Description
2003年9月30日に発行されたYoungによる特許文献2は、クランプにより脊椎アンカーに取り付ける脊椎ロッドを教示する。このクランプは、クランプにねじ込まれたコレットにより、アンカー周囲に締め付けられる。ロッドは、コレットにより大きさを縮小されたスプリットリングによってクランプに保持される。アンカーはトルクによって骨に設置され、追加トルクによってコレットが締め付けられる。
2003年8月26日に発行されたKumar他による特許文献3、2003年8月26日にBailey他に発行された特許文献4、2003年5月20日にYuan他に発行された特許文献5、2002年4月16日にRalph他に発行された特許文献6、2002年11月12日にHowlandに発行された特許文献7、1996年12月17日にErrico他に発行された特許文献8は、クランプによって複数の骨アンカーに連結され、骨ネジが骨に着座した後に組立構造体に追加トルクを付加する必要のある1つの脊椎ロッドを教示する。
これら従来技術の脊椎固定装置は全て、骨ネジのそれぞれにロッドを締め付け、固定するため、組立構造体に追加トルクを与え、それにより骨ネジに追加トルクを加える結果となる。この追加印加によって骨と骨ネジのネジ山との間の引っかかりをすり減らすことがある。このような結果を防ぐため、ロッドとネジを係止するトルクに対抗する何らかの道具を用いなければならない。トルク対抗道具の利用は、外科医による調整や、外科医がずれを防ぐことを必要とする。
この技術に必要なのは、トルクではなく圧縮力を用いて1つのロッドと1つの埋め込み骨ネジを接続するシステムである。
外傷や疾病による骨格要素の位置を補正するため、特定の空間関係で骨および骨片を安定および固定しなければならない場合が多くある。ある装置群は、骨の1つの所定の空間位置を維持するため、1つのロッドで接続された、骨折、または隣接する椎骨、または関節など、骨または骨片の1つの不連続部分を横切る骨の内部に配置される、多数の骨ピン、アンカーまたはネジを設置する。これらの装置は、ある場合には一時的でその後取り外し、または人工補充物の形の恒久的な場合もある。装置は、体内または体外でもよい。本発明の装置はこれら用途に用いることもできる。しかしながら、好適な実施例は脊椎固定に関し、例としてこれを説明するが、これに限定されない。

図1は、より自然な湾曲を維持するため、椎骨を互いの椎骨および仙骨に関して安定させかつ固定するための1つの片側性外科整形固定装置10を配置した腰椎Sの一部を示す。必要に応じて両側性設置を用いることができる。図2により詳細に示すように、細長いコネクタバー11が椎骨と骨ネジ13との間の不連続部分を繋ぐ。図に示すように、このバーは円形の断面を有するが、図13のリンク29に示すように、他の形状も使用可能である。骨アンカーは、図5および図6に示す外部螺旋状ネジ山60を有し、これにより骨ネジ13はトルクの印加中、海綿質の骨の中で引っかかりを得る。
Claims (11)
- 骨に侵入するための軸を有する、一端に頭を持つ1つの骨ピンと、
1つの細長いコネクタに前記骨ピンを係止するための第1のクランプと、
を有する骨を安定させるための外科整形装置において、
前記第1のクランプは、
前記頭を包むよう適合された内側ソケットを形成する外壁を含む本体と、
1つの位置において前記外壁の外側を取り囲む1つの圧縮リングと、
前記本体の前記外壁を介して前記ソケットに対向する1つのスロットと、
ここにおいて前記スロットは前記コネクタと摺動可能に接触するよう適合され、
前記圧縮リングは前記外壁の前記外側に沿って第2の位置に直線移動可能であり、これにより、前記ソケットと前記スロットの前記外壁は、前記第1のクランプを前記頭と前記コネクタの周りに確実に係止するよう適合された圧縮力を印加し、
前記スロットを覆う1つの保持手段と、を有し、
前記外壁は前記スロットと前記圧縮リングを超えて伸長し、また外溝を有し、
ここにおいて前記保持手段は、前記外壁の前記外溝と係合し、前記コネクタの上方で前記スロットを閉鎖するように適合される、
ことを特徴とする装置。 - 前記コネクタは細長いロッド形状を有し、前記ロッドはその長さに沿って複数のリンクと骨ピンと接続するよう適合され、前記コネクタと前記複数の骨ピンは、1つの固定位置で骨を安定させるよう適合されることを特徴とする請求項1に記載の外科整形装置。
- 前記第1のクランプは、一つの端に前記スロットと摺動可能に接触するよう適合された1つのリンクと、前記リンクの他の端の1つのジャーナルとを含み、前記ジャーナルは前記コネクタをその中に固定するための第2のクランプを有し、これにより、前記コネクタが前記骨ピンの位置から離れて配置されることが出来る、ことを特徴とする請求項1に記載の外科整形装置。
- 前記リンクは、1つの分割スリーブと、前記分割スリーブ周囲の1つの圧縮リングとを有し、前記圧縮リングは、1つの開位置から1つの閉位置に前記分割スリーブに沿って移動可能で、前記開位置は、前記第2のクランプ内での前記コネクタの調整を可能にし、前記閉位置は、前記コネクタと前記ジャーナルを共に固定する、ことを特徴とする請求項3に記載の外科整形装置。
- 前記骨ピンの前記頭はほぼ球形で、前記ソケットは1つの補完的形状を有し、これにより、前記骨ピンと前記クランプは自在に方向を向くことができることを特徴とする請求項1に記載の外科整形装置。
- 前記コネクタは細長い棒状であり、前記ロッドはその長さに沿って複数の前記クランプ及び前記骨ピンと接続するよう適合され、前記コネクタと前記複数の骨ピンは、1つの固定位置で骨を安定させるよう適合されることを特徴とする請求項5に記載の外科整形装置。
- 前記第1のクランプは、一端で前記スロットと摺動可能に接触するよう適合された1つのリンクと、前記リンクの他の一端にある1つのジャーナルとを含み、前記ジャーナルは前記コネクタをその中に固定するための手段を含み、これにより、前記コネクタが前記ピンからずれることが出来ることを特徴とする請求項5に記載の外科整形装置。
- 前記ジャーナルは、前記リンクを囲む1つの分割スリーブと、前記分割スリーブ周囲の1つの圧縮リングとを有し、前記圧縮リングは、1つの開位置から1つの閉位置に前記分割スリーブに沿って移動可能であり、前記開位置は、前記ジャーナル内での前記コネクタの調整を可能にし、前記閉位置は、前記コネクタと前記ジャーナルを共に固定する、ことを特徴とする請求項7に記載の外科整形装置。
- 前記軸は1つの外部螺旋状ネジ山を有して形成されることを特徴とする請求項1に記載の外科整形装置。
- 前記スロットが前記ソケットに隣接し、これにより、前記頭と前記コネクタが近接して低い高さ寸法を形成することを特徴とする請求項1に記載の外科整形装置。
- 前記保持手段は前記外壁にスナップ適合で係合し、前記スロットを前記コネクタの上方で閉鎖する、ことを特徴とする請求項1に記載の外科整形装置。
