図21〜24は本発明にかかるステントの例として実施例5を示している。図21は実施例5のステント収縮時においてステント軸方向から見た断面図を示している。このように実施例5では、カテーテルもしくはバルーンにステントが固定された時に、ストラットが半径方向に重なりを有して固定されており、部分的に2層の状態で固定されることになる。図22には実施例5のステント収縮時の展開図を示している。図23に実施例5の拡張後のステント軸方向から見た断面図を示している。また図24は実施例5の拡張後の展開図を示している。拡張された後は、従来の例である比較例1と同様にストラットが半径方向に重なりを有していない。実施例5のステントをISO5832−7に規定される金属(コバルトクロム合金)を用いて作製した。ストラットの幅300um、厚33um、ステント長さ13mmとした。
図25は実施例6のステント収縮時においてステント軸方向から見た断面図、図26は実施例6のステント拡張時においてステント軸方向から見た断面図を示している。このように、ステントが第1の直径にあるときに、薬剤が該ステントの外表面に存在しないが、ステントが第2の直径にあるときに、薬剤が該ステントの外表面に存在するステントが可能であった。
上述の比較例1および実施例1〜6について、ミニブタ(クラウン、雌、月齢8から12ヶ月)へのステント留置実験を実施し、評価を行った。麻酔下でミニブタの右大腿動脈にシース(6Fr)を挿入し、シースから挿入したガイディングカテーテル(6Fr)の先端を左冠状動脈入口部にエンゲージさせた。ガイディングカテーテル経由で左冠状動脈前下行枝および左冠状動脈回旋枝へとステントをデリバリーした後、拡張・留置した。ガイディングカテーテルおよびシースを抜去した後、右大腿動脈を結紮し止血した。ステントを留置する部分はステント径/血管径の比を約1.25となるようにステントを留置した。ステントは左冠状動脈前下行枝、左冠状動脈回旋枝、および右冠動脈の各血管に各1個のステントをランダムに留置した。留置実験を実施する前日より剖検日まで、アスピリン330mg/day、チクロピジン250mg/dayを混餌投与した。留置28日後にミニブタを安楽死させ心臓を摘出した。比較例1および実施例1〜6の各群についてn=3のステントを留置し評価を実施した。すべての群、すべてのステントにおいて、ステント留置手技に問題は生じず、留置期間である28日間でステント閉塞等の問題は生じなかった。ステントを留置した冠状動脈を心臓より摘出し、10%中性緩衝ホルマリン溶液中で浸漬固定した。樹脂包埋後、各ステントの中央部の切片を作製し、H.E.染色(ヘマトキシリン・エオジン染色)を行い、拡大観察を実施した。ステントストラットの血管への障害度を評価指標とし、各ストラット毎に障害スコアをスコアリングし、全ストラットの平均をそのステントの障害度とした。障害スコアは、ストラットが血管内弾性板と接触していない状態を0、ストラットが血管内弾性板と接触しているが、内弾性板に損傷がない場合には1、ストラットが内弾性板を貫通して中膜に接触している場合には2,ストラットが血管外弾性板と接触している場合に3,ストラットが外弾性板を貫通して外膜と接触している場合を4、として実施した。(これらの評価方法はKornowsk、et al.JACC.1998;Vol31、No.1:224−230に開示された方法を引用している)。各群の障害度の平均を表2に示した。
次に各ステント群の血管閉塞率の比較を実施した。各ステント断面の血管内腔面積(LA:Lumen Area)、血管内弾性板内側面積(IELA:Area within the Internal Elastic Lamina)を測定した。血管内腔面積(LA)および血管内弾性板内側面積(IELA)を用いて血管閉塞率を次式に従い算出した。式:血管閉塞率(%)=(1−(LA/IELA))×100
各群の血管閉塞率の平均を表3に示した。

- 管内ステントであって、該管内ステントは、圧縮された第1の直径から拡大された第2の直径まで半径方向に拡張でき、第1の直径にある時には管内ステントを構成するストラットが少なくとも1カ所において半径方向に重なりを有し、
前記ストラットの少なくとも1カ所において閉塞を抑制する薬剤が固定化されており、
管内ステントが第1の直径にあるときに、前記閉塞を抑制する薬剤が該管内ステントの外表面に存在しないが、管内ステントが第2の直径にあるときに、前記閉塞を抑制する薬剤が該管内ステントの外表面に存在することを特徴とする管内ステント。 - 該管内ステントは、さらに、管内ステントを構成するストラットが、第2の直径にある時に、半径方向に重なりを有しないことを特徴とする、請求項1記載の管内ステント。
