらに含む。第一の非直線のバックスパンの部分は、第一の横の部分と第三の横の部分の第一の端部分との間に延び、第二の非直線のバックスパンの部分は、第二の横の部分と第三の横の部分の第二の端部分との間に延びている。

本開示において、本明細書中に開示されているステープルは、オープン用ステープラおよび腹腔鏡的／内視鏡的ステープラを含む円形吻合ステープラ、直線形ステープラ、横形ステープラなどの様々なステープル留め器具によって利用され得ることが想定される。図１は、本開示の外科用ステープルを適用するために用いられ得る一タイプの外科用器具を例として提供し、図２は、外科用ステープルを含んでいるステープラ１０のカートリッジを例示する。器具１０は、米国特許第７，１４０，５２７号（その内容全体は参考として本明細書中に援用される）に開示されている器具などのオープン外科用直線形ステープラである。図１を参照すると、ステープラ１０は、カートリッジアセンブリ１２と、アンビルアセンブリ１４と、摺動可能なノブ２６とを有することにより、ステープルプッシャを接触させるためのカムバーを前進させて、ステープルをカートリッジアセンブリ１２からアンビルアセンブリ１４のアンビルとの接触状態へと前進させる。
図面において、および以下の説明においては、慣習的であるように、用語「近位」が、オペレータにより近い本開示の外科用デバイスまたは器具の端部を表し、一方で、用語「遠位」が、オペレータからより遠いデバイスまたは器具の端部を表している。
図２を参照すると、外科用ステープル留めカートリッジアセンブリ１２が例示され、カートリッジアセンブリ１２は、ステープル列の間の組織を切断するためのナイフの遠位方向の動きに適応するために、間にナイフスロット２４を有する複数のステープルスロット２２を有するカートリッジ１６を有する。示されているカートリッジ１６においては、ナイフスロット２４の各々の側に２つの並行するステープルスロット２２の列が存在するが、異なる数のステープルスロットが、ステープルの異なる列の構成に適応するために提供され得ることが認識されるべきである。例えば、１つの列のステープル（およびステープルスロット）または３つの列のステープルが、ナイフスロットの各々の側に提供され得る。ステープルは、概して参照番号３０によって示され、ステープルプッシャは、概して参照番号８０によって示されている。ステープラ１０の動作（例えば、カートリッジとアンビルアセンブリとのクランプ動作、およびステープルを発射する発射ノブの前進動作）の詳細は、上で参考として本明細書中に援用された米国特許第７，１４０，５２７号に開示されている。
図３および図４を参照すると、ステープル３０がここで記載される。ステープルは、丸い断面のステンレス鋼ワイヤから好ましくは構成されるが、他の材料とワイヤ形状とが想定される。ステープル３０は、バックスパン５０によって結合された一対の脚部３２、４２を一般に備え、バックスパン５０は、間隔をおいて配置された第一および第二の近位の横の部分５２および６２と、第一および第二の横の部分５２、６２から遠位方向に間隔をおいて配置され、第一および第二の横の部分５２、６２の間の空間（ギャップ）７０を通過する下記のステープルプッシャ８０によってアクセス可能である第三の横の領域５９とを有する。
より具体的には、ステープル３０は、第一の変形可能な脚部３２と第二の変形可能な脚部４２とを有する。バックスパン５０は、説明の都合上、ステープル脚部３２と関連づけられる第一の領域５１と、ステープル脚部４２と関連づけられる第二の領域６１とに分割されている。好ましくは、バックスパン領域５１および６１は、一体であり、単一のワイヤから形成されるようにステープル脚部３２、４２と一体である。各々の脚部３２、４２は、組織を貫通するように構成されている遠位の先端部３４、４４において、それぞれ終端する。第一の脚部３２の近位部分３５は、バックスパン５０の第一の横のバックスパンの部分５２へと移行する。同様に、第二の脚部４２の近位部分４５は、バックスパン５０の第二の横のバックスパンの部分６２へと移行する。第一および第二のバックスパンの部分５２、６２は、上記のバックスパン領域５１、６１のそれぞれ一部分である。好ましくは、ステープル脚部３２、４２は、それぞれのバックスパンの部分に対して実質的に垂直に延び、互いに対して実質的に並行に延びている。しかしながら、脚部が収束するかまたは発散し、それによって、示されている約９０度とは異なる角度で脚部がバックスパンの部分から延びることもまた想定される。
第一の横のバックスパンの部分５２は、第二の脚部４２に向かって内側に延び、そして、脚部３２の遠位の先端部３４に向かって部分５４で遠位方向に延び、そして、部分５５で第一の脚部３２に向かって内側から後戻りに延び、そして、部分５６で遠位方向に延び、それから、部分５７で第二の脚部４２に向かって内側に延びている。これらの部分は、第一のバックスパン領域５１の一部分と考えられ得る。
バックスパン５０の第二のバックスパン領域６１は、第一のバックスパン領域５１の鏡像である。さらに具体的には、第二の横のバックスパンの部分６２は、第一の脚部３２に向かって内側に延び、そして、脚部４２の遠位の先端部４４に向かって部分６４で遠位方向に延び、そして、部分６５で第二の脚部４２に向かって内側から後戻りに延び、そして、部分６６で遠位方向に延び、それから、部分６７で第一の脚部３２に向かって内側に延びている。これらの部分は、第二のバックスパン領域６１の一部分と考えられ得る。
別の方法で述べると、第一のバックスパン領域５１は、いくぶんＳ字形の領域によって、第三の横の領域５９の部分５７に接続された第一の横の部分５２を有し、第二のバックスパン領域６１は、いくぶんＳ字形の領域によって、第三の横の領域５９の部分６７に接続された第二の横の部分６２を有する。部分５７および６７は、示されているように、第三の横の領域５９が実質的に直線で、連続的であるように好ましくは結合される。第一の横の部分５２と第二の横の部分６２とが好ましくは、実質的に直線で、間隔をおいて配置されて、それらの間にギャップ７０を形成する。このギャップ７０は、下記のように、ステープルプッシャ８０の、第三の横の領域５９へのアクセスを可能にする。
第一および第二のバックスパン領域５１および６１の様々な部分における屈曲は好ましくは、実質的に直線部分を有するように約９０度で形成され、従って、実質的に直線部分および約９０度の角度を有するいくぶんＳ字形を有する。異なる角度によって結合されている曲線および／または領域を有する領域もまた想定される。
カートリッジアセンブリ１２のプッシャ８０は、ステープルスロット２２に隣接して位置決めされ、個々のステープル３０をアンビルアセンブリ１４のアンビルと係合するように前進させるように構成されている。複数のカムバー（図示されていない）が、カートリッジアセンブリ１２内に含まれており、摺動ノブ２６によって遠位方向に移動可能である。カムバーがステープルプッシャ８０の斜めの内側表面８１を係合するので、プッシャ８０が、ステープルスロット２２に向かって前進させられることにより、ステープル３０をカートリッジ１６からアンビルとの接触状態へと前進させ、ステープル脚部３２、４２が、アンビルポケットに接触し、お互いに向かって曲がるようにアンビルポケットによって変形させられる。プッシャ８０の突出フィンガ８２は、ステープル３０の第一および第二の横の部分５２、６２の間のギャップ７０を通過し、第三の横の領域５９との接触に、好ましくは、図５に示されるような実質的にそれの中央の領域内に進む。肩部（すなわち、ステープル脚部への移行部に隣接するバックスパン領域）ではなく中央領域に接触することによって、ステープル３０は、バックスパン５０が組織を圧縮し、その後の曲がりを可能にするように位置決めされた状態で組織の中に配置される。すなわち、ステープル３０の適用後、組織が膨張したり、伸びたりする場合には、バックスパン５０が曲がることにより、組織の動きに適応する。このことは、例えば、図５と図６とを比較することによって示される。
図５は、ステープルプッシャ８０によるステープル３０の適用を示している。プッシャ８０は、最大の所定の量の動き／力がフィンガ８２によって領域５９に適用された後、プッシャ８０の表面８３ａ、８３ｂが肩部（すなわち、脚部３２、４２に隣接するバックスパン５０の横の部分５２、６２の部分）と接触し、従って、ステープル適用の間、屈曲要素の過大な変形を避けるように構成されている。
図６は、ステープル適用後、組織Ｔによってバックスパンが近位方向に動く場合のバックスパン５０の曲がりを示している。より具体的には、バックスパン５０の屈曲領域に起因して、第三の横の領域５９が、第一および第二の横の部分５２、６２のそれぞれに向かって曲がり得ることにより、組織の膨張（組織の押し戻し）を可能にする。第一および第二の横の部分５２、６２はまた、形成されたステープル脚部３２、４２から離れる方向に曲がり得、組織の伸び／膨張に対する余地を提供する。従って、ステープル３０のバックスパン５０のコンプライアンス／ばね反応性は、ステープルへの組織の押し戻しに適応する。換言すると、ステープルの剛性は、その脚部ではなく、複数の曲がりを有するばねのようなバックスパンによってほぼ決定され、組織圧縮の量によって決定され、かつ、その量に比例する量だけステープルが跳ね返るので、それによって、より一定に近い力／圧力を組織上に加えることができるので、特定の用途における引き裂きのリスクを緩和し、低減する。
本開示のいくつかの実施形態が、図面中に示され、かつ／または本明細書において論じられてきたが、本開示は、当該分野が許容するような広い範囲であり、本明細書は、同じように読み取られることが意図されているので、本開示がそれらの実施形態に限定されることは意図されていない。従って、上記の説明は限定としてではなく単に特定の実施形態の例示として解釈されるべきである。当業者は、添付の特許請求の範囲の精神および範囲の内において他の修正を想定するであろう。

Claims (15)
- 外科用ステープルであって、該外科用ステープルは、
第一の近位部分と、第一の組織貫通先端部を有する第一の遠位部分とを有する第一の変形可能な脚部と、
第二の近位部分と、第二の組織貫通先端部を有する第二の遠位部分とを有する第二の変形可能な脚部と、
バックスパンと
を備え、
該バックスパンは、該第一の脚部の該第一の近位部分から延びる第一の横の部分と、該第二の脚部の該第二の近位部分から延びる第二の横の部分とを有し、
該第一の横の部分および該第二の横の部分は、それらの間にギャップを作り出すために、間隔をおいて配置され、
該バックスパンは、
第一の端部分と第二の端部分とを有する第三の横の部分であって、該第三の横の部分は、該第一の横の部分および該第二の横の部分から遠位方向に、かつ該ギャップの遠位方向に間隔をおいて配置されている、第三の横の部分と、
該第一の横の部分と該第三の横の部分の該第一の端部分との間に延びている第一の非直線のバックスパンの部分と、

- 前記非直線のバックスパンの部分の各々は、横の領域によって結合された２つの遠位方向に延びる領域を備えている、請求項１〜請求項５のうちのいずれか一項に記載の外科用ステープル。
- 外科用ステープルであって、該外科用ステープルは、
第一の近位部分と、第一の組織貫通先端部を有する第一の遠位部分とを有する第一の変形可能な脚部と、
