吸収/透過、吸収/反射、拡散反射および減衰全反射IR分光技術のような任意のタイプのIR分光技術を使用して、振動スペクトルを得ることができる。1実施形態においては、フーリエ変換IRマイクロ分光技術が使用される。IRマイクロ分光技術に関して、少量の微生物の懸濁液は典型的に、たとえば、約37℃で約16乃至24時間のあいだ適切な栄養になる寒天プレート上で培養される。少量の微生物は寒天プレートから除去され、蒸留水中で懸濁され、その後、この懸濁液の小滴はキャリアに移される。スペクトルは、適切なキャリア上に落とされた溶液、懸濁液あるいは粘性または固体の薄膜から得られてもよい。適切なキャリアはIR透過プレートであり、このIR透過プレートは水に溶けず、CaF2、BaF2、ZNSe、ZnSおよびゲルマニウムのような材料を含んでいる。スペクトルが薄膜から得られた場合、その小滴はデシケータ中において適度の真空下でシリカゲルのような乾燥剤の上方において脱水され、透過性薄膜を形成する。その後、微生物プレパラートを含む光学プレートが気密キュベットカートリッジ中に密封され、分光計中に位置される。微生物からスペクトルが得られたとき、希釈されたバイオマス懸濁液が栄養になる寒天プレートから適当なIR透過ウインドウに移される前に、それらは典型的に約37℃で約6乃至10時間のあいだ培養される。コロニーおよびマイクロコロニーの両者に関するIR分光法に対して、均一な薄膜および再生可能な結果を得るために懸濁液は典型的に標準化され、接種原濃度が調節される。もっとも、これは厳密に言えば常に必要とは限らない。計器のパラメータは、比較される必要のある全ての測定値に対して一定に維持されることが好ましい。適切なパラメータのセットは、1乃至15cm-1の公称物理解像度、および1000:1乃至10000:1の信号対雑音比を得るために十分な走査数である。1実施形態において、公称の物理解像度は6cm-1であり、信号対雑音比は3000:1よりよいものである。

処理手段は、典型的に、動作においてコンピュータプログラムを実行するコンピュータであり、この処理手段もまた本発明の一部である。コンピュータはパーソナルコンピュータであってもよいし、あるいは単一のプロセッサまたはマルチプロセッサシステムのような任意の他のタイプの処理装置であってもよい。プログラムは、たとえば、フロッピー(登録商標)ディスクまたはCD ROMのような記憶媒体中に記憶されていてもよく、たとえば、フロッピーディスクドライブまたはCD ROMドライブのような媒体ドライブ装置により読取られる。その代りに、プログラムは、たとえば、ハードドライブまたは別のメモリ装置のようなコンピュータの一部を形成する記憶媒体中に記憶される。
スペクトルデータベース中のスペクトルに関するさらに多くの情報は、そのスペクトルデータベースが随意に接続されることのできる情報データベースから得られてもよい。
本発明の計器は、ローカル、地域的または広域ネットワークのようなネットワークの一部であってもよい。“ネットワーク”という用語は、コンピュータ間でメッセージおよび情報が伝送されるように接続された2以上のコンピュータまたは処理システムを示している。このようなコンピュータネットワークにおいて、典型的に1以上のコンピュータは“サーバ”として動作し、1つのコンピュータはハードディスクドライブのような大型記憶装置と、プリンタおよびモデムのような周辺装置を動作させるための通信ハードウェアとを有している。“ワークステーション”または“クライアント”と呼ばれる別のコンピュータは、コンピュータネットワークのユーザが共用データファイル、共通周辺装置およびワークステーション間通信のようなネットワークリソースにアクセスできるようにユーザインターフェースを提供する。ユーザはコンピュータプログラムまたはネットワークリソースを起動して、コンピュータプログラムの一般的な動作と、入力変数およびその環境によって決定される特定の動作特性とを含んでいる“プロセス”を生成する。ネットワークは、1以上のサーバおよび1以上の、典型的には、数台のワークステーションとを含んでいる。サーバおよびワークステーションは、イーサネット(登録商標)ケーブルまたは無線接続のような別の適切な装置であることのできる通信ラインにより接続されている。ネットワークはまたいくつかの共有周辺装置を備えていることができる。本発明の1実施形態において、本発明の分光計は、サーバによってコンピュータに接続された遠隔機構である。
関心のあるサンプル中の微生物の振動スペクトルが第1のスペクトルデータベース中にすでに存在していることを示すプロンプトをユーザに与え、
別の特徴づけ手段を使用するようにユーザを促し、
第2の情報データベース中に情報を入力するようにユーザを促し、
適切な抗菌性治療を示唆し、
抗菌剤の効きめ(susceptibility)のプロフィールの変化についてユーザに警告し、
大発生の存在についてユーザに警告し、
持続する汚染をユーザに警告し、
地理的存在の異常な変化が発生したことをユーザに警告する。
別の特徴において、本発明は、関心のあるサンプル中の微生物に関する情報を得る方法を提供し、この方法は、
(a)関心のあるサンプル中の微生物の亜種特異性の振動スペクトルを得るために本発明による振動分光計を使用し、
(b)関心のあるサンプル中の微生物のスペクトルを、スペクトルを含まないか、あるいは1以上のスペクトルを含んでいる微生物のスペクトルの第1のデータベースに追加し、この第1のデータベースはコンピュータのような処理手段によりアクセスされることのできる記憶媒体中に位置しており、
(c)関心のあるサンプル中の微生物に関する情報を第1の情報データベースに追加するステップを含んでいる。追加される可能性のある情報のタイプは分離の時間および場所と、および関心のあるサンプルが分離された患者に関する情報とを含んでいる。第1の情報データベースは、そのスペクトルが第1のスペクトルデータベース中にすでに存在している微生物に関する情報を含んでいる。第1のスペクトルデータベースがスクラッチから形成されるためにこのデータベース中にまだスペクトルが存在しない場合には、当然ながら、何の情報も第1の情報データベース中に存在しない。この方法はさらに、
(d)自己生成アルゴリズムを実行し、第1のスペクトルデータベース中への新しいエントリに自己適合するコンピュータのような処理手段を使用して、未知の微生物のスペクトルが最も高いスペクトル類似性を示す第1のスペクトルデータベース中のスペクトルを決定し、
(e)関心のあるサンプル中の微生物が、第1のスペクトルデータベース中にそのスペクトルがすでに存在する微生物と同じ系統であると推断するのにこのスペクトル類似性が十分であるか否かを決定するための基準を与え、
(f)ステップ(e)において関心のあるサンプル中の微生物と第1のデータベース中のスペクトルとの間のスペクトル類似性が十分であると推断された場合には、第1の情報データベース中の関心のあるサンプル中の微生物系統に関する情報を検索するステップを含んでいる。第1の情報データベースは、第1のスペクトルデータベース中にそのスペクトルが含まれている微生物系統に関する情報を含んでおり、このような情報は、別の手段によって決定される微生物系統の同一性に関する情報、ある抗菌物質に対するその微生物系統の敏感度、その微生物系統が前に分離された時間および場所、その微生物系統により引起された感染の最中またはその後に観察された臨床的合併症、ならびにその微生物系統が分離された患者に関する情報を含んでいる。この方法はまた、
(g)第1のスペクトルデータベース中にすでに存在しているスペクトルとの間に十分なスペクトル類似性が見出されなかった場合には、関心のあるサンプル中の微生物に関して利用できる情報がないことを報告するステップを含み、それによってステップ(e)の前または後にステップ(b)および(c)が行なわれてもよい。
例1 サンプル準備処理
実験において使用された微生物系統は表1に示されている。10%のグリセリンを含む脳−心臓浸出液(infusion broth)中に−80℃で蓄積された系統(Becton Dickinson,Franklin Lakes,New Jersey,USA)が解凍され、一晩中Mueller−Hinton(MH)培養媒体上において培養された(37℃で約16乃至20時間)[Merck,Darmstadt,Germany]。その後、その培養媒体上で成長したコロニーからのバイオマスが1mmのプラチナループで収集され、厚さが1mmの25×40mmのCaF2ウインドウ上に移された。長さほぼ5mm、幅2mmの25の塗沫標本が5×5のマトリックスにロードされた。
ラマンスペクトルは、Renishawシステム1000のラマンマイクロ分光計を使用して得られた(Renishaw plc,Gloucestershire,UK)。Renishawシステムの顕微鏡部分を形成するライカDM−LM顕微鏡は80x近赤外線対物レンズ(MIR Plan80x/0.75、オリンパス)をつけられた。分光計の入射スリット幅は30μmに設定された。分光計は300個のレンズ/mmの格子を備えていた。ラマン信号は250cm-1から2150cm-1までのスペクトルインターバルで約8cm-1のスペクトル解像度により収集された。ラマン測定は、100mWのレーザパワーをサンプル上に供給するアルゴンイオンレーザ(モデル2000、Spectra Physics,Mountain View,CA,US)によってポンプされたチタニウムサファイアレーザ(モデル3900、Spectra Physics,Mountain View,CA,US)からの830nmの励起を使用して行われた。
測定されたスペクトルを系統識別用のアルゴリズムにかける前に、スペクトルは以下のように予備処理される。レーザ光伝播路中の光学素子から生じた一定の背景信号の影響が全てのスペクトルから減算して除去された。スペクトルの波数および強度軸が共に較正された。CCDカメラ上に投影される波数の範囲は、絶対ピーク位置が知られている2つの較正ランプ(ネオンランプおよびネオン・アルゴンランプ)のスペクトルと、ラマンピークの相対位置が正確に決定されている1以上のラマン規格(たとえば、シクロヘキサン、4-アセトアミドフェノール)とを測定することによって決定された。最初に、3次の多項式が絶対較正ポイント(画素数の関数としての、ナノメートルでのピーク位置)を通るように適合され、絶対波長軸が得られた。その後、正確なレーザ波長を計算するためにラマンピークの相対位置が使用された。絶対波長軸およびレーザ波長から、相対波数軸が決定された。この結果、波数軸の正確さは波数中の画素幅の約1/10、すなわち約0.3波数となった。
管理されていない分類方法によりデータベース中のスペクトルをスペクトルサブスペースに区分することは、データベースを18のサブスペースに分割する2つの階層レベルでの4つの自動生成LDAモデルによって行われる。最大のサブスペースは240のスペクトルから構成され、最小のサブスペースは20のスペクトルから成る。
実際の状況をシミュレートする3つのテストが行われた:
(a)A.baumanniiの単一の散発性系統のスペクトルがモデルに与えられた。この例は、これまで遭遇されたことのない系統のスペクトルが解析されたときに得られた結果を解析するように機能する。
(b)次に、スペクトルデータベースから省かれたDNAグループ3の大発生からのA.baumannii分離物のスペクトルが分類モデルに与えられた。ここで、4つの疫学的に関連づけられた別のA.baumannii分離物はスペクトルデータベース中にすでに含まれていた。これは、新しい大発生分離物に遭遇したときの大発生中の状況をシミュレートしている。
(c)最後に、ニューカッスルの大発生からのA.baumannii分離物のスペクトルがモデルに与えられた。5つの分離物のスペクトルは1つもスペクトルデータベースに含まれなかった。その後、これらの5つの各分離物のスペクトルは分類モデルに与えられた。したがって、大発生が始まるときの状況がシミュレートされる。
第1のテストにおいて、データベース中にスペクトルが存在しない散発性のA.baumannii系統のスペクトルが分類モデルに与えられた。それは、A.baumannii系統のみを有するスペクトルサブスペースに属するものとしてLDAモデルにより認識された。しかしながら、このクラスタ内のスペクトルとデータベース中にすでに存在しているスペクトルの任意のものとの類似性は、その分離物の分光学的識別を可能にするには不十分であった。分類モデルはその分離物を未知のものとして分類し、この系統に関する情報を供給するようにユーザを促した。その後、スペクトルはデータベースに自動的に追加された。それに続いて、分離された同じものから得られた残りの9つのスペクトルが分類モデルに与えられ、全て正しく分光学的に新しく追加された系統として識別され、ユーザにより供給されたその情報が利用可能になった。
第2のテストにおいて、A.baumanniiの分離物のスペクトルが解析され、それはバジルドンの大発生に属するものであった。異なった患者から得られたこの大発生の系統の別の4つの独立した分離物のスペクトルは、データベース中にすでに存在していた。この解析の第1のLDAモデルは、これらのスペクトルを、アシネトバクター系統だけを含むサブグループに割当てた。第2のLDAモデルは、これらのスペクトルを、バジルドンおよびヴェンロー大発生からのA.baumannii系統だけにより占有されているスペクトルサブスペースに属するものとして分類した。このスペクトルサブスペースに関するクラスタ解析は、分離物のスペクトルの全てがデータベース中にすでに存在するバジルドン大発生の分離物のスペクトルに類似しており、その類似の程度は、新しい分離物をバジルドン大発生系統のものとして識別するのに十分であることを示した。
第3のテストにおいて、ニューカッスルの大発生の5つの分離物から得られたスペクトルが分類モデルにより解析され、データベースに1つづつ追加された。第1のLDAモデルは、それに与えられた第1のニューカッスルの大発生スペクトルを、アシネトバクター系統だけを有するグループに属するものとして分類した。このグループをさらに再分するLDAモデルは、これらのスペクトルを、2つの別の独立した大発生(すなわち、バジルドン大発生およびヴェンロー大発生)の分離物のスペクトルを含むスペクトルサブスペースに属するものとして分類した。しかしながら、その新しいスペクトルを含むこのスペクトルサブスペースに関して行われたクラスタ解析の結果は、新しいスペクトルとすでに存在するスペクトルの任意のものとの間のスペクトル類似性が識別に対して十分ではないことを示した。スペクトルは、それがLDAモデルにより割当てられたスペクトルサブスペースでスペクトルデータベースに追加され、ユーザはこの分離物に関する情報を情報データベースに提供するように促された。それに続いて、この分離物およびこの大発生系統から得られた他の4つの分離物の他のスペクトルが直ぐにこの新しいエントリとして識別された。これは、依然として知られていない系統により引起された感染病の大発生を自動的に検出するために本発明がどのようにして使用されることができるかを示している。情報データベース中に入力された情報は、原則的に、未知の系統に対するコード名に限定される可能性が高い。それにもかかわらず、本発明の分光学的分類法システムは、別の分離物が同じ系統を含んでいるときにその大発生をユーザに自動的に知らせるものである。
Claims (13)
- 微生物の発生の事前の疑いなしに、および関係のある微生物の事前の分類学的分類を必要とせずに微生物の発生を検出する方法において、
(a)関心のあるサンプル中の微生物についての情報を得るために、振動分光計と、コンピュータと、第1のスペクトルデータベースと、第1の情報データベースとを具備している装置を使用して関心のあるサンプル中の微生物の振動スペクトルを獲得し、
(b)前記第1のスペクトルデータベースは1以上の振動スペクトルを含んでおり、前記第1のスペクトルデータベース中に存在している微生物の振動スペクトルに対する関心のあるサンプル中の微生物の獲得した振動スペクトルの類似性を確立することによって、獲得した振動スペクトルに基づいて関心のあるサンプル中の微生物を分類し、
(c)関心のあるサンプル中の後続する微生物を分類するために第1のスペクトルデータベースに獲得した振動スペクトルを追加し、
(d)前記ステップbにしたがって同じ微生物が分類されている異なる対象物の表面または個人から得られたサンプルの数を決定し、微生物の発生を示している期間にわたってサンプル中に特定の微生物を含んでいるサンプルの数が正常の場合のその特定の微生物を含んでいるサンプルの数よりも大きい状態が発生したことを発見するステップを含み、
前記振動分光計は前記コンピュータに結合され、前記サンプル中の微生物の振動スペクトルを決定し、
前記コンピュータは、前記振動分光計および前記第1のスペクトルデータベースならびに前記第1の情報データベースに結合されて、前記振動分光計から得られた信号の解析および関心のあるサンプル中の微生物を分類のための実行可能なソフトウエアコードを含むコンピュータプログラムを実行し、
前記第1のスペクトルデータベースは前記コンピュータに結合され、振動スペクトルが第1のスペクトルデータベース中に存在している微生物の振動スペクトルの類似性を確立するための1以上の振動スペクトルを含んでおり、
前記第1の情報データベースは前記コンピュータに結合され、関心のあるサンプルの分離された時間と場所についての情報と、他のサンプルから分離された、振動スペクトルが前記第1のスペクトルデータベース中に存在しているサンプルの対象物または個人についての情報とを含んでいる微生物の発生を検出する方法。 - 前記関心のあるサンプルは、(i)固体培養媒体上で成長させたバイオマスのコロニー、または(ii)処理せずにキャリア材料に移された液体培養媒体からの濃縮されたペレットの一部である請求項1記載の方法。
- 前記コンピュータは、ローカルまたはグローバルネットワーク中の別のコンピュータにリンクされている請求項1または2記載の方法。
- さらに、前記装置は、第1のスペクトルデータベース中に存在しない振動スペクトルを含んでいる第2のスペクトルデータベースと、
関心のあるサンプルが分離された時間と場所についての情報と、他のサンプルから分離された、振動スペクトルが前記第2のスペクトルデータベース中に存在しているサンプルの対象物または個人についての情報とを含んでいる第2の情報データベースとを備えている請求項1乃至3のいずれか1項記載の方法。 - さらに、第2の情報データベース中に情報を入力するようにユーザを促す場合および微生物の発生をユーザに警告する場合の少なくとも一方の場合において可視または可聴の信号を発生する請求項4記載の方法。
- 前記コンピュータは、関心のあるサンプル中の微生物の振動スペクトルと第1のスペクトルデータベース中に存在する微生物の振動スペクトルとの類似性を確立するアルゴリズムを自身で生成する請求項1乃至5のいずれか1項記載の方法。
- コンピュータは、第1のスペクトルデータベースに基づいたアルゴリズムを実行する請求項1乃至5いずれか1項記載の方法。
- 第1のスペクトルデータベースまたは第2のスペクトルデータベースをそれぞれ第1の情報データベースまたは第2の情報データベースと組合せて1つのデータベースとしている請求項1乃至7のいずれか1項記載の方法。
- 振動分光計はラマン分光計であり、振動分光計から出力される振動スペクトルはラマンスペクトルである請求項1乃至8のいずれか1項記載の方法。
- 関心のあるサンプルは、個人の患者から得られた物質または医療器具、工場生産ライン、給水システム、食品、化粧品または医薬品である請求項1乃至9のいずれか1項記載の方法。
- 関心のあるサンプル中の微生物は、葡萄球菌、腸球菌、ミコバクテリウム属、アシネトバクター属、連鎖状球菌、シュードモナス属、サルモネラ属、エシェリヒア属の任意のもののメンバーである請求項1乃至10のいずれか1項記載の方法。
- 微生物の分類は、種または系統レベルのものである請求項1乃至11のいずれか1項記載の方法。
- 院内感染の事前の疑いを必要とせずに、関係のある微生物の事前の分類学的分類を必要とせずに院内感染を決定する方法において、
(a)サンプル中の微生物についての情報を得るために、振動分光計と、コンピュータと、第1のスペクトルデータベースと、第1の情報データベースとを具備している装置を使用して個人から関心のあるサンプル中の微生物の振動スペクトルを獲得し、
(b)前記第1のスペクトルデータベースは1以上の振動スペクトルを含んでおり、前記第1のスペクトルデータベース中に存在している微生物の振動スペクトルに対する関心のあるサンプル中の微生物の獲得した振動スペクトルの類似性を確立することによって、獲得した振動スペクトルに基づいて関心のあるサンプル中の微生物を分類し、
(c)関心のあるサンプル中の後続する微生物を分類するために第1のスペクトルデータベースに獲得した振動スペクトルを追加し、
(d)前記ステップbにしたがった微生物の分類により特定された微生物が存在することを検出した日付を決定し、検出した日付が病院に入院した日付よりも後であり、潜伏期間が現時点までの入院期間よりも短い場合には院内感染であることが示され、
前記振動分光計は前記コンピュータに結合され、前記サンプル中の微生物の振動スペクトルを決定し、
前記コンピュータは、前記振動分光計および前記第1のスペクトルデータベースならびに前記第1の情報データベースに結合され、前記振動分光計から得られた信号の解析および関心のあるサンプル中の微生物の分類のための実行可能なソフトウエアコードを含むコンピュータプログラムを実行し、
前記第1のスペクトルデータベースは前記コンピュータに結合され、前記第1のスペクトルデータベース中に存在している微生物の振動スペクトルとの類似性を比較するための1以上の振動スペクトルを含んでおり、
前記第1の情報データベースは前記コンピュータに結合され、関心のあるサンプルの分離された時間と場所についての情報と、他のサンプルから分離された、振動スペクトルが前記第1のスペクトルデータベース中に存在しているサンプルの個人についての情報とを含んでいる院内感染を決定する方法。
