Description
本出願の原出願は、2005年9月14日に出願された米国特許出願第11/227981号の優先権を主張する一部継続特許出願であり、その開示全体は、本明細書に参考として組み込まれる。
Claims (22)
- 縫合針とともに使用する縫合装置であって、
近位端と遠位端を有する本体と、
本体の遠位部分の近くの第1クランプと、
本体の遠位部分の近くの第2クランプと、
把持形状と移動形状の間で前記第1と前記第2クランプの動きを行うリンケージであって、各クランプは前記把持形状で関連した把持位置で縫合針を把持しかつ前記移動形状で前記本体の軸線方向に沿って移動され、前記把持位置は本体に対して固定される、リンケージと、そして
外科医の手によって把持されるように構成されたハンドルであって、ハンドル作動サイクルを規定するように閉じた形状から手で拡げた形状へかつ手で拡げた形状から閉じた形状へ作動可能である、ハンドルと、
を含み、
前記リンケージは、交替可能な駆動要素を含み、前記交替可能な駆動要素は、各ハンドル作動サイクルにより第1形状と第2形状の間で駆動され、前記交替可能な駆動要素は、前記第1形状で前記第1クランプに連結されるように駆動され、かつ前記第2形状で前記第2クランプに連結されるように駆動され、
前記ハンドルは前記リンケージによって前記第1と前記第2クランプに連結され、前記ハンドル作動サイクルを介して前記ハンドルの前記繰り返し作動は、前記把持形状の前記第1クランプおよび前記移動形状の前記第2クランプの状態と前記把持形状の前記第2クランプおよび前記移動形状の前記第1クランプの状態との間で前記縫合装置を繰り返し交替させる、
ことを特徴とする縫合装置。 - 前記ハンドルが前記本体の前記近位端に配置され、作動中に前記リンケージは、前記第2クランプが前記針を把持した後に、前記針の長さ方向の軸から横方向に前記第1クランプを移動させ、かつ作動中に前記リンケージは、前記第1クランプが前記針を把持した後に前記針の長さ方向の軸から横方向に前記第2クランプを移動させることを特徴とする請求項1に記載の縫合装置。
- 前記縫合針の支持は、各ハンドル作動サイクル中に前記第1クランプと前記第2クランプの間で少なくとも1回交替し、かつ前記縫合針がクランプの一方によって把持される間に、外科医は前記ハンドル上の前記外科医の手を閉じて前記ハンドルを操作することによって前記縫合針を組織に挿入することができることを特徴とする請求項2に記載の縫合装置。
- 両方のクランプの各々は、前記本体に対して固定位置で前記縫合針を維持するように、他方のクランプがハンドル作動サイクル中に閉鎖した後で、前記他方のクランプとは別の一方のクランプが開放することを特徴とする請求項3に記載の縫合装置。
- 作動中に前記リンケージは、前記第2クランプが前記針を把持した後に、前記針の近くで横方向に前記第1クランプを移動させ、かつ作動中に前記リンケージは、前記第1クランプが前記針を把持した後に針の近くで横方向に前記第2クランプを移動させることを特徴とする請求項1に記載の縫合装置。
- 各クランプは、前記クランプから本体に近位に伸びる往復動可能なシャフトを有し、前記リンケージは、各クランプを遠位に移動させるように、前記作動サイクル中に本体に対してシャフトを往復動させ、かつその後前記リンケージは、前記針の近くで遠位に伸長したクランプを閉鎖することを特徴とする請求項1に記載の縫合装置。
- 前記リンケージは、引っ込めた位置と伸長した把持位置の間で前記シャフトの軸の周りで各クランプを回転させることを特徴とする請求項6に記載の縫合装置。
- 少なくとも1つの関連したばねは、クランプが針に隣接する前に各クランプの閉鎖を妨げることを特徴とすることを特徴とする請求項6に記載の縫合装置。
- 前記第1と前記第2クランプから針を解放するように前記リンケージに連結された解放入力を更に含むことを特徴とする請求項1に記載の縫合装置。
- 標準的開放又は腹膜鏡外科手術のために販売される既製の針をさらに含むことを特徴とする請求項1に記載の縫合装置。
- 前記第1と前記第2クランプの締め付け位置は、前記針の長さ方向の軸に沿って片寄せられ、かつ前記針の軸方向湾曲に適応するように角度的に片寄せられることを特徴とする請求項1に記載の縫合装置。
- 前記ハンドルは、ハンドル作動平面に沿って作動するように前記本体に連結され、
前記第1と前記第2クランプは、前記針の軸が前記ハンドル作動平面を横断するように前記針の支持に向けられることを特徴とする請求項1に記載の縫合装置。 - 前記縫合装置は、化学的に滅菌可能な材料からなることを特徴とする請求項1に記載の縫合装置。
- 前記本体は、近位端から前記第1および前記第2クランプに隣接した遠位端への軸に沿って伸長し、前記交替可能な駆動要素は、前記第1形状と前記第2形状との間で前記本体の軸線方向に沿って移動して、前記第1および前記第2クランプを交互に作動させ前記針から移動させることを特徴とする請求項1に記載の縫合装置。
- 前記駆動要素の動きに応答して、前記第1形状と前記第2形状の間で前記本体の軸線方向に沿って可動な交替可能なラッチを更に含み、前記第1形状の前記交替可能なラッチは、前記第1クランプの動きを妨げ、かつ前記第2形状の前記交替可能なラッチは、前記第2クランプの動きを妨げることを特徴とする請求項1に記載の縫合装置。
- 前記本体の伸長部が、前記クランプと前記本体の近位ハウジングの間に配置され、前記伸長部は、前記本体の軸線方向に沿って伸長し、前記本体は、前記クランプが前記伸長部により支持される間、特定の患者に望まれる軸方向屈曲を有するために、可塑的に屈曲可能であり、かつ屈曲した前記伸長部を通って伸びる駆動部材が、前記本体が前記軸方向屈曲を有する間、前記クランプが前記針を把持し解放できるように、前記近位ハウジングから屈曲可能な前記伸長部を通って前記第1と前記第2クランプのそれぞれを前記交替可能な駆動要素に連結することを特徴とする請求項1に記載の縫合装置。
- 前記第1と前記第2クランプのそれぞれは、関連したシャフトによって前記リンケージに接続され、前記シャフト内で移動可能なくさびを含み、
前記くさびは、前記クランプそれぞれの一対の作業ジョーの対応する面と係合可能なくさび面を有し、移動する前記くさびの遠位での動きが、それぞれのクランプを閉鎖している一対の作業ジョーの対応する表面を係合させることを特徴とする請求項1に記載の縫合装置。 - 前記第1と前記第2クランプは、交替可能なラッチによって交互にラッチされ、かつ駆動要素の動きに応答して、第1形状と第2形状の間で横方向に前記交替可能なラッチを移動させることを更に含み、前記第1形状の前記交替可能なラッチは、前記第1クランプの動きを妨げ、かつ前記第2形状の前記交替可能なラッチは、前記第2クランプの動きを妨げることを特徴とする請求項1に記載の縫合装置。
- 各クランプが、1人だけの患者の組織の縫合に使用できるように、前記第1と前記第2クランプを本体に解放可能に連結する切り離し可能な連結器、使い捨て可能な前記第1と前記第2クランプ、前記第1と前記第2クランプの代わりに本体に解放可能に連結する第3、第4の使い捨てクランプを更に含むことを特徴とする請求項1に記載の縫合装置。
- 前記リンケージのサイクルは、
前記リンケージの繰り返しのサイクルの間、前記本体に対する針の漸進的な移動が妨げられるように、前記クランプを前記把持形状の把持位置を僅かに越えて伸長させること、
前記針上で伸長したクランプを閉鎖すること、および
前記把持位置に前記クランプにより前記針を取り外すこと
によって、各クランプによる前記針の把持を行うことを特徴とする請求項1に記載の縫合装置。 - 前記クランプは、近位又は遠位縫合のために、前記クランプにおいて予め各クランプの軸線に対して近位に又は遠位に角度が付けられた前記針を把持することができ、かつ前記リンケージのサイクルは、前記クランプの軸線に対して近位に又は遠位に付けられた前記角度を維持することを特徴とする請求項1に記載の縫合装置。
- 前記リンケージの第1シャフトおよび第2シャフトは前記本体により支持され、
前記第1および第2クランプは第1クランプ・ユニットに含まれ、前記第1クランプ・ユニットはまた第1の本体伸長部、第1シャフト伸長部および第2シャフト伸長部を備え、前記第1本体伸長部が前記本体からリリースされるときに前記第1本体伸長部は前記第1および第2クランプと前記第1および第2シャフト伸長部とを支持する
ことを特徴とする請求項19に記載の縫合装置。
