Description
図1は、本発明の原理による前庭刺激装置30の典型的な実施の形態を示すブロック図である。前庭刺激装置30は、患者の平衡感覚に関連する迷路部分及び/又は関連神経を刺激して、患者に治療の恩恵を与える装置である。より具体的には、本発明は、侵襲性であると非侵襲性であるとを問わず、平衡感覚神経に関連する迷路の受容体及び/又は前庭神経及びその神経分岐を含む前記受容体に関連する複数の神経又は神経分岐を刺激することを企図する。本発明は、前記刺激部位の少なくとも1つ又はそれ以上に刺激を印加して、a)患者の自然な呼吸機能を増大し又は支援し、b)患者の気道を開放し、c)睡眠を誘発し及びd)目眩を防止する機能の1つ又はそれ以上を実行することを企図する。前記各機能を達成する特定の刺激部位及び好適な刺激メカニズムの詳細を以下説明する。しかしながら、本発明の刺激装置の概要を最初に説明する。本発明で重要なシミュレーション部位は、前庭系と一般に指称される平衡感覚に関連する人間の内耳の前記構造及び/又は組織であるから、本発明の刺激装置を「前庭刺激装置」として本明細書中に示す点で留意を要する。
本発明の一実施の形態では、前庭刺激装置30, 30'を使用して、患者の呼吸作用を増大させる生理学的機能を達成することができる。図3及び図4に示すように直接的に又は図2及び図5に示すように間接的にの何れかで、患者の呼吸サイクルに同期して前庭神経を刺激することによってこれを達成することができる。図2〜図7について、本実施の形態を以下詳細に説明する。
多くの個人に対する閉塞性睡眠無呼吸症が発生する場合に、例えばオトガイ舌筋等の上部気道に関連する筋肉の弛緩は、第一の要因ではないが、重要要因であると思われる。呼吸サイクルの吸気段階の間に少なくとも前記上部気道筋を緊張させることは、上部気道の崩壊を最小にすることも判明した。このように、本発明の別の実施の形態は、少なくとも呼吸サイクルの吸気段階の間、上部気道筋を緊張させることによって、閉塞性睡眠無呼吸症の発生又は上部気道抵抗を減少し又は最小化することを企図する。前庭神経の刺激は、上部気道の筋群に関連する第一の神経である舌下神経及び反回神経の励振を誘発するので、前庭神経の刺激により、上部気道筋を緊張させ、これにより上部気道の圧壊を最小化するものと認められる。このように、本発明は、前庭系を刺激して上部気道の崩壊を最小化することを企図する。
本発明は、前庭系に印加する刺激量を変更して、吸気作用又は呼気作用の力及び持続時間を制御することも企図する。例えば、時々深いため息をつくことは呼吸作用に有益であることは公知である。このように、本発明の前庭刺激装置を使用して、呼吸サイクルの吸気段階の間に、深いため息を誘導することができる。
本発明の他の実施の形態は、適切に計時された方法で前庭系の適当な部分に刺激を印加し、患者の揺動感を誘発させることを企図する。前庭系の人工的刺激から発生する揺動感は、当に患者を物理的に揺するように患者に睡眠を誘導し、睡眠中の患者はより安穏な睡眠を促進すると思われる。
本発明のなお更なる実施の形態は、適切に計時された方法で前庭系の適当な部分に刺激を印加して、目眩及び/又は患者の環境が回転している感覚である目眩を防止することを企図する。目眩は、脳が独楽感覚と認める励振パターンにより神経学上の信号を出力する前庭系の結果である。目眩は、必ずしも患者を物理的に回転させる結果ではない。しかしながら、この種の物理的回転から目眩が生じることがある。
Claims (8)
- 電源と、
患者の前庭系に接近して前記患者の表面上に配置される非侵襲性の電極と、
前記電源及び前記電極に作動連結される制御ユニットであって、前記患者において揺動感覚を誘導して睡眠を刺激するために正弦波形の刺激エネルギを生じるように、前記電極を介した前記患者の左側又は右側前庭系の少なくとも一部に対する前記電源からの刺激エネルギの印加を制御する制御ユニットと、
前記患者の体位を検出する体位センサであって、前記制御ユニットが、前記体位センサの出力に基づいて前記電極に印加される刺激エネルギを制御する、体位センサとを備える前庭刺激装置。 - 前記患者の生理学的特性を検出する手段をさらに備え、前記制御ユニットが、検出された生理学的特性に基づいて前記電極に対する刺激エネルギの印加を制御する、請求項1に記載の前庭刺激装置。
- 前記生理学的特性が、前記患者が睡眠中であるかどうかを表す特性であり、当該装置が、前記検出された生理学的特性に基づいて前記患者の睡眠時を決定する睡眠検出手段をさらに含み、前記制御ユニットが、前記睡眠検出手段の出力に基づいて前記電極に対する刺激エネルギの印加を制御する、請求項2に記載の前庭刺激装置。
- 前記患者の運動を検出する運動センサをさらに含み、前記制御ユニットが、前記運動センサの出力に基づいて前記電極に対する刺激エネルギの印加を制御する、請求項1に記載の前庭刺激装置。
- 電源と、
患者の前庭系に接近して前記患者の表面上に配置される、第1の非侵襲性の電極と、
前記電源及び前記第1の電極に作動連結される制御ユニットであって、前記患者において揺動感覚を誘導して睡眠を刺激するために正弦波形の刺激エネルギを生じるように、前記第1の電極を介した前記患者の左側又は右側前庭系の少なくとも一部に対する前記電源からの刺激エネルギの印加を制御する制御ユニットと、
前記患者の睡眠時を決定する睡眠検出手段であって、前記制御ユニットは、前記睡眠検出手段の出力に基づいて前記第1の電極に対する刺激エネルギの印加を制御する、睡眠検出手段とを備える前庭刺激装置。 - 前記睡眠検出手段が、前記患者の動きを検出する運動センサを含む、請求項5に記載の前庭刺激装置。
- 前記患者の表面上に配置される第2の非侵襲性電極をさらに含み、前記刺激エネルギが、前記第1の電極と前記第2の電極との間に変化する電流波形を印加することによって生じ、前記変化する電流波形が1つの極性又は交互に換わる極性を有する、請求項5に記載の前庭刺激装置。
- 電源と、
患者の前庭系に接近して前記患者の表面上に配置される、第1の非侵襲性の電極と、
前記患者の表面上に配置される、第2の非侵襲性電極と、
前記電源、前記第1の電極、及び前記第2の電極に作動連結される制御ユニットであって、前記患者において揺動感覚を誘導して睡眠を刺激するために正弦波形の刺激エネルギを生じるように、前記第1の電極と前記第2の電極との間に変化する電流波形を印加することによって前記患者の左側又は右側前庭系の少なくとも一部に対する前記電源からの刺激エネルギの印加を制御し、前記変化する電流波形が1つの極性又は交互に換わる極性を有する、制御ユニットと、
前記患者の運動を検出する運動センサであって、前記制御ユニットが、前記運動センサの出力に基づいて前記第1の電極に対する刺激エネルギの印加を制御する、運動センサとを備える前庭刺激装置。
