Description
本発明は、概して、医療装置および方法に関する。より具体的には、本発明は、尿管結石および他の体の内腔内の閉塞物を減圧する方法および装置に関する。
特許文献1および特許文献2は、尿管内で固定するための一連の隣り合う拡大構造を含む尿管ステントについて記述している。薄い、引き伸ばされたポリテトラフルオロエチレンでできた反転スリーブを、カテーテルを体の内腔に導入するために使用することについては、特許文献3〜特許文献9に記載されている。砕石術の間、体の内腔を通して石の破片を前進させるワイヤのかごは、Stone Coneの商標名においてBoston Scientific社から入手可能である。特許文献10を参照されたい。その全体の開示は参照により本願に組み込まれる、2004年3月5日出願の同時係属出願第10/794,337号は、本願に記載のいくつかの方法を行うために使用されることのできるシース送達システムについて記述している。
次に図12を参照すると、本発明の装置のまた別の実施形態150が説明される。装置150は、ガイドワイヤ様の前進部材152、管状ガイド154、およびガイド管の遠位端に取り付けられている一定の長さの材料の遠位端160を含む。このようにすると、ガイド管(取り外し可能なハブ162を有する)は別個のガイドワイヤ152の上を前進することができる。一定の長さの材料156は次に、ガイド管154を近位に引くことによって固定質量に拡大されることができる。あるいは、固定質量は、リング158を近位に引くことによって真っ直ぐにされ、によって引き伸ばされ得る。
図12Aおよび図12Bは、ワイヤ172によって管174の中央内腔内の所定の位置に押し入れられる閉鎖式の折り畳み可能構造170を説明する。縫合糸または他のフィラメント、あるいは柔軟なコードであってもよい制御ロープ176は、管172の内腔を通って折り畳み可能構造170の遠位端に達し、このロープは構造を外側へ貫通して、折り返して管に取り付けられているところ、好ましくは説明されるように管の遠位端の近傍に戻る。所定の位置につくと、ワイヤ174は管172から取り外され、ロープ176は近位に(矢印178)引っ張られて構造を図12Bに示すように折り畳み、さもなければこの構造が厳密な軸圧縮において可能とする幅よりも広い幅の球状の質量を形成する。得られる形状は、「トイレブラシ」に類似したものであってもよい。折り畳み可能構造は、本発明のリボン構造用に上述されるあらゆる材料から形成され得る。
次に図12Cおよび図12Dを参照すると、本発明の装置のさらに別の実施形態が説明される。装置180は、外部コイル182および内部芯ワイヤ184を有するガイドワイヤ構造である。内部芯ワイヤ184は、その近位端にアクチュエータグリップ186を有し、通常ポリウレタンや他のやわらかいポリマーで形成される柔軟な先端188に、その遠位端に取り付けられている。略管状の膜構造190は、シャフト182の近位端192に、および柔軟な先端188の遠位端194に取り付けられている。ゆえに、グリップ186を近位に引くことによって、柔軟な先端188はシャフト182に向かって引かれ、ゆえに図12Dに示すように、管状の構造190を圧縮する。管状の構造190は種々に形成されることができ、連続的であっても非連続的であってもよい。管状の構造190は、編まれた、または他の小孔のある構造、あるいは複数の軸要素としてさらに形成されてもよい。全ての場合において、管状の構造190は、軸方向に圧縮されるにつれて半径方向に拡大される。説明される実施形態において、平坦なシートは、拡大された構造が、説明されるように一連の層状の長方形の部品のように見えるように、芯ワイヤの周囲に熱融着される。
Claims (3)
- 腎臓を減圧するデバイスであって、
膀胱にある尿管口から尿管内の尿管結石を通り越して前進可能なガイド部材と、
該ガイド部材の遠位端近傍に配置され、該石の直遠位に配備されるように適合された圧縮性アンカとを備え、拡大されたアンカは非閉塞性であって、該ガイド部材は該石を通り越して漏出経路を生成し、該ガイド部材はワイヤまたは管を備え、該アンカは一定の長さの材料を含み、該一定の長さの材料はストリップ、スリーブ、リボン、または管を含み、該材料はポリマー薄膜、織布、不織布、およびその合成物と積層から成る群から選択され、そして該ガイド部材は、遠位区分および分離型近位区分を含む、デバイス。 - 前記一定の長さの材料は、圧縮された際に、そこを通る尿の漏出を促進するために多孔性または有孔性である、請求項1に記載のデバイス。
- 前記遠位区分は、前記近位区分が分離された際に、アンカ構成を形成するために予成形される近位端を有する、請求項1に記載のデバイス。
