Description
関節軟骨の欠陥は、伝統的には、軟骨形成術、シェービング、微小骨折術、研磨関節形成術、ごく最近には、自家移植を用いて処置されてきた。早期には、関節軟骨の欠陥の処置は、主として、欠陥の進行を防止することに関して懸念事項が存在していた。より最近では、欠陥を実際に修復し、関節軟骨を治療する方法に注意が向けられた。
造血細胞形成移植と称されている、一つの処置では、健康な関節軟骨及びその下にある骨のプラグが、ドナー箇所から採取され欠陥箇所へと移植される。この技術は、有効であることが判っているが、それは、典型的に、深刻な損傷をドナー箇所に引き起こす。加えて、適切な表面プロフィールを備えたドナー箇所を見出すことは困難となり得、欠陥箇所の層と、移植プラグの層(即ち、軟骨及びその下の骨)とを適切に整列させることは困難となり得る。
本発明の別の目的は、微小骨折術及び研磨関節形成術で使用することができる、関節軟骨の欠陥の修復のための改善された方法を提供することである。
本発明のなお更に別の目的は、自家細胞移植で使用することができる、関節軟骨の欠陥の修復のための改善された方法を提供することである。
カプセル形成装置5は、関節軟骨の欠陥の修復を容易にするように、単一部材から形成されてもよく、又は、製造中に一緒に連結された複数の部材から形成されてもよい。
(微小骨折術及び研磨関節形成術)
図5を参照すると、関節軟骨の欠陥55が最初に微小骨折術及び研磨関節形成術により処置されることが示されている。
(万能幹細胞を用いた微小骨折術及び研磨関節形成術)
傷部内にカプセル形成装置5を配置する前に、万能幹細胞を用いて、傷部箇所に種蒔き状態を作り出すことも可能である。カプセル形成装置は、該カプセル形成装置が吸収され、再形成されるまで、傷部箇所で万能幹細胞を覆うことになる。本発明の一つの好ましい形態では、万能幹細胞は、身体の様々な領域(例えば、大腿切痕、腸骨稜、脊椎骨)から骨髄を吸い上げることにより採取され、次に、カプセル形成装置5の配置前又はこれと同時に、骨髄を傷部箇所に配置する。代替として、採取された骨髄は、万能幹細胞のより効能のあるスラリーを生成するように、種蒔き前に、フィルター処理されてもよい。
(自家細胞移植)
欠陥箇所でカプセル形成装置5を配置する前に、自家関節軟骨細胞を用いて、関節軟骨の欠陥55に種蒔きすることも可能である。カプセル形成装置は、該カプセル形成装置が吸収され、再構成されるまで、傷部箇所で自家関節軟骨細胞を覆うことになる。本発明の一つの好ましい形態では、自家関節軟骨細胞は、身体外部で採取、準備され(例えば、分離され、及び/又は、成長因子で強化され、及び/又は、増殖される)、次に、カプセル形成装置5の配置前又はこれと同時に、欠陥箇所に配置される。
(他の代替実施例)
次に図13を参照すると、本発明の代替形態を備えるカプセル形成装置5Aが示されている。カプセル形成装置5Aは、前述したカバー14というよりも、ボディ10Aがボディ10A内部の1つ以上の位置でマトリックス又はメッシュ85Aを備えている点を除いて、上述したカプセル形成装置5に類似している。1つ以上の脚部25Aがボディ10Aから延在している。マトリックス又はメッシュ85Aでボディ10Aを形成することは、例えば、微小骨折術及び研磨関節形成術の場合に、中実ボディ10を超える効果を奏することができる。それは、当該箇所に栄養物の優れた流れを提供し、当該箇所から浪費物の優れた流れを提供するか、又は、万能幹細胞を用いた微小骨折術及び研磨関節形成術の場合には、それは、欠陥箇所でカプセル形成装置を展開する前にカプセル形成装置上にそのような細胞のスラリーを配置することを可能にする。自家細胞移植の場合には、それは、移植関節軟骨細胞のスラリーを、欠陥箇所にカプセル形成装置を配置する前にカプセル形成装置上に配置することを可能にする。
図15Bはカプセル形成装置に固定された採取骨膜の一断片部分を示していることを除いて、図15及び図15Aと同様の概略図である。
図16Dは、図16CのラインXVI−XVIに沿って取られた断面図である。
Claims (14)
- 関節軟膏の欠陥の修復のためのカプセル形成装置であって、
前記軟骨欠陥の領域で骨に隣接して配置するためのボディと、
前記軟骨欠陥の領域で前記骨に配置するため前記ボディから延在する、細長い脚構成部であって、前記ボディの厚さよりも大きい複数段階の長さを有し、略錐体の形状を有する、前記脚構成部と、
を備え、
前記ボディは、周辺フレーム部と、カバー部と、を備え、
前記脚構成部は、前記フレーム部の末端部から延在する複数の細長い脚部を備える、カプセル形成装置。 - 前記脚構成部には、前記骨を把持するため、突部が形成されている、請求項1に記載のカプセル形成装置。
- 前記脚部の各々は、内部に中央チャンネルが形成され、該チャンネルの各々は、前記フレーム部の基端側で開放している、請求項1に記載のカプセル形成装置。
- 前記ボディは、メッシュ部を備える、請求項1に記載のカプセル形成装置。
- 前記ボディは、コラーゲンの足場を備える、請求項1に記載のカプセル形成装置。
- 前記ボディは、フレーム部と、該フレーム部に固定された骨膜のシートと、を備える、請求項1に記載のカプセル形成装置。
- 前記シートを前記フレーム部に付着させる縫合糸を更に備える、請求項6に記載のカプセル形成装置。
- 前記ボディは、カバー部の境界を形成する周辺フレームを備える、請求項1に記載のカプセル形成装置。
- 前記ボディは、シェル部と、該シェル部に固定され、該シェル部の中央から径方向に延在する補強支柱と、を備え、前記脚構成部は、前記ボディの中央から末端方向に延在する単一脚を備える、請求項1に記載のカプセル形成装置。
- 前記脚は、挿管されている、請求項9に記載のカプセル形成装置。
- 前記ボディは、周辺フレームと、該フレームから内側に延在し、前記ボディの中央部で連結されている、支柱と、を備え、前記脚構成部は、前記ボディの中央の末端表面から末端方向に延在する単一脚を備える、請求項1に記載のカプセル形成装置。
