Description
本発明の別の側面は、2型糖尿病の治療のための医薬の製造におけるTCF7L2療法剤の使用に関する。一つの態様において、該TCF7L2療法剤は、Wntシグナル伝達経路の又はカドヘリン経路の活性を改変する剤である。別の態様において、該TCF7L2療法剤は、剤表(Agent Table)に示した群から選択される剤である。
本発明の好ましい態様の説明が以下に続く。
2型糖尿病に関連する座位
2型糖尿病は、インスリン分泌障害、末梢組織におけるインスリン耐性及び肝臓による増加したグルコース排出のような機構を介して生じることが可能である高血糖により特徴付けられる。ほとんどの2型糖尿病患者は、腎症、神経障害、網膜症及び心血管疾患の加速された発生を含む、慢性的高血糖の重篤な合併症に苦しんでいる。全世界の2型糖尿病の有病率は現在6%であるが、次の10年で上昇すると予想されている(1)。2型糖尿病の有病率におけるこの増加は、人口の高齢化及び肥満の増加によるものである。
本発明は、T細胞転写因子4(TCF4−公式遺伝子記号TCF7L2)をコードする遺伝子内の2型糖尿病関連LDブロック(「TCF7L2のエクソン4LDブロック」)の同定に関する。マイクロサテライトDG10S478及びSNPマーカーrs7903146及びrsl2255372を含むTCF7L2のエクソン4LDブロック内のいくつかのマーカーが2型糖尿病と関連していることが観察された。アイスランド人コホートで最初に観察された、DG10S478の関連の最初の観察結果(P=1.3x10−9;相対リスク=1.45;人口寄与リスク=22.7%)が、続いてデンマーク人2型糖尿病コホート及び米国白人コホートにおいて再現された。DG10S478は、10q25.2上のTCF7L2遺伝子のイントロン3中、及びイントロン3の一部、エクソン4のすべて及びイントロン4の一部を包み込む74.9kbの明確に定義されたLDブロック内に位置する。TCF7L2遺伝子産物は、APC3/β−カテニン/TCF経路としても知られているWntシグナル伝達経路において役割を果たしている高移動度群(HMG)ボックス含有転写因子である。TCF7L2は、Wnt経路メンバーβ−カテニン及びグリコーゲン合成酵素キナーゼ−3ベータを介してプログルカゴン遺伝子発現(血中グルコースホメオスターシスで重要な役割を果たしているもの)の細胞型特異的調節を仲介する(12)。加えて、Wntシグナル伝達は、脂肪生成転写因子CCAAT/エンハンサー結合タンパク質アルファ(C/EBPアルファ)及びペルオキシソーム増殖因子活性化レセプターガンマ(PPARガンマ)の阻害を介して未分化状態の前脂肪細胞を維持する(13)。前脂肪細胞のWntシグナル伝達が、優性阻害TCF7L2の過剰発現により妨げられた場合、これらの細胞は脂肪細胞に分化する(13)。加えて、Wnt/β−カテニンシグナル伝達経路は、結腸癌細胞でβ−カテニンとTCF7L2との物理的相互作用を介してPPARガンマ活性を標的とすることが報告されている(14)。多機能β−カテニンタンパク質は、そのカドヘリンの結合を介した細胞接着を仲介するためにも重要である(15)。
本明細書に記載した核酸、プローブ、プライマー及び抗体は、2型糖尿病への感受性の診断の多様な方法において、ならびにキット(例えば、2型糖尿病への感受性の診断について有用である)において使用し得る。同様に、該本明細書に記載した核酸、プローブ、プライマー及び抗体は、2型糖尿病への減少した感受性の診断の方法において、ならびに2型糖尿病に対する保護の診断の方法において、及びキットにおいて使用し得る。一つの側面において、該キットは、目的のマーカーを増幅するために使用し得るプライマーを含む。
もしくは、ペプチド核酸(PNA)プローブを、上記のハイブリダイゼーション法の核酸プローブの代わりに使用し得る。PNAは、N−(2−アミノエチル)グリシンユニットのような、ペプチド様無機主鎖を有し、メチレンカルボニルリンカーを介して該グリシン窒素に有機塩基(A、G、C、T又はU)が結合されているDNA模倣物である(例えば、Nielsen, P.E. et al, Bioconjugate Chemistry 5, American Chemical Society, p. 1 (1994) を参照されたい) 。PNAプローブは、TCF7L2核酸に特異的にハイブリダイズするように設計し得る。TCF7L2核酸へのPNAプローブのハイブリダイゼーションは、2型糖尿病への感受性、又は2型糖尿病への減少した感受性(又は2型糖尿病に対する保護的アレルを示す)の診断であり得る。
遺伝的多様性を示している個体の集団は同一のゲノムを有していない。むしろ、ゲノムはゲノム中の多くの位置で、個体間の配列可変性を示し、言い換えると、集団内に多くの多型部位がある。いくつかの例において、基準アレルを選ぶことなく多型部位で異なったアレルへの基準が作製される。もしくは、基準配列が特定の多型部位について参照される。基準アレルは時には「野生型」アレルとして参照され、それは通常第一の配列決定されたアレルとして、又は「非罹患」個体(例えば、疾患又は異常な表現型を示さない個体)からのアレルとして通常選ばれる。基準と異なるアレルは「変異体」アレルと称される。
患者及び対照群中のハプロタイプの頻度は期待値最大化(expectation-maximization)アルゴリズム(Dempster A. et al., J. R. Stat. Soc. B, 39:1-38 (1977)) を使用して見積もることが可能である。失われた遺伝子型及びフェーズでの不確かさを取り扱うことが可能なこのアルゴリズムの実行を使用し得る。帰無仮説下、患者及び対照は同一の頻度を有すると仮定した。尤度アプローチを使用し、対立仮説を試験し、そこでは、本明細書に記載したマーカーを含むことができる候補アットリスクハプロタイプは対照よりも患者でより高い頻度にし、一方、他のハプロタイプの頻度は両群で同一とした。両方の仮説下で尤度を別々に最大化し、対応する1−df尤度比統計を、統計的有意性を評価するために使用した。
ハプロタイプ解析
ハプロタイプ解析の一つの一般的アプローチはネステッドモデル(NEMO)(Gretarsdottir S., et al., Nat. Genet.35:131-38 (2003))に応用した尤度に基づいた推論を使用することを含んでいる。該方法は、多くの多型マーカー、SNPs及びマイクロサテライトを許容するプログラムNEMOで実行される。方法及びソフトウェアーは、患者対照研究について具体的に設計され、異なったリスクを与えるハプロタイプ群を同定することが目的である。それはLD構造を研究するためのツールでもある。NEMOにおいて、最大尤度見積もり、尤度比及びP値は、観察されたデータについてそれを失われたデータ問題として取り扱う、EMアルゴリズムの助けを借りて、直接的に計算される。
フェーズの不確かさ及び失われた遺伝子型による情報損失を取り込んだ観察データを直接計算される尤度に基づいた尤度比試験は信頼できて、有効なP値を与えるが、不完全である情報のため、どのくらいの情報が失われていたかを知ることはそれでも興味あることであろう。ハプロタイプ解析についての情報量は、連鎖解析について定義された情報量の自然な流れとしてNicolae and Kong (Technical Report 537, Department of Statistics, University of Statistics, University of Chicago ; Biometrics, 60(2):368-75 (2004)) により記載されており、NEMOで実行された。
疾患への単一マーカー関連について、フィッシャーの直接確率検定を使用することができ、各個体アレルの両側P値を計算した。すべてのP値は特別に示されていない限り、複数比較のために調整されずに示されている。示された頻度(マイクロサテライト、SNPs及びハプロタイプについて)は、キャリアー頻度に対立するものとしてのアレル頻度である。連鎖解析のための家族として動員された患者の同系性による偏向を最小化するため、一等親及び二等親血縁者類は患者リストから除去し得る。さらに、Risch, N. & Teng, J.(Genome Res., 8: 1273-1288 (1998)) に記載されている分散調節法を拡大することにより、一般的家族関連性に適用できるように、及び比較のため調整及び未調整P値の両方に提示できるように同胞群についてDNAをプール化することにより(前記文献)、患者間に残った同系性を補正して関連について試験を繰り返し得る。相違は、予測されたごとく一般的に非常に小さかった。多重試験のために補正された単一マーカー関連の有意性を評価するため、同一の遺伝子型データを使用してランダム化試験を実施し得る。患者及び対照のコホートをランダム化でき、関連解析を複数回(例えば、500,000回まで)やり直し、及びP値は、本来の患者及び対照のコホートを使用して観察されたP値より低いか又は等しい、いくつかのマーカーアレルについてのP値を生み出す反復の一部である。
マーカーの対間のLDは、D’及びR2の標準定義を使用して計算し得る(Lewontin, R., Genetics 49:49-67(1964); Hill, W.G. & Robertson, A. Theor. Appl Genet. 22:226-231 (1968)) 。NEMOを使用し、二つのマーカーアレルの組み合わせを、最大尤度により見積もり、及び連鎖平衡からの逸脱を尤度比試験により評価した。D’及びR2の定義を、辺縁アレル確率により加重した二つのマーカーのすべての可能なアレル組み合わせについての値を平均することによりマイクロサテライトを含むように拡張した。特定の領域中のLD構造を評価するためにすべてのマーカーの組み合わせをプロッティングする場合、D’を左上の隅に及びP値を右下隅にプロットした。LDプロットにおいて、もし望むなら、マーカーはそれらの物理的位置に従うよりもむしろ等間隔にプロットし得る。
複数点、患者のみ、アレル共有法を連鎖についての証拠を評価するための解析に使用し得る。LODスコア及びノンパラメトリック連鎖(NPL)スコアの両方の結果は、プログラムAllegro(Gudbjartsson et al., Nat. Genet. 25:12-3 (2000)) を使用して得ることが可能である。われわれのベースライン連鎖解析はSpairsスコアリング関数(Whittemore, A.S., Halpern、 J. Biometrics 50:118-27(1994); Kruglyak L. et al., Am. J. Hum. 2型糖尿病を発生するリスクを与える遺伝子変異体についての知識は、疾患を発生する増加したリスクを有する個体(即ち、アットリスク変異体のキャリアー)及び疾患を発生する減少したリスクを有する個体(即ち、保護的変異体のキャリアー)間を区別するための遺伝子検査を適用する機会を与える。上記の群の両方に属する個体についての遺伝子検査の中核的意義は、早期段階で疾患を診断することができる可能性であり、最も適切な治療を適用することが可能であるように疾患の予後/攻撃性について、臨床医に情報を提供することである。例えば、2型糖尿病についての遺伝子検査の適用は、早期段階での疾患の検出のための機会を提供することが可能であり、それは早期段階での治療手段の適用を導くことができ、そしてそれ故、2型糖尿病による症状の有害効果及び重大な健康上の影響を最小にし得る。

本発明はさらにTCF7L2療法剤への応答する個体の可能性を評価する方法に関する。該方法においては、2型糖尿病への感受性について個体を評価することに関して上に記載したように、TCF7L2遺伝子に関係するマーカー又はハプロタイプを評価する。2型糖尿病についての感受性(増加したリスク)に関連するアレル、マーカー、SNP又はハプロタイプ(例えば、マーカーDG10S478中の非0アレル;SNP rs12255372中のTアレル;SNP rs7895340中のAアレル;SNP rs11196205中のCアレル;SNP rs7901695中のCアレル;SNP rs7903146中のTアレル;SNP rs12243326中のCアレル;SNP rs4506565中のTアレル;TCF7L2のエクソン4LDブロックに関連するアットリスクハプロタイプのようなTCF7L2のエクソン4LDブロックに関連するマーカー)の存在は、TCF7L2療法剤への陽性応答の可能性を示す。「陽性応答の可能性」は、個体が、本明細書に記載した2型糖尿病についての感受性(増加したリスク)に関連するアレル、マーカー、SNP又はハプロタイプを有していない個体よりも、TCF7L2療法剤に対する陽性応答を有する可能性が高いことを示す。TCF7L2療法剤への「陽性応答」は、2型糖尿病の治療を示す、生理学的応答である。上記のように、「治療」は、2型糖尿病に関連する症状を寛解させるだけでなく、2型糖尿病の発症を防止する又は遅延させる;2型糖尿病の症状の重度又は頻度を軽減する;及び/又は2型糖尿病に関連する症状を寛解する他の薬剤での併用療法の必要性も軽減することを指す。
本発明は、TCF7L2活性を改変するか、又はさもなければWntシグナル伝達経路又はカドヘリン経路に影響するか、又はTCF7L2療法剤として使用し得る剤を含む医薬組成物にも関する。該医薬組成物は、薬学的に許容できる担体又は賦形剤と配合することができて、医薬組成物が製造される。担体及び組成物は滅菌し得る。製剤は投与の様式に適していなければならない。
本発明は、TCF7L2の活性を改変する(例えば、増加又させる又は減少させる)、さもなければWntシグナル伝達経路又はカドヘリン経路(例えば、βカテニン)のTCF7L2又は別のメンバーと相互作用する剤(例えば、融合タンパク質、ポリペプチド、ペプチド模倣体、プロドラッグ、レセプター、結合剤、抗体、小分子又は他の薬剤又はリボザイム)を同定するための方法も提供する。例えば、特定の態様において、こうした剤は、TCF7L2に結合する;例えば、TCF7L2の活性に刺激的又は抑制的効果を有する;又はWntシグナル伝達経路の他のメンバーと又はカドヘリン経路のメンバーと相互作用するTCF7L2の能力を変化させる(例えば、増強する又は抑制する)、又はTCF7L2の翻訳後プロセッシングを改変する剤であり得る。他の態様において、こうした剤は、Wntシグナル伝達経路又はカドヘリン経路の活性又は機能を改変する剤であり得る。
TCF7L2核酸、部分及び変異体
本発明はヒトTCF7L2を含む、単離された核酸分子にも関する。本発明のTCF7L2核酸分子は、RNA(例えば、mRNA)又はDNA(例えば、cDNA及びゲノムDNA)であり得る。DNA分子は、二本鎖又は一本鎖であり得;一本鎖RNA又はDNAは、コード、又はセンス鎖、又は非コード、又はアンチセンス鎖であり得る。該核酸分子は、遺伝子のコード配列のすべて又は一部を含むことが可能であり、及びさらにイントロン及び非コード3’及び5’配列(例えば、調節配列を含んで)のような追加の非コード配列を含むことが可能である。
関連する側面において、本発明の核酸断片は、本明細書に記載したアッセイにおいてプローブ又はプライマーとして使用される。「プローブ」又は「プライマー」は、核酸分子の相補鎖に塩基特異的様式でハイブリダイズするオリゴヌクレオチドである。こうしたプローブ及びプライマーには、Nielsen, P. et al, Science 254:1497-1500(1991) に記載されているポリペプチド核酸(PNA)が含まれる。

遺伝子産物の一つの形態に特異的に結合するが、遺伝子産物の他の形態には結合しないポリクローナル抗体及び/又はモノクローナル抗体も提供される。変異体又は多型部位(単数又は複数)を含有する基準遺伝子産物の一部を結合する抗体も提供される。本明細書で使用する用語「抗体」は、免疫グロブリン分子及び免疫グロブリン分子の免疫学的に活性な部分、即ち、抗原を特異的に結合する抗原結合部位を含有する分子を指す。本発明のポリペプチドに特異的に結合する分子は、そのポリペプチド又はそれらの断片に結合するが、サンプル、例えば、天然に該ポリペプチドを含有する生物学的サンプル中の他の分子には実質的に結合しない分子である。免疫グロブリン分子の免疫学的に活性な部分の例には、該抗体をペプシンのような酵素で処理することにより発生し得る、F(ab)及びF(ab’)2断片が含まれる。本発明は、本発明のポリペプチドに結合するポリクローナル抗体及びモノクローナル抗体を提供する。本明細書で使用する用語「モノクローナル抗体」又は「モノクローナル抗体組成物」は、本発明のポリペプチドの特定のエピトープと免疫反応することが可能である抗原結合部位のただ一つの種を含有する、抗体分子の集団を指す。モノクローナル抗体組成物は、それ故典型的には、それが免疫反応する本発明の特定のポリペプチドに対する単一の結合親和性を示す。
アイスランドコホート
アイスランドのData Protection Authority及びアイスランドのNational Bioethics Committeeは本研究を承認した。本研究の全ての関与者にはインフォームドコンセントを与えた。血液サンプル、医療情報及び系統学に関連するすべての個人情報は、サードパーティー暗号化システムを使用して、Data Protection Authorityにより最初に暗号化された(18)。
デンマーク研究グループはデンマークにおけるPERF(予測疫学的リスク因子)研究から選択された(19)。228人の女性が2型糖尿病を有すると診断されており、及び/又は>=7mMグルコースが測定された。対照として、539人の非罹患(2型糖尿病に関して)女性が同一研究コホートから無作為に選ばれた。
新規配列反復(即ち、ジヌクレオチド、トリヌクレオチド及びテトラヌクレオチド反復)は、タンデムリピートファインダーソフトウェア(20)を使用して同定され、及び94人の対照中の多型性について試験した。CEPHサンプル1347−02(CEPHゲノミクスレポジトリー)の下位アレルの塩基対のサイズをマイクロサテライトアンプリコンのサイズから差し引き、基準として使用した。SNP遺伝子型同定は、直接DNA配列決定(Applied BioSystems)又はCentaurus プラットフォーム(Nanogen) を使用して実施した。
2型糖尿病への単一マーカー関連について、各アレルの両側p値を計算するために尤度比検定を使用した。用いたマイクロサテライトについてのキャリア頻度よりむしろアレル頻度を示した。
全座位関連研究
我々は以前に、アイスランド人集団における、2型糖尿病についての染色体5qへの全ゲノム有意連鎖を報告した(10);我々は、10q及び12qへの連鎖の示唆的証拠も報告した。10q座位を追跡するため、この座位に対応する10.5Mb領域(NCBI Build 34: ChrlO:l14.2 - 124.7 Mb) にわたり、遺伝子型同定マイクロサテライトマーカーの高密度を用いる関連アプローチを使用した。228のマイクロサテライトマーカーを同定し、分類した――即ち、46kb毎に一つのマーカーの平均密度(表1)。全マーカーを1185人のアイスランド人2型糖尿病患者及び931人の非相関集団対照で分類した。
2型糖尿病へのDG10S47の関連を確認するため、228の症例及び539の対照のデンマーク2型糖尿病コホートにおいてマイクロサテライトを遺伝子型同定した。デンマーク人コホートはデンマークにおけるPERF(予測疫学的リスク因子)研究から選択された(19)。この女性2型糖尿病コホートは、以前に2型糖尿病と診断されている。アイスランド人で観察された関連が追試された(表3)。
続いて、マイクロサテライトを、PENN CATH研究からの361の症例及び530の対照の米国白人2型糖尿病コホートで遺伝子型同定した。この研究は、University of Pennsylvania Medical Centerで心臓カテーテル法を受けた患者の連続コホートにおける、冠動脈硬化と生化学的及び遺伝的因子の関連の横断研究である。2型糖尿病は、空腹時血糖>126mg/dl、食後2時間グルコース>200mg/dl、年齢40歳より高齢な対象での経口血糖降下薬又はインシュリン及び経口血糖降下薬の使用の履歴として定義された。アイスランド人で観察された関連がこの集団でも追試された(表4)。
表6に、TCF7L2のエクソン4LDブロック内に位置するマイクロサテライト及びSNPマーカーを収載した。該表は公的に利用可能なSNPs、ならびに全LDブロック領域の配列決定により発見されたSNPsを含んでいる。該表はさらに、ブロック内に位置する多型マイクロサテライトマーカーを提供する。
この研究において、我々は、10q25.2上の転写因子7−様2(TCF7L2−以前はTCF4)をコードする、以前に報告した10q連鎖領域(10)内の2型糖尿病についての新規候補遺伝子の同定を記述する。それはアイスランド、デンマーク及び米国において、類似の頻度及び相対リスクで、2型糖尿病のリスクを与えることが示された。該変異体は、2型糖尿病の家族性クラスター化の実質的分画は説明しないが、少なくとも20%の集団寄与リスクは、公衆衛生の観点から重要である。非キャリアと比較して、アットリスク複合アレルのヘテロ接合性キャリア(集団のおよそ38%)及びホモ接合性キャリア(集団のおよそ7%)の相対リスクは、それぞれ1.45及び2.41である。それ故、この変異体は、臨床使用するために十分な的中率値を有する。
