5未満のS.I.を有するT細胞株は、限界希釈によってクローニングされる。方法は、
PBMCまたはCSFMCを、MBPまたはそのフラグメント(好ましくは、アミノ酸8
3〜99に対応するフラグメントおよびアミノ酸151〜170に対応するフラグメント
)とともに7日間、培地を交換せずにインキュベートすることによって、MBPおよび/

る。両方のウェルセットにおける細胞は、この抗原ウェルに上記のMBPまたはそのフラ

MBP反応性T細胞は、インビボでの活性化およびクローン性増殖を受け、そして所定

己免疫成分がT細胞ワクチン接種によって除去または抑制された後でさえ、炎症病変が消
散するまでに依然として長期間かかり得、そして既存の組織損傷の一部は永続性である可

T細胞ワクチン、ならびにMSの処置のためにこのワクチンを使用するための方法を提供
する。
において実証されたMSの通常の病歴の評価と比較され、プラシーボコントロールと比較
されていないことが指摘されるべきである。本研究はまた、本研究のオープンラベル臨床
的設計に関連した潜在的プラシーボ効果によって制限される。それゆえ、本研究は、MS
におけるT細胞ワクチン接種の役割に有利な重要な臨床的指標を提供したとはいえ、T細
胞ワクチン接種の処置効力は、二重盲検およびプラシーボでコントロールした臨床試験に
おいて最良に評価される。
、より初期のT細胞ワクチンよりも調製が容易であって、患者における改善された免疫学
的応答を協調して提供するように作用し得る不均質細胞集団(非クローン性)を提供し、
そしてエピトープ拡散またはクローンシフトについての潜在的問題を回避し、そして疾患
を担う、より多様性の大きなT細胞をより良く除去するように設計される。
MS患者の血液中のMBP反応性T細胞の頻度を、Zhangら,1994,Zhan
gら,1993,Medaerら,1995(これらの各々は、本明細書中に参考として
援用される)によって記載される方法を用いて評価した。各場合、細胞の処理および細胞
培養のために用いた材料は、厳密に自己であった。末梢血単核細胞(PBMC)を、ヘパ
リン処理した静脈血から、標準的なFicoll勾配分離によって調製した。これらのP
BMCを、2つの免疫優性領域(アミノ酸残基83〜99およびアミノ酸残基151〜1
70,Tejada−Simonら,Eur.J.Immunol.,2001,Mar
;31(3)907−917)に対応するヒトミエリン塩基性タンパク質(MBP)の2
つの合成ペプチド(それぞれ、20μg/mlの濃度)の存在下で、10%熱不活化自己
血清および50IU/mlの組換えインターロイキン−2(IL−2)を補充したRPM
I 1640(Hyclone,Logan,Utah)中に(合計96ウェルについて
)200,000細胞/ウェルでプレーティングした。インキュベーションを、37℃で
行った。7日後、全ての培養物を、パルス照射した自己PBMC(凍結または新鮮)で再
刺激した。使用の前に、PBMCのパルス処理を、PBMCを100μg/mlの濃度の
各ペプチドとともに37℃で3時間インキュベートすることにより行い、続いて60Co
供給源で4,000ラドで照射した。さらに1週間のインキュベーション後、各培養物を
、MBPペプチドに応答した特異的増殖について、以下に記載の増殖アッセイにおいて試
験した。

ンキュベートし、そして最後の16時間の培養の間、[3H]−チミジン(Amersh

した。次いで、細胞を、自動化細胞収集機を用いて収集し、そして[3H]−チミジン取
り込みを、βプレートカウンター中で測定した。細胞中に取り込まれた3H−チミジンの
1分間あたりのカウントが、1,500よりも多く、そしてコントロール(ペプチドの不
存在下)の1分間あたりのカウントを少なくとも3倍超えた場合、細胞を、MBPペプチ
ドについて反応性と定義した。次いで、充分な反応性を示すウェルの数を、最初の培養物
中に播種したPBMCの総数(19.2×106細胞)によって除算することによって、
MBP反応性T細胞の頻度を評価した(例えば、Zhangら,1994,Zhangら
,1993,Medaerら,1995を参照のこと)。同じ算出方法を一貫して用いて
、本研究全体を通してのMBP反応性T細胞の頻度の変化を比較した。
細胞の頻度は、約14×10−5であった。これは、Zhangら,(1994)、およ
びOtaら,Nature,346:183−187(1990)によって報告された約
10×10−5の頻度と匹敵する(実施例5も参照のこと)。
(PBMCの調製および一次刺激)
新鮮な血液標本を、収集2時間以内に処理した。あるいは、単核細胞は、MS患者の脳
脊髄液(CSFMC)から入手され得る。末梢血単核細胞(PBMC)を、標準的なFi
coll勾配分離法によって、全血から単離した。特に、ヘパリン処理した血液をハンク
ス平衡塩溶液(HBSS)(1:1の血液/HBSS)で希釈し、次いで遠心管中のFi
coll−hypaque溶液上にゆっくりと重層した。そして1800rpmで18℃
〜25℃、ブレークなしで20分間遠心分離する。次いで、PBMCを、過剰のHBSS
を添加して1700rpmで10分間、18℃〜25℃で遠心分離することによって洗浄
した。精製したPBMCを、遠心分離によってRPMI 1640培地中で3回洗浄し、
続いてAIM V培地(Gibco,Grand Island,N.Y.)中に再懸濁
した。細胞数を数え、そして細胞を、200,000細胞/ウェルの濃度で96ウェルU
字底型培養プレート上にプレーティングした。全てのプレートを、患者数および患者のイ
ニシャルでラベルした。実施例1で考察したミエリンペプチドを、それぞれ、20μg/
mlでこの培養物に添加した。プレートを、CO2インキュベーター中に配置し、そして
毎日目視試験試験した。細胞を、培養培地を交換することなく7日間培養して、ペプチド
特異的T細胞を選択的に増殖させた。
全てのウェル由来の細胞の約50%を取り出し、そして2つのウェル(抗原ウェルおよ
びコントロールウェル)に均等に分けた。新鮮なPBMCまたは解凍したPBMCを、8
,000(60Co源を用いる)ラドで照射し、そして供給源の抗原提示細胞(APC)
として、100,000細胞/ウェルで用いた。細胞を、5%ヒトAB+血清を含むRP
MI 1640中で培養した。上記の実施例1に記載されるミエリンペプチドを、それぞ
れ、20μg/mlで、この抗原ウェルに対して添加した。ミエリンペプチドを含まない
培地を、一対のコントロールウェルに対して添加した。あるいは、以下によって記載され

をBetaplateカウンター中で測定した。対応するミエリンペプチドに対する各T
細胞株/ウェルの反応性を、[3H]チミジン取り込み増殖アッセイによって決定した。
特に、各ウェル由来の細胞を、4つのアリコート(1アリコートあたり約104細胞)に
分け、そして2連で、ミエリンペプチドの存在下および不存在下で、APCの供給源とし
ての、照射した105個の自己PBMCとともに培養した。培養物を3日間インキュベー
トし、そして培養の最後の16時間の間、1μCi/ウェルの[3H]チミジンでパルス
した。この抗原ウェルの1分間あたりのカウント(cpm)/コントロールウェルのcp
mの商が、3以上である;およびこの抗原ウェルの総cpmが1,500よりも大きいの
両方である場合、T細胞株は、ミエリンペプチド特異的であると定義される。ミエリン反
応性T細胞の頻度を、ポアッソンの統計学に従って評価した。同定されたミエリン反応性
T細胞株の残りの50%の細胞を、照射したPBMCで、増大のために再刺激する。
T細胞株が、ミエリンペプチド反応性であると同定され、続いて、1回再刺激された後
、これをさらに増殖して、以下の方法のうちの1つを用いて、ワクチン接種のために十分
な細胞を産生する:直接増大法およびTクローン化方法。増殖方法の選択は、そのミエリ
ンペプチドに対するT細胞株の特異性および反応性に依存する。これらの特異性は、刺激
指数(SI)によって測定される。SIは、上記の[3H]−チミジン取り込み増殖アッ
セイによる結果から計算される。SIは、抗原ウェルの1分間あたりのカウント(cpm
)/コントロールウェルのcpmの商である。SIが5以上である場合、この直接増大法
が用いられる。SIが5未満である場合、クローニング方法が用いられる。
手短に述べると、次いで、5以上のS.I.を有すると同定されたミエリン反応性T細
胞を、照射した自己PBMCの存在下で、対応するミエリンペプチドでの刺激サイクルと
PHAでの刺激サイクルとを交互に行う、直接増大法(DEM)によって増大させた。各
刺激サイクルを、7〜10日間実施した。より詳細には、上記のとおりに同定されたミエ
リン反応性T細胞を、照射したPBMC(APC)(1ウェルあたり100,000細胞
)の存在下で、1ウェルあたり20,000〜40,000細胞でプレーティングした。
それぞれ、対応するミエリンペプチドを抗原刺激サイクルのために20μg/mlで添加
した。そしてPHAを各PHA刺激サイクルのために1μg/mlで添加する。組換えヒ
トIL−2もまた、刺激サイクルの2日目に100IU/mlで添加した。培養物を、1
0%ヒトAB+血清および100IU/ml rIL−2を含むRPMI 1640培地
で3〜4日毎にリフレッシュした。ミエリン反応性T細胞株を、総細胞数が約2000万
個に達するまで、交互刺激サイクルで増殖させた。
このクローニング方法において、T細胞株を、限界希釈アッセイを使用してクローニン
グした。各ミエリンペプチド反応性T細胞株の細胞をプールし、そして10%ヒトAB+
血清および100IU/mLのrIL−2を含むRPMI 1640培養培地中に約0.
3〜約20細胞/ウェルで播種した。PHAを1μg/mLで添加し、そして照射した自
己APCを100,000細胞/ウェルで添加した。100IU/mLのrIL−2を含
む培養培地RPMI 1640を、3〜4日毎に交換した。培養の14日後、増殖陽性ウ
ェルをアッセイして、上記のような対応するミエリンペプチドに対する特異的反応性を決
定した。これらのペプチド特異的T細胞株のさらなる増殖を、対応するミエリンペプチド
およびPHAを用いる代替的刺激サイクルにおいて、上記の直接的増殖方法に従って実施
した。
(T細胞ワクチン接種による、MBP反応性T細胞の枯渇)
RR−MS(n=28)を有する患者およびSP−MS(n=26)を有する患者54
人を、この非盲検研究のために採用した。これらの患者のベースラインの臨床的特徴を、
表1に示す。各患者は、上記のように準備された2ヶ月間隔で、照射した自己MBP反応
性T細胞クローン(このクローニング方法によって調製された)を用いる皮下注射の3つ
の過程を受けた。患者を、MBP反応性T細胞の前駆体頻度、再発率、EDSSおよびM
RI病変活性における変化について、24時間モニタリングした。これらの結果を、自己
対合様式でワクチン接種前の値と比較した。さらに、β−インターフェロン−1a臨床試
験におけるRR−MS(Jacobsら、1996)および最近のβ−IFN−1b研究
におけるSP−MS(European Study Group、Lancet、35
2:1491−1497(1988))の偽薬対照集団の臨床データを、比較のためのM
Sの自然歴データを提供するために含めた。この研究において記載される偽薬コントロー
ル被験体のベースライン特徴は、より低い平均EDSS以外は、本明細書中に記載される
患者集団研究のベースライン特徴と類似であった。
4×10−5)にてこれらのMS患者において検出される循環MBP反応性T細胞の前駆
体頻度は、以前の研究において報告された頻度(末梢血単核球中に約10×10−5)(
Zhangら、1994、Otaら、1990)に高度に匹敵した。RR−MS集団とS
P−MS集団との間のMBP反応性T細胞の前駆体頻度においては、有意な差異は見出さ
れなかった。T細胞頻度は、92%の患者において検出不能であったか、または3つの過
程のワクチン接種の終了後2〜3ヶ月に、残りの患者において実質的に減衰した(14×
10−5 対 1.9×10−5、p<0.0001)。これらの結果により、MSを有
する患者におけるT細胞ワクチン接種による、MBP反応性T細胞の枯渇を確認した。
(自己MBP反応性T細胞を使用するMS患者のワクチン接種)
54人のMSを有する患者を、この試験に参加させた。包括基準は、少なくとも2年間
にわたる臨床的に明確なMS、RR−MSについて1.5〜6.5の、そして二次進行性
MS(SP−MS)を有する患者について4.0〜8.0の、ベースラインの拡大身体障
害状態スケール(EDSS)、および緩和−軽減MS(RR−MS)集団について、研究
に入る前の過去2年間での少なくとも1回の再燃、であった。約25%の患者は、β−イ
ンターフェロンまたはグラチラマー(glatiramer)を用いた処置に対して応答
または許容することに以前に失敗し、そして残りの患者は、研究に入る前の少なくとも1
ヶ月間および研究の間中、これらの薬剤で処置されていなかった。これらの患者は、研究
に参加する前の少なくとも3ヶ月間、いかなる免疫抑制薬物(ステロイドを含む)をも摂
取していなかった。悪化が生じる場合には、ステロイドをこの研究の間に許容した。疲労
、痙縮および膀胱病訴についての全身処置は、禁止しなかった。インフォームドコンセン
トを、実験手順の説明後に患者から得た。このプロトコルは、Institutiona

ege of Medicineによって承認された。

ニング方法によって調製したMBP反応性T細胞クローンを、アクセサリー細胞の供給源
としての照射したPBMCの存在下で、フィトヘマグルチニン(PHA)(1μg/ml
)を用いて予め活性化した。次いで、細胞を、10%心臓不活化自己血清および50単位
のrIL−2を補充したRRIM 1640中で5〜6日間培養した。活性化MBP反応
性T細胞を、引き続いて滅菌生理食塩水で3回洗浄して、残留PHAおよび細胞細片を除
去した。照射(8,000rads、60Co源)後、細胞を、2mlの生理食塩水中に
再懸濁し、そして2本の腕に皮下注射した(1ml/腕)。ワクチン接種のために使用し
たT細胞数は、1回の注射当たり40×106〜80×106細胞の範囲であり、そして
相対的皮膚表面積基準で、実験動物において有効なT細胞用量の外挿によって選択した(
Ben−Nunら、1981)。各患者は、2ヶ月間隔で3回の皮下注射を受けた。

験に対する客観的な変化(EDSSに対する少なくとも0.5ポイントの悪化)を伴う、
少なくとも48時間続く既存の神経学的症状の悪化、によって定義した。患者に、計画さ
れた定期的な訪問の間の事象を報告するように指示し、そして症状が悪化を示唆した場合
に、神経学者が患者を試験した。安全性評価は、定期的な訪問時の有害事象、バイタルサ
インおよび身体試験を含んだ。T細胞ワクチン接種の前後の研究患者における臨床的変数
の差異を、Wilcoxonの順位和検定を使用して分析した。
(ワクチン接種後のMSの臨床過程の変化)
T細胞ワクチン接種による循環MBP反応性T細胞の枯渇がMSの臨床過程を変更する
か否かを解決することを試みた。患者は、上記のように調製した自己T細胞ワクチンを受
けた。幾人かの患者において注射部位で見られた弱く一過的な紅斑以外には、T細胞ワク
チン摂取に伴う有害効果は存在せず、そして全ての患者を、外来診療所で処置した。表2
に示されるように、平均EDSSは、ワクチン接種後24ヶ月間に亘って、RR−MSを
有する患者において僅かに減少した(開始時3.21 対 終了時3.1)。比較として
、β−IFN−1a試験を使用して実施された試験(Jacobsら、1996)におい
て報告されたのと同じ観察期間にわたって、RR−MS(n=56)の自然歴において、
平均EDSSが0.61増加した。さらに、EDSSを変化させなかったかまたは改善し
たのいずれかであった患者集団は、自然MS歴の患者集団と比較して、かなり高かった(
75% 対 50%)。処置されたRR−MS群中の一人(3.5%)の患者だけが、M
Sの自然歴中の患者の18%と比較して、24ヶ月以内に2.0のEDSSを超えて進行
した(表2)。
0.6と比較して、24ヶ月間の期間にわたって僅かに進行した(+0.12)(Eur
opean Study Group、Lancet、1988;352:1491−1
497)。さらに、Kaplan−Meier法を使用して確認された進行までの時間の
概算は、MS患者の自然歴(RR−MSについて12ヶ月およびSP−MSについて9ヶ
月で、20%の進行)と比較して、かなりの遅延(両方の処置群について、18ヶ月で2
0%の進行)を示した(Jacobsら、Ann.Neurol、1996;39;28
5−294、European Study Group、1998)。しかし、進行は
、両方の研究群において、18ヶ月後(最後のワクチン接種の12ヶ月後)に加速するよ
うであった。
(臨床的悪化速度の変化)
表3に示されるように、再発の年率は、T細胞ワクチン接種後のRR−MSを有する患
者において減少し、ベースラインの再発率からの40%の減少を示した。再発率における
有意な差異は、処置の1年目と2年目との間には見出されなかった。比較として、再発の
年率における25%の減少が、RR−MSの自然歴において観察された(Jacobsら
、1996)。さらに、攻撃を示さないかまたは攻撃をほとんど示さない患者集団は、自
然MS歴における患者集団よりもかなり高かった(表3)。再発率は、SP−MS集団に
おいて50%減少したが、本明細書中で試験した少数の二次進行性患者だけ(6/26)
が、研究に入る前の2年間に再発した。
(画像共鳴像試験による脳病変活性)
画像共鳴像(MRI)を、ガドリニウム増強T2加重画像として実施した。より高い信
号強度の領域を、半定量的様式でスコアリングした(Scheltenら、Brain
1992;115:735−748、Truyenら、J.Neurol.Sci.、1
990;96:173−182)。このスコアリング方法は、増大した信号過剰強度(h
yperintensity)を有する病巣のサイズおよび数の両方に関連するスコアを
生じた。信号過剰強度を、以下の領域においてスコアリングした:(i)前頭および後頭
領域の脳室周囲、ならびに側脳室に対して平行;(ii)前頭、側頭、頭頂骨および後頭
領域中の別々の葉性白質;(iii)脳幹神経節、尾状核、被殻、淡蒼球および視床、な
らびに(iv)テント下領域、小脳、中脳、橋および髄質。病変を、以下のようにスコア
リングした:直径0.5cm未満の病変には、スコア「1」を与え、0.5cmと1.0
cmとの間には「2」を与え、1.0cmと1.5cmとの間には「3」を与え、1.5
cmと2.0cmとの間には「4」を与えそして2.0cmより大きいものには「5」を
与えた。融合性病変を、以下のように測定した:上記のような目的の領域の25%未満が
異常な信号強度であるとみなされた場合、スコア「5」を与え、目的の可視化領域の50
%より多くが罹患している場合には、25%および50%については「10」および「1
5」を与えた。次いで、これらの値を、「個々の」病変スコアに加算した。
および終了時(24ヶ月)で実施して、疾患進行の指標として脳病変活性の変化をモニタ
リングした。異なる医療センターで実施されたいくつかのスキャンの技術的不適合性に起
因して、34人の患者だけからのMRIスキャンを分析し得た。全てのMRIスキャンを
、この臨床試験に関与しなかった外部の神経学者によって評価した。本発明者らの予備臨
床試験および他の関連の研究において以前に使用した半定量的スコアリング方法を使用し
て、病変活性を評価した(Medaerら、1995、Scheltensら、1992
、Truyenら、1990)。このスコアリング方法は、T2加重画像の増大した信号
過剰強度を有する病巣のサイズおよび数の両方に関連するスコアを生じた。表4に示され
るように、これらの結果は、試験した患者の70%において、MRI病変スコアが、病変
スコアにおける少なくとも1ポイントの減少によって規定されるように、不変であったか
または改善されたかのいずれかであったが、一方で残り30%の患者は、研究過程の間に
増加した病変スコアを有したことを明らかにした。集団的に、平均MRI病変スコアにお
ける変化は、1年目には1.2%の減少を示したが、2年目にはベースラインMRIから
3.3%の増加を示した。しかし、これらの変化は有意ではなかった(P>0.4)。こ
れらの結果は、T細胞ワクチン接種に寄与する安定化またはいくらかの改善を反映し得る
。なぜなら、MRI病変は、一般に、種々の臨床試験において報告されたように、未処置
のRR−MA患者において1年基準で約10%進行するからである(European
Study Group、1998、IFNB Multiple Sclerosis
Study Group、Neurol.、1993;43:655−661)。まと
めると、これらの知見は、T細胞ワクチン接種によるMBP反応性T細胞の枯渇と試験し
たMS患者における臨床的改善との間の有利な相関を示唆する。

Claims (9)
- 多発性硬化症の処置のためのT細胞ワクチンであって、少なくとも1つの多発性硬化症関連抗原の、フラグメントの組合せを用いてインビトロで初回免疫された、弱められたT細胞を含み;該フラグメントの組合せの少なくとも1つのフラグメントが、ミエリン塩基性タンパク質(MBP)のアミノ酸83〜99からなるフラグメントまたはMBPのアミノ酸151〜170からなるフラグメントであり;該T細胞は、該ワクチンで処置される患者から得られたものである、T細胞ワクチン。
- 多発性硬化症の処置のためのT細胞ワクチンであって、以下の工程:
