Description
[0002]従来の脊椎ケージ又は移植片の特徴とするところは、図1に示す隣接する椎骨セグメント又はエンドプレートに接触するように外側表面に設けた骨無機質被覆表面を備えた腎臓形豆形状の本体である。従来の脊椎ケージは典型的には、試験的移植片が経路を発生させた後に、伸延された脊椎の神経椎間孔を通して後方から並んで挿入される。
骨と医用生体材料との間の貧弱な境界面
[0004]従来の椎体間安定装置における別の問題は骨と医用生体材料との間の貧弱な境界面を含む。従来の静的椎体間スペーサは骨と医用生体材料との間に脆弱な境界面を形成する。このような移植片の表面は典型的には一連の隆起部を具備するか又は骨無機質で被覆されるが、隆起部は適用される水平ベクトル又は並列運動に対して平行になることがある。すなわち、隆起部又は被覆物はエンドプレートのいずれかの側104に適用される運動に対して抵抗を殆ど与えない。したがって、移植片と被移植骨との間の運動のため、アログラフト(同種移植)、チタン及びポリマースペーサにおいて非結合が普通である。従来の装置は典型的には隣接する椎骨間で膨張しない。
従来の大型静的装置は神経構造の大幅の露出を必要とする
[0006]従来の椎体間スペーサの別の問題は大幅の露出を必要とするその大きな直径である。椎体間スペーサに使用する既存の装置は構造的なアログラフトのネジ付きケージ、円筒状ケージ及びブーメラン形状のケージを含む。従来の装置は安全性及び効能に関して重大な制限を有する。椎体間スペーサの安全性に関しては、前方又は後方アプローチからの配置において神経素子の損傷を生じることがある。従来の脊椎ケージは膨張能力が欠如しており、その固定能力を減少させる。
椎体間骨形成のための限られた能力
[0011]既存の椎体間移植片は骨グラフトのために利用できる空間が限られる。十分な骨グラフト又は骨グラフト代用品は固体椎体間関節固定にとって重要である。椎骨間空間を満たすためにケージ内及びそのまわりでの大量の骨グラフト材料の配置を許容する膨張可能な椎体間ケージを提供することが望ましい。その上、従来の椎体間移植片はエンドプレートを完全に安定化させ、エンドプレートが移動するのを阻止する能力に欠けている。それ故、必要なものは、椎骨のエンドプレートを離間させるように伸延し、移動しないようにエンドプレートを保持する力をエンドプレートに受けされるような膨張脊椎ケージである。このような椎体間ケージは運動セグメントを安定させることができ、それによって、微細運動を減少させ、偽関節症(不完全癒合)及び痛みをやめさせる。
マスターシリンダは純アクリル材料で作った使い捨て品である。
挿入工具のハンドルは使い捨てで、ポリマーが硬質化したときに容易に分解できる。
[0032]楔プレートは右方又は左方での0ないし20度の前方/後方脊椎修正を提供する。随意のベースプレート又は前彎プレートはまた前方から後方への前彎角度を修正するためにほぼ0−20度(前方/後方)だけ角度づけられる。
本発明の態様によれば、マスターシリンダから奴隷シリンダ又は注射器へ機械的なてこ作用を伝える液圧流体は有利にはメチルメタクリレートのような時間制御される硬化可能なポリマーである。粘度及び硬化時間は周知のように適当な添加される触媒の組成により調整することができる。このような触媒は米国コネチカット州ロッキーヒル06067(Rocky Hill Connecticut 06067)、1001トラウト・ブルック・クロッシング(1001 Trout Brook Crossing) のロクタイト社(LOCTITE Corp.) から入手できる。ポリマーが硬化したとき、ポリマーは硬質化し、ピストン従って医師により決定された所望の量の脊椎修正を適所で不動に固定する。
三次元における最小侵入液圧制御操作
[0088]図6を参照すると、SECの代わりの実施の形態は複数の奴隷シリンダを有し、対応するピストン110a、110b、110nはSEC本体106内に設けられる。複数の奴隷シリンダ及びピストン110a、110b、110nの各々は、三次元(X、Y、Z軸)における複数の高さでの奴隷シリンダの独立に制御される膨張のために、マスターシリンダ内の複数のシリンダの対応する1つと独立に連通する別個の関連する液圧ライン122a、122b、122nを具備する。
代わりのマスターシリンダシステム
[0098]図8を参照すると、マスターシリンダの代わりの実施の形態はSEC移植片内の各奴隷ピストンの個々の液圧制御のために設けられる。マスターシリンダ154は2又はそれ以上のシリンダ156a、156b及び関連するピストン157a、157bを具備する。医師により制御されるレバー158は各ピストンに取り付けられる。液圧流体はライン148a、148bを通して挿入されたSEC移植片内へ送られる。レバーはSEC内の奴隷シリンダの内部の圧力それ故椎骨エンドプレートに対する圧力の1ポンドないし10ポンドの比率を生じさせる。機械的には、これは医師のために持ち上げ力において10:1の利点を提供する。医師が必要とする力の適用は三次元における任意の所望の脊椎椎骨修正効果を生じさせるために上述したようなエンドプレートに対するSECの制御された膨張を生じさせるようにレバー及び液圧システムを介して倍化される。
光硬化性ポリマー
[0109]当業者ならよく理解できるように、液圧力を運ぶための第1の流動可能な状態を有し、硬化時に第2の固形状態に移る任意の等価の硬化可能なポリマーを使用することができる。第1の状態においては、硬化可能なポリマーは、修正作用が奴隷シリンダを持ち上げることにより達成されるように、マスターシリンダから奴隷シリンダへ力の適用を液圧的に伝達する。硬化可能なポリマーは硬化時に第2の固形状態へ移行し、そのため、奴隷ピストンの修正高さは適所で固定される。このような等価の硬化可能なポリマーは可視光又は紫外光又は固形状態に移行するようにポリマーを活性化する他の放射線源の適用により硬化されるポリマーである。粉末と組み合わされたときの別のメチルメタクリレート液体ポリマーは、粉末と液体とが混ぜ合わされた直後に粘性流体となり;この流体は最初薄くて自由に流れる。徐々に、数分で、この流体は濃くなり始め、ピストンの内部で糊状及びプリン状を経てセメント様の固体へと状態を変換させ、従って、膨張位置において精確な修正量でSECを固定する。
液圧制御ライン
[0113]図10A、10Bを参照すると、液圧ライン200はマスターシリンダからのSECの複数の奴隷シリンダの遠隔液圧制御のために設けられる。液圧ライン200は中央軸線のまわりに位置する複数の個々の液圧ライン202を有する。各液圧ライン202は上述のようにマスターシリンダからの別個の奴隷シリンダの独立の作動を提供する。骨グラフト供給ライン204はライン200の中央軸線に沿って設けられる。個々の液圧ライン202は、各奴隷シリンダに対する独立の液圧制御を提供するために、SECの挿入前に整合させることができ、対応する奴隷シリンダの入力ポートに接続することができる。

Claims (9)
- 椎骨本体間で支持を提供する装置において、
a. 第1の圧力適用骨係合表面を有するベースと;
b. 各々が複数の相互嵌合段階を有する、上記ベースと共働する複数の膨張可能部材であって、該複数の膨張可能部材のうちの少なくとも1つが少なくとも3つの相互嵌合段階を有する前記複数の膨張可能部材と;
c.上記膨張可能部材に結合され、第2の圧力適用修正骨係合表面を有するエンドプレートと;
を有することを特徴とする装置。 - 上記膨張可能部材が液圧的に膨張するようになっていることを特徴とする請求項1に記載の装置。
- 上記ベースと共働する少なくとも3つの相互嵌合段階を有する複数の膨張可能部材を含むことを特徴とする請求項1又は2に記載の装置。
- 上記複数の膨張可能部材が独立に膨張するようになっていることを特徴とする請求項1〜3のうちのいずれか一項に記載の装置。
- 上記複数の相互嵌合段階が入れ子式に相互嵌合することを特徴とする請求項1〜4のうちのいずれか一項に記載の装置。
- 患者の脊椎の再構築のために患者の対向する上位及び下位の椎骨エンドプレート間に挿入するように形状づけられることを特徴とする請求項1〜5のうちのいずれか一項に記載の装置。
- 上記3つの相互嵌合段階の各々の高さが15ミリメートルである、ことを特徴とする請求項1〜6のうちのいずれか一項に記載の装置。
- 総合膨張高さが70ミリメートルであり、閉じた高さが25ミリメートルである、ことを特徴とする請求項1〜7のうちのいずれか一項に記載の装置。
- 幅が55ミリメートルであり、深さが45ミリメートルである、ことを特徴とする請求項1〜8のうちのいずれか一項に記載の装置。
