本願は、2003年3月28日出願の仮出願第60/457,911号の利益を主張する。
この発明は、真菌感染の治療に関する。特定的には、この発明は、骨髄移植に関連した侵入性アスペルギルス症といったアスペルギルス感染の治療および予防に関する。
侵入性アスペルギルス症(IA)は、菌糸の侵入、肺組織の破壊そして他の臓器への播種を特徴とし、同種骨髄移植(BMT)における院内肺炎および死亡の両方の最も大きな原因であり、その推定感染率は5〜10%、関連の死亡率は90〜100%である。歴史的に、IAについての最も重要な危険因子は好中球減少であったため、骨髄前駆体での再構成が同種BMTのマウスモデルでのIAに対しての防御を提供してきた。しかしながら、BMT被移植者におけるIAの疫学に関する最近の研究においては、好中球減少に関係する感染の減少および「遅発型(late-onset)」感染の増加、そしてこれに伴い対宿主性移植片病の発生が示されている。
この発明に従うと、哺乳動物におけるアスペルギルス感染を治療または予防するための方法であって、抗真菌性の有効量のチモシンα1(thymosin alpha 1:TA1)を含む医薬組成物を上記哺乳動物に投与するステップを含む。
臨床的および実験的証拠では、IAの抑制においてTh1細胞反応の有する役割が示唆されている。樹状細胞(DC)が、インビボおよびインビトロで真菌に対するTh1初回抗原刺激(priming)を命令する。真菌抗原に対する適当なT細胞応答を命令する肺DCの能力は、Toll様受容体(TLR)を通じてのシグナリングを含む局所的な免疫制御事象から影響を受ける場合があることが証拠によって示されている。DCは、免疫療法およびワクチン開発における将来有望な介入のターゲットである可能性があり、医薬品による介入についての関心は「アジュバント」の方へとシフトしている。感染と戦うよう最適化された適当な種類の応答を刺激するとともに免疫抑制の各状況下で有効である能力のあるアジュバントであれば有利である。
細胞機能の増強を中心とするその免疫調節活動に関係すると考えられている。生得免疫系における細胞に対する免疫調節機能、たとえばマクロファージにおける有糸分裂促進因子活性化タンパク質キナーゼ(MAPK)および遺伝子発現を活性化する能力などから、TA1は、DCをアスペルギルスに対するTh1初回抗原刺激へと活性化する能力のあるアジュバントであると発明者は考えてきた。この発明は、アスペルギルス感染の治療であって、TA1が、TLRを通じてのシグナリングによって抗真菌Th1初回抗原刺激へとDCを活性化し得るというものを提供する。

メス、8〜10週齢のBALB/cおよびC57BL6マウスは、チャールズ・リバー(Charles River)からのものとした。NOD/SCIDは、ザ・ジャクソン(The Jackson)からのものとした。C57BL6の環境で育てられた、同型接合のTLR2、TLR9およびMyD88欠損マウス、および、BALB/cの環境で育てられた、同型接合のIFN−γおよびIL−4欠損マウスの繁殖対を、特定病原体のない条件下で繁殖させた。
アスペルギルス・フミガトゥス(A. fumigatus)に感染させるために、分生子2×107個/20マイクロリットル塩水の懸濁液を3日間続けてマウスに鼻腔内注入した。肺内の真菌成長の定量化にはキチン質試験が用いられた。キチン質の含有量は、臓器当りでグルコサミンのマイクログラムとして表わされた。非感染マウスの肺のグルコサミン含有量が陰性対照実験として用いられ、グルコサミン0.80〜2.25マイクログラム/臓器の範囲内であった。肺を摘出して直ちにホルマリンで固定し、組織分析できるようにした。パラフィンに埋込まれた組織の切片(3〜4ミクロン)を周期的な酸シッフ法で染色した。チモシンα1(TA1)およびスクランブルされたポリペプチドは、標準的なカブトガニ細胞分解産物試験による、内毒素レベルが0.03pg/ml未満の純化した無菌凍結乾燥アセチル化ポリペプチドである。配列は以下のとおりであった。Ac−Ser−Asp−Ala−Ala−Val−Asp−Thr−Ser−Ser−Glu−Ile−Thr−Thr−Lys−Asp−Leu−Lys−Glu−Lys−Lys−Glu−Val−Glu−Glu−Ala−Glu−Asn−O(チモシンα1)およびAc−Ala−Lys−Ser−Asp−Val−Lys−Ala−Glu−Thr−Ser−Se
r−Glu−Ile−Asp−Thr−Thr−Glu−Leu−Asp−Glu−Lys−Val−Glu−Val−Lys−Ala−Asn−Glu−OH(sチモシンα1)。これらを凍結乾燥した粉末を無菌水で戻した。
チモサン(Zymosan)は、サッカロマイセス・セレヴィシアエ(Saccharomyces cerevisiae)からのものとし、リポテイコ酸(LTA)はスタフィロコッカス・アウレウス(Staphylococcus aureus)から、そしてリポ多糖(LPS)はサルモネラ・ミネソタ(Salmonella Minnesota)Re 595からのものとした。CpGオリゴヌクレオチド1826および2006は、過去に実績のある免疫刺激配列であった。
C57BL6マウスを致死量の9Gyに晒し、BALB/cマウスからのT細胞枯渇ドナー細胞を注入した。95%超のマウスが生存し、脾臓の細胞でのドナー型MHCクラスI抗原発現で見られるような安定のドナー型造血キメラ現象を呈した。
CD14+単核細胞から磁気細胞分離で血液CD11c+骨髄DC(MDC)を作製し、50ng/ml rヒトGM−CSFおよび200U/ml rヒトIL−4の存在下で、10%のウシ胎仔血清、50マイクロモルの2−メルカプトエタノール、ピルビン酸ナトリウム(1mM)、2mMのL−グルタミン、HEPES(10mM)、および50マイクログラム/mlのゲンタマイシンを含むイスコーブ(Iscove)変性培地において5日間培養した。1000ng/ml三量体ヒトCD40リガンド−ロイシン−ジッパー融合タンパク質で未熟MDCを24時間培養して成熟DCを得た。BDCA−4分離キットを用いてCD123+形質細胞様DC(PDC)を分離した。CD123+細胞の純度は>96%であった。
がわかった。HLAクラスII、CD80およびCD86の発現は、未熟DCおよび成熟DCの両方で高かった。磁気細胞分離によってマウス肺CD11c+DC(CD8αについて5〜7%陽性、Gr−1について30〜35%陽性)を分離した。
試料をFITCまたはPE複合ラット抗マウス抗体で反応させることによって、細胞表面表現型を評価した。無関係の、アイソタイプ(hisotype)を一致させた抗体を対照実験として用いた。
貪食作用を決定する際には、気管支肺胞マクロファージおよび周辺の好中球を100ng/mlのTA1に対して予め60分間晒し、非オプソニン化アスペルギルス分生子とともに37℃で60分間インキュベートした。これに加え、コロニー形成単位の数と、殺分生子活性と呼ばれるコロニー形成単位阻害のパーセンテージ(平均±SE)とを求めることによって、殺分生子活性を評価した。
野生型または、ヒトTLR2、TLR9およびTLR4/CD1427を安定にトランスフェクトした、ヒト胚腎臓細胞系HEK293を、10%FCS、HEPES(10nM)、L−グルタミン(2マイクログラム/ml)およびゲンタマイシン(50マイクログラム/ml)を補った低グルコースのダルベッコ(Dulbecco)変性イーグル(Eagle)培地内で培養した。さらに、形質転換体にピューロマイシン(100マイクログラム/ml)を補った。刺激実験を行なうために、細胞を、12ウェル組織培養プレートにおいて、細胞3〜5×105個/ウェルの密度で一晩中培養した。細胞を、単独またはTLRリガンドとともに5時間100ng/mlのTA1に通して刺激し、それから上澄み中でIL−8の産生を評価した。
培養上澄みにおけるTNF−α、IL−10、IL−12 p70、IFN−αおよびIL−8のレベルをキットELISAで判定した。試験の検出限界(pg/ml)は、TNF−αについては<3(ヒト)および<32(マウス)、IL−10については<12(マウス)および<5(ヒト)、IL−12 p70については<16(マウス)および<3(ヒト)、そしてIL−8については<25(ヒト)であった。ヒトIFN−αについては<3ng/mlであった。サイトカイン産生細胞の個数を数えるために、肺からの純化したCD4+T細胞およびDCに対してELISPOT試験を採用した。
10マイクログラム/mlのCon Aまたは肺DCの存在下の熱不活性化分生子で刺激した肺CD4+Tリンパ球の増殖を、CFSE 5(6)−カルボキシフルオレセイン・ジアセタート・サクシニミジルエステルで標識することによって評価した。
初期の実験で示唆されているように、未熟DCを100ng/mlのTA1で予め60分間処置してから非オプソニン化アスペルギルス分生子に対して60分間晒し、そこから全RNAを抽出した。順および逆PCRプライマーで、そしてマウスおよびヒトTLRおよびHPRTに用いられるサイクルで、cDNAの合成およびPCRを実行した。合成されたPCR産生物を2%アガロースゲルにおいて電気泳動により分離し、臭化エチジウム染色によって視覚化した。
肺DCにおいて、p38およびNF−kBを、アスペルギルス分生子および/または100ng/mlのTA1に対して37℃で20分間晒すことによって活性化した。製造元の指示により、細胞溶解物のブロットは、p38 MAPKについてのリン酸化されていない形態または二重にリン酸化された(Thr−180/Tyr−182)p38 MAPKのいずれかを認識するウサギポリクローナルAbsか、Rel Aに特異的なAbs、ヒトNF−kBの65kDa DNA結合サブユニットとともにインキュベートし、これにワサビペルオキシダーゼ複合ヤギ抗ウサギIgGが続いた。ブロットは、高感度化学発光検出キットで現像された。このブロットのコダック(Kodak)RXフィルムに対する露出後、バンドが視覚化された。各々のレーンにつき類似のタンパク質のローディングを確実にするために、リンのブロットを剥がして膜をp38およびNF−kBに対しAbsで再び精査した。
マウスDCは、インビトロおよび感染の部位でアスペルギルスを貪食することが以前から示されている。スクランブルされたペプチドでなくTA1が、(菌糸よりもむしろ)非オプソニン化分生子の貪食、共刺激抗原発現およびサイトカイン産生へと肺DCを活性化する。これとは対照的に、アスペルギルス分生子だけでは、DCの活性化を誘導するのに十分な刺激を表わしておらず、TA1に対する組合せの露出によって、MHCクラスII抗原、CD86およびCD40分子の発現、ならびにIL−12 p70産生DCの頻度、が顕著に増加した。興味深いことに、IL−12 p70産生DCは、チモシンのみでも増加した。TA1はまた、ヒトMDCおよびPDCサブセットを活性化する。未熟DCおよび成熟DCの両方のサブセットが分生子を貪食する。TA1は、未熟DCの貪食活動を増大させ、DC形態に影響を与え(未熟MDCにおいてより多くの細胞質突起が検出され得る)、分生子に応答してHLAクラスII抗原および共刺激分子発現をアップレギュレートする。TA1は、未熟MDCによる分生子およびチモサンに対する応答としてのIL−12 p70の放出を大幅に増加させるとともに、未熟PDCによる分生子に対する応答としてのIL−10の放出を大幅に増加させる。INF−αは、PDCにより、TLR9リガンドCpGに対する応答として産生され得るが、この産生はTA1によって大幅に増強される。これとは対照的に、スクランブルされたペプチドは、クラスII抗原および共刺激分子発現をアップレギュレートすることも、分生子に対する応答としてのDCによるサイトカイン産生を誘導することもできなかった。
TLRシグナリングはアスペルギルス分生子に応答して起こっており、これは真菌に対する機能的応答を媒介する。TA1は、マウスDCにおけるTLR2、TLR5およびTLR9の発現を強力に活性化する。TLR2およびTLR9は、分生子およびTA1に対する組合せの露出の際になお活性化されるのに対し、TLR5の発現については、その発現は阻害される。ここでもまた、スクランブルされたペプチドは、単独でまたは分生子への応答としてTLR2およびTLR9の発現を活性化させることができなかった。
TLR2およびTLR9欠損マウスにおける真菌の成長は、野生型マウスのそれと同等であり、チモシン治療の際に同様に減退する。MyD88欠損マウスにおいても、真菌成長は同等であるが、これらのマウスにおいてはTA1での治療の際に減退しなかった。よって、或る程度TLRの使用における冗長があるにもかかわらず、MyD88依存のシグナリング経路は、インビトロおよびインビボの両方でTA1の活動に本質的であると考えられる。
肺内の真菌成長の減少と並行した生存率の上昇でわかるように、スクランブルされたペプチドでなくTA1での治療によって、IAを持つBMTマウスを治癒できたと考えられる。防御に対する効果は用量に依存し、完全な防御(60日超の生存)は、200〜400マイクログラム/kgのTA1で治療されたマウスで達成され、アンフォテリシンBよりも優れている。さらに、両方の薬剤で治療されたマウスの生存率上昇および真菌負荷の
減少に示されるように、TA1はアンフォテリシンBの治療効力を増加させる。さらには、TA1は肺の病状を減少させる。感染したマウスの肺切片においては、肺の実質組織に浸潤した多数のアスペルギルス菌糸の存在が示され、気管支壁の損傷および壊死の重大な兆候があるとともに炎症細胞補充が少なかった。対照的に、これらの特徴は、TA1で治療されたマウスには認められず、その肺は炎症細胞の治癒浸潤物で特徴付けられ、真菌成長および気管支壁破壊の証拠がなかった。したがって、TA1は、IAにおける治療効力を有し得て、BMT状況において活動が低下することが知られている抗真菌薬との組合せで有益であり得る。
TA1治療後、循環するリンパ球および好中球の絶対数は大幅に増加する。より重要なことに、血中好中球レベルでは、アスペルギルス症に対する感染性は予測されない。しかしながら、細胞蛍光測定分析においては、肺CD4+およびCD8+細胞ならびに好中球の数は、TA1でのBMTマウスの治療の際に大幅に上昇した。これら肺CD4+Tリンパ球は、抗原に特異的な増殖およびIFN−γ産生によって示されるように機能的に活性である。TA1で治療されたマウスにおいては、Th1細胞(IFN−γを産生)の頻度はより高く、Th2細胞(IL−4を産生)はより低い。さらに、エフェクター食細胞の抗真菌活性に関し、TA1で治療されたマウスにおいてはマクロファージおよび好中球両方の殺分生子活性がより高くなる。したがって、TA1は、DC成熟を促進するばかりでなく、迅速な真菌の貪食および殺菌へと局所的なエフェクター細胞を活性化させると考えられる。
Claims (12)
- 抗真菌性の有効量のチモシンα1(thymosin alpha 1:TA1)を含む、哺乳動物におけるアルペルギルス感染を治療または予防するための医薬組成物。
- 前記TA1は、Th1細胞促進サイトカインを産生するように樹状細胞を活性化させるのに十分な用量で投与されるものである、請求項1に記載の医薬組成物。
- 前記TA1は、200〜400マイクログラム/kg体重/日の用量で投与されるものである、請求項1に記載の医薬組成物。
- 前記哺乳動物は免疫不全の宿主である、請求項1に記載の医薬組成物。
- 前記哺乳動物はヒトである、請求項4に記載の医薬組成物。
- 前記ヒトは骨髄移植の被移植者である、請求項5に記載の医薬組成物。
- 前記TA1は、Th1細胞促進サイトカインを産生するように樹状細胞を活性化させるために投与されるものである、請求項5に記載の医薬組成物。
- 前記TA1は、200〜400マイクログラム/kg体重/日の用量で投与されるものである、請求項5に記載の医薬組成物。
- 少なくとも1つの追加の抗真菌剤とともに投与されるものである、請求項1に記載の医薬組成物。
- 前記追加の抗真菌剤はアンフォテリシンBである、請求項9に記載の医薬組成物。
- 前記アンフォテリシンBは、4000マイクログラム/kg体重/日の用量で投与され
るものである、請求項10に記載の医薬組成物。 - 前記アルペルギルス感染は、侵入性アルペルギルス症である、請求項1に記載の医薬組成物。
