Description
前記初期肢閉塞圧を自動的に測定するために、制御器48は、止血カフ4を徐々に加圧し、血流変換器インターフェース74により伝えられるために血流変換器26により発せられた信号を解析する。制御器48は、止血カフ4の位置に対して末梢側の位置への動脈血流を阻止するために止血カフ4に必要な最低圧力の値を決定する。この最低圧力が前記初期肢閉塞圧である。前記初期肢閉塞圧は、肢閉塞圧が測定された時における患者2の生理状態を反映する。前記初期肢閉塞圧(ILOP)の値が決定された後、制御器48は適応肢閉塞圧(ALOP)の値を計算し、自動的に更新する。肢閉塞圧を直接に測定するために止血カフ4に対して末梢側の位置において血流が観察されなければならない。前記適応肢閉塞圧の値は、手術中に血流が止血カフ4を通過するのを阻止するために止血カフ4が加圧された時のような、肢閉塞圧の直接的な測定が可能でない又は安全でない時における前記肢閉塞圧の推定値である。様々な要素が肢閉塞圧に影響を与える。止血カフの種類及び肢に取り付けられた止血カフの位置のようないくつかの要素は手術中に比較的一定である。動脈圧のような他の要素は手術中に大きく変化する。止血器具8により計算された前記適応肢閉塞圧の値は、直接的な肢閉塞圧測定の後に生じる、肢閉塞圧に対する血圧及び他の生理的特徴の変化の影響を反映する。前記初期肢閉塞圧の値が制御器48により決定された後、制御器48は、肢閉塞圧測定時又はこれに近い時に測定された血圧値及び他の生理的特徴の値を生理的特徴取得モジュール80に要求する。肢閉塞圧測定時又はこれに近い時における患者の血圧は肢閉塞圧の基準血圧(BPLOPREF)と定義される。制御器48は、前記適応肢閉塞圧の値を計算するために、前記基準血圧の値を前記初期肢閉塞圧の値及び該初期肢閉塞圧の決定後に測定された血圧値とともに使用する。制御器48は、前記適応肢閉塞圧の値を計算するときに収縮期血圧又は平均血圧のいずれかを使用する。制御器48は、前記適応肢閉塞圧の現在の値を決定するときに心拍数のような他の生理的特徴を使用してもよい。前記適応肢閉塞圧の値は、前記初期肢閉塞圧の値と、現在の血圧値(BP)と前記基準血圧の値との差との合計に等しい(ALOP = ILOP + ( BP - BPLOPREF))。

Claims (6)
- 手術用の止血装置であって、
患者の肢を取り巻く膨張可能な止血カフと、
前記止血カフを加圧する加圧手段と、
前記止血カフを減圧する減圧手段と、
カフ圧を検出するカフ圧検出手段と、
前記患者の初期肢閉塞圧を検出する肢閉塞圧検出手段と、
前記患者の生理的特徴を検出する生理的特徴検出手段と、
前記初期肢閉塞圧及び前記生理的特徴の予め決められた関数である適応肢閉塞圧を設定する圧力調整手段とを含み、
前記生理的特徴は、前記患者の手術されない肢の血圧であり、
前記圧力調整手段は、前記初期肢閉塞圧が検出された後、手術中に前記カフ圧を前記適応肢閉塞圧の値より高い値に維持するように前記加圧手段及び前記減圧手段を作動させる、止血装置。 - 前記肢閉塞圧検出手段は、前記止血カフに対して末梢側の位置における前記肢の内部の血流の動脈拍動を測定すること、前記カフ圧を、零に近いレベルから、前記動脈拍動が検出されない最低レベルへ増加させること、該最低レベルを示す前記初期肢閉塞圧を発生させること及び該初期肢閉塞圧の発生後に前記カフ圧を減少させることにより、前記初期肢閉塞圧を検出する、請求項1に記載の止血装置。
- 前記予め決められた関数は、前記初期肢閉塞圧の値と、前記初期肢閉塞圧の検出時に前記生理的特徴検出手段により検出された第1の血圧値と前記初期肢閉塞圧の検出後に前記生理的特徴検出手段により検出された第2の血圧値との差との合計と定義されている、請求項1に記載の止血装置。
- 前記圧力調整手段は、前記カフ圧を、前記適応肢閉塞圧に対する予め決められた関係を有する止血基準圧力レベルに近いレベルに維持する、請求項1に記載の止血装置。
- 前記圧力調整手段は、前記カフ圧を、予め決められた第1の圧力レベルと前記適応肢閉塞圧に比例する第2の圧力レベルとの合計に等しい止血基準圧力レベルに近いレベルに維持する、請求項1に記載の止血装置。
- 前記生理的特徴検出手段は前記患者の、肢の温度を含む第2の生理的特徴を検出し、前記圧力調整手段は前記適応肢閉塞圧を前記初期肢閉塞圧、前記血圧、及び前記第2の生理的特徴の予め決められた関数と設定する、請求項1に記載の止血装置。
