（関連技術の背景）

例えば、ＳＵＬＵ１６は、米国特許第６，９５３，１３９号に開示されているように、その近位端回りで旋回可能であるツールアセンブリ１７を含み得、該米国特許の開示は、本明細書に参考として援用される。この実施形態において、ユーザは、ハンドルアセンブリ１２に位置決めされた関節運動レバー３０によって、ツールアセンブリ１７の旋回可能の動きを制御する。関節運動レバー３０は、ハンドルアセンブリ１２の内側に位置決めされたカム部材（図示されず）と旋回可能に係合する。カム部材は、並進部材（図示されず）と動作可能に結合される。並進部材は、ハンドルアセンブリ１２内に位置決めされ、カム部材と並進部材との間の係合は、並進部材の回転を抑制するが、関節運動レバー３０の旋回運動に応答して、並進部材の直線的な動きを可能にする。並進部材の遠位端は、第１の関節運動リンク（図示されず）の近位端に結合される。動作の間、並進部材の直線状の動きは、第１の関節運動リンクの対応する直線状の動きを引き起こす。ハンドルアセンブリ１２内に主に位置している第１の関節運動リンクは、第２の関節運動リンク（図示されず）に動作可能に接続される。第２の関節運動リンクは、第１の関節運動リンクの直線的な動きに応答して直線的に動くように構成され、取り付けアセンブリ２３の横にずれた内部の部分と係合する。取り付けアセンブリ２３は、ピボットピン２５を含み、ツールアセンブリ１７と細長い部分３１とを相互に接続する。第２の関節運動リンクは、ピボットピン２５から横にずれているので、第２の関節運動リンクの直線的な動きは、取り付けアセンブリ２３をピボットピン２５回りに旋回させ、ツールアセンブリ１７を関節運動させる。
ツールアセンブリ１７の旋回運動を制御することに加えて、ハンドルアセンブリ１２は、ツールアセンブリ１７の作動を指示する。この目的で、ハンドルアセンブリ１２は、固定ハンドル部材２２と、可動ハンドル部材２４と、長手方向軸「Ｘ」を画定するバレル部分２６とを含む。バレル部分２６は、その遠位部分に取り付けられた回転可能部材２８を有する。使用に際して、回転可能部材２８は、ハンドルアセンブリ１２に対する細長い本体１４の回転を容易にする。
図４、図４Ａ〜図４Ｃ、および図５を参照すると、バレル部分２６は、発射駆動部２を取り囲み、発射駆動部２は、歯付きラック３８と駆動つめ４４とを含む。駆動つめ４４は取り付け部分４４ａと、歯付きラック３８と係合するように適合された係合部分４４ｂとを含む。ピボットピン５１は、可動ハンドル部材２４（図１）の一端と駆動つめ４４とを旋回可能に相互に接続し、可動ハンドル部材２４の固定ハンドル部材２２に向かう動きに応答して、駆動つめ４４の係合部分４４ｂが歯付きラック３８に向かって旋回することを可能にする。したがって、駆動つめ４４は、歯付きラック３８と選択的に係合し、作動ストロークによる可動ハンドル部材２４（図１を参照）の操作に応答して、遠位方向に発射駆動部２を前進させるように取り付けられる。ねじりばねであり得る付勢部材４７は、駆動つめ４４の係合部分４４ｂを歯付きラック３８に向かって促すように位置決めされる。つめ４４の取り付け部分４４ａは、バレル部分２６内に取り付けられた当接壁４５と相互に作用するように適合される。上に論議されたように、可動ハンドル２４は、駆動つめ４４の係合部分４４ｂを動かして、歯付きラック３８と接触させ、発射駆動部２を遠位方向に直線的に前進させることができるようにつめ４４に旋回可能に接続される。結果として、駆動つめ４４は、可動ハンドル部材２４の操作により回転し、発射駆動部２の歯付きラック３８と係合し、そして係合から外れる。発射駆動部２は、制御ロッド（図示されず）に動作可能に接続される。発射駆動部２の直線的動きに応答して直線的に動くように適合される制御ロッドは、アンビルアセンブリ２０と機械的に協働するように配置されたカムローラ（図示されず）に動作可能に結合される。カムローラは、制御ロッドが遠位方向に動かされると、アンビルアセンブリ２０の一部分に沿って、遠位方向に動くように構成される。カムローラが、アンビルアセンブリ２０に沿って遠位方向に動くと、アンビルアセンブリ２０は、カートリッジアセンブリ１８に向かってピボットピン２１回りに旋回する（図１〜図３を参照）。制御ロッドは、カム部材（図示されず）にも動作可能に接続され、該カム部材は、ステープルをカートリッジアセンブリ１８の外へ促すように構成される。動作において、カム部材は、制御ロッドの遠位方向の動きに応答して、近位の位置から遠位の位置へ動く。カム部材が遠位方向に動くと、カム部材は、ステープルプッシャ（図示されず）に作用し、これらのステープルプッシャ上に休止しているステープルを排出する。ステープルは、カートリッジアセンブリ１８から排出されながら、保持スロット１８ａを通ってカートリッジアセンブリ１８から出て、アンビルアセンブリ２０の方に動き、組織をステープルする。
動作において、ユーザは、ねじりばね４７の付勢に抗して可動ハンドル部材２４を固定ハンドル２２に向かって旋回させることによって、外科用装置１０を発射し、駆動つめ４４を動かして、発射駆動部２上の歯付きラック３８と係合させる。したがって、可動ハンドル部材２４の旋回運動は、駆動つめ４４を歯付きラック３８と係合させ、発射駆動部２を遠位方向に駆動する。発射駆動部２の遠位方向の前進は、制御ロッドを遠位方向に動かす。制御ロッドが遠位方向に動くと、カム部材は、アンビルアセンブリ２０に沿って動き、アンビルアセンブリ２０をカートリッジアセンブリ１８に向かって促し、アンビルアセンブリ２０とカートリッジアセンブリ１８との間に位置決めされた組織を締め付ける。制御ロッドの遠位方向の動きは、カム部材を遠位方向にも動かす。カム部材が遠位方向に動くと、カム部材は、ステープルプッシャをアンビルアセンブリ２０に向かって動かす。ステープルはステープルプッシャ上に着座しているので、ステープルは、ステープルプッシャがアンビルアセンブリ２０に向かって動くと、カートリッジアセンブリ１８から排出される。
一実施形態において、ステープル発射の動作を完成するために、可動ハンドル部材２４は、固定ハンドル２２に向かって再度接近させられ、駆動つめ４４を歯付きラック３８と係合させ、発射駆動部２を遠位方向にあと１５ｍｍ前進させる。このようにして、この実施形態において、作動ハンドル２４の２つの完全なストロークが、バレル部分２６内で発射駆動部２を３０ｍｍ前進させ、カートリッジアセンブリ１８の中のすべての外科用ステープルを連続して排出する。必要であれば、オペレータは、複数の短いストロークによって、発射デバイス２を漸増的に前進させ得、この場合、最小の前進は、ラック３８上の歯間の直線距離によって決まる。したがって、２つの完全な１５ｍｍのストローク距離のストロークが、３０ｍｍの使い捨てローディングユニットを発射するために使用され得るが、完全なストロークは必要ではないし、必要ともされない。外科用装置１０は、様々なストローク距離を有するように構成され得る。
一対のレトラクタノブ３２が、発射駆動部２の近位端に接続される。結合ピン４８が、両引込みノブ３２を相互に接続し、バレル部分２６に沿ったレトラクタノブ３２の並進を容易にする。詳細には、結合ピン４８の少なくとも一部分が、図１に見られるように、バレル部分２６に沿って形成された一対の長手方向のスロット３４ａ内を並進するような大きさおよび構成とされる。解放プレート６４が、発射駆動部２と動作可能に関連付けられ、レトラクタノブ３２の操作に応答して、発射駆動部２に対して動くように取り付けられる。図４Ａに示されるように、一対の間隔が置かれて離されたピン２９が、発射駆動部２の側面から外向きに延び、解放プレート６４に形成された一対の対応する角度のついたカムスロット４０と係合する。その開示が本明細書に参考として援用される米国特許第７，０４４，３５３号が、発射駆動部２および解放プレート６４の構造および手動による動作を詳細に記述する。要するに、ユーザは、レトラクタノブ３２を近位方向に手動で並進させ、発射駆動部２をその近位位置まで引込む。発射駆動部２が近位方向に動くと、発射駆動部２と動作可能に関連付けられたカム部材も、近位方向に動く。
引込みを可能にするために、発射駆動部２は、遠位端４２ａおよび近位端４２ｂを有する遠位方向に付勢されたコネクタ４２を含む。コネクタ４２は、発射駆動部２の上部部分に取り付けられ、結合ピン４８を受け入れるような大きさ、および構成とされた穴４２ｃを有する。可撓性部材１９は、コネクタ４２と引込みプーリ９とを相互に接続する。本開示は、可撓性部材１９が、コネクタ４２と引込みプーリ９とを相互に接続することのできるケーブル、チェーン、または任意の他の適切なデバイスであり得ることを想定している。特に、可撓性部材１９の第１の端１９ａは、コネクタ４２の近位端４２ｂに取り付けられる。図１１に見られるように、可撓性部材１９の第２の端１９ｂは、引込みプーリ９に取り付けられる。
図５〜図９を参照すると、引込みメカニズム３は概して、ハンドルアセンブリ１２内で長手方向軸「Ｘ」に対して平行に位置決めされ、発射駆動部２を近位の位置まで引込むことを容易にする。この目的で、引込みメカニズム３は、可撓性部材１９によって、発射駆動部２と動作可能に関連付けられる。さらに、引込みメカニズム３は、減速メカニズムまたはトランスミッション８と電気機械的に協働するように配置されたモータ６と、随意的なギアボックス６ａとを含む。特に、伝動シャフト４６は、モータ６に取り付けられる。結合部７は、伝動シャフト４６と減速メカニズム８とを相互に接続する。減速メカニズム８は、モータ６によって供給される回転運動を効果的に低減する。一実施形態において、減速メカニズム８はウォーム歯車セットであり、該ウォーム歯車セットは、第２の歯車８ｂと噛み合うように構成された第１の歯車８ａを含む。第１の歯車８ａは結合部７に対して固定され、動作において、伝動シャフト４６の回転に応答して回転する。続いて、第１の歯車８ａの回転は、第２の歯車８ｂの回転を引き起こす。
図８〜図１１を参照すると、第２の歯車８ｂは、中空のシャフト７８に回転可能に接続される。中空のシャフト７８は、第２の歯車８ｂが回転すると回転するように適合される。さらに、中空のシャフト７８は、そこを貫通するボア７６を有する。ボア７６は、シャフト１３の少なくとも一部分を受取るように適合される。中空のシャフト７８は、第２の歯車８ｂとクラッチメカニズム１１とを動作可能に接合する。したがって、クラッチメカニズム１１は、減速メカニズム８と回転可能に協働するように配置される。図示された実施形態において、クラッチメカニズム１１は、遊星クラッチである。しかしながら、本開示は、クラッチメカニズム１１が、任意の他の適切なクラッチであり得ることを想定している。クラッチメカニズム１１は、係止された状態および係止解除された状態を有する。ストラップ６２およびばねで負荷されたレバー５（図６および図７）は共に作動して、クラッチメカニズム１１を係止された状態から係止解除された状態へおよびその逆に切り替えるように設計される。
特に、ストラップ６２は、図６に示されるように、レバー５が作動すると、クラッチメカニズム１１と係合して、これを係止するように適合される。一実施形態において、レバー５は、トリガ５ａと、細長い区間５ｂと、穴５ｃとを含む。穴５ｃは、ピボットピン５２を受け入れるように構成される。ピボットピン５２、または任意の適切なデバイスは、レバー５を引込みメカニズム３に旋回可能に接続する。レバー５は、ユーザによって手動により操作されると、ピボットピン５２回りに回転する。レバー５は、ストラップ６２の少なくとも一部分を受け入れるように適合された開口部５ｄをさらに含む。ストラップ６２の一部分は、リング歯車７７を取り囲み、ストラップ６２の別の部分は、レバー５に動作可能に接続される。一実施形態において、ストラップ６２の一部分は、引込みメカニズム３のフレームに直接的に接続される。動作の間、レバー５のトリガ５ａの遠位方向の並進は、ストラップ６２の一部分を遠位方向に動かし、クラッチメカニズム１１を係止する。レバー５が遠位方向に動くと、リング歯車７７の円周に沿ってストラップ６２によって及ぼされる力が増し、リング歯車７７の回転を抑制する。リング歯車７７を係止解除するために、ユーザは、レバー５をその最初の位置に戻し得る。あるいは、レバー５は、付勢デバイス、例えばコイルばねの影響の下で、その最初の位置に戻り得る。
図８〜図１１を参照すると、クラッチメカニズム１１は、遊星歯車システムで構成され得る。クラッチメカニズム１１は、複数の遊星歯車８６と、センタリング１５と、太陽歯車８４と、リング歯車７７とを含む。太陽歯車８４は、歯８４ａとそこを貫通する管腔５４とを含む。管腔５４は、シャフト１３を受け入れるように適合される。シャフト１３の一部分は、中空のシャフト７８内に配置される。動作の間、中空のシャフト７８は、シャフト１３の少なくとも一部分の回りに回転する。中空のシャフト７８は、遊星歯車８６、および既に論議されたように、歯車８ｂと機械的に協働するように配置される。
遊星歯車８６は、太陽歯車８４の周りに位置決めされる。したがって、各遊星歯車８６は、その中心回りおよび太陽歯車８４の中心回りに回転するように構成される。さらに、各遊星歯車８６は、複数の歯８６ａとボア８６ｂとを含む。歯８６ａは、太陽歯車８４の歯８４ａと噛み合うように構成される。各ボア８６ｂは、シャフト６０を受け入れるように構成される。シャフト６０は、各遊星歯車８６をセンタリング１５に接続する。
センタリング１５は、図１０および図１１に示されるように、すべての遊星歯車８６を相互に接続し、遊星歯車８６と機械的に協働するように配置された２つの環状のプレート５６、５８を含む。環状のプレート５６、５８は各々、遊星歯車８６の両側に動作可能に取り付けられる。特に、環状のプレート５８は、中空のシャフト７８にも動作可能に結合される。各環状のプレート５６、５８は、シャフト６０を受け入れるように適合されたボア５７を含む。
使用の間、レバー５の作動は、ストラップ６２をリング歯車７７の周りに押さえつけ、リング歯車７７を動かないようにし、それによってクラッチメカニズム１１を係止された位置に配置する。リング歯車７７の中心は、太陽歯車８４の中心と一致する。
上で論議されたように、太陽歯車８４の管腔５４は、シャフト１３を受け入れるように構成される。シャフト１３は太陽歯車８４、引込みプーリ９、およびばねモータ８０と動作可能に関連付けられる。ばねモータ８０（図８）は、第１のスプール６４と、第２のスプール６６と、第１のスプール６４と第２のスプール６６とを相互に接続するばね６８とを含む。使用において、ばねモータ８０は、可撓性部材１９の張力を維持し、そのもつれを防止する。
可撓性部材１９は、図１１に見られるように、引込みプーリ９に動作可能に固定される。引込みプーリ９は、その円周に沿ってリセス７０を有する。リセス７０は、可撓性部材１９を受け入れるように適合される。さらに、引込みプーリ９は、相対するフランジ７２、７４を含む。フランジ７２、７４は、リセス７０内に可撓性部材１９を固定する。可撓性部材１９は、引込みプーリ９と発射駆動部２とを相互に接続する。特に、可撓性部材１９の第２の端１９ｂは、引込みメカニズム２の引込みプーリ９に取り付けられ、可撓性部材１９の第１の端１９ａは、コネクタ４２の近位端４２ｂに取り付けられる。
引込みメカニズム３は、発射の後、外科用装置１０の発射駆動部２の引込みを容易にする。動作において、医師は、ハンドルアセンブリ１２を手動で作動させることによって外科用装置１０を発射する。オペレータは、片手だけでハンドルアセンブリ１２を作動させ得る。詳細には、ユーザオペレータは、可動ハンドル部材２４を固定ハンドル部材２２に向かって手動で接近させ、発射駆動部２を遠位方向に動かす。発射駆動部２の遠位方向の並進は、アンビルアセンブリ２０を接近された位置に動かす。さらに、発射駆動部２の遠位方向の動きは、カートリッジアセンブリ１８の保持スロット１８ａに配置された外科用ステープルの排出を引き起こす。このプロセスの間、発射駆動部２は、近位の位置から遠位の位置へ並進する。可撓性部材１９は、発射駆動部２に動作可能に接続されているので、発射駆動部２の遠位方向の動きは、可撓性部材１９の遠位方向の並進も引き起こす。可撓性部材１９が遠位方向に動くと、可撓性部材１９は、引込みプーリ９から巻きを解く。この瞬間に、モータ６はオフとなり、クラッチ１１は係止解除され、センタリング１５は、固定されたままとなり、遊星歯車８６に対する基盤として役立つ。
一旦外科用装置１０が発射されると、外科医は、再使用するために、駆動２をその近位の位置に引込めなければならない。手動での引込みが必要である場合、外科医は、ノブ３２を手動で操作することにより、発射駆動部２をその近位の位置へ動かし得る。特に、オペレータは、ハンドルアセンブリ１２を片手で保持しながら、別の片手でノブ３２を近位方向に並進させ得る。ノブ３２の近位方向の並進は、発射駆動部２の近位方向の動きを引き起こす。その後、外科用装置１０は再度、発射に対して準備ができる。
動力式引込みが必要である場合、オペレータは、引込みメカニズム３を使用することによって、その近位の位置に発射駆動部２を戻し得る。外科医は、レバー５によって、引込みメカニズム３を作動させる。特に、外科医は、レバー５のトリガ５ａを押し下げ、モータ６を作動させ、クラッチメカニズム１１をその係止された位置に動かす。この瞬間、ストラップ６２は、リング歯車７７の周囲を締め付け、それらの間の摩擦を有意に増加させる。したがって、リング歯車７７は基盤に固定される。
モータ６は伝動シャフト４６を駆動する。伝動シャフト４６の回転は、減速メカニズム８の第１の歯車８ａを回転させる。第１の歯車８ａは、第２の歯車８ｂと噛み合う。第１の歯車８ａの回転運動は、第２の歯車８ｂの回転を引き起こす。第２の歯車８ｂが回転すると、中空のシャフト７８が回転し、センタリング１５の対応する回転を引き起こす。したがって、遊星歯車８６は、リング歯車７７の内側で、かつ太陽歯車８４の周囲を回転する。遊星歯車８６は、それらの中心回りに、かつ太陽歯車１４の中心回りに回転する。遊星歯車８６の全体的な動きは、太陽歯車８４を駆動する。太陽歯車１４が回転する間、シャフト１３は、プーリ９を回転させる。プーリ９は、反時計回りの方向に回転し、その周りに可撓性部材１９を巻く。プーリ９の回転運動はまた、近位方向に可撓性部材１９を並進させる。可撓性部材１９の近位方向の並進は、発射駆動部２をその近位の位置へ動かす。一旦上述のプロセスが完了すると、外科用装置は、再使用の準備ができる。
引込みプロセスの間、外科医は、引込みに対する抵抗が大きすぎてモータ６単独では作動することができないことを感知し得る。この場合、外科医は、引込みメカニズム３を手動でオーバーライドし得る。オペレータは、レバー５を係合解除することによって、モータ６を止める選択肢を有する。このとき、外科用装置１０の発射駆動部２は、その近位の位置へ、ノブ３２によって手動で戻され得る。ノブ３２を使用して発射駆動部２をその近位の位置へ戻す方法は、上に論議された。
様々な変更が本明細書に開示された実施形態に対してなされ得ることは、理解される。したがって、上述は、限定するものとして解釈されるべきではなく、実施形態の単なる例示として解釈されるべきである。当業者は、本明細書に添付された請求項の範囲および精神内で他の変更に想到する。例えば、外科用装置１０はセンサを含み得、引込みメカニズム３による引込みに対する抵抗をモニタし、モータ６の警報またはフィードバック制御を作動させ得る。センサは、発射駆動部２がその近位の位置に達したとき、モータ６を止めるためにも使用され得る。

Claims (13)

  1. ハンドヘルド外科用ステープリング装置に対して使用される引込みメカニズムであって、
    モータによって駆動されるように適合された第１の減速メカニズムと、
    係止された状態および係止解除された状態を有する第２の減速メカニズムを組み込んだクラッチメカニズムであって、該クラッチメカニズムは、該第１の減速メカニズムと機械的に協働するように配置されている、クラッチメカニズムと、
    該第２の減速メカニズムが係止された状態にあるとき、該モータが作動すると回転するように構成されたシャフトであって、該シャフトの回転が、外科用装置の発射駆動部を引込む、シャフトと、
    該シャフトに結合されたプーリであって、該プーリは、該発射駆動部に動作可能に接続されている、プーリと、
    該プーリと該発射駆動部とを相互に接続する可撓性部材と
    を備えている、引込みメカニズム。
  2. 前記第２の減速メカニズムは、遊星クラッチである、請求項１に記載の引込みメカニズム。
  3. 前記プーリを回転可能に付勢するように適合されたばねモータをさらに備えている、請求項１に記載の引込みメカニズム。
  4. 前記ばねモータは、第１のスプールと、第２のスプールと、該第１のスプールと該第２のスプールとを相互に接続するばねとを含む、請求項３に記載の引込みメカニズム。
  5. 前記第１の減速メカニズムと前記遊星クラッチとを相互に接続する中空のシャフトをさらに備えている、請求項１に記載の引込みメカニズム。
  6. 前記中空のシャフトはそこを貫通するボアを有し、該ボアは、前記シャフトの少なくとも一部分を受け入れるように適合されている、請求項５に記載の引込みメカニズム。

    外科用ツールを作動させるように適合された発射駆動部であって、該発射駆動部は、該係止された状態にある間に該遊星クラッチによって駆動されたシャフトの回転に応答して、近位方向に動くように適合され、該シャフトは該モータが作動すると回転する、発射駆動部と、
    該シャフトに結合されたプーリであって、該プーリは、該発射駆動部に動作可能に接続されている、プーリと、
    該プーリと該発射駆動部とを相互に接続する可撓性部材と
    を備えている、引込みメカニズム。
  9. 前記可撓性部材のもつれを防止するように適合されたばねモータをさらに備え、該ばねモータは、前記プーリに動作可能に取付けられている、請求項８に記載の引込みメカニズム。
  10. 前記ばねモータは、第１のスプールと、第２のスプールと、該第１のスプールと該第２のスプールとを相互に接続するばねとを含む、請求項９に記載の引込みメカニズム。
  11. 前記減速メカニズムと前記遊星クラッチとを相互に接続する中空のシャフトをさらに備えている、請求項８に記載の引込みメカニズム。
  12. 前記中空のシャフトはそこを貫通するボアを有し、該ボアは、前記シャフトの少なくとも一部分を受け入れるように適合されている、請求項１１に記載の引込みメカニズム。
