Description
(a)ステントフィラメントの外面領域を、少なくとも約20μインチ(0.5μm)の表面粗さ及び約300〜700μインチ(7.5〜17.5μm)の表面粗さ範囲(山−谷間の最大値)まで粗面化するステップと、
(b)ステントフィラメントの粗面化された領域を、粗面化されたステント表面の表面粗さ範囲よりも大きなコーティング厚さ、すなわち粗面化された表面を覆い隠す厚さのリムス薬物のポリマーフリーコーティングでコーティングするステップと
を含む。
特に指示しない限り、下記の用語は本明細書において以下の意味を有する。
図1は、本発明に従って構築された、収縮状態にあるステントを示す。このステントは、後に詳述するように、少なくとも1つの表面が、少なくとも抗再狭窄化合物を保持し放出するために少なくとも部分的に粗面化され又は研磨された構造部材又はボディ(body)を含む。
一実施形態では、この方法が、マスクを使用して、ステントの少なくとも一部分が研磨されないようにすることを含む。このマスクは、PARAFILM(登録商標)などの炭化水素フィルムであることが好ましいが、これらの方法において使用するものとしては、研磨を妨げる適当な任意の障害物が適当であることが理解される。これに応じて、好ましい一実施形態では、少なくともステントの管孔側表面(luminal surface)が研磨されない。一実施形態では、約5mm×60mmの1枚のマスクが、1.4mmのガラス毛細管などのマンドレル(mandrel)の直径の周囲に巻き付けられる。このマンドレル上にステントが配置され、炭化水素マスクの中へ手で押し縮められる。研磨しない部分のステントがマスクによって覆われることを保証するため、10倍から40倍の立体顕微鏡を使用してもよい。好ましい一実施形態では、全ての表面のステント壁の厚さの少なくとも80%が炭化水素フィルム層によってマスクされる。
この項では、本発明に基づく血管治療法及び本発明に従って構築されたステントの性能特性を説明する。
ステントからのBiolimus A9(登録商標)のin vitro薬物放出
抗増殖薬Biolimus A9(登録商標)を含むポリマーでコーティングされたBiomatrix(登録商標)IIステントと、Biolimus A9(登録商標)を含んだ反管孔側微小構造を含むステントとを用い、37℃のPBS pH7.4/Tween媒質中でin vitro薬物放出を実施した。試料採取は定期的に実施し、Biolimus A9(登録商標)の総量はHPLCによって測定した。図5は、Biomatrix(登録商標)IIステント及び微小構造ステントからの薬物放出を示す。
粗さファクタ卓上試験
Bioflex II 6クラウン(crown)ステントの外面を研磨材で処理して、Bio−Freedom Stent(FS)と呼ばれる、薬物装填容量のために選択的に微小構造が形成されたこのステントの外面を生成した。治療薬は、選択的に微小構造が形成されたこのステントの表面に直接にコーティングすることができる。
動物埋込み試験
Biolimus A9(登録商標)を含むテクスチャが付与された実施例2のステント及びBiolimus A9(登録商標)を含まないテクスチャが付与された実施例2のステントを、異系交配の若齢のブタに埋め込んだ。バルーンカテーテルを使用して、10〜20%の過伸展を有する標準ブタ過伸展モデルに従いステントを配置した。ステントを配置する前に、知られている血管形成術によって若齢のブタの標的血管を予め拡張させた。
3か月ブタ埋込み実験
A.ステントの埋込み
実施例2と同様にサンドブラスト処理されたBiolimus A9(登録商標)を225μg含むポリマーフリーBioMatrixステント又は裸のBioFlex IIステントを、表3に基づく交雑家畜ブタモデルに埋め込んだ。

*ステンティングに適さないサイズのためLCXにはステントを埋め込まなかった。
光学顕微鏡法に関しては、ステントが埋め込まれた血管分節をメチルメタクリル酸プラスチックに包埋し、近位、中間及び遠位ステントから切片標本を切り出し、チャージドスライド(charged slide)に載せ、ヘマトキシリン−エオシン及びエラスティックワンギーソン(Elastic Van Gieson:EVG)で染色した。ステントが埋め込まれていない動脈の近位及び遠位部分をパラフィンに包埋し、4ないし5ミクロンの切片とし、ヘマトキシリン−エオジン及びEVGで染色した。全ての切片標本について、炎症、血栓、新内膜形成及び血管壁損傷の有無を光学顕微鏡法によって調べた。形態計測分析。線分長2.0mm、円直径2.0 mmのNISTトレース可能顕微鏡対物ミクロメータを使用して、全ての対物レンズについて、形態計測ソフトウェア(IP Lab for Macintosh、Scanalytics社、米メリーランド州Rockville)を較正した。Klarmann Rulings,Inc.社(米ニューハンプシャー州Manchester)が全てのミクロメータの目盛りを認証した。測定領域はEEL(外弾性板)、IEL(内弾性板)及び管腔を含む。新内膜の厚さは、ステントストラットの位置及びステントストラット間の位置で測定し、ブタごとに平均した。EELからIELを差し引くことによって中膜面積を求めた。狭窄パーセントは、式[1−(管腔面積/ステント面積)]×100によって導き出した。血管損傷スコアは、Schwartzの方法(Schwartz RS他、J Am Coll Cardiol 1992;19:267−274)を使用して決定した。0(=炎症/フィブリン/損傷なし)から値3(=著しい炎症/フィブリン/損傷)までの等級付けされた分析に基づいて、切片標本ごとに、炎症、フィブリン及び損傷スコアを生成した。2つ以上の肉芽腫反応が存在する切片標本には炎症スコア4を与えた。内皮の被覆率は半定量化し、管腔周長の百分率として表現した。
組織形態計測連続データは平均±SDとして表現した。スチューデントのt検定を使用して、正規分布させたパラメータの統計解析を実施した。非正規分布パラメータ又は離散値の解析にはウィルコクソン検定を使用した。分布の正規性は、ウィルク−シャピロ(Wilk−Shapiro)検定で検定した。<0.05のp値を統計学的に有意とみなした。
ステントは全て、広く均一に拡張しているように見えた。破損又は屈曲の証拠は見られなかった。

埋込み後3か月しても、ステントは全て広く拡張し、開いており、血栓の証拠は見られなかった。新内膜形成は軽度で、平均新内膜厚さは0.16mmであり、ゆるく充填された平滑筋細胞及びプロテオグリカンに富んだ基質によって構成されていた。血管損傷は軽度であった。ストラットの周囲に限局性の軽度のフィブリン沈着が見られた。ブタ5のLCXに肉芽腫反応が見られたが、残りの全ての血管で炎症は最小限だった。時折、巨細胞が観察され、それらについては記録した。内皮化は完了しており、管孔側の炎症性細胞及び/又は小板の癒着は見られなかった。特に、裸金属ステントを含むブタ2のLCXの近位切片標本の新内膜に密実な石灰化が見られた。
埋込み後3か月しても、ステントは全て広く拡張し、開いており、血栓の証拠は見られなかった。新内膜形成は軽度で、平均新内膜厚さは0.21mmであり、ぴったりと充填された平滑筋細胞を含んでいた。ブタ2の左冠動脈前下行枝(LAD)に中膜裂傷が見られた。おそらくは埋込み手技によって形成された損傷のため、この血管は主にストラットの周囲に重症の炎症を示した。しかしながら、このブタを除けば、血管損傷及び炎症は全体的に軽度であった。フィブリン沈着及びマルアポジション(malapposition)はどのステントにも見られなかった。内皮化は完了しており、管孔側の炎症性細胞及び/又は小板の癒着は存在しなかった。
Claims (17)
- 相互接続された金属フィラメントから形成された裸金属拡張性ステントを対象者の血管損傷部位に配置し且つリムス薬物を含むポリマー担体のステントフィラメントの外面上にコーティングを有することにより観察される場合と比べて、該対象者の血管損傷に起因した再狭窄又は血栓症の発生率及び/又は程度を低減させるステントを製造する方法であって、該方法は、
ステントフィラメントの外面領域を、少なくとも約0.5μmの表面粗さ(Ra)及び約7.5〜17.5μmの表面粗さ範囲(Rt)まで粗面化すること、並びに
ステントフィラメントの粗面化された領域を、粗面化されたステント表面の表面粗さ範囲よりも大きなコーティング厚さまでリムス薬物のポリマーフリーコーティングでコーティングすること
を含む、上記方法。 - 相互接続された金属フィラメントから形成された、抗再狭窄薬投与用の拡張性ステントを製造する方法であって、
該ステントフィラメントの外面領域を、少なくとも約0.5μmの表面粗さ(Ra)及び約7.5〜17.5μmの表面粗さ範囲(Rt)まで粗面化すること、及び
該ステントの外面をポリマーフリーリムス薬物コーティングによってコーティングすること、
を含む、上記方法。 - ステントフィラメントが約0.5から1μmの表面粗さまで粗面化される、請求項1又は2に記載の方法。
- 前記粗面化することが、ステントフィラメントの外面領域を加圧された研磨粒子流で研磨することによって実施される、請求項1又は2に記載の方法。
- 前記粗面化することが、ステントフィラメントの外面領域の上に炭化水素フィルムのマスクを形成し、マスクによって露出したステント材料を選択的に除去し、マスクを除去することによって実施される、請求項1又は2に記載の方法。
- 前記粗面化することが、ステントフィラメントの外面領域をレーザエッチングすることによって実施される、請求項1又は2に記載の方法。
- 前記粗面化することが、フィラメントの外面領域をピーニングして、外面領域に、あるパターンを刻印することによって実施される、請求項1又は2に記載の方法。
- 前記コーティングすることが、前記薬物の粘性溶液をステントフィラメントの外面に塗布し、塗布した前記溶液を乾燥させて、ステントフィラメント上に固体薬物コーティングを形成することによって実施される、請求項1又は2に記載の方法。
- 前記コーティングすることが、最終的にステント長1cm当たり80から240μgの量のリムス薬物をステントの表面に塗布するように実施される、請求項1又は2に記載の方法。
- 前記コーティングすることが、最終的に厚さ5から15μmの薬物コーティングを形成するように実施される、請求項1に記載の方法。
- 前記ステントをコーティングするリムス薬物がBiolimus A9である、請求項1又は2に記載の方法。
- ステントが血管損傷部位に配置されたときに、リムス薬物溶出ポリマーコーティングを有するステントにより生じる炎症反応を生じさせることなく、再狭窄又は血栓症の発生率及び/又は程度を低減させるために使用される拡張性ステントであって、
相互接続された金属フィラメントから形成された拡張性ステントボディと、
該ステントフィラメントの外面領域に形成された、少なくとも約0.5μmの表面粗さ(Ra)及び約7.5〜17.5μmの表面粗さ範囲(Rt)を特徴とする粗面化された表面と、
ステントフィラメントの該粗面化された領域に担持された、該粗面化されたステント表面の表面粗さ範囲よりも大きなコーティング厚さを有するリムス薬物のポリマーフリーコーティングと
を含むステント。 - ステントフィラメントが約0.5から1μmの表面粗さまで粗面化された、請求項12に記載のステント。
- ステントフィラメントが、約7.5〜12.5μmの表面粗さ範囲を有するように粗面化された、請求項12に記載のステント。
- リムス薬物がBiolimus A9である、請求項12に記載のステント。
- リムス薬物のポリマーフリーコーティングが、ステント長1cm当たり80から240μgの量で広がる、請求項12に記載のステント。
- リムス薬物のポリマーフリーコーティングが5から15μmの厚さを有する、請求項12に記載のステント。
