本発明は、埋め込み型機器で疾患を治療するための方法およびシステムに関する。
リンパ系および心臓血管系は、密接に関係した構造であり、毛管系によって間接的に連結される。リンパ系は、疾患を引き起こす有機体を除去すること、および外来有機体を攻撃して抗体を生成するリンパ球を産出することに関し、身体の防御機構にとって重要である。また、リンパ系は、間質組織から過剰な流体およびタンパク質を排出するため、身体内の流体および栄養の分布に関しても重要である。血管から体組織の間質腔およびその他の間質成分へ浸透する流体は、次に、細リンパ管によって吸収され、リンパ管を通って血流に戻って流入するリンパを形成する。リンパ管の末端構造は、横隔の上および正中線の下の身体の右上の4分の1の部分からリンパ液を排出する右リンパ本幹と、残りの身体を排出する胸腔の縦隔に位置する胸管とを含む。動脈を出入りする血流を通って静脈に入り、リンパ管およびリンパ節を通って、身体は、細胞損傷および異物侵入の産物を排除可能である。
リンパ系を介して薬物を体内の位置に送達するための方法および装置が本明細書において記載される。それゆえ、一実施形態において、薬物容器および制御回路を含む埋め込み型機器は、リンパ管内に配置されるように適合される薬物送達カテーテルに連結される。また、埋め込み型機器は、療法の送達を制御するためセンサ信号を受信するために、リードによってまたは無線で構成されてもよい。また、埋め込み型機器は、遠隔測定リンクを介して外部システムまたは外部制御機器と通信してもよい。
リンパ管は、身体の循環系の一部であり、間質腔から血液に流体が流入可能な経路として役立つ。また、リンパ管は、リンパ節とも連通し、間質腔からリンパ節へ外来抗原を運ぶことによって身体の免疫機能を促進する。脳および中枢神経系等のいくつかの例外を除き、体組織の全ては、リンパ管と連通する。このため、リンパ系は、選択された体内位置に薬物を直接送達するための便利な管である(本明細書において用語「薬物」を使用する際、その用語は、治療効果または診断効果を有するように意図される任意の化学的物質または生物学的物質である)。後述のように、埋め込み型機器は、薬物送達装置と、リンパ管内に配置するように適合されるカテーテルとを備えて構成されてもよく、カテーテルは、通常リンパ管が排出する薬物を標的位置に送達する。カテーテルは、埋め込み型機器によって作動される膨張式バルーンまたはその他の閉塞構造を備えてもよく、これらは、リンパ液の通常の順行性リンパ流を阻止し、治療剤が管に注入されて、バルーンから遠位に流れ、逆行的に標的位置に流れることを可能にする。その他の実施形態は、閉塞構造を使用せずに、順行性に流れるように治療剤を送達してもよい。機器は、1つ以上の感知様式を備えてさらに構成されてもよく、この感知様式は、生理学的モニタリングに従って、および/または療法送達の患者側制御を可能にする磁気作動型または接触式作動型スイッチに従って療法の送達を可能にする。また、機器は、無線測定コマンドを介して薬物送達の制御を可能にする無線遠隔測定能力も備えてもよい。
Claims (10)
- 埋め込み型ハウジングと、
該埋め込み型ハウジング内に含まれる薬物送達装置と、
該薬物送達装置と通信し、リンパ管内に配置するように適合され、かつ内腔を有するカテーテルと、
該リンパ管におけるリンパ液の流れを阻止するための、該カテーテルに組み込まれる閉塞構造と、
該閉塞構造から遠位への該カテーテルの外へ、かつ該リンパ管への薬物の送達を可能にするように、該閉塞構造および薬物送達装置の動作を制御するための、該埋め込み型ハウジング内に含まれる制御回路と
を備える、機器。 - 前記閉塞構造は、前記カテーテル上に取り付けられる膨張式バルーンであり、該カテーテルのバルーン膨張内腔を介して該バルーンを膨張させるための、前記埋め込み型ハウジングに含まれる膨張装置をさらに備える、請求項1に記載の機器。
- 前記カテーテルは、該カテーテルの異なる部分の選択的補強をもたらし、そしてリンパ管を通る該カテーテルの通過を促進するように個々に加圧され得る複数のチャンバを組み込む、請求項1または請求項2に記載の機器。
- 前記カテーテルは、複数の同心円層を備え、該複数の同心円層の各々は、該カテーテルからの薬物の流れを制御可能にするために、異なる分子に対して選択的に透過性である、請求項1から請求項3のいずれかに記載の機器。
- 前記薬物送達装置は、薬物容器に連結される機械的ポンプである、請求項1から請求項4のいずれかに記載の機器。
- 生理学的信号を感知するための感知回路をさらに備え、前記制御回路は、該生理学的信号に応答する方法で前記薬物を送達するように構成される、請求項1から請求項5のいずれかに記載の機器。
- 心臓内に配置するように適合されるリードによって前記感知回路に連結される電極をさらに備え、該感知回路は、心臓の電気的活動を感知するように構成される、請求項6に記載の機器。
- 前記リンパ管内における前記リンパ液を感知するための前記感知回路に連結されるリンパセンサをさらに備える、請求項6に記載の機器。
- 前記薬物の送達を開始または停止するように動作可能な、接触式作動型または磁気的作動型のスイッチをさらに備える、請求項1から請求項8のいずれかに記載の機器。
- 前記機器との無線通信を可能にするための、前記制御回路にインターフェース接続される遠隔測定送受信器をさらに備える、請求項1から請求項9のいずれかに記載の機器。
